Should we be worried? Investigation of signals generated by an electronic syndromic surveillance system--Westchester County, New York.

INTRODUCTION: In January 2003, the Westchester County Department of Health (WCDH) began conducting electronic syndromic surveillance of hospital emergency department (ED) chief complaints. Although methods for data collection and analysis used in syndromic surveillance have been described previously, minimal information exists regarding the responses to and investigations of signals detected by such systems. This paper describes WCDH's experience in responding to syndromic surveillance signals during the first 9 months after the system was implemented. OBJECTIVES: The objectives of this analysis were to examine WCDH's responses to signals detected by the county's syndromic surveillance system. Specific goals were to 1) review the actual complaints reported by hospital EDs to determine whether complaint data were accurately identified and classified into syndrome categories, and provide feedback from this review to data collection and analysis staff to refine text terms or filters used to identify and classify chief complaints; 2) develop procedures and response algorithms for investigating signals; 3) determine whether signals correlated with reportable communicable diseases or other incidents of public health significance requiring investigation and intervention; and 4) quantify the staffing resources and time required to investigate signals. METHODS: During January 27-October 31, 2003, electronic files containing chief-complaint data from seven of the county's 13 EDs were collected daily. Complaints were classified into syndrome categories and analyzed for statistically significant increases. A line listing of each complaint comprising each signal detected was reviewed for exact complaint, number, location, patient demographics, and requirement for hospital admission. RESULTS: A total of 59 signals were detected in eight syndrome categories: fever/influenza (11), respiratory (6), vomiting (11), gastrointestinal illness/diarrhea (8), sepsis (7), rash (7), hemorrhagic events (3), and neurologic (6). Line-listing review indicated that complaints routinely were incorrectly identified and included in syndrome categories and that as few as three complaints could produce a signal. On the basis of hospital, geographic, age, or sex clustering of complaints, whether the complaint indicated a reportable condition (e.g., meningitis) or potentially represented an unusual medical event, and whether rates of hospital admission were consistent with medical conditions, 34 of 59 signals were determined to require further investigation (i.e., obtaining additional information from ED staff or medical providers). Investigation did not identify any reportable communicable disease or other incidents of public health significance that would have been missed by existing traditional surveillance systems. Nine staff members spent 3 hours/week collectively investigating signals detected by syndromic surveillance. CONCLUSIONS: Standardized sets of text terms used to identify and classify hospital ED chief complaints into syndrome categories might require modification on the basis of hospital idiosyncrasies in recording chief complaints. Signal investigations could be reasonably conducted by using local health department resources. Although no communicable disease events were identified, the system provided baseline and timely objective data for hospital visits and improved communication among county health department and hospital ED staff.

[Evolutionary quality assurance. A new concept for improving process and outcome quality].

BACKGROUND: The concept evolutionary quality assurance is a new, non-mandatory, open peer review process for in-patients settings. METHOD: Ten medical charts (with radiographs) from participating hospitals were randomly chosen and assessed using a prespecified questionnaire (total 134). Individual inadequacies had to be justified on a case by case level. Reviewer and reviewee are known by names, allowing subsequent discussions on content between reviewer and reviewee prior to a final judgement. The final analysis was performed anonymously and communicated to the participants after completion of the process. Patients had to give their consent to the review process. The report, however, is not part of the medical patient file, so that access is not warranted. For the first cycle, all lung clinics and departments throughout Germany were asked to participate. 35 chest hospital (approximately 50%) agreed to participate. RESULTS: Data analysis revealed that almost all detected inadequacies were apparent, i.e. discussions on the discordant interpretation of diagnostic and therapeutic strategies rarely occurred (0.25%). Final analysis of the evaluation performance of reviewers judged less than 5% and 14% judged more than 30% of all quality inadequacies. CONCLUSIONS: The quality assurance process is comparably cheap and can be implemented without delay, because standards for reference values are not required. The structure allows adaptation in all areas of clinical medicine.

[Surgical management of posterior communicating artery aneurysms based on computed tomographic angiography with three-dimensional reconstruction and without preoperative angiography].

