[TAC-278. A new 5-FU derivative, by the oral route for its efficacy and safety in 36 patients with advanced cancers].

Tumor regression was observed in only one patient. Twelve (33%) of the 36 patients reported adverse effects: gastrointestinal symptoms in 8, CNS symptoms in 3, and others in 1. The antitumor efficacy of TAC-278 was insufficient while incidence of adverse effects was similar to other 5-FU derivatives in man and the clinical usefulness of the drug was hardly found. TAC-278 has been developed with an aim to give higher plasma concentrations of 5-FU in man. Even at a dose causing adverse effects, however, the clinical efficacy of TAC-278 was unsatisfactory. The study is a meaning tal eince the results suggest that there might be a more important factor than blood concentration of 5-FU to enhance an antitumor effect of 5-FU derivatives.

[Immunity state of oncological patients during long-term treatment with tramal].

Narcotic analgesics have an inhibiting effect on the immunity and that is why their use for pain relief in cancer patients cannot be regarded as adequate. The effect of a new synthetic analgesic tramal (tramal hydrochloride) on the immune system has been studied. Cellular immunity indexes, immunoglobulin level, and phagocytic activity of neutrophils have been determined before treatment and 1 and 2 weeks after the treatment started. The data obtained have shown that tramal does not inhibit the immune system. It has been also noted that pain relief in cancer patients improved the indexes of cellular and humoral immunity.

[Carcinoid of the small intestines with right cardiac involvement. Clinical, phonomechanographical hemodynamic and anatomical study].

The authors report a case of carcinoid of the small intestine with liver metastases in whom the entire right side of the heart was affected, with severe tricuspid incompetence and pulmonary stenosis. The extent of the fibrosis, which affected all three layers of the heart on the right side has lead the authors to reconsider the various factors which cause the heart failure in carcinoid syndrome. This must now be included in the wider category of APUD. The severity of the tricuspid lesions compared with the derangement of the valvular and subvalvular structures makes an argument in favour of surgical correction.
