[Effect of progesterone on the immune system in consideration of bovine placental retention].

Effects of progesterone on the immune system of different species are presented by a literature study. Elevated progesterone concentrations in blood result in more severe course and longer persistence of bacteria in infectious diseases and decrease of the clearance of antibody-coated erythrocytes in vivo. Progesterone suppresses specific components of the immune system and natural killer (NK) cell activity while it has a mainly positive influence on other nonspecific components. It suppresses blastogenesis and cytotoxicity of lymphocytes and increases synthesis of asymmetric antibodies without effector function to block fetal antigens. Progesterone increases the concentration of leucocytes in blood, random migration and chemotactic ability, synthesis of reactive oxygen intermediates (ROI) and antibody-dependent cellular cytotoxicity of polymorphonuclear leucocytes (PMN) and expression of complement receptors, synthesis of ROI and phagocytosis of peritoneal macrophages. In the uterus mainly immunosuppressive effects of progesterone are described (decreased migration of PMN into the gravid uterus, persistence of bacteria or transplants, synthesis of immunosuppressive uterine milk proteins, decreased random migration of PMN) except for an increased content of IgA in the secretory products indicating a significance of progesterone in the pathogenesis of endometritis mainly in the cow and bitch. No correlation could be found between blood concentrations of progesterone and placental retention in cattle. Progesterone takes part in avoiding immunologic aggression of the maternal organism against the semiallogenic conceptus by suppressing specific components of the immune system and NK cell activity. Positive effects on nonspecific components of the immune system could be regarded as compensatory mechanisms to maintain the efficiency of the complete immune system.

[Curettage of uterine cavity in late puerperal bleeding].

Sixty eight (68) female patients were subjected to curettage caused by late puerperal bleeding. The above group is considered as 0.8% of all the female patients who delivered children during the period of research. The most frequent causes of bleeding were: abnormal desquamation of decidua (48.5%), residua of secundines (32.4%), and endometritis (14.7%). Prior to curettage, every patient was subjected to ultrasonographic examination. The suspicion of residual secundines, found in ultrasonographic examination was later confirmed in 36.8% of patients. No significant interdependence between the results of ultrasonographic and histopathological examinations, were noted. The disturbances in natural process of decidua excretion were rarely diagnosed in ultrasonographic pictures (3%) when puerperal bleeding required curettage of uterine cavity.

[Initial management of primary postpartum hemorrhage].

Accurate management of primary postpartum hemorrhage requires perfect knowledge of a recent protocol accessible at all times. Staff members and equipment need to be adjusted to the emergency situation. The first step is to establish a rapid diagnosis. This could be done with collecting bags, placed immediately after the birth. Once the diagnosis has been established, it has to be communicated to the whole team including obstetricians, midwives and anesthetists. A search for the cause of the hemorrhage is premordial. Abdominal uterine palpation confirms the diagnosis of uterine atony, the most frequent cause of postpartum hemorrhage. Two management schemes are possible depending on whether or not there is retention of the placenta. When the placenta is retained, manual removal has to be performed to empty the uterine cavity and to rule out uterine rupture. After placental delivery, manual control of the uterine cavity is always required. Uterine massage has to be performed after the intrauterine manoeuvres and must not be stopped too quickly. Rapidly other origins can be excluded: episiotomy or low genital tract lacerations requiring rapid repair, coagulation disorders requiring specialised care, or hematomas. During the same time, uterotonic drugs have to be used. Oxytocin (intravenous injection of 5 to 10 IU) is generally employed. Prostaglandins cannot be proposed as publications reporting their efficiency are not validated. Meanwhile, the anesthetists will have begun the first measures of resuscitation.

Modified B-Lynch technique for the control of massive postpartum hemorrhage. An alternative to hysterectomy.

The B-Lynch, brace suture technique is a life saving procedure. It is a simple and safe procedure, which has the advantage of preserving the uterus and thus fertility. The efficacy of the procedure can be assessed immediately and complete hemostasis is obtained in most cases. The special advantage of the B-Lynch technique is that it is an alternative to major surgical procedures such as hysterectomy and internal iliac artery ligation. A modified technique of the B-Lynch embracing suture has been performed in the case reported without any apparent complications.

[Myocardial infarction during post-abortion DIC].

We report the case of a 40 year old woman admitted for dilatation and curettage at 12 weeks of amenorrhoea with post-abortion partial placental retention. Clinically she had developed disseminated intravascular coagulation (DIC) complicated by an acute coronary syndrome with persistent ST elevation in the inferior leads. Management of her myocardial ischaemia was symptomatic, due to the underlying complex coagulation disorder making coronary reperfusion techniques impossible. After treatment for the cause of DIC, coronary angiography revealed no detectable angiographic lesion or spasm. In this context, the most likely aetiological hypothesis is a thrombus related to the DIC in an otherwise healthy heart, which was spontaneously lysed.

[Deferred deliveries in multiple pregnancies. Discussion on a new case report].

The authors report a new case of a triplet pregnancy in which an abortion occurred in the second trimester and twins were delivered at the 33rd week of gestation. The authors point out that it is possible to carry out a cerclage when a fetus and placenta are retained without necessarily causing an infection with clinical symptoms; but the authors do point out that there is always the possibility of infecting the mother and the fetus. As to the prognosis for the fetus, it is better it is given an opportunity to grow further in spite of the risk of infection.

[Wound healing and wound irrigation in cesarean section of cattle].

Calves were delivered by Caesarean section in 128 cases during the early months of 1984. All animals were allocated alternately to a trial group and a group of controls. When the peritoneum and transverse muscle had been sutured, the wounds of the animals in the trial group were irrigated and washed with 300 ml. of Betadine (10 per cent of PVP-iodine in water). This was followed by closure of the wound. The animals of the group of controls were not treated. The procedure was performed in ninety-four primiparae (73 per cent) and thirty-four multiparae (27 per cent). The indication for Caesarean section consisted in fetal oversize in 119 cases (93 per cent). Eight calves (6 per cent) were stillborn or died immediately post partum. The proportion of animals in which the placentae were retained, was 9 per cent. Two animals died from peritonitis and intra-abdominal haemorrhage respectively. Irrigation of the wound did not have any effect on the number of wound infections (Table 3). Wound infection occurred in nineteen animals (15 per cent). The operations were performed by six veterinary surgeons (Table 4). The trial group and group of controls treated by each veterinarian did not differ essentially as regards wound healing.
