[Study on lung carcinogenesis associated genes in human lung squamous cell carcinoma and malignant transformation of human bronchial epithelial cells induced by carcinogen].

OBJECTIVE: To investigate lung carcinogenesis associated genes in human lung squamous cell carcinoma and malignant transformation of human bronchial epithelial cells induced by chemical carcinogens with cDNA microarray. METHODS: The gene expression patterns were detected in all specimens by cDNA microarray which representing 4 096 different human genes. The differences in gene expression among 6 cases of human lung squamous cell carcinoma tissues and 6 normal lung tissues were analyzed. The different gene expression patterns between the normal human bronchial epithelial cell lines (16HBE) and the malignant transformation of human bronchial epithelial cells induced by Benzo(a)pyrene metabolite BPDE (anti-Benzo(a)pyrene diol-epoxide,BPDE) and crystalline nickel sulfide were also studied by that method. The similar changed genes among those gene expression patterns were identified as lung carcinogenesis associated genes. RESULTS: Among the 4096 genes of cDNA microarrays, there were 171 genes expressed differently among lung cancer tissues and normal lungs, 143 genes expressed differently between BPDE transformed cells and normal 16HBE cell lines, 151 genes differed between nickel sulfide transformed cells and normal 16HBE cell lines. By comparing the gene expression profiles, there were 89 similar changed genes which might be associated with human lung carcinogenesis, 39 of which were up regulated: 6 oncogenes, 4 cell cycle control genes, 6 cell proliferation genes, 8 metastasis genes, 3 neuroendocrine genes, 1 drug-resister gene, 1 anti-apoptosis gene, 1 oxidative gene and other 9 genes. 50 genes were down-regulated: 7 tumor suppression genes, 11 DNA repair genes, 1 antioxidant genes, 3 GST family genes, 3 cell framework genes, 2 apoptosis induced genes, 5 signal conduction genes, 5 cytokines and their receptor genes, 7 metabolization genes, 1 cell matrix genes, and other 5 genes. CONCLUSION: cDNA microarray can be applied to study gene expression profiles effectively and to screen human lung carcinogenesis associated genes.

[Study on apoptosis of lung tissue cells in patients with severe brain injury complicated with aspiration-induced lung injury].

OBJECTIVE: To investigate the role of apoptosis of pulmonary cells in aspiration induced lung injury in patients with severe brain injury with or without aspiration-induced lung injury. METHODS: The Glasgow scale (GCS) of 11 dead patients with severe closed brain injury was 3-8. There 11 cases were divided into aspiration-induced lung injury (AILI) and non-aspiration-induced lung injury (NAILI) groups. The plasma levels of tumor necrosis factor-alpha (TNF-alpha) and interleukin-8 (IL-8) were measured, and the ratio of apoptosis in lung tissue cells was also determined. RESULTS: The plasma levels of TNF-alpha and IL-8 in NAILI and AILI groups were (2.17+/-0.41)nug/L vs. (3.14+/-0.28)nug/L and (0.42+/-0.05)nug/L vs. (0.91+/-0.08) nug/L (P<0.05) respectively. Lung tissue cell apoptosis ratio was significantly higher in AILI group than NAILI group (P<0.01). CONCLUSION: TNF-alpha and IL-8 may induce apoptosis in lung tissues through different signaling pathway. During the early phase of aspiration-induced lung injury complicating severe closed brain injury, apoptosis in cells of lung tissue may play a role in the pathogenes.

Right and left ventricular performance after single and double lung transplantation. The Toronto Lung Transplant Group.