OBJECTIVE: To demonstrate the usefulness of three-dimensional computed tomographic angiography (CT-3D-angiography) in the microsurgical management of aneurysms of the posterior communicating artery (PComA). MATERIALS AND METHODS: A series of 27 patients with aneurysms of the PComA diagnosed by means of CT-3D-angiography and without preoperative angiography (group A) were compared with a series of 34 cases diagnosed by cerebral angiography. The findings of the CT-3D-angiography, angiography, microsurgical exploration and clinical data were evaluated. RESULTS: A total of 75 aneurysms were diagnosed preoperatively in 66 patients and 3 additional lesions were found postoperatively in group A. The sensitivity of the CT-3D-angiography was 91.7% for diagnosis of any aneurysm with an specifity of 100%, being the snsibility of angiography a 100% and its specifity 94.9%. Mortality rate was 4.5% without differences between groups regarding clinical results or complications. Both the preoperative timing and hospitalization time were shorter in group A. CONCLUSION: The study of patients with acute subarachnoid hemorrhage with CT-3D-angiography allows a reliable diagnosis of PComA aneurysms. Moreover, provides usefull information for the microsurgical clipping. When compared with angiography in the diagnosis of PComA aneurysms, CT-3D-angiography allows to improve some health indicators with similar clinical results and complications.

Preceptorship and interpersonal conflict: a multidisciplinary study.

BACKGROUND: The impetus for the study was the concern of teachers about the potentially deleterious effects of conflict in the preceptee-preceptor relationship. Preceptorship, in which students work alongside designated professionals in clinical settings, can provide opportunities to integrate theoretical and practice knowledge, and can play a part in professional development. However, students may experience communications and interpersonal problems with their preceptors and, if the conflict thereby generated is not challenged and resolved, negative outcomes can occur for students, preceptors and teachers. AIM: The purpose of the study was to explore the nature of conflict in preceptorship experiences. METHODS: A modified, simultaneous quantitative/qualitative triangulated method was used, with the quantitative results reported in this paper. A total of 548 questionnaires were mailed to students and preceptors in the final clinical experience of undergraduate programmes in four disciplines. The return rate was 42.7% (234 questionnaires). FINDINGS: Data revealed the prevalence of conflict, the degree to which this affected preceptorship relationships, factors occurring in the practicum that may contribute to conflict, the degree to which the conflict was resolved, and the outcomes of conflict. CONCLUSIONS: It is only through an understanding of the dynamics involved in conflict, and its negative effects on students, that teachers, preceptors and students alike can respond proactively to conflicts and become adept at seeking and implementing effective solutions.

A bioinformatics-based approach for the prediction and identification of novel proteins potentially involved in phosphorylation signalling pathways.

Together with the explosion in the availability of genome data of a number of organisms including human and mouse, various methods and programs for computational prediction of protein-coding genes and annotation of functional proteins have dramatically increased. For the last decade there has been intense interest in the role of protein phosphorylation which is involved in post-translation modification mechanisms critically regulating inter/intracellular communication, patho/physiological responses and homeostasis during many biological processes. In the present study a total of 202 functionally uncharacterized human full-coding cDNA sequences were investigated using a bioinformatics-based approach. Ten novel potential substrates for protein kinases have been identified which may play multiple roles in regulating intracellular phosphorylation signalling pathways. In addition, 5 of those may be involved in the human-only post-translation mechanism regulated by specific protein kinases. The data presented here therefore would greatly contribute toward the understanding of human molecular basis and cellular signalling networks.

Health care experiences of African American teen women in eastern North Carolina.

BACKGROUND AND OBJECTIVES: Women, especially women of color, are a vulnerable population in eastern North Carolina. This study investigated the health care experiences of Southern, rural, African American adolescent women. METHODS: Four focus groups were conducted in three counties in eastern North Carolina in the spring of 2000. A total of 44 teen women ages 16-19 participated; 38 were African American. RESULTS: Participants emphasized the following themes: (1) seeking help for sexual health, (2) obtaining health information from sources other than the doctor, (3) gender and race preferences, (4) communication with physicians, and (5) good and bad doctors' visits. CONCLUSIONS: To provide quality care to African American teenage women, it is critical to listen and understand their experiences. Most importantly, medical educators must be cognizant of cultural issues and how they can impede as well as enhance patient care. This study supports the need to educate students, residents, and practicing physicians on how to provide culturally sensitive and competent care.

[Quality of life before and after thyroplasty for vocal fold paralysis].

