Characteristics of and reasons for the floating population in contemporary China.

This is an analysis of the floating population in China, defined as "those people who travel beyond a certain limit, leave their registered permanent residence temporarily, and travel to and from their homes on an irregular basis. It includes temporary residents (including those who have temporary residency permits for six months or longer), those who travel to and fro on the same day, and those who change trains on the way." The author notes that the main concentration of these migrants is in the major cities.

Correlations between epidermal cell structure and endogenous hormone titers during the fifth larval instar of the tobacco hornworm, Manduca sexta.

Epidermal cell morphology and cuticle production in Manduca sexta are directly influenced by both ecdysterone and juvenile hormone. Up to day 6 of the last larval instar, post-molt endocuticle is continuously deposited even though cells undergo a partial and temporary separation from the overlying cuticle at the time when a small ecdysteroid peak is detected (approximately day 3.5). At about days 6--7 when another, larger ecdysteroid peak is present, apolysis occurs accompanied by the appearance of edcysial droplets. Following apolysis, layers of pupal cuticle are deposited. Increased quantities of rough endoplasmic reticulum characterize the epidermis at times of peak endocuticle deposition (day 3, larval cuticle; day 9, pupal cuticle). Dense pigment inclusions are found in epidermis from the day of ecdysis to the last larval instar until they are eliminated 5 days later. These dense bodies migrate from cell apex to base in the absence of juvenile hormone (or in the presence of a negligible amount of juvenile hormone) and probably contain insecticyanin.

CD3 directed bispecific antibodies induce increased lymphocyte-endothelial cell interactions in vitro.

Bispecific antibody (BsMAb) BIS-1 has been developed to redirect the cytolytic activity of cytotoxic T lymphocytes (CTL) to epithelial glycoprotein-2 (EGP-2) expressing tumour cells. Intravenous administration of BIS-1 F(ab')2 to carcinoma patients in a phase I/II clinical trial, caused immunomodulation as demonstrated by a rapid lymphopenia prior to a rise in plasma tumour necrosis factor-alpha and interferon-gamma levels. Yet, no lymphocyte accumulation in the tumour tissue and no anti-tumour effect could be observed. These data suggest a BsMAb-induced lymphocyte adhesion to blood vessel walls and/or generalized redistribution of the lymphocytes into tissues. In this study, we describe the effects of BIS-1 F(ab')2 binding to peripheral blood mononuclear cells (PBMC) on their capacity to interact with resting endothelial cells in vitro. Resting and pre-activated PBMC exhibited a significant increase in adhesive interaction with endothelial cells when preincubated with BIS-1 F(ab')2, followed by an increase in transendothelial migration (tem). Binding of BIS-1 F(ab')2 to PBMC affected the expression of a number of adhesion molecules involved in lymphocyte adhesion/migration. Furthermore, PBMC preincubated with BIS-1 F(ab')2 induced the expression of endothelial cell adhesion molecules E-selectin, VCAM-1 and ICAM-1 during adhesion/tem. These phenomena were related to the CD3 recognizing antibody fragment of the BsMAb and dependent on lymphocyte-endothelial cell contact. Possibly, in patients, the BIS-1 F(ab')2 infusion induced lymphopenia is a result of generalized activation of endothelial cells, leading to the formation of a temporary sink for lymphocytes. This process may distract the lymphocytes from homing to the tumour cells, and hence prevent the occurrence of BIS-1 F(ab')2 - CTL-mediated tumour cell lysis.

Endoscopic removal of metallic airway stents.

