Repurposing anti-phage defenses to differentially arrest the viral lifecycle reveals the regulatory logic of a parasitic satellite.

Mobile genetic elements frequently encode defense mechanisms to protect their bacterial hosts from viral attack. In Vibrio cholerae, these defensive elements include phage-inducible chromosomal island-like elements (PLEs), which are phage satellites that act as highly specialized parasites of the lytic phage ICP1. While PLE transcriptional activation upon ICP1 infection is known to be temporally regulated, the underlying regulatory logic and dependencies on the progression of the phage's developmental program required for activation remain unclear. In this study, we took a novel approach to define these dependencies by introducing independent anti-phage defense systems, BREX and DarTG, as molecular roadblocks to impede the ICP1 lifecycle. We discovered that, for both ICP1 and PLE, late-stage gene expression is fundamentally uncoupled from genome replication, representing a striking departure from the standard paradigm for double-stranded DNA phages. While BREX restricts ICP1 to an immediate-early transcriptional state that stalls PLE activation, DarTG allows the phage to execute its full transcriptional cascade despite the total block in DNA replication. This permissive environment provides the necessary cues for complete PLE induction, revealing that the extent of ICP1 transcriptional progression is a key determinant of PLE transcriptional activation. Unlike other phage satellites that rely on a single cue for activation, our results demonstrate that PLE uses a progressive licensing strategy that relies on multiple cues tied to milestones in the phage's developmental program. This regulatory architecture ensures robust PLE activation resilient to phage escape.

A logical approach to process validation.

KISS stands for "Keep it simple, stupid!" Sometimes those of us who cringe at the inelegance of this adage, nevertheless resort to it to point out that quality system policies, procedures, and work instructions should be described in clear and simple terms. This should also apply to process validation. This article will describe a logical approach to certain key elements of process validation that should help in meeting that objective.

NLP techniques associated with the OpenGALEN ontology for semi-automatic textual extraction of medical knowledge: abstracting and mapping equivalent linguistic and logical constructs.

This research project presents methodological and theoretical issues related to the inter-relationship between linguistic and conceptual semantics, analysing the results obtained by the application of a NLP parser to a set of radiology reports. Our objective is to define a technique for associating linguistic methods with domain specific ontologies for semi-automatic extraction of intermediate representation (IR) information formats and medical ontological knowledge from clinical texts. We have applied the Edinburgh LTG natural language parser to 2810 clinical narratives describing radiology procedures. In a second step, we have used medical expertise and ontology formalism for identification of semantic structures and abstraction of IR schemas related to the processed texts. These IR schemas are an association of linguistic and conceptual knowledge, based on their semantic contents. This methodology aims to contribute to the elaboration of models relating linguistic and logical constructs based on empirical data analysis. Advance in this field might lead to the development of computational techniques for automatic enrichment of medical ontologies from real clinical environments, using descriptive knowledge implicit in large text corpora sources.

Myeloproliferative Disease: Polycythemia Vera: The Packed Cell Volume and The Curious Logic of The Red Cell Mass.

Since the first systematic blood volume studies of polycythemia in the 1920s, measurement of blood volume and red cell mass (RCM) has become routine. However, the radionuclide-labeling methods promulgated by the International Committee for Standardization in Haematology (ICSH) remain complex and poorly understood. Many hematologists and other clinicians err in the belief that these methods permit "direct measurement" of RCM, whereas the ICSH method is indirect: it requires calculation of RCM from (PCV) x (whole blood volume). The use of an elevated value of PCV to calculate RCM in order to evaluate the same elevated value of PCV is a curiously circular logic that is embraced by most clinicians and most hematologists. Analysis of published data in 186 cases of polycythemia vera indicates that RCM is an exponential function of PCV. In most cases, PCV alone suffices to document normal or increased RCM. Relative polycythemia results from dehydration, not from stress. Clinicians need to be aware of the range of physiologic fluctuations that normally occur in plasma volume. Realistic criteria for normal ranges of PCV, Hb concentration and RCM should be adopted in clinical laboratories so that clinicians will not be misled to undertake futile and costly investigations of results that are in the upper percentiles of the normal distribution, as exemplified by the Ulysses Syndrome.

[Logical and mathematic substantiation of selection of a model for health care system in a big city].

Based on the study and analysis of results of the health system activities it can be ascertained that the existing system of care of public health is not in a position to accomplish tasks it faces related to maintenance and promotion of health of the population. A logical-and-mathematical substantiation of an essentially new variant of the public health system is submitted. The results of the study made formed a basis for carrying a reform of the health system in the city of Chernigov.

