Hospice patients are more likely to die at home than patients involved in conventional medical care, but the proportion of home deaths varies between programs. A study of the effect of hospice administrative characteristics on inpatient death rates is presented. Data from hospice programs that operated in New York State between 1993 and 1995 were obtained from the State Health Department and from publications of the National Hospice Organization. Inpatient death rates varied significantly between different operating structures: 11% for hospices that were divisions of home health care agencies; 34% for hospices that were divisions of hospital corporations; and 20% for hospices that were independent hospice corporations. This difference was explained by the presence of dedicated inpatient hospice units: Programs with such units demonstrated higher inpatient death rates than those without (42% versus 11%; P < .00001). Though not necessarily causal, these associations should be of interest to patients, physicians, families, and policy-makers.

The relationships between infant and preschool age mortality, malnutrition, and land tenure patterns in the State of Rio Grande do Sul, Brazil, were investigated with data from demographic and agricultural censuses, vital statistics, and a nutritional survey in urban and rural areas. These studies employed a variety of analytical methods and revealed that young children in areas with large ranches, livestock raising, and high proportion of agricultural wage-earners presented a higher mortality and a greater prevalence of malnutrition than children in areas with small farms, crop agriculture, and self-employed family workers. Children of landowners showed better nutritional status and smaller risk of death compared to children of laborers, although the differential seems to have narrowed in recent years. The conclusion is that land tenure patterns play a very important role in determining mortality and malnutrition of children in this Brazilian State.

This Decree-Law provides that a buyer may become the owner of abandoned land by paying 10% of the land price to the Government of Equatorial Guinea. The buyer is required to present and carry out a five-year plan for cultivation and may not sell the land to a third person. If these conditions are not fulfilled, the land must be returned to the State, which will then distribute the land to villages.

It is not clear if medical records belong to the physician, the institution or the patient. There is a bill being discussed in the Chilean Congress that establishes that "the patient, personally or through a representative, has the right to access and review his medical record. In case of death, this right may be exerted by his inheritors". In this article we postulate that this bill infringes a number of legal norms in force and universally accepted ethical principles. We distinguish between patient's advocacy to be informed and their free access to medical records. The main ethical principles violated are those of beneficence and non maleficence.