BACKGROUND: Complications of metallic airway stents include granulation tissue formation, fracture of struts, migration, and mucous plugging. When these complications result in airway injury or obstruction, it may become necessary to remove the stent. There have been few reports detailing techniques and complications associated with endoscopic removal of metallic airway stents. We report our experience with endoscopic removal of 30 such stents over a 3-year period. METHODS: We conducted a retrospective review of 25 patients who underwent endoscopic stent removal from March 2001 to April 2004. The patients ranged in age from 17 to 80 years (mean, 56.3 years). There were 10 male and 15 female patients. The stents had been placed for nonmalignant disease in 20 patients (80%) and malignant disease in 5 patients (20%). All procedures were done under general anesthesia with a rigid bronchoscope. Special attention was focused on the technique of stent removal and postoperative complications. RESULTS: Thirty metallic airway stents were successfully removed from 25 consecutive patients over a 3-year period. The basic method of removal involved the steady application of traction to the stent with alligator forceps. In all cases, an instrument such as the barrel of the rigid bronchoscope or a Jackson dilator was employed to help separate the stent from the airway wall before removal was attempted. In some instances, the airway wall was pretreated with thermal energy prior to stent removal. Complications were as follows: retained stent pieces (n = 7), mucosal tear with bleeding (n = 4), re-obstruction requiring temporary silicone stent placement (n = 14), need for postoperative mechanical ventilation (n = 6), and tension pneumothorax (n = 1). CONCLUSIONS: Although metallic stents may be safely removed endoscopically, complications are common and must be anticipated. Other investigators have described airway obstruction and death as a result of attempted stent removal. Placement and removal of metallic airway stents should only be performed at centers that are prepared to deal with the potentially life-threatening complications.

Endovascular stent-graft deployment: temporary vena caval occlusion with balloons to control aortic blood flow-experimental canine study and initial clinical experience.

Vena caval occlusion was evaluated in animal experiments. In five patients with thoracic aortic aneurysms, two balloon catheters were introduced via the femoral vein to the inferior vena cava and superior vena cava and inflated before stent-graft deployment. Aortic pressure and flow were immediately decreased, which minimized the downstream shift of the stent-grafts. Temporary vena caval occlusion is safe and effective for precise aortic stent-graft deployment.

Immigration Act 1988, 10 May 1988.

This Act amends the Immigration Act 1971 to do the following, among other things: 1) remove from the 1971 Act the obligation of immigration rules to preserve the statutory rights of Commonwealth citizens settled in the UK as of 1 January 1973, and their wives and children, to enter and remain in the UK; 2) ensure that only one polygamous wife or widow of a polygamous marriage, who otherwise would have the right of abode in the UK because of marriage, has a right to enter as long as more than one such wife or widow is living; 3) restrict the right to appeal of a person refused leave to enter to those persons who have a certificate of entitlement to the right of abode or have been granted citizenship; 4) restrict the issues to be considered on appeal of a person who has been ordered deported after less than seven years in the UK, subsequent to legal entry, to liability to deportation, in effect removing consideration of all relevant circumstances; 5) provide that any appeal against a refusal to vary leave automatically expires if a deportation order is made; 6) make the offense of overstaying one's granted period of leave to enter a continuing offense so that the statute of limitations on the offense does not apply; 7) provide that persons with the right of freedom of movement in the European Community do not need leave to enter or remain in the UK; 8) provide for the immigration examination of passengers to take place prior to arrival in the UK; 9) provide that leave to enter for six months is deemed granted after an examination if notice for granting or refusing leave is not given within 24 hours; previously the leave was indefinite in time limit and was deemed granted after 12 hours; 10) extend from two months to indefinitely the time limits within which removal directions may be given for persons refused leave to enter; and 11) authorize employment and occupation restrictions to be placed on persons given temporary admission or released on bail. The Immigration (Restricted Right of Appeal Against Deportation) (Exemption) No. 2 Order 1988 (S.I. No. 1203) makes an exception to 4) above for persons claiming asylum.

Plasma chemotactic activity during dipyridamole induced myocardial ischemia.