Dorsal placement of the pedicled preputial/penile onlay flap for anterior urethral stricture: is it more logical?

OBJECTIVES: We present our preliminary experience in the management of inflammatory anterior urethral strictures with a dorsally/dorsolaterally placed penile/preputial vascularized flap and discuss the distinct advantages of this procedure over a traditional ventrally placed flap. METHODS: Twelve patients (age, 20-66 years; mean age, 40.5 years) with recurrent inflammatory strictures of the penile and/or bulbar urethra (penile, 2; bulbar, 5 and bulbopenile, 5) were treated with dorsally/dorsolaterally placed penile/preputial (penile skin, 5; prepuce, 7) vascularized flap substitution urethroplasty. Prior to surgery, 7 patients had suprapubic cystostomy for acute urinary retention and 5 had a mean peak flow rate of 6.2 ml/sec. Inferior pubectomy was performed as an adjunct in two patients to facilitate proximal placement of the flap. Follow-up (5 to 24 months; mean, 15.5) included uroflowmetry and retrograde urethrograms at 6, 12 and 18 months, and thereafter as required. RESULTS: The mean stricture length was 5.8 cm (range, 3 to 12 cm) and all strictures were associated with dense spongifibrosis. Temporary urethral fistulae, which healed spontaneously, occurred in 4 patients. The clinical and radiological outcome during the mean follow up of 15.5 months was gratifying. The mean peak flow rate improved to 24.2 ml/sec. No stricture recurrence has been noted to date. Sacculation of the flap, diverticulum formation and post-void dribble have not been encountered. None of the patients developed chordee or erectile dysfunction. CONCLUSIONS: Dorsal onlay preputial/penile flap urethroplasty is a versatile procedure in the treatment of inflammatory anterior urethral strictures. The dorsal pedicle flap may be anatomically and functionally more logical as compared to the traditional ventrally placed flap.

[The European Union and the migrations of populations. Government logic and human rights].

"The first part of this article consists of a summary of the major [immigration] reforms undertaken by the EU [European Union] states over the past decade. The second chapter discusses the ways in which these reforms were drafted, and, in particular, the influence of intergovernmental negotiations on national policy. The third part, relative to the regulation of job markets, underscores the contrast between the tough restrictions on permanent employment of foreigners and flexibility...for administering seasonal immigration or entry visas granted within the context of temporary work contracts. The fourth part, which analyzes the impact of these legislative changes on individual rights, logically leads to a discussion of the foundation of the procedure followed by the wealthy countries, and points out the shortcoming of a strictly punitive or even military solution to what is chiefly a problem of development and democracy." Performance of a brain PET camera based on anger-logic gadolinium oxyorthosilicate detectors.

UNLABELLED: A high-sensitivity, high-resolution brain PET scanner ("G-PET") has been developed. This scanner is similar in geometry to a previous brain scanner developed at the University of Pennsylvania, the HEAD Penn-PET, but the detector technology and electronics have been improved to achieve enhanced performance. METHODS: This scanner has a detector ring diameter of 42.0 cm with a patient aperture of 30.0 cm and an axial field of view of 25.6 cm. It comprises a continuous light-guide that couples 18,560 (320 x 58 array) 4 x 4 x 10 mm(3) gadolinium oxyorthosilicate (GSO) crystals to 288 (36 x 8 array) 39-mm photomultiplier tubes in a hexagonal arrangement. The scanner operates only in 3-dimensional (3D) mode because there are no interplane septa. Performance measurements on the G-PET scanner were made following National Electrical Manufacturers Association NU 2-2001 procedures for most measurements, although NU 2-1994 procedures were used when these were considered more appropriate for a brain scanner (e.g., scatter fraction and counting-rate performance measurements). RESULTS: The transverse and axial resolutions near the center are 4.0 and 5.0 mm, respectively. At a radial offset of 10 cm, these numbers deteriorate by approximately 0.5 mm. The absolute sensitivity of this scanner measured with a 70-cm long line source is 4.79 counts per second (cps)/kBq. The scatter fraction measured with a line source in a 20-cm-diameter x 19-cm-long cylinder is 39% (for a lower energy threshold of 410 keV). For the same cylinder, the peak noise equivalent counting rate is 60 kcps at an activity concentration of 7.4 kBq/mL (0.20 micro Ci/mL), whereas the peak true coincidence rate is 132 kcps at an activity concentration of 14 kBq/mL (0.38 micro Ci/mL). Images from the Hoffman brain phantom as well as (18)F-FDG patient scans illustrate the high quality of images acquired on the G-PET scanner. CONCLUSION: The G-PET scanner attains the goal of high performance for brain imaging through the use of an Anger-logic GSO detector design with continuous optical coupling. This detector design leads to good energy resolution, which is needed in 3D imaging to minimize scatter and random coincidences.

