Valaciclovir in herpes zoster ophthalmicus: new indication. An empty clinical assessment file.

(1) Valaciclovir, a metabolic precursor of aciclovir, improves the bioavailability of the active compound. It is licensed for the prevention of ocular complications of herpes zoster ophthalmicus in immunocompetent subjects. (2) The clinical file on valaciclovir in this indication is very thin. Only two (uninterpretable) trials have been done, both versus aciclovir. Note that oral aciclovir has also been assessed inadequately in this indication. (3) Over 10% of patients treated with valaciclovir in the two trials had nausea or headache.

Creating and indexing teaching files from free-text patient reports.

Teaching files based on real patient data can enhance the education of students, staff and other colleagues. Although information retrieval system can index free-text documents using keywords, these systems do not work well where content bearing terms (e.g., anatomy descriptions) frequently appears. This paper describes a system that uses multi-word indexing terms to provide access to free-text patient reports. The utilization of multi-word indexing allows better modeling of the content of medical reports, thus improving retrieval performance. The method used to select indexing terms as well as early evaluation of retrieval performance is discussed.

Et Tu, Counselor: may an in-house attorney file a qui tam action against the attorney's employer?

Most corporations probably do not consider their in-house counsel to be potential qui tam threats. That may be a naive assumption. Case law provides an illustrative view of the legal ramifications involved when an attorney brings a qui tam suit. In general, there is no prohibition on attorneys who wish to bring these actions. Nevertheless, a corporation can take preventive steps to eliminate the likelihood of attorney qui tam actions. In addition, the corporation can take advantage of state professional ethics laws to mount a defensive action against the attorney who files any such action.

Medical confidentiality and electronic patient files.

When considering electronic patient files some conditions have to be respected. They are, in particular, confidentiality and informed consent by the patient. Furthermore, undue access to data can be prevented by PET, access authorization and the like. Patients should be allowed to exert control over their medical data. National and international good privacy practice guidelines should be drawn up in order to avoid privacy-erosion.

New drug applications; drug master files. Food and Drug Administration, HHS. Final rule.

The Food and Drug Administration (FDA) is revising its regulation governing drug master files (DMF's). FDA is removing the provision for submitting Type I DMF's and will no longer permit information submitted in a Type I DMF to be incorporated by reference in investigational new drug applications (IND's), new drug applications (NDA's), abbreviated new drug applications (ANDA's), or amendments or supplements to any of these. This rule is intended to eliminate submissions of information that are not necessary either to conduct inspections of manufacturing facilities or to review the chemistry, manufacturing, and controls sections of IND's, NDA's, and abbreviated applications.

Incorporating new recipes into the Armed Forces Recipe File: determination of acceptability.

As part of a project of decrease fat, cholesterol, and sodium in soldiers' diets, new ethnic and breakfast items were developed and standardized for 100 portions. Acceptability data were collected after initial recipe development, during recipe validation at a collaborating university, and in an actual Army garrison. Acceptability was determined using a nine-point hedonic scale; products rating > or = 6.0 in initial tests were prepared in garrison. Acceptability data were compared among test settings, ethnic categories, and food type. When grouped by ethnic categories, acceptability ratings varied more than when grouped by food type. Ratings varied most between development and validation settings (7.2 vs. 6.6; p < 0.05) and least between validation and actual Army settings (6.6 vs. 6.6; not significant). Because acceptability ratings were similar between the validation site and the Army garrison, future recipe development may continue without additional testing at actual Army garrisons, leading to more timely armed forces recipe file additions.

Probabilistic record linkage: relationships between file sizes, identifiers and match weights.

This study investigates relationships between file sizes, amounts of information contained in commonly used record linkage variables, and the amount of information needed for a successful probabilistic linkage project. We present an equation predicting the amount of information needed for a successful linkage project. Match weights for variables commonly used in record linkage are measured using artificially created databases. Linkage algorithms were successful when the sum of minimum weights for variables used in a linkage exceeded the predicted cutoff. Linkage results were acceptable when this sum was near the predicted cutoff. This technique enables researchers to determine if enough information exists to perform a successful probabilistic linkage.

The Swiss solution for anonymously chaining patient files.

Being able to link the successive hospitalizations of a given patient for epidemiological surveys together by preserving patient anonymity requires a specific procedure. For its medical hospital statistics, Switzerland has adopted anonymous record linkage procedure, ensuring an irreversible transformation of identity and allowing the linkage of anonymous files. Concepts, algorithms and organization have been developed by the Federal Statistical Office. Algorithms are closed to those developed in Dijon University Hospital.