BACKGROUND: We have previously reported that transient myocardial ischemia induced during exercise or dipyridamole challenge leads to the release of increased amounts of hydrogen peroxide into circulating blood. It would indicate that the temporary functional changes within myocardial cells may constitute there a sterile inflammatory area. Therefore we decided to evaluate the chemotactic properties of plasma in patients undergoing dipyridamole provocative test, as a sign of released inflammatory mediators. The ischemia occurrence was evaluated with 99mTc-SestaMIBI followed by single photon emission computed tomography (SPECT). METHODS: Blood samples were obtained from the peripheral vein of 42 patients (18 men and 24 women, mean age 61 years). Plasma chemotactic activity was determined by the use of the Boyden chamber method: immediately before dipyridamole challenge (time 0), 7, and 30 min after drug infusion. The migration of control polymorphonuclear neutrophils towards evaluated plasma samples was estimated. RESULTS: Chemotaxis of control PMNs towards plasma isolated from patients without signs of myocardial ischemia 7 min after dipyridamole administration was significantly diminished in comparison with baseline values (p=0.003). Plasma obtained 7 min after dipyridamole infusion from patients manifesting signs of myocardial ischemia by SPECT attracted control PMNs significantly more intensively in comparison to plasma isolated at time 0 (p=0.0005). CONCLUSIONS: The obtained results indicate that transient myocardial ischemia induced by dipyridamole challenge leads to generation of chemotactic factors detectable in peripheral blood plasma.

Gastric electrical stimulation using endoscopically placed mucosal electrodes reduces food intake in humans.

BACKGROUND: Implantable gastric stimulation (IGS) has been proposed for treating obesity. The aim of this study was to investigate the effects of temporary mucosal electrical stimulation on water and food intake as well as gastric emptying in healthy humans. METHODS: The study was designed to study the effects of temporary gastric electrical stimulation (GES) on symptoms, gastric accommodation, food intake, and gastric emptying. It was performed in 12 healthy volunteers on 3 consecutive days. GES was performed using mucosal electrodes endoscopically placed in the fundus. RESULTS: The amount of maximum water intake was reduced with GES (894 +/- 326 mL) compared with sham-GES (1,093 +/- 417 mL, p = 0.01). The food intake was also reduced with GES (p = 0.012). In comparison with sham stimulation, GES delayed gastric emptying during the first 45 min after the meal but not during the remaining time. GES with parameters effective in reducing water and food intake and delaying gastric emptying did not induce significant dyspeptic symptoms, compared with sham stimulation. CONCLUSIONS: GES using temporary mucosal electrodes decreases food intake as well as maximum intake of water, and has a tendency of delaying gastric emptying. It may have a potential application for the treatment of obesity.

Partial amino acid sequence of purified von Willebrand factor-cleaving protease.

von Willebrand factor-cleaving protease (vWF-cp) is responsible for the continuous degradation of plasma vWF multimers released from endothelial cells. It is deficient in patients with thrombotic thrombocytopenic purpura, who show unusually large vWF multimers in plasma. Purified vWF-cp may be useful for replacement in these patients, who are now treated by plasma therapy. In this study, vWF-cp was purified from normal human plasma by affinity chromatography on the IgG fraction from a patient with autoantibodies to vWF-cp and by a series of further chromatographic procedures, including affinity chromatography on Protein G, Ig-TheraSorb, lentil lectin, and heparin. Four single-chain protein bands, separated by sodium dodecyl sulfate-polyacrylamide gel electrophoresis under nonreducing conditions, showed M(r) of 150, 140, 130, and 110 kd and were found to share the same N-terminal amino acid sequence, suggesting that they were derived from the same polypeptide chain that had been partially degraded at the carboxy-terminal end. A hydrophobic sequence (Ala-Ala-Gly-Gly-Ile-Leu-His-Leu-Glu-Leu-Leu-Val-Ala-Val-Gly) of the first 15 residues was established. The protease migrates in gel filtration as a high-molecular-weight complex with clusterin, a 70-kd protein with chaperonelike activity. vWF-cp bound to clusterin is dissociated by the use of concentrated chaotropic salts. vWF-cp in normal human plasma or serum is not associated with clusterin, suggesting that the observed complex is due to vWF-cp denaturation during the purification procedure. Activity of vWF-cp is unusually stable during incubation at 37 degrees C; its in vitro half-life in citrated human plasma, heparin plasma, or serum is longer than 1 week. There was even a temporary increase in protease activity during the first 3 days of incubation.

