Efficacy and safety of cefotiam hexetil in the treatment of ear, nose and throat infections.

In 60 patients suffering from acute ear, nose and throat infections the efficacy and safety of cefotiam hexetil (SCE-2174, CAS 61622-34-2) which is a pro-drug of cefotiam was studied. The clinical success rate (improvement/cure) achieved with a daily administration of 600 mg was 90%. In 63% of the cases being evaluable for bacteriological response an eradication of the causative pathogens at the end of the treatment period was found. The majority of the patients tolerated well the compound. Side effects which were possibly, likely or definitely related to the treatment were observed in 12 patients. In 7 of these patients the treatment had to be stopped due to side effects which is equivalent to an intolerance rate of 12%. The data obtained in the present study indicate that cefotiam hexetil is a potent drug for the treatment of acute and acute on top of chronic infections of the upper aerodigestive tract.

[A pathologic analysis on 9,666 cases of tumors of nose, pharynx and throat].

OBJECTIVE: To analyze nasal, pharyngeal and laryngeal tumor patients age, sex and tumors' sites, to find out the relationship of above indices and tumor's pathologic types. METHODS: 9,666 cases of tumors on nose, pharynx and throat diagnosed in West-China Hospital in 1984 to 1999 were analyzed for the constitution of histological type. RESULTS: 1. the ratio of benign and malignant tumor is 1:8.6, it is 1:62.0 in nasalpharynx; 2. papilloma, angiofibroma and angioma are more commonly seen in all 21 kinds of benign tumors; 3. differentiated squamous cell carcinoma, non-keratinized is the most common types in 25 kinds of malignant tumors, the ratio of carcinoma and sarcoma is 11.98 to 1; 4. The average age of patients with malignancy is about 10 years older than that of the patients with benign tumors, and malignant patients are male predominant; 5. squamous cell papilloma of oralpharynx was the most common in all benign tumors; nasopharyngeal carcinoma is the most common in all malignancies, nasal lymphomas are the most common seen sarcoma. CONCLUSION: The histologic types of nasal, pharyngeal and laryngeal tumors are related with patients' age, sex and sites of tumors.

An open multicentre study to compare the efficacy and safety of sultamicillin with that of cefuroxime axetil in acute ear nose and throat infections in adults.

A total of 110 adults with acute ear, nose and throat infections were treated orally with 750 mg/day (n = 9) or 1500 mg/day (n = 46) sultamicillin, or 500 mg/day (n = 51) or 1000 mg/day (n = 4) cefuroxime axetil for a minimum of 5 days. Variations in dose and duration of treatment were due to severity of symptoms. After treatment with sultamicillin for 8.1 +/- 1.5 days or with cefuroxime axetil for 7.9 +/- 1.6 days, local pain, erythema, exudate, oedema and adenopathies were improved in both treatment groups and all sultamicillin-treated patients were apyretic. All sultamicillin-treated and all but three cefuroxime axetil-treated patients experienced cure or improvement; only one cefuroxime axetil-treated patient discontinued treatment due to treatment failure. Gastrointestinal adverse events occurred in both treatment groups (eight sultamicillin-treated patients and three cefuroxime axetil-treated patients); one patient receiving cefuroxime axetil discontinued treatment due to nausea. Pruritus was reported by one sultamicillin-treated patient.

[Complications in ear-nose-throat day surgery].

BACKGROUND: More and larger operations are performed on an outpatient basis with patient leaving the hospital the same day and complications may remain unknown to the hospital unless the patients are asked explicitly. MATERIAL AND METHODS: Complications from 383 ear-nose-throat operations were registered continuously by the surgeon and supplemented with data from postoperative consultations and reports from other hospitals or doctors, and then compared with data from 328 (86%) postoperative patient interviews. RESULTS: According to the patient interviews, the rate of complications (16.7%) was much higher than the hospital was aware of (4.7%). Most unknown complications (40 out of 64) were mild but five were serious enough to require reoperation under general anesthesia. INTERPRETATION: Registration of surgical complications should include postoperative interviews with patients.