OBJECTIVES: The aim of our prospective study was to evaluate the vocal handicap in relation with the quality of life in patients with unilateral vocal fold paralysis (UVFP), before and after thyroplasty with Montgomery implant (type I). METHODS: Ten patients had to file the VHI questioner before and three months after thyroplasty. The 30 items of the Voice Handicap Index scale are sub-divided in 3 categories referring to "Functional", "Physical" and "Emotional" dimensions. RESULTS: Before thyroplasty, the global score and the 3 sub-scales rating are high reflecting severe impairment in speech communication (VHI total = 65,9/120, F = 25.7/40, E = 19.9/40, P = 20.8/40). Three months after thyroplasty, a significant improvement is found predominantly in the field of functional and emotional evaluation, corresponding to an improved quality of social life (VHI total = 20,9/120, F = 6.3/40, E = 3.6/40, P = 10.6/40). CONCLUSION: These results show the interest in using the autoevaluation VHI scale as a complementary evaluation associated to perceptual, acoustic and aerodynamic evaluation to objectify the functional improvement in voice quality after thyroplasty.

Reestimations of the protein concentrations of birch pollen allergen extracts selected as candidates for the international standard (IS) preparation.

The total protein concentrations of six selected candidates for an international standard (IS) of birch pollen were previously estimated (Ann Allergy 1987;58:71-77). The results of analyses showed large variations depending both on the method and the reference protein used. More than 2-fold variations in the protein content were obtained using Bradford's protein binding method. In the present communication the protein contents of these birch IS-candidates were reestimated by Lowry, a modified Lowry technique, Bradford's protein-dye binding, bicinchoninic acid reagent and amino acid compositions. Great variations were obtained for the protein contents, depending on the technique employed, emphasizing the difficulties of choosing a proper method of protein analyses of pollen allergens. The Lowry and the bicinchoninic acid reagent methods gave the highest obtained protein estimates, while the modified Lowry and Bradford's protein-dye binding methods showed the lowest values. The amino acid analysis gave concentrations similar to those obtained by Lowry and bicinchoninic acid reagent methods. The Lowry method seemed to be adequate for the analysis of plant proteins. The amino acid analysis is currently the most precise method for estimation of the protein concentration irrespective of additional prosthetic groups. A serious shortcoming of the method is that it requires an amino acid analyser or other liquid chromatrographic systems, which are not commonly in laboratory use.

[Pathways of transcranial blood flow].

The injection of the injectional mass into the internal and external carotid arteries has shown the presence of perforant arteries in the bones of the cranial fornix. The method of impregnation of vessels by silver nitrate in sections of decalcified cranium bones enabled the author to establish the penetration of thin blood vessels into the cranial cavity through sutures. Blood vessels simultaneously found to be present in the spongy and compact substance of the cranial fornix bones. The totalities of the above vessels are considered as transcranial vascular communications. Their possible significance in hemodynamics is discussed.

Patients' resistance to risk information in genetic counseling for BRCA1/2.

BACKGROUND: Risk information from health care providers is relevant to and used in nearly all medical decisions. Patients often misunderstand their risks, yet little is known about the risk perception that patients derive from risk communications with health care providers. This study examines patients' risk perceptions following communication with health care providers during genetic counseling about the risks of breast cancer and BRCA1/2 mutations. METHODS: A prospective, longitudinal study was conducted from October 2002 to February 2004 of women who received genetic counseling. The women completed a survey before their counseling and a telephone interview in the week after the counseling. Main outcome measures included change from precounseling in risk perception and accuracy of postcounseling risk perception (relative to actual risk information communicated). RESULTS: A total of 108 women agreed to participate in the study. The women's postcounseling risk perceptions were significantly lower than their precounseling risk perceptions (breast cancer: 17%, P<.001; mutation: 13%, P<.001) but were significantly higher than the actual risk information communicated (breast cancer: 19%, P<.001; mutation: 24%, P<.001). Accuracy of breast cancer risk perception but not mutation risk perception was associated with precounseling worry (P = .04), even after adjusting for trait anxiety (P = .01). CONCLUSIONS: This research demonstrates patients' resistance to risk information. Inappropriately high risk perception derived from a risk communication with a health care provider can lead patients to make different, and potentially worse, medical decisions than they would with an accurate risk perception and to be unnecessarily distressed about their risk.

Incidental liposarcomas identified during hernia repair operations.