Experience with a new ureteral stent made of a biocompatible copolymer.

A double-pigtail ureteral stent made from a biocompatible copolymer was designed for antegrade insertion with a new coaxial system. Thirty-eight of these stents were successfully placed in 33 patients. Of eight stents used for benign temporary indications, two (two patients) occluded prematurely. One of these patients had retained stone fragments, which caused the 10-F stent to occlude 4 months after balloon dilation of a midureteral stricture. The second patient had a ureteroconduit stricture that was dilated and stented, but mucus occluded the 10-F stent 5 days after insertion. In 25 of the patients, 30 stents were placed for ureteral obstruction due to malignant neoplasms. Three patients died with patent stents, while surviving patients with malignancies continue to have functioning stents, for an overall mean patency of 5.1 months in these patients. No problems related to stent migration or brittleness have been encountered.

Potential role of natural enemies during tree range expansions following climate change.

Recent investigations have shown how chance, long-range dispersal events can allow tree populations to migrate rapidly in response to changes in climate. However, this apparent solution to Reid's paradox applies solely within the context of single species models, while the rapid migration rates seen in pollen records occurred within multispecies communities. Ecologists are therefore presented with a new challenge: reconciling the macroscopic dynamics of spread seen in the pollen record with the rules and interactions governing plant community assembly. A case that highlights this issue is the rapid spread of Beech during the Holocene into a landscape already dominated by a close competitor, Hemlock. In this study, we analyse a simple model of plant community assembly incorporating competition for space and dispersal dynamics, showing how, even when a species is capable of rapid migration into an empty landscape, the presence of an ecologically similar competitor causes Reid's paradox to re-emerge because of the dramatic slowing effect of competitive interactions on a species' rate of spread. We then show how the answer to the question of how tree species dispersed rapidly into occupied landscapes may lie in secondary interactions with host-specific pathogens and parasites. Inclusion of host-specific pathogens into the simple community assembly model illustrates how tree species undergoing range expansions can temporarily outstrip specialist predators, giving rise to a transient Jansen-Connell effect, in which the invader acts as temporary 'super-species' that spreads rapidly into communities already occupied by competitors at rates consistent with those observed in the paleo-record.

Contacts between regenerating axons and the Schwann cells of sciatic nerve segments grafted to the optic nerve of adult rats.

The relation between Schwann cells, basal laminae and axons during retinal ganglion cell regeneration was studied by using cellular, acellular and partially acellular sciatic nerve autografts into the optic nerve. Acellular grafts were achieved by temporary compression which eliminates living Schwann cells and axons. The compressed sciatic nerve together with the intact portion was used as a partially acellular graft. The compressed portion was anastomosed to the optic nerve and the intact portion was situated distally. After 3-21 days post-operation, the grafts were studied by thin sectioning and freeze-fracture. Axons were seen to regenerate into cellular grafts in contact with Schwann cells after one week, but not into acellular grafts for the entire period. In the partially acellular grafts, regenerating axons were first observed after two weeks and were always in contact with Schwann cells migrating from the intact portion. Moreover, membrane specializations, fuzzy materials in the space between apposed membranes, and putative tight junctions, were found between regenerated axons including growth cone and Schwann cells, and between adjoining Schwann cells. An extensive meshwork of putative tight junctions was displayed between reforming perineurial cells surrounding the groups of Schwann cells and associated axons. Gap junctions were seen between adjoining Schwann cells, and between reforming perineurial cells. These results suggest that the axonal contact with Schwann cell surfaces plays an important role in retinal ganglion cell regeneration.

Long-duration temporary vena cava filter: a prospective 104-case multicenter study.