Twelve single lung and nine double lung transplant recipients were studied before transplantation and at 3 months and 1 year after transplantation with serial right and left ventricular radionuclide angiograms. The resting right ventricular ejection fraction increased in the double lung recipients from 31% +/- 5% before transplantation to 43% +/- 11% at 3 months after transplantation (p less than 0.05) and then remained stable to 1 year. The single lung recipients also demonstrated a significant rise in ejection fraction from 25% +/- 11% before transplantation to 36% +/- 12% at 3 months after transplantation. Again, this remained stable to 1 year. The exercise right ventricular ejection fraction also showed a significant posttransplantation rise in the double lung recipients (p less than 0.005) that remained stable to 1 year. In the single lung recipients a trend was seen for a rise in the exercise right ventricular ejection fraction that did not reach statistical significance by 1 year after transplantation. Neither group had a significant change in rest or exercise left ventricular ejection fraction. The systemic blood pressure increased significantly by 1 year after transplantation in both groups. The heart rate increase with exercise at 3 months after transplantation was significantly greater in the single lung group (42 +/- 13 beats/min) than in the double lung group (14 +/- 13 beats/min) (p less than 0.005). These data indicate that a significant improvement in right ventricular function occurs after single and double lung transplantation.

Cyclooxygenase inhibition in lungs or in neutrophils attenuates neutrophil-dependent edema in rat lungs perfused with phorbol myristate acetate.

Results from previous studies indicate that injury in isolated rat lungs perfused with buffer containing phorbol myristate acetate (PMA) and rat neutrophils (PMNs) is dependent on the production of reactive oxygen species and thromboxane (Tx) A2. The purpose of this study was to determine whether the lung or the PMN was the source of TxA2 required to produce lung injury in this model. Prostanoid synthesis by rat lungs or PMNs was inhibited selectively by pretreatment of either rats or isolated PMNs with aspirin (100 mg/kg p.o. or 100 microM, respectively). Unbound aspirin was removed from the lungs and PMNs before use in experiments. Lungs from vehicle-pretreated rats that were perfused with PMA and untreated PMNs exhibited increases in weight, lavage fluid albumin content and TxB2 production with respect to lungs perfused with PMA but no PMNs. Increases in these markers were prevented when cyclooxygenase from either the lungs or the PMNs was inhibited. These results indicate that TxA2 is produced by both PMNs and by lung cells in this preparation, and that TxA2 production by both of these sources is required for the manifestation of edema.

Metabolism of acrylonitrile to 2-cyanoethylene oxide in F-344 rat liver microsomes, lung microsomes, and lung cells.

The metabolism of acrylonitrile to the epoxide, 2-cyanoethylene oxide (ANO) was examined in rat liver microsomes, lung microsomes, and isolated enriched lung cell preparations. GC/high resolution MS was used to quantitate ANO in microsomal and cellular extracts by monitoring the fragment ion C2H3N (m/z 41.0265). The limit of detection was 0.05 pmol of ANO/0.5 microliter of standard solution, microsomal extract, or cellular extract injected onto the column, and the linear range of analysis was 0.05 to 12.5 pmol of ANO. Kinetic parameters of Vmax, V/K, and Km were calculated for microsomal ANO formation. Liver microsomes were quantitatively more active than lung microsomes on a mg of protein basis. The Vmax (pmol of ANO formed/min/mg of protein) was 666.61 for liver and 45.07 for lung microsomes. The V/K (pmol of ANO/min/mg of protein/microM) was 12.83 for liver and 0.02 for lung microsomes. The apparent Km was 51.93 microM and 1853.83 microM for liver and lung microsomes, respectively. When calculated as nmol of ANO formed/min/nmol of microsomal P-450, the Vmax for lung was equivalent to the Vmax for liver. ANO formation in the rat lung was cell specific. The rates of metabolism in the Clara cell-enriched fraction, the alveolar type II cell-enriched fraction, and the cell suspension were 2.55, 0.38, and 0.67 pmol of ANO formed/min/mg of protein, respectively. No metabolism was observed in the endothelial (small) cell-enriched fraction or in the alveolar macrophages. The results suggest that the lung contributes to the metabolism and disposition of inhaled acrylonitrile.

The influence of Fluosol-DA on the occurrence of lung metastases in Lewis lung carcinoma and B16 melanoma.