Ear-nose-throat manifestations of autoimmune rheumatic diseases.

Ear-nose-throat (ENT) manifestations of connective tissue disorders represent a diagnostic challenge for clinicians as they often constitute the initial sign of an otherwise asymptomatic autoimmune disease. Moreover, in patients with known autoimmune rheumatic diseases, ENT manifestations can be overlooked. Hearing disturbances may be seen in patients with systemic lupus erythematosus, Wegener's granulomatosis, relapsing polychondritis, polyarteritis nodosa, Cogan's syndrome, Sjögren's syndrome, and less frequently in Churg-Strauss syndrome and Adamantiades-Behçet's disease. Nose and paranasal sinuses are variably affected during the course of Wegener's granulomatosis, Churg-Strauss syndrome, relapsing polychondritis and sarcoidosis. Recurrent mucosal ulcerations are common in systemic lupus erythematosus and Adamantiades-Behçet's disease. Xerostomia is a common feature of primary and secondary Sjögren's syndrome; salivary gland enlargement may be also seen in these patients, as well as in patients with sarcoidosis. The cricoarytenoid joint can be involved during the course of rheumatoid arthritis, ankylosing spondylitis and gout; osteoarthritic changes have also been described. Motility disorders of the upper and/or the lower portions of the esophagus have been reported in patients with dermatomyositis/polymyositis, systemic sclerosis and systemic lupus erythematosus. Trigeminal nerve dysfunction may occur in patients with Sjögren's syndrome, systemic sclerosis, systemic lupus erythematosus and mixed connective tissue disease. Peripheral facial nerve palsy has been described to complicate the course of Sjögren's syndrome and sarcoidosis.

The rate of asymptomatic throat carriage of group A Streptococcus in school children and associated ASO titers in Duzce, Turkey.

A prospective study was conducted to investigate the rate of group A streptococci (GAS) carriers and associated anti-streptolysin O (ASO) titers in serum samples in asymptomatic school children in spring in Duzce, Turkey. Pharyngeal swabs were obtained to detect the presence of GAS and blood samples were collected to determine elevated ASO titers in serum. A total of 351 asymptomatic primary school children were included in the study, and 91 (25.9%) of these were found to be GAS carriers. Of the 91 carrier students, ASO titers were elevated (> or =200 IU/ml) in 34 students (37%). Of the 260 non-carrier children, ASO titers were found as elevated (> or =200 IU/ml) only in 27 (10.3%) students. The difference between the ASO-positivity rate of the GAS carrier group (34 in 91 students) and that of the non-carrier group (27 in 260 students) was found to be statistically significant (P < 0.05). The finding of the significant relationship between ASO positivity and GAS carriage indicated that ASO measurement might be used together with throat culture to identify of GAS carriers.

[Management of recurrent post thyroidectomy abductor vocal cord paralysis at the Ear, Nose and Throat (ENT) department of the Dakar University Teaching Hospital: report of 14 cases].

Latrogenic injury of the recurrent laryngeal nerve is a serious accident in the thyroid gland surgery. Bilateral losses of the abduction of the larynx, occurring during thyroidectomy, determine several dyspneas which often necessitate emergency tracheostomy. In second hand, 14 patients, who had undergone thyroidectomy, were admitted in the Ear, Nose and throat departement of Dakar university hospital with bilateral abductor vocal cord paralysis. Material consits of 12 women and 2 men. They were aged between 15 and 58 years old. Clinically, laryngeal dyspnea was noticed for all the patients and, emergency tracheostomy was performed for 10 patients (71,42%). Bilateral abductor vocal cord paralysis occurred after total or subtotal thyroidectomy in 12 cases. In the most cases (71,42%), the treatment was carried out, with arytenoidopexy, by extralaryngeal route. Successful results were noticed for 11 cases (78,6%) and decanulation was realised between 2 to 26 days after procedure operative. If results were satisfied for respiration, voice quality was bad.