US files WTO complaints against Brazil over requirement for "local working" of patents.

At the end of May 2000, the US (later joined by the European Communities) filed a complaint against Brazil at the World Trade Organization (WTO), alleging Brazil was in violation of its obligations under the Agreement on Trade-Related Aspects of Intellectual Property Rights (the TRIPS Agreement) and the 1994 General Agreement on Tariffs and Trade. Brazilian legislation that came into force in 1997 establishes that, in order to enjoy exclusive patent rights in Brazil, the holder of a patent on an invention must satisfy a "local working" requirement. In other words, the patent holder must "work" the patent in Brazil to enjoy full patent protection. If it fails to do this, the law says it shall be subject to the possibility of the government issuing a compulsory license, allowing someone else to use the invention and pay a royalty fee to the patent holder.

Computer-retrievable surgical pathology files. A low-cost, shareable system.

Reading descriptions of computer-retrievable surgical pathology files, the tissue pathologist might like to institute a similar system, but faces two problems. He finds little cost information and, without either the time or the skill for computer programming, he is unable to proceed with system development. We have a system that was developed to maximize cost efficiency. All of the programs are written in FORTRAN so that the system will run on almost any time-sharing computer. There is a need for computer-retrievable systems in pathology, and after careful consideration of affordability and shareability, computerization is workable.

Order on the guidance file provided for in Article L. 162-3 of the Public Health Code, 29 December 1987.

Guidance files (dossiers-guides) are intended for physicians practicing in the department; in public and private hospital establishments; in family planning and family education centers; in establishments providing family information, advice, or education; in social services; and in other agencies approved by French departmental authorities responsible for health and welfare matters. The Annex sets out, on the one hand, conditions for voluntary termination of pregnancy (the legislation on voluntary termination of pregnancy, procedures to be followed, medical data, consultations following voluntary termination of pregnancy and contraception, and the costs of termination of pregnancy) and, on the other, the availability of assistance for women deciding to carry their pregnancy to term.

[Genealogical files and the analysis of the genetic structure of non-isolated populations. An application to an agricultural population of the Limousin region].

"This article shows the methodological problems appearing in the production of genealogical files for non-endogamous or 'open' populations. [The authors conclude that] the study area must be extended to a geographic space larger than the one where the [designated] population resides." The authors reconstitute family structures using data from the eighteenth, nineteenth, and twentieth centuries for an agricultural community in Limousin, France. (SUMMARY IN ENG AND SPA)

Dutch international migration statistics, 1820-1880: an analysis of linked multinational nominal files.

"International migration statistics in the nineteenth century are acknowledged to be deficient and biased, but there are few source-critical studies to determine the extent of underreporting and omissions. This article provides a critical analysis of the statistics of Dutch emigration to North America in the period 1835-1880, based on the method of nominal record-linkage of computer files derived from Netherlands emigration lists and U.S. ship passenger manifests. Published and unpublished official records in the Netherlands, U.S.A. and Canada are used to determine the extent of underreporting, the structural biases in the migration data and the 'true' annual Dutch immigration rate to the United States."

Haunted by Enron's ghost. National Century Financial Enterprises files for Chapter 11, leaving a string of broken healthcare chains and hospitals.

Some are calling it the Enron of the healthcare industry. Ryder trucks hauled possible evidence from embattled financier National Century Financial Enterprises during an FBI raid. NCFE filed for Chapter 11 bankruptcy protection last week, sending ripples through the industry and contributing to the bankruptcies of a string of national healthcare chains and at least six hospitals.

[János Bolyai's health according to his military files].

János Bolyai (1802-1860) one of the greatest mathematicians of the 19th century, the founder and inventor of non-Euclidean geometry, was born in Transylvania, as son of a Hungarian nobleman and scientist, Farkas Bolyai. Having finished his studies at the Academy for Engineering Corps in Vienna, he joined the army. From 1823 to 1833 he served at different places all over the the Empire. Being however physically and mentally unbalanced, he underwent several medical examinations during his military service. Based on archival documents and files of medical examinations and on those of the following resolutions, the author of the present article gives a comprehensive and very detailed account of the young genius's military career and on the circumstances determining his mental activities while creating the most admirable mathematical theory of the age.
