Fibrous hyperplasia of the gingiva in organ transplant patients.

Recent improvements in immunosuppressive therapy have improved the survival rate of post organ transplant patients. These developments have introduced a new group of medically compromised patients to the general dental practitioner. This paper reviews the medical and dental complications of organ transplant patients with special reference to heart transplant patients. A case report of the management of Cyclosporin A induced gingival hyperplasia in a post heart transplant patient is presented. Guidelines for dental management of heart transplant patients are suggested.

Soft-tissue plastic surgery.

Soft-tissue plastic surgery was previously referred to as mucogingival surgery by Friedman (Tex Dent J 1957, 75:358-362). This term referred to any surgery "designated to preserve attached gingiva, to remove frena or muscle attachment, and to increase the depth of the vestibule." The aim of this type of surgery was to maintain an adequate mucogingival complex, focusing on the importance of the amount of attached gingiva. Soft tissue plastic surgery is important not only for reasons of health, but also for cosmetic purposes.

[Cyclosporin induced gingival hyperplasia].

Cyclosporine is a strong immunosuppressive drug, which is used specially in transplantation patients, however in some cases, like others medicines it causes, gingival overgrowth. In the present article we present five transplantations patients, (one of liver, and four of kidney), whom have taken this drug since more than a year at different dosages. One of them, never has presented overgrowth, whereas the rest cases have needed this specific periodontal treatment to solve hyperplasy. After this work, we don't find clear relationship between drug dosage-age-effect over tissues.

[Diazepam versus midazolam for "wakeful sedation" in oral surgery].

I.v. midazolam was compared with diazepam in 112 dental outpatients in order to obtain a control of sedation. Midazolam has shown better results in all the parameters evaluated (memory scale, pharmacokinetics and pharmacodynamics values) and in controlling sedation. The Authors consider midazolam as a preferred drug in dental outpatients and hope to get it available also in Italy.

[A practical method for the surgical uncovering of a buried implant].

This method puts forward a new and easy process to uncover an endosseous implant before the prosthetic rehabilitation. We use a "punch", a dermatologist's instrument, composed of two parts: a plastic handle and a cylinder blade. We choose the diameter's blade instrument according to the diameter of implant and we change the punch for each implant to clear. The punch is held like a pencil, vertically. The punch goes deep into the gum to the bone and is turned simultaneous. A perfect cylinder part of gum is filled out. In case of propitious circumstances, this method is safe quick and avoids doing a second flap.

The provision of periodontal services in general dental practice in Australia.

Recent studies have documented the low percentage of professional services which are associated with periodontal care. The aims of the present study were to describe the provision of periodontal services by private general practice dentists in Australia and to investigate the variation of provision of periodontal services. Using a day log approach via a mailed questionnaire one in eight services provided were found to be related to periodontal care. Most were of a preventive nature. There was great variation between dentists in the number of services provided indicating the possibility of both under and overservicing. The findings are discussed with regard to the changing philosophies of provision of periodontal services and known methods of eliciting change among providers.