BACKGROUND: Nonpermanent caval filters are placed in critical thromboembolic situations in which anticoagulation therapy is transiently contraindicated, ineffective, or the source of complications. The purpose of this study was to assess the safety and effectiveness of a second-generation long-duration temporary caval filter in these situations and compare its utility with that of other temporary filters. METHODS: This prospective study, including patients who underwent temporary caval filtration with the Tempofilter II, was conducted in nine European centers. All filters were successfully implanted. The filter was removed when the indication for caval filtration ceased. RESULTS: A total of 104 filters were inserted in 103 patients with an average age of 60 +/- 15.5 years (range, 22-92 years). Most patients (85%) had pulmonary embolism, deep venous thrombosis, or both. The main indications for caval filter placement were complications of or contraindications to anticoagulation therapy (n = 85; 82.5%) or for ineffectiveness of anticoagulation therapy (n = 12; 11.7%). The average duration of implantation was 29.5 +/- 14.0 days (range, 2-86 days). One filter migrated in the right atrium, followed by pulmonary embolism. No other case of pulmonary embolism or of infectious or mechanical complications related to the filter was observed. Thrombus was trapped within the filter in 24 cases (23.3%). All filters but one were removed, regardless of whether thrombus had been trapped. Retrieval was always successful after implantation periods up to 12 weeks. In 16 cases (15.5%), the filter was replaced by a permanent filter. CONCLUSIONS: The Tempofilter II is safe, effective, and useful in critical thromboembolic situations. It offers a valuable alternative to retrievable optional filters.

Between two cultures: identity, roles, and health.

Population shifts and migration are increasing worldwide, and with it there is a renewed interest in maintaining uniqueness and cultural heritage. Immigrants are constantly attempting to balance the values of their cultural heritage with those of the new host society. To deal with immigrant women's health-care needs, scientists from the host country need to understand the delicate interplay between several dynamic properties inherent in being a woman and an immigrant and to develop ways to describe the health-care needs and responses that are congruent with the properties of immigrant women's situations. Properties selected for this article are uniqueness versus stereotyping, permanence versus temporariness, and host-country values versus country-of-origin values. Appropriate methods of research discussed are involvement, lived experiences, and satisfaction and stress in roles. Proposed agendas for future study of immigrant women include identifying high-risk populations among immigrant women and developing appropriate methodological strategies and population-specific interventions.

[Prevention of temporary prostatic obstruction after high-energy microwave thermothery by placement of prostatic bridge catheter].

OBJECTIVES: The clinical utility of a novel intraurethral prostatic bridge-catheter (PBC) was evaluated for prevention of temporary prostate obstruction following targeted high-energy transurethral microwave thermotherapy (TUMT) in patients with benign prostatic hyperplasia (BPH). MATERIAL AND METHODS: High-energy TUMT was administered to 54 BPH patients under topical urethral anesthesia followed by placement of a PBC, which remained indwelling up to 1 month. Patient evaluation included determination of peak urinary flow rate (Qmax), International Prostate Symptom Score (IPSS), and quality of life (QOL) score at baseline, immediately following TUMT and PBC placement, and at periodic intervals thereafter up to 1 month. Results were compared retrospectively with those of 51 patients who underwent TUMT followed by standard temporary urinary catheterization, generally for 24 h. RESULTS: Immediately following TUMT and PBC placement significant improvements (p < 0.0005) were observed in mean Qmax, IPSS and QOL score of 59.3%, 33.5% and 23.6% respectively, compared with baseline values. Further improvements were demonstrable up to 1 month, at which time mean Qmax, IPSS and QOL score had improved 79.0%, 54.9% and 56.5%, respectively, vs baseline means (p < 0.0005). In a retrospective comparison at baseline and 14 days between PBC recipients (PBC group) and a cohort of TUMT patients who had undergone temporary standard catheterization and subsequent catheter removal (standard catheterization group), mean baseline Qmax, IPSS and QOL score were similar between the two groups. However, at the 14 day follow-up evaluation in the PBC group mean Qmax was 101.8% higher, and IPSS and QOL score were 47.9% and 51.1% lower, respectively, than the corresponding values in the standard catheterization group (p 0.0005). The PBC was well tolerated and remained in situ throughout the entire 1 month follow-up period in 48/54 (88.9%) patients. Early PBC removal was performed in 3/54 patients (5.6%) because of urinary retention and in 3/54 patients (5.6%) due to PBC migration. During the acute post-TUMT recovery period, PBC recipients experienced impairment in sexual function which, though statistically significant, was comparatively small in magnitude. CONCLUSION: PBC provides an efficacious and well-tolerated option for preventing prostatic obstruction in the acute post-TUMT period. This approach avoids the inconvenience and infection risk of standard indwelling catheters or intermittent self-catheterization. PBC insertion and removal are rapid, facile and non-traumatic. PBC placement may prove useful in improving the early results of TUMT.