The development of lung metastases from subcutaneously implanted tumors or the development of lung nodules from intravenously injected tumor cells are model systems for metastases formation. Animals bearing subcutaneous Lewis lung tumors (50-100 mm3) were treated with a single dose of Fluosol-DA followed by 1 h of breathing carbogen or maintenance in air. Their lungs were examined for metastases 25 or 40 days after tumor cell implantation. Treatment with Fluosol-DA and carbogen or air breathing reduced by almost 4-fold the number of lung metastases seen. The addition of Fluosol-DA with air or carbogen breathing to treatment of the tumor-bearing limb with 20 Gy reduced the number of lung metastases by 2-fold compared to radiation treatment alone. If Fluosol-DA was administered immediately before or up to 3 days prior to an intravenous challenge with Lewis lung tumor cells, there was a 2- to 3-fold reduction in the number of lung nodules formed. Fluosol-DA administered immediately before or up to 4 days prior to B16 melanoma cells caused a 2- to 3-fold reduction in the number of lung nodules observed. The vascular endothelial cell monolayer adhesion assay was used to test the effects of prior exposure to Fluosol-DA on the attachment of radiolabelled B16 melanoma cells in vitro. There was a trend toward increasing attachment of B16 cells to the endothelial monolayer with prior exposure to increasing concentrations of Fluosol-DA; however, this difference did not reach statistical significance.

Isolated lung transplantation; initial experience at the University Hospitals Leuven. Leuven Lung Transplant Group.

Lung transplantation nowadays has become a therapeutic modality in the treatment of patients with a variety of end-stage lung diseases. Between July 1991 and December 1992, twelve patients received an isolated lung transplant (eight single lungs and four double lungs) at the University Hospitals of Leuven. The indication for transplantation was emphysema in five patients, pulmonary fibrosis in three, cystic fibrosis in three and primary pulmonary hypertension in one. There were four early, in-hospital deaths (30%): two from sepsis and multi-organ failure, one from anoxia following a bronchial dehiscence and another patient exsanguinated following stent insertion for a partial bronchial dehiscence. Three more patients have died during follow-up: two from chronic respiratory failure secondary to the development of obliterative bronchiolitis (one at 8 months and one at 17 months), and one from a late bronchovascular fistula 4 months following transplantation. The overall actuarial one and two year-survival was 50.0% and 41.6% respectively. All patients discharged from hospital were oxygen free with an improved lung function and exercise capacity. We conclude that lung transplantation is a viable therapeutic option for selected patients with end-stage, irreversible lung disease. In our experience, the bronchial anastomosis remains an important keystone in the early success. Lung transplantation provides a good quality of life in patients free from infection and rejection. Nevertheless, chronic rejection resulting in obliterative bronchiolitis is a major problem in long-term survivors.

Pathological lesions in the right and left porcine lung: evaluation of an alternative method for scoring pneumonic lesions based on right lung examination.

Plucks from 2,372 sows, gilts and slaughterswine (originating from 3 independent sources) were evaluated for presence and severeness of 6 different pathological entities. Results were calculated for each of the lungs separately and compared to the combined findings of both lungs. Both qualitatively (comparing relative test sensitivities) and quantitatively (comparing lung scores), the right lung results were more optimal than the left lung outcomes and relatively consistent among the 3 data sets. The over-all right lung relative sensitivities for the detection of catarrhal pneumonia or chronic pleuritis were 81.3% and 72.0%, respectively. The over-all differences in the right minus left lung scores for the 2 lesions were +13.0% and +9.0%, respectively. The necessary sample size of right lungs to detect at least 1 lung with catarrhal pneumonia or chronic pleuritis is presented along with a method for calculating confidence intervals.

Proliferating alveolar macrophages in BAL and lung function changes in interstitial lung disease.

In interstitial lung disease, the number of alveolar macrophages (AMs) can be increased. This may be caused by recruitment of precursor cells from peripheral blood and/or local proliferation in the lung. We therefore analysed proliferation, by studying both the expression of the nuclear proliferation antigen, Ki67, and the deoxyribonucleic acid (DNA) content, using the Feulgen reaction followed by cytometry. The patients had interstitial lung disease, i.e. sarcoidosis (n = 20), extrinsic allergic alveolitis (n = 20), idiopathic lung fibrosis or lung involvement in collagen-vascular disease (n = 19). In all patient groups there was a significant increase in proliferating AMs compared to healthy controls (4.2 versus 1.4% Feulgen, 2.1 versus 0.5% Ki67), with a significant correlation between these two parameters. A positive correlation was also found in bronchoalveolar lavage (BAL) between numbers of lymphocytes and proliferating cells in sarcoidosis and in fibrosis. In fibrosis, numbers of eosinophils and proliferating cells were also positively correlated. Our main finding was, however, a positive correlation between numbers of proliferating cells (Feulgen) and lung function parameters, especially vital capacity and oxygen tension (PO2) at rest, in patients with sarcoidosis and lung fibrosis. By contrast, in extrinsic allergic alveolitis, no correlation could be observed between proliferating cells and cell population or lung function. Our results suggest that local proliferation of macrophages is an important element in interstitial lung disease.