Multisystem disorder of Punjabi children exhibiting spontaneous dermal and submucosal granulation tissue formation: LOGIC syndrome.

We describe a multisystem disease that affects children of Muslim families originating in the Punjab region of Pakistan and India. An altered cry due to vocal cord thickening, skin ulceration, nail abnormalities, and conjunctival scarring appear in the first few months of life. Progression and spread of the disease in these sites may be accompanied by involvement of other epithelial surfaces. The teeth may exhibit defective enamel formation. Histology reveals the formation of simple granulation tissue arising in the dermis and submucosa which become massively thickened and ulcerated. There is good evidence for an autosomal recessive gene defect, but the actual mechanism of the disease is not known. Medical and surgical therapy have been ineffective in altering the course of this devastating and usually fatal condition. We suggest the term LOGIC (laryngeal and ocular granulation tissue in children from the Indian subcontinent) for this newly established disease.

Bureaucratic logic in new social movement clothing: the limits of health promotion research.

This paper discusses theoretical, methodological and political problems in the field of health promotion research. It argues that these problems result from a partial and contradictory appropriation of the discourse of new social movements. Politically, the health promotion movement is largely confined within the state, rather than the expression of a social movement against the state. The direction of health promotion research and policy is, therefore, caught in the bureaucratic logic of "trapped administrators", and results in contradictory emphases on problems like the development of "health promotion indicators", which show little result in informing a broader but coherent conceptualization of health, let alone in effecting change in health policy and outcomes. Such political problems reflect parallel confusions about theory and methodology. Theoretically, the field relies heavily on a critique of bio-medical science, but fails to move beyond a rhetorical outline of an alternative to systematic arguments about what promotes health. In this regard, the literature on health promotion remains unaware of important conceptual developments in the social sciences, relies on imprecise specifications of major constructs like community empowerment, and has no conception of the state. Methodologically, the literature is influenced by contradictory epistemological tendencies which reflect a positivist inspiration (as in the search for indicators) and an anti-positivist emphasis on agency and social change through the collective action and the discursive reconstitution of social identity, value and meaning. In regard to these questions, this paper is critical of observers who suggest that the way ahead is to embrace post-modern research strategies.(ABSTRACT TRUNCATED AT 250 WORDS)

["Unpredictablity of the quantum world" and logical catastrophe].

The fundamental gnoseological question that is always present in a basic science is: Can this science adequately predict the future? In classical mechanics, we deal in many cases with dynamic chaos; therefore, this prediction is most often impossible. In the quantum theory, the dynamic chaos associated with the disturbances of initial conditions is formally absent. Nevertheless, as it was shown in this work, there are other reasons that directly relate to what is called the paradoxes of formal logic, which, in the case of quantum theory, too, do not enable one to precisely determine what will happen in the future.

The internal logic of the Canadian Cardiovascular Society scale for grading angina pectoris: a first appraisal.

OBJECTIVE: To assess the internal logic (content validity) of the Canadian Cardiovascular Society (CCS) scale for grading angina pectoris. PATIENTS: Forty-one consenting patients with stable angina of at least two months duration, admitted to a tertiary centre for coronary angiography. METHODS: Patients completed a supervised questionnaire with closed-ended questions. Key questions included: usual numbers of blocks walked on the level or flights of stairs climbed before onset of chest pain; frequency with which chest pain occurred at the usual threshold distance; presence of rest pain; and influence of modifiers suggested for class II of the scale such as walking uphill and into the wind. RESULTS: Agreement of four questionnaire-defined 'stair-climbing grades' and 'walking grades' was statistically significant (P < 0.001) but only 37% better than expected by chance alone (weighted kappa). Frequency of angina at a patient's self-defined exercise threshold varied; only 22 of 41 patients (54%) had symptoms always or often. Higher classes of angina were more likely to be associated with frequent symptoms at threshold, eg, class I/II, six of 23 versus class III/IV, 16 of 17; 2P = 0.00002). Pain at rest was reported as 'definitely' present by 23 of 41 patients, and was similar in incidence across angina classes. All suggested modifiers reduced distances walked in a significant majority of patients (P values uniformly < 0.01) except for walking in the first few hours after awakening. However, the proportions of subjects for whom these factors were relevant were statistically similar for all angina grades, rather than for class II patients alone. CONCLUSIONS: These findings suggest that internal inconsistencies in the CCS scale are identifiable with simple validity checks. Further research appears warranted to improve this popular and useful clinical tool.

[Logical-mathematical "ND" software development].

