[Cornea-crystalline lens needle-prick wounds from self-mutilation].

Automutilation is a voluntary autoprovoked lesion, which determines infirmities of different degrees. The paper presents the case of a 25-year-old patient, who, being in detention, pricked his both eyes using a needle. This action provoked endophthalmitis and bilateral traumatic cataract. The particularity of the case consists of the bilaterality of the autoagression, with serious evolution at the both eyes.

The use of double latex gloves during hip fracture operations.

A prospective study was undertaken to determine the perforation rate of surgical gloves when double-gloving techniques were employed in 120 hip fracture operations. One or more perforations occurred in 30 operations (25%). In 10 operations (8.3%), perforations of both the outer and inner gloves occurred at corresponding sites. The surgeons were able to recognize the perforations during the surgery on five occasions only. Of the 64 perforations identified, 41 (64%) occurred in the left (nondominant) hands, and 42 (65.6%) occurred at the tips of thumbs and index fingers. Nearly half of the perforations (46.9%) occurred at the thumb and index finger of left (nondominant) hands. To reduce the perforation rate, further protection of the thumb and index finger of the nondominant hand of surgeons is therefore required.

Perceived barriers to implementation of a successful sharps safety program.

IN RESPONSE TO INCREASING needle sticks and sharps injuries at a large, urban trauma center in the northeastern United States, a nurse educator assembled a focus group of OR staff members to determine what they perceived to be barriers to successful implementation of a sharps safety program. THE FOCUS GROUP IDENTIFIED inadequate communication, powerlessness, resistance to change, intimidation, inconsistencies in practice, negative attitudes, inexperienced staff members, and time constraints as barriers to a successful change implementation. USING THIS INFORMATION, the nurse educator identified strategies to implement a practice change to decrease the rate of needle sticks and sharps injuries.

Bisected macula following retrobulbar steroid injection.

A case of bilateral accidental globe penetration during administration of retrobulbar steroid for bilateral optic neuritis is discussed. One eye with bisected macula was managed successfully by vitrectomy, internal gas tamponade, and postoperative laser to the edges of the retinal tear. The fellow eye was blind due to central retinal artery occlusion.