Regional danofloxacin lung tissue concentrations and their relationship to regional pulmonary blood flow in consolidated and nonconsolidated bovine lung.

Six calves with areas of pulmonary consolidation attributable to bronchopneumonia, and 6 calves with no areas of consolidation were given i.v. injections of danofloxacin. This injection was followed approximately 55 minutes later by injection of 15-microns radio-labeled microspheres to measure regional pulmonary blood flow. Calves were euthanatized exactly 1 hour after the danofloxacin injection. Six samples for determination of danofloxacin concentration, each surrounded by 4 samples for determination of gamma emission counts, were taken from each lung. Additional samples focusing on the line of demarcation between consolidated and nonconsolidated tissue were taken from calves with pulmonary consolidation. Data from calves with no areas of pulmonary consolidation indicated that blood flow was significantly reduced in the caudodorsal position of the left lung and the caudodorsal and cranioventral positions of the right lungs. Danofloxacin concentrations in the cranioventral positions of the right and left lungs were significantly lower than those in the middle-dorsal positions. Differences in danofloxacin concentrations and blood flow were analyzed in consolidated and non-consolidated cranioventral and middle-ventral positions of the lungs from calves with pulmonary consolidation. Decreases in blood flow in consolidated lung tissue ranged from 83.3 to 91.7%. Danofloxacin concentrations in consolidated lung tissue were significantly reduced by 41% in the middle-ventral position of the left lung. The line of demarcation step study revealed a significant reduction of blood flow at 2 and 4 cm into consolidated lung tissue, with reductions of 84 and 88%, respectively. Danofloxacin concentration did not significantly decrease in consolidated tissue.

Experimental reovirus serotype 1/strain Lang infection of the lung: a model for the study of the lung in the context of mucosal immunity.

A number of studies have examined the nature of the respiratory immune response to particular pathogens. Although many pathogens stimulate specific immunity in the lung, they frequently are not effective immunogens at other mucosal sites. Because the gastrointestinal tract is a major inductive site for mucosal immunity, a pathogen that is an effective respiratory and gut immunogen would allow studies of the interaction of the lung with gut mucosal immune system. Reovirus, a respiratory isolate that previously has been shown to be an effective gut mucosal immunogen, provides a potential model of the relationship of the lung to the gut mucosal immune system. In this report, we demonstrate that intranasal application of reovirus serotype 1/strain Lang (1/L) to CD-1 mice elicits an acute lymphocytic inflammatory infiltration of the lung and hyperplasia of the lung-associated lymph nodes. The initial inflammatory response occurs in the airspaces and interstitium of the lung. As the infection progresses, the initially diffuse cellular infiltrate becomes more focused around small bronchioles. Viral replication occurs predominantly during the first week of the infection, and infectious virions are eliminated during the second week. After the elimination of infectious virions, a secondary response consisting of the appearance of plasma cells adjacent to pulmonary arteries develops as the primary infiltrate organizes into peribronchiolar follicles, resembling the human inflammatory lung condition termed follicular bronchiolitis. These two infiltration patterns were also observed by immunohistochemical analysis of the the infected lung. Whereas CD4+ and CD8+ lymphocytes and Mac-1+ cells were found to be more closely associated with the primary infiltration process, B220+ lymphocytes were observed adjacent to pulmonary arteries. These results establish respiratory reovirus 1/L infection as a viable model for future investigations of the mucosal immune response in the lung and its relationship to the common mucosal immune system.