In this article, we propose a mathematical analysis model for the generation of a proportional value of defining characteristics in nursing diagnoses. The study results are based on the logical-mathematical development of software specifically built for the generation of diagnosis hypotheses. The final calculation creates the sum of all defining characteristics found in each diagnosis, comparing it with the value of the V variable of the diagnosis, according to a mathematical rule that was set. It is concluded that the software needs to go through clinical validation in order to evaluate the pertinence of the rule proposed here.

Thrombolysis: the logical approach for the treatment of vascular occlusions.

Thrombotic and thromboembolic occlusions of arteries and veins represent acute and often life threatening complications requiring immediate therapeutic intervention. The most important clinical manifestations of vascular occlusions are myocardial infarction, peripheral arterial occlusion, pulmonary embolism, deep vein thrombosis and ischemic stroke. The logical approach for the treatment in these indications is the early restoration of blood circulation in order to preserve the organ deprived from oxygen supply and to prevent chronic sequelae. Recanalization by surgical intervention is only possible in some indications and is restricted to special clinics. Thrombolysis induced by agents activating plasminogen imitates the physiologic way of dissolving an occlusive clot by shifting the balance of the hemostatic and fibrinolytic system towards fibrinolysis. Streptokinase was the first effective thrombolytic drug used in patients. In the first years of its usage the identification of the appropriate indication and the dosage and application regimens used were based on little pharmacological knowledge and lack of appropriate dose finding. This resulted in suboptimal therapeutic efficacy and severe bleeding. Development of advanced diagnostic methods, more appropriate dose and application regimens and the development of more specific fibrinolytic drugs like rt-PA led to a remarkable improvement of its benefit-risk ratio and made thrombolysis to a widely accepted form of therapy in thrombotic and thromboembolic diseases. Early restoration of blood flow however is only the starting point of a therapeutic strategy, aiming at minimizing the risk of recurrence.(ABSTRACT TRUNCATED AT 250 WORDS)

Optimizing moulding conditions: a logical approach.

Setting and optimizing the moulding process for a new mould tool is critical to output efficiency. Using suitable statistical tools can help increase the number of components the equipment makes in an hour and improve the quality by reducing dimensional variation. This article looks at new methods that are turning a traditionally hit-and-miss endeavour into one that is logical and verifiable.

Interactive cutting simulation with adaptive refinements using digital logic design analogy.

A major requirement for surgical simulation is to allow virtual tissue cutting. This paper presents a scalable and adaptive cutting technique based on a mass-spring mesh. By the analogy of digital logic design, an arbitrary incision is modeled systematically by translating the cutting process into a state diagram. Subdivision of mesh elements is driven by the state transitions. Node redistribution, local re-meshing and deformation are applied to refine the subdivided mesh.

The REACH 2010 logic model: an illustration of expected performance.

The Centers for Disease Control and Prevention (CDC) supports 40 Racial and Ethnic Approaches to Community Health (REACH 2010) community coalitions in designing, implementing, and evaluating community-driven strategies to eliminate health disparities in racial and ethnic groups. The REACH 2010 logic model was developed to assist grantees in identifying, documenting, and evaluating local attributes of the coalition and its partners to reduce and eliminate local health disparities. The model emphasizes the program's theory of change for addressing health disparities; it displays five distinct stages of evaluation for which qualitative and quantitative measurement data are collected. The CDC is relying on REACH 2010 grantees to provide credible evidence that explains how community contributions have changed conditions and behaviors, thus leading to the reduction and elimination of health disparities.

[Prognosis of remote consequences of combat mental trauma using logical statistical methods].

Prognosis of remote consequences of acute stress disorders and adaptation disorders as posttraumatic stress disorders (PTSD) is possible using relevant mathematical methods. The authors have used the following logical statistical methods: algorithms for search and statistical verification of optimal fragmentation of variables in multi-dimensional space ("Q-nearest neighbors", "statistically weighed syndromes") as well as a program of searching for multiple conjunctions of variables that allowed to single out statistically significant combinations of variables (p<0,001) and to divide the sample into two groups: with favorable and with unfavorable outcome both at the acute and remote stages of mental trauma. Predictors of unfavorable outcome found in the acute period of mental trauma were as follows: affective tension, impulsivity, high personal anxiety and depressive tendencies with reduced activity. Comparing to clinical evaluation, subjective criteria of mental state (self-rating) retained its informativeness for prognosis of protracted PTSD types. Using this set of variables, it became possible to predictt remote consequences (2-4 years) of psychogenias of combat situation with reliability of recognition of 85% for the favorable outcome and 79% for cases of protracted types.