BACKGROUND AND OBJECTIVES: Since the inguinal region communicates with the retroperitoneum, both retroperitoneal as well as de novo spermatic cord liposarcomas may be detected during hernia repair operations. We assessed the incidence of liposarcomas presenting at hernia repair in our hospital. METHODS: We performed a clerical review of pathology reports on adult tissue accessioned during hernia repair operations and reviewed operating room logs to obtain information concerning the total number of hernia repair operations (since some operations afford no accessioned tissue). RESULTS: Between 1992 and 1997, 1,736 adult hernia repair specimens were accessioned from approximately 2,000 operations. Among these, 22% had an associated cord lipoma; 2 cases were well-differentiated liposarcomas. These were from males aged 56 and 64 years in contrast to the mean age of 35 years for cord lipoma and measured 13 and 10 cm, whereas the mean size for cord lipomas was 5.5 cm. One of the liposarcomas had radiographic evidence of extension from a retroperitoneal lesion; the other appeared confined to the groin. On surgical exploration, the lesion was restricted to the spermatic cord region in both cases despite the suggestion of retroperitoneal extension/involvement in one. CONCLUSIONS: Incidental liposarcomas identified during hernia operations are rare (<0.1% at our institution) but their presence merits histologic evaluation of adipose tissue from these cases. However, if efforts to contain costs are implemented and histologic review of such tissue is deemed generally unrewarding, large (>10 cm) fatty masses from this area should still be sampled.

Psychological well being and quality of life in ovarian cancer survivors.

BACKGROUND: This report offers a unique analysis of the psychological distress associated with ovarian cancer in a review of natural correspondence between ovarian cancer survivors and an ovarian cancer newsletter. METHODS: A review of 21,806 letters, cards, and e-mails reflecting correspondence from January 1994 to December 2000 between ovarian cancer survivors and the founding editor of Conversations!: The International Newsletter for those Fighting Ovarian Cancer was performed using ethnographic qualitative research methods. Statements related to the impact of disease were bracketed and coded within physical, psychological, social, and spiritual domains according to the City of Hope Quality of Life Ovarian Cancer instrument. Statements that reflected psychological well being were then evaluated with respect to the disease trajectory (i.e., diagnosis, treatment, remission, recurrence, and advanced disease/end of life). RESULTS: A total of 1282 communications were identified that pertained to psychological well being. Findings based on major themes derived from the analysis included descriptions of stressors associated with disease status. Significant stressors were identified within all phases of diagnosis, treatment, remission, and recurrence. Women described both positive and negative effects of disease and frequently demonstrated resourcefulness and perseverance by sharing coping mechanisms and survival strategies. CONCLUSIONS: The natural correspondence from women with ovarian cancer provided a rare opportunity to capture the psychological concerns of women throughout all stages of the cancer trajectory. Health care professionals' awareness of the common psychological stressors throughout the ovarian cancer trajectory may allow them to identify more readily the needs for support, leading to improvement in overall quality of life.

Stricture associated with primary tracheoesophageal puncture after pharyngolaryngectomy and free jejunal interposition.

BACKGROUND: Free jejunal interposition has been one of the standard reconstructive options for patients undergoing total laryngopharyngoesophagectomy. Tracheoesophageal puncture (TEP) done primarily is a well-accepted means of voice restoration. The rapid recovery of swallowing and communication in patients who have advanced cancer of the upper aerodigestive tract is a valid goal. The objective of this study was to evaluate the functionality and complications of primary TEP in patients with a free jejunal interposition graft. METHODS: Twenty-four consecutive patients who had free jejunal interposition were studied. Thirteen of these patients had a primary TEP. Stricture was assessed through barium swallow, laryngoscopy, and CT scan. RESULTS: A statistically significant greater number of patients had stricture develop after primary TEP (p < .0325). All these patients had stricture develop at the distal anastomosis. These patients also tended to have a poorer quality of diet. Moreover, speech with a TEP prosthesis in patients with a free jejunal interposition was less intelligible and functional than speech with a TEP prosthesis after simple laryngectomy. CONCLUSIONS: This article recognizes an increased incidence of stricture formation after primary TEP as compared with non-TEP in patients undergoing pharyngolaryngectomy with free jejunal interposition. The morbidity and possible etiology of this complication are discussed. This clinical data have been translated into a change in clinical practice.

Sign language and autism.

Research findings and issues in teaching sign language to nonspeaking autistic children are reviewed. Data on over 100 children indicate that nearly all autistic children learn receptive and expressive signs, and many learn to combine signs. These children also exhibit marked improvement in adaptive behaviors. Speech skills are acquired by fewer children and may be developed through simultaneous speech and sign training. Possible explanations for these results are given, together with suggestions for future research and data collection. Recommended innovations include exposure to fluent signers and training in discourse and code-switching. Different sign language teaching methods need to be investigated more fully, including emphasis on training sign language within the children's total environment and with greater staff and parental participation.