Comparison of regional variability in lung-specific gene expression using a novel method for RNA isolation from lung subcompartments of rats and mice.

The lung is composed of a complex assemblage of more than 40 different cell types. Therefore, investigative techniques that rely on samples derived from whole lung homogenates, whether for biochemical measurements of metabolism or the analysis of gene expression, are inherently insensitive to cell type or region-specific differences. Microdissection has previously been successful for defining region-specific metabolic activity in the lung. Tissues obtained by this technique exhibit good viability and permit reproducible enzyme activity measurements. In this paper, a technique for isolating RNA from lung subcompartments obtained by microdissection is described. The method is straight forward and results in high quality RNA that can be used to quantify specific mRNAs in microscopically selected lung subcompartments by complementary DNA or RNA hybridization techniques. This technique provides a significant increase in sensitivity over techniques based on whole lung homogenates because RNA contributed by relevant lung subcompartments is enriched. The high sensitivity of the method makes it feasible to compare differences in mRNA expression 1) within different regions of the lung in the same animal, 2) in the same region in different animals and between different species, and 3) between susceptible and nonsusceptible sites in conditions of focal lung injury.

[Method and technique of experimental autotransplantation of a lung and a lung lobe].

The authors have accumulated an experience of 350 canine autotransplantations of a lung and its lobe. The peculiarities of the operative technique are described, and special emphasis is laid on its technical details that largely determine the success of a lung autotransplantation. The described method and technique permit the performance of orthotopic autotransplantation of a whole lung and of its lobe, and of heterotopic autotransplantation of the lower lobe of the right lung into the left pleural cavity in place of a removed lower lobe or whole lung. The first stage of autotransplantation of a lung or its lobe consists in applying an atrio-venous or venous anastomosis and that of the pulmonary artery with continuous U-shaped eversion sutures, with a re-establishment of the circulation, thus reducing the period of lung ischaemia. Upon restoring the blood flow in the lung, a bronchial anastomosis is applied with interrupted or continuous sutures, and the ventilation is restored. The vascular and bronchial anastomoses are applied with synthetic suture material (Orsylon) on atraumatic needles. The extensive experience of the authors in long-term follow-up of the animals after transplantation (up to 5 years) demonstrates that the described technique and method of lung autotransplantation ensures good competence and patency of the vascular and bronchial anastomoses, without any stenoses observed in the late postoperative period. As shown by histological examinations, the inflammatory reaction around the suture material is minimal.

Asbestos exposure among Finnish lung cancer patients: occupational history and fiber concentration in lung tissue.

In a series of 65 surgically treated lung cancer patients, past exposure to asbestos was evaluated by personal interviews, and by scanning electron microscopy analyses of the mineral fibers in lung tissue. Lung tissue samples of 17 autopsied male office workers were analyzed as referents. According to occupational history, 37% of the lung cancer patients had definite or probable, 31% possible, and 32% unlikely exposure to asbestos. The fiber concentration in the lung tissue ranged from < 0.1 to 65 million fibers per gram dry tissue in the lung cancer group, and from < 0.1 to 0.8 million fibers per gram dry tissue in the reference group. In 26% of the lung cancer patients, but in none of the referents, the fiber concentration exceeded 1 million fibers per gram dry tissue. Most of the exposed patients had been employed in various construction jobs, and valuable information about the exposure levels could be obtained by the mineral fiber analyses. In general, there was a good accord between the exposure categorization and the fiber burden measured in the lung.

Detection of K-ras gene mutations in non-neoplastic lung tissue and lung cancers.

Oncogene and tumor suppressor gene mutations are candidate biomarkers for cancer risk assessment and lesion detection. The K-ras oncogene has previously been associated with non-small cell lung cancer (NSCLC), particularly adenocarcinomas in which reported rates of mutation have approached 30-40%. We have analyzed non-malignant lung tissue from patients with lung cancer and primary lung cancers for K-ras gene mutations. Mutations were detected in 32% cancers and 29% non-malignant lung tissue from patients with cancer. The majority of tumors testing positive were adenocarcinoma of the lung. Normal DNA controls, including peripheral blood lymphocytes and normal lung from non-smokers, were negative. The ability to detect genetic alterations in non-malignant lung tissues is consistent with the concept that genetic alterations are involved in field cancerization of the aerodigestive tract.