Influence of complementary Viscum album (Iscador) administration on microcirculation and immune system of ear, nose and throat carcinoma patients treated with radiation and chemotherapy.

With the techniques of vital microscopic and reflection spectrometric imaging, representative characteristics of microcirculation and immunology of white blood cells were evaluated before, during and after radiotherapy and chemotherapy of patients suffering from ear, nose and throat carcinomas. Adverse effects of radiotherapy and chemotherapy on the microcirculation and the immune system were decreased and reconstitution processes were accelerated by complementary administration of a standardized mistletoe extract (Iscador).

Ear nose and throat injuries in children.

BACKGROUND: This study was carried out to determine types and mechanisms of injuries encountered in Ear, Nose and Throat (ENT) regions of children at two localities of NWFP province of Pakistan. METHODS: This study was carried out at ENT departments of DHQ Hospital, Swabi and Khyber Teaching Hospital, Peshawar, Pakistan from June 2001 to June 2003. Children up to age 15 years presenting in emergency with ENT injuries or reporting at OPDs were included in the study. RESULTS: 160 children reported with injuries to ENT in this 2 years period. Most of the injuries were in the nasal region (50%) and nasal bone fracture was the commonest (26%). Fall and trauma during playing were the mode of injuries. In children of less than 5 years, the commonest cause of injury was pointed objects in the mouth while in children between 5-10 years the cause was fall. In children of 10-15 years of age the common causes of injury were road traffic accidents (270%) and fall (23 %M). CONCLUSION: It is concluded that injuries of ENT in children are different than in adults. The mechanism of these injuries is also different.

[The optimization of patient positioning and irradiation-field documentation in irradiation in the neck-nose-throat area].

The large mobility of head and neck requires, when irradiating in this area, positioning and fastening aids, which go beyond simple head bolsters or head moulds. This paper presented describes a positioning system developed by us for irradiating the neck-nose-throat area, which, due to its construction, consists of a non-slipping positioning ground plate and an individually adjustable masking system, which is physiological safe, can be proceeded quickly and exactly, is relatively inexpensive, and is accepted by patients without objections, furthermore since skin markings by drawing field limits onto the face mask are no longer necessary. Various technical aids allow the placement of satellites to spare those body parts which are not irradiated and to install compensators for the direction of radiation. An integrated cartridge mount enables in a simple way easy production of field control documentation photos.

[High prevalence of beta hemolytic streptococci isolated from throat swabs in Buenos Aires].

Beta hemolytic streptococci, particulary group A, are the most frequently isolated pathogens in cases of pharyngoamigdalitis. Other beta hemolytic streptococci also produce this pathology. An increase of positive cultures for group A streptococci was detected during 2004 in relation to previous years. The aim of this study was to determine the isolation rates of beta hemolytic streptococci groups A, C and G during a period of 5 years. Pharyngeal exudates were obtained from children (aged 6 months to 18 years) and adults. Swabs were cultured on Columbia agar plates containing 5% sheep blood. Lancefield grouping was performed using a latex immunoagglutination test. Group A beta hemolytic streptococci were isolated significantly more frequently from pediatric population than from adults. Groups A, C and G beta hemolytic streptococci were isolated significantly more frequently during 2004 than in previous years. Group G beta hemolytic was more prevalent in adult population than in patients less than 18 years of age. Among the isolated beta hemolytic streptococci, in adults and children, 18.9% and 5.8% were non-group A streptococci, respectively. Therefore special attention should be paid not only to group A beta hemolytic streptococci but also to other groups. Throat culture is the most reliable method for detecting the presence of the beta hemolytic streptococci and should also be indicated in adult patients.

School-age girl with sore throat.

In summary, the evidence to date from multiple reviews provides guidance for some of the questions most frequently raised when caring for children like Jasmin who come to ambulatory care settings with the chief complaint of a sore throat. It is also clear that while this presenting symptom is one of the most common seen in everyday practice, the evidence to support practice for this clinical entity is evolving and requires continued monitoring for new developments in the state of the science.