Quantified multidimensional assessment of autism and other pervasive developmental disorders. Application for bioclinical research.

A large number of investigation techniques are used to establish the relationships between the clinical and biological data which are necessary for physiopathological analysis in the field of developmental disorders. It therefore seemed necessary to develop a quantified grouping system, based on developmental assessments, which could allow closer matching between clinical evaluations and biological numerical data. Two hundred and two subjects presenting developmental disorders (autistic disorder, pervasive developmental disorder not otherwise specified and mental retardation) were examined. For each child, a quantification of autistic behaviour, intellectual impairment, neurological signs and language and communication disorders was performed. A cluster analysis of these quantified data elicited four subgroups according to the scores obtained in these four different areas. We showed the value of this approach by applying it to one of the studies of monoamines routinely examined in childhood autism--dopamine and HVA, its main urinary derivative. Moreover, this method revealed a subgroup within the total population which was independent of nosographic classification and which had a particular clinical and biochemical profile. Other applications could follow, for example in the fields of neurophysiology, cerebral imaging, molecular biology and genetics.

Communication training for health professionals who care for patients with cancer: a systematic review of effectiveness.

BACKGROUND: Effective communication is increasingly recognised as a core clinical skill. However, there is evidence that health and social care professionals still lack basic communication skills. PURPOSE: To assess the effectiveness of different communication skills training courses for health professionals in cancer care. METHODS: We searched six computerised databases and augmented this with a follow-up of references and grey (unpublished) literature. We included all studies evaluating communication training and assessed methodological quality according to the standard grading system of the Clinical Outcomes Group. Data on author, year, setting, objectives, study design and results were extracted and compared in tabular format. RESULTS: A total of 47 studies potentially assessing communication training in the area of cancer care were identified. Sixteen papers were included describing 13 interventions. Four were randomised controlled trials (RCTs) (grade I), with samples ranging from 72 to 233 subjects. The others were all grade III. Eleven interventions trained health professionals, two trained medical students. The outcomes measured included communication skills as assessed on audio or video, professionals' self-report and patient assessment. All the interventions demonstrated modest improvements (effect sizes ranged 0.15-2) and one found deterioration in the outcomes measured. CONCLUSION: Communication training improves basic communication skills. Positive attitudes and beliefs are needed to maintain skills over time in clinical practice and to effectively handle emotional situations.

Atrial defect size after Blalock-Hanlon atrioseptectomy.

In transposition of the great arteries, a Blalock-Hanlon closed atrial septectomy is performed to improve intracardiac mixing at the atrial level. Although the Blalock-Hanlon septectomy is a common surgical procedure in cyanotic congenital heart disease, it has not been adequately assessed pathologically. In 14 heart specimens from patients (aged 3 days to 19 years) with transposition of the great arteries and Blalock-Hanlon septectomy, the margins of the septectomy, fossa ovalis and atrial septum were identified. The total area of the septum and its defects was calculated using planimetry. The ratio of defect size to atrial septal area was expressed as percent communication, which ranged from 5 to 39 (mean 18) percent in eight specimens with intact limbus of the foramen ovale and 26 to 57 (mean 42) percent in six specimens in which the limbus had been excised. The finding that specimens in which the Blalock-Hanlon defect extended into the fossa ovalis had the largest total communication emphasizes that to obtain optimal bidirectional atrial mixing the surgeon should extend the Blalock-Hanlon procedure across the limbus into the foramen ovale.

The relationship between interpersonal problems and occupational stress in physicians.

OBJECTIVE: This article examined the associations between occupational stress and interpersonal problems in physicians. METHOD: A nationwide representative sample of Norwegian physicians received the 64-item version of the Inventory of Interpersonal Problems (IIP-64) (N=862, response rate=70%) and six instruments measuring occupational stress. Comparison of means, correlation and reliability statistics and multiple regression analyses were applied. RESULTS: The IIP-64 total score had a significant impact on job satisfaction, perceived unrealistic expectancies, communication with colleagues and nurses and on stress from interaction with patients. Being overly subassertive was related to low job satisfaction. Being overly expressive was linked to the experience of unrealistic expectancies from others and lack of positive feedback, whereas overly competitive physicians tended to have poorer relationships with both colleagues and nurses. CONCLUSIONS: Addressing interpersonal problems in medical school and postgraduate training may be a valuable measure to prevent job stress and promote quality of care.