Human duodenal myoelectric activity after operation and with pacing.

We sought to determine the influence of operation on the pattern of human duodenal myoelectric activity and to assess whether electrical pacing might correct any postoperative disturbances. Three pairs of temporary bipolar serosal electrodes were placed on the duodenums of ten patients undergoing cholecystectomy. Electrical recordings were obtained daily until the patients' discharge, at 3 to 7 days, after operation. On each postoperative day, a regular rhythmic pattern of pacesetter potentials (PPs) was detected in all patients. The PP frequency (mean +/- SEM) was greater at the proximal electrode than at the distal electrode on the first postoperative day (12.3 +/- 0.1 cpm vs 11.9 +/- 0.1 cpm, p less than 0.01) and on the day of feeding (12.0 +/- 0.2 cpm vs 11.6 +/- 0.2, p less than 0.01). Spontaneous periods when spike potentials accompanied each PP (phase III of the migrating myoelectric complex), were found in only one patient on the day after operation, while they were recorded in five patients after 3 to 7 days, when postoperative ileus had resolved (p less than 0.05). Pacing with electric pulses (50 msec, 5 to 15 mA, 11 to 13 cpm) did not alter the pattern of duodenal PPs or entrain them in the duodenum of any patient at any time after operation. In conclusion, the pattern of duodenal pacesetter potentials changed little during the period of postoperative ileus, while the incidence of phase IIIs of the migrating myoelectric complex was greatly decreased.(ABSTRACT TRUNCATED AT 250 WORDS)

Clinical utility of "blind placement" prostatic stent in patients with benign prostatic obstruction: a prospective study.

OBJECTIVES: To assess the efficacy and safety of two versions of a blind placement temporary prostatic stent in the treatment of patients with benign prostatic obstruction. METHODS: Two versions of a prostatic stent (blind placement stent 1 [BPS-1] and BPS-2) were blindly inserted in an outpatient setting under local anesthesia. An assessment of voiding function, symptom scores, and complications was performed at baseline, directly after stent insertion, and at 14 and 28 days after stent placement. RESULTS: A total of 55 men were enrolled in the trial. Most stents of both types were successfully placed. Spontaneous voiding was achieved in all patients immediately after stent insertion, with improvements in voiding parameters and symptom scores. In patients with the BPS-1, migration occurred in 85%. In patients with the BPS-2, migration occurred in 5%. The median indwelling time of the stent was 16 and 38 days for the BPS-1 and BPS-2, respectively. Removal was successful in all but 1 case (BPS-2). CONCLUSIONS: The BPS-1 is not suitable for clinical practice because of the significantly high migration rate. The bulbar segment of the BPS-2 can prevent stent migration. However, patients experienced significant discomfort after insertion of the BPS-2. The voiding parameters and symptom scores of the BPS-2 were not significantly improved. Therefore, the BPS-2 is also not useful for clinical practice. Additional adjustment in the design of the BPS is needed to overcome these problems.

[Temporary and permanent vena cava filter for prevention of pulmonary embolism].

Inferior vena cava filters have been used for prophylaxis of pulmonary embolism since the end of the sixties. Complication rates depend on the types of filters used and are reported to be 2% with the Guenther-Filter and 21% with the Titanium-Greenfield-Filter. Depending on the exact indication both filter types are useful instruments for the prevention of pulmonary embolism.