Evolutionary significance of geographic variation in a plumage-based foraging adaptation: an experimental test in the slate-throated redstart (Myioborus miniatus).

Geographic variation in the plumage pattern of birds is widespread but poorly understood, and in very few cases has its evolutionary significance been investigated experimentally. Neotropical warblers of the genus Myioborus use their contrasting black-and-white plumage to flush insect prey during animated foraging displays. Although previous experimental work has demonstrated that white plumage patches are critical to flush-pursuit foraging success, the amount of white in the plumage shows considerable interspecific and intraspecific geographic variation. We investigated the evolutionary significance of this geographic variation by experimentally decreasing or increasing the amount of white in the tail of slate-throated redstarts (Myioborus miniatus comptus) from Monteverde, Costa Rica, to mimic the natural extremes of tail pattern variation in this species. In addition to measuring the effects of plumage manipulation on foraging performance, we performed field experiments measuring the escape response of a common insect prey species (an asilid fly) using model redstarts representing four different Myioborus plumage patterns. Our experiments were designed to test four hypotheses that could explain geographic variation in plumage pattern. Compared to controls, experimental birds with reduced-white tails that mimic the plumage pattern of M. miniatus hellmayri of Guatemala showed significant reductions in flush-pursuit foraging performance. In contrast, the addition of white to the tail to mimic the plumage pattern of M. miniatus verticalis of Bolivia had no significant effect on foraging performance of Costa Rican redstarts. In field experiments with asilid flies, model redstarts simulating the plumage of M. miniatus comptus of Costa Rica and M. miniatus verticalis of Bolivia elicited greater responses than did models of other Myioborus taxa with either less or more white in the plumage. The results of our experiments with both birds and insects allow us to reject two hypotheses for geographic variation in plumage pattern: (1) that geographic variation is a nonadaptive result of genetic drift, and (2) that selection for enhanced flush-pursuit foraging performance generally favors increased white in the plumage, but evolutionary trade-offs constrain the evolution of extensive patches of white in some geographic regions. Instead, our results suggest that geographic variation in the plumage pattern of Myioborus redstarts reflects adaptation to regional habitat characteristics that enhances flush-pursuit foraging performance.

[Potential of optical coherence tomography for diagnosing mucositis in cancer of the nasal cavity and throat in the course of radio- and chemoradiotherapy].

The monitoring data on mucosa of the nasal cavity and throat using optical coherence tomography imaging in 14 cases during radiochemotherapy are presented. Typical alterations such as reduction in contrast distinctions between tissue layers to an extent that it blurs down completely at the peak of radiation-related response are discussed. Due to use of the technique valuable information on individual response was obtained.

Susceptibility to 5-fluorocytosine, miconazole and amphotericin B of Candida albicans strains isolated from the throat of non-AIDS patients.

Eighty Candida albicans strains, isolated from throat of patients at the Universitary Clinic of the Faculty of Superior Studies Iztacala of the National Autonomous University of Mexico, were analyzed. They were identified by microscopic and colony morphologies, germ tube test, and by auxanogram and zimogram. Minimal inhibitory concentrations (MIC) of 5-fluorocytosine, miconazole and amphotericin B were determined by microtiter broth dilution. MIC frequency distribution of 5-fluorocytosine showed a single peak (0.25-8.0 microg/ml), with 65% susceptible strains (MIC < or = 1.0 < or =g/ml) and 35% intermediate susceptible strains (MIC = 1.1-8 microg/ml). MIC frequency distribution of miconazole was threemodal with 6.25% susceptible (MIC = 1.562 microg/ml), 48.75% intermediate susceptible (MIC = 3.125-12.5 microg/ml), and 45% resistant (MIC = 25-50 microg/ml) strains. All strains were susceptible to amphotericin B (MIC= 0.0156-0.125 microg/m). These results shows that amphotericin B was the more active antimycotic, followed by 5-fluorocytosine, against the strains analyzed, and that miconazole was the less effective one.
