[Determination of secretory immunoglobulin A in the saliva of children with recurrent and chronic respiratory tract diseases during a stay at a health resort].

Investigations of sIgA-concentrations in saliva of 101 children in age of 2 to 4 years with recurrent and chronic airways diseases were performed by means of a sIgA specific enzyme linked immunoassay. The aim was to find out immunodeficiencies in comparison to healthy children (n = 50), and to test whether or not there is an influence of medical and climate treatment over a period of 8 weeks onto sIgA values in saliva. Only 3 children showed decreased sIgA concentrations. There were no differences between patients and healthy children because of the great interindividual variances of sIgA concentrations. An effect of climate cure to sIgA values could not be shown.

[Changes in atrial natriuretic peptide in patients with lung embolism and obstructive respiratory tract disease].

Plasma levels of atrial natriuretic peptide (ANP) correlate closely with the pressure and mural tension of the right atrium and with pulmonary arterial pressure. We determined ANP levels in eight patients with scintigraphically confirmed pulmonary embolism, before and after reperfusion. Furthermore, ANP was measured in ten patients with acute obstruction of the airways, and during the course of clinical recovery. Peripheral venous blood was saved for determination of ANP plasma levels. The hormone concentrations were measured by radioimmunoassay. In ten patients with scintigraphically confirmed pulmonary embolism clearly higher ANP plasma levels were determined than after complete reperfusion (p less than 0.05). In patients with acutely exacerbated airways obstruction, hormone levels were measured which were significantly higher than those observed after clinical improvement (p less than 0.01). Since it is known, that ANP reduces pulmonary arterial smooth muscle tone and resistance, the hormone appears to be of physiological significance in pulmonary embolism and acute airways obstruction by lowering right ventricular afterload.

[A new inhalation system for bronchodilatation. Study of the acceptance of the Ingelheim M inhaler in chronic obstructive respiratory tract diseases].

The aim of this study was to investigate the acceptance and convenience of a new inhalation system for the treatment of chronic obstructive pulmonary disease. The Ingelheim M inhalator, an inhalation system with a supply magazine for six powder-filled capsules represents a further development of the Ingelheim inhalator in which only a single capsule can be utilized. The study was designed as an open, randomized, crossover trial involving 60 ambulatory adult patients. Each treatment period lasted two weeks. The medication tested was Berodual Inhaletten. The success of treatment was checked on the basis of peak flow metering each morning prior to and after initial inhalation. The results of the study show a highly significant preference on the part of the patients for the new inhalation system. The advantage most emphasized by the patients was the fact that they were able to carry with them their entire daily dose. Differences in the symptomatology of chronic obstructive pulmonary disease were not observed with these inhalation systems.

[Inhalation therapy with selective beta 2-sympathomimetic drugs. Effect of pirbuterol administered via the Autohaler and salbutamol administered via the Diskhaler in obstructive respiratory tract disease].

The therapeutic success of salbutamol inhalation with the powder inhalator (Diskhaler) and of pirbuterol with an inhalation-triggered metered aerosol (Autohaler) was determined by comparing the degrees of bronchodilation in 13 patients suffering from chronic obstructive airways disease. Rt and IGV were determined by whole-body plethysmography and FEV1, peak flow and MEF50 by spirometry, and pulse rates were also measured. On average, Rt values decreased by 40 per cent; the other lung function parameters responded correspondingly. Duration of action extended to 4 hours after dosage. Pulse rates and lung functions measured after administration of the products did not differ, either. In such studies as this, whole-body plethysmographic results are superior to those obtained by spirometry.

[A strategy for optimal standardized education of patients with obstructive respiratory tract diseases].

The contraindication between optimal treatment and, nevertheless, increasing morbidity and mortality in bronchial asthma results in interest being focussed on patient education aimed at improving the patient's own responsibility. The decisive object of any educational measures must be to increase this sense of own responsibility. In Germany, such educational measures are rarely provided, for the most part being offered by rehabilitation centres. At the present time, statistical data on the effect of such measures are available for some 500 patients. These patients are better informed and are better able to cope with their disease on a day-to-day basis, while therapeutic discipline is improved. The patients themselves desire such education in close conjunction with hospitals (rehabilitation centres) and their own physicians. The newly founded working group Patient Education at the Germany Society for Pneumology and Tuberculosis, has set itself the task of coordinating presently available activities. In an initial phase, current activities offered are made known and matched to one another. The next step is to work put the common essentials of the educational aims, contents and forms for children and adults with asthma, bronchitis and emphysema. The final objective is to establish standards for educational programmes to be provided by hospitals and rehabilitation centres, and for pre- and post-instruction by the patient's own physician.

[Incidence and social medicine significance of chronic obstructive respiratory tract diseases in West Germany 1986].

On the basis of the statistics provided by health insurance organisations and pension insurance carriers, the incidence, socio-medical significance and cost to the general economy of chronic obstructive airways diseases in the Federal Republic of Germany can be calculated. In 1986, in the Federal Republic of Germany, these disorders (ICD No. 490-496 in the Key to the International Classification of Diseases) resulted in early pensioning in 9,611 cases, 36,752 inpatient curative measures that were carried by the pension insurance scheme, and also 7.8 million lost working days and 1.4 million days of hospitalisation within the area of activity of the general local health insurances. Of the costs thus incurred, 5.3 billion DM can be itemized (outpatient drugs, inpatient curative measures, early pensioning, lost working days, hospital treatment), but the overall costs to the country's economy in 1986 must be estimated at approximately 7-9 billion DM.
