Hemangioendothelial sarcoma of the liver after thorotrast injection.

An autopsy case of hemangioendothelial sarcoma of the liver which occurred 33 years after the injection of thorotrast was reported. A fifty-nine years old male with complaint of abdominal swelling and progressive jaundice died of hepatic failure and hemorrhage from gastric ulcer after 10 days of medical treatment in the Kumamoto University Hospital. Autopsy revealed hemangioendothelial sarcoma and multiple capillary hemangiomas in the liver. Histologically, a yellowish brown crystalline substance was intensively accumulated in the connective tissue in and adjacent to the thickened capsule of the liver, the lymph nodes of hepatic hilus, the spleen, the bone and the bone marrow. The material was certified as thorotrast with the Th-232 activity measurement and the autoradiographical examination. The absorbed dose of irradiation of the local liver tissue was calculated to be approximately 2,597 rads in 33 years. The relation among capillary hemangioma, hemangioendothelial sarcoma, and thorotrast was discussed.

Lung cancer in a Thorotrast administered patient.

A 79-year-old man developed small cell carcinoma in the lung 43 years after Thorotrast-injection. The tumor with interstitial fibrosis arose in the periphery of the lung. Thorotrast particles were observed in the liver, spleen, bone marrow, and lymph nodes but not in the lungs. Four other Japanese cases of lung cancer after Thorotrast injection were reviewed.

Thorotrast-induced hepatic angiosarcoma with 39 years latency. A pathologic and immunohistochemical study.

An autopsy case of hepatic angiosarcoma which occurred in a 64-year-old man who had been examined by Thorotrast angiography 39 years previously, is reported together with the pathologic, radiologic and immunohistochemical findings. Thorotrast deposits were found in the spleen, bone marrow, right elbow joint and lymph nodes as well as the liver. The angiosarcoma was accompanied by markedly distended sinusoids with hypertrophic and hyperplastic endothelial cells, which were considered to be closely associated with angiosarcoma. The results of immunostaining indicated that only a very limited number of tumor cells and not many sinusoidal endothelial cells showed a positive staining reaction to anti-factor VIII-related antigen (anti-F VIII-RAG). On the other hand, endothelial cells lining blood vessels in the portal areas were distinctly positive for anti-F VIII-RAG. This would suggest that production of F VIII-RAG by tumor cells was dependent on the degree of cell dedifferentiation or on the heterogeneity of endothelial cells.

Effect of PSK, a protein-bound polysaccharide preparation, on liver tumors of Syrian hamsters induced by Thorotrast injection.

The contrast medium Thorotrast, an agent well known to be carcinogenic, was injected into 400 congeneic Syrian hamsters. The resulting incidence of malignant hepatic tumors such as cholangiocarcinoma, hepatocellular carcinoma and hemangiosarcoma, was significantly higher in the male experimental group than in the control group, and the 50% survival period in the male group was shortened by about 100 days (P < 0.01). However administration of the antitumor drug PSK (Polysaccharide Kureha), a protein bound-polysaccharide extracted from basidiomycete fungi, prevented this carcinogenic effect. The incidence of malignant hepatic tumors in the experimental group was 22.5% compared with 2.8% in the control group (P < 0.01) and 10.5% in the PSK-treated group (P < 0.01). PSK also increased the 50% survival period by 61 days (P < 0.01).

The effect of Arvin on reticuloendothelial activity in rabbits.

1. Arvin stimulated the reticulo-endothelial uptake of colloidal carbon in rabbits. The onset of effect after intravenous administration was immediate and the minimum stimulant dose by this route was very low-0.05 unit/kg (0.1 mug protein/kg).2. Carbon opsonized in vitro with serum taken 4 h after an intravenous injection of Arvin was cleared more rapidly than carbon treated with control serum. Serum collected 10 min after Arvin did not promote carbon uptake.3. Reticulo-endothelial block with Thorotrast or excess carbon did not prevent the stimulant effect of Arvin but prevented the stimulant effect of opsonization with 4 h serum.4. The reticulo-endothelial stimulant effect of Arvin appears to be unrelated to its coagulant action on fibrinogen. The mechanism may involve a direct action on phagocytic cells and an effect on colloidal particles via serum opsonins.

Peliosis hepatis as a late and fatal complication of thorotrast liver disease. Report of five cases.

Five autopsy cases of peliosis hepatis occurring as a late complication of thorotrast (ThO2) liver disease are described. The liver contained many blood-filled cystic spaces of various sizes. Marked sinusoidal dilatation, disruption of cell cords and reticulin fiber framework, and cystic dilatation of sinusoids seem to represent the developmental stages of peliosis hepatis in sequence. Of the five cases, two had no other liver disease except for hepatic fibrosis, and the other three had associated neoplasms, such as angiosarcoma, hepatocellular carcinoma, cholangiocarcinoma, benign hemangioma, and their combinations. Peliosis hepatis seemed to have directly contributed to the patient's death in four cases. The most characteristic clinical feature was the fulminant terminal course with massive ascites, deep jaundice, and hepatic failure, often accompanied by hepatorenal syndrome and tendency to hemorrhage. Liver function study suggested progressive hepatic insufficiency with reduction in serum albumin, prothrombin and the clearance rate for test dyes, and increase in bilirubin. Clinical diagnosis was almost impossible without biopsy.
