Tympanomety from 200 to 2000 Hz probe tone.

Previous investigations have shown that the probe tone frequency is the main factor in determing the shape of the tympanogram. These had, however, been limited up to 800 Hz. In the present paper, a multifrequency investigation (from 200 to 2000 Hz) has been performed on 72 ears with different pathophysiological conditions of the middle ear. From the analysis of the result some new critical aspects of tympanometry emerge. The multifrequency tympanometric investigation proves to be, according to our experience, a far more sensitive method for checking small changes in the transmission characteristics of the tympano-ossicular system than the monofrequency traditional analysis. In this connection, the technique seems to furnish a simple and reliable method for discriminating between stiffness versus mass-controlled systems.

[Auricular cartilage palisade technique for repairing tympanic membrane perforation].

OBJECTIVE: To study the auricular cartilage palisade technique for repairing tympanic membrane perforation. METHOD: Sixty-six cases of large tympanic membrane perforation were treated with auricular cartilage. The results were compared with that of temporal fascia repairing in 60 ears. RESULTS: The closure rates of tympanic membrane perforation were 92.4% in cartilage group and 80% in temporal fascia group, respectively. There was no significant difference between two groups in hearing improvement. The cartilage is quite suitable for repairing large perforation of the tympanic membrane and for treating adhesive otitis media. CONCLUSION: The auricular cartilage palisade technique is an ideal method for repairing tympanic membrane perforation.

[The measurement of cochlear flow by laser Doppler in man: the preliminary results].

Laser-Doppler flowmetry is presently one of the methods of choice in measuring cochlear blood flow. The techniques is non-invasive and is based on the frequency shift of the laser beam induced by the red blood cell movement. Previous studies of cochlear blood flow carried out on animals and humans demonstrated the reliability of laser-Doppler flowmetry and its usefulness in understanding inner ear microcirculation physiology. In this paper we present preliminary data obtained from three patients examined under general anesthesia while undergoing tympanoplasty. Results showed that tracings, whose baseline is proportional with the blood flow, are characterized by waves correlated to pulse beat and automatic ventilation. Moreover, intrinsic contractions of inner ear vessels (waves of vasomotion) as exist in cerebral microcirculation, were observed. Pharmacological hypotension hypocapnia and the application of epinephryne determine a significant reduction of cochlear blood flow. These results suggest that while cochlear blood flow is related to systemic pressure, it has an intrinsic control system. As well since we did not obtain any modification in bone conduction threshold after surgery, we conclude that laser-Doppler flowmetry is a safe technique.

Diagnostic value of tympanometry in otitis media with effusion.

Three particular applications of tympanometry are stressed. In the first place, tympanometric screening fails to give any appreciation of the tubal dysfunction evolution. During general anaesthesia, nitrogen oxide diffusion in the middle ear can hide effusion. Secondly, the correlation between tympanometric curves and middle ear effusion is far lower than usually assumed.