The incorporation of SPECT functional lung imaging into inverse radiotherapy planning for non-small cell lung cancer.

BACKGROUND AND PURPOSE: Patients with non-small cell lung cancer (NSCLC) often have inhomogeneous lung perfusion. Radiotherapy planning computed tomography (CT) scans have been accurately co-registered with lung perfusion single photon emission computed tomography (SPECT) scans to design radiotherapy treatments which limit dose to healthy 'perfused' lung. PATIENTS AND METHODS: Patients with localised NSCLC had CT and SPECT scans accurately co-registered in the planning system. The SPECT images were used to define a volume of perfused 'functioning' lung (FL). Inverse planning software was used to create 3D-conformal plans, the planning objective being either to minimise the dose to whole lungs (WL) or to minimise the dose to FL. RESULTS: Four plans were created for each of six patients. The mean difference in volume between WL and FL was 1011.7 cm(3) (range 596.2-1581.1cm(3)). One patient with bilateral upper lobe perfusion deficits had a 16% reduction in FLV(20) (the percentage volume of functioning lung receiving >or=20 Gy). The remaining patients had inhomogeneous perfusion deficits such that inverse planning was not able to sufficiently optimise beam angles to avoid functioning lung. CONCLUSION: SPECT perfusion images can be accurately co-registered with radiotherapy planning CT scans and may be helpful in creating treatment plans for patients with large perfusion deficits.

Collagenase-3 induction in rat lung fibroblasts requires the combined effects of tumor necrosis factor-alpha and 12-lipoxygenase metabolites: a model of macrophage-induced, fibroblast-driven extracellular matrix remodeling during inflammatory lung injury.

The mechanisms responsible for the induction of matrix-degrading proteases during lung injury are ill defined. Macrophage-derived mediators are believed to play a role in regulating synthesis and turnover of extracellular matrix at sites of inflammation. We find a localized increase in the expression of the rat interstitial collagenase (MMP-13; collagenase-3) gene from fibroblastic cells directly adjacent to macrophages within silicotic rat lung granulomas. Conditioned medium from macrophages isolated from silicotic rat lungs was found to induce rat lung fibroblast interstitial collagenase gene expression. Conditioned medium from primary rat lung macrophages or J774 monocytic cells activated by particulates in vitro also induced interstitial collagenase gene expression. Tumor necrosis factor-alpha (TNF-alpha) alone did not induce interstitial collagenase expression in rat lung fibroblasts but did in rat skin fibroblasts, revealing tissue specificity in the regulation of this gene. The activity of the conditioned medium was found to be dependent on the combined effects of TNF-alpha and 12-lipoxygenase-derived arachidonic acid metabolites. The fibroblast response to this conditioned medium was dependent on de novo protein synthesis and involved the induction of nuclear activator protein-1 activity. These data reveal a novel requirement for macrophage-derived 12-lipoxygenase metabolites in lung fibroblast MMP induction and provide a mechanism for the induction of resident cell MMP gene expression during inflammatory lung processes.

Clinical value of lung scintigraphy with Tc-99m gluconate in distinguishing benign from malignant lung diseases.

One hundred patients were studied with three-phase lung scintigraphy with Tc-99m gluconate. The results showed that there were statistically significant differences between benign and malignant lung lesions in the intensity of accumulation, the blood supply index ratio, and the radioactive uptake ratio. However, no difference was observed between the benign lung lesions and the healthy lungs. Analysis of false negatives and false positives revealed that false negatives had something to do with the cell types of the malignancies, in addition to the relatively small size of the lesions; the false positives were mostly caused by acute inflammation, for which obstructive pneumonitis might be responsible in part. It is concluded that Tc-99m gluconate is tumor-avid and can be used as an agent for positive imaging of lung cancer, because quantitative parameters, blood supply index ratio, and radioactive uptake ratio are more objective in distinguishing malignant lung lesions from benign ones, and are of relatively higher sensitivity and specificity. Thus, lung scintigraphy with Tc-99m gluconate provides an efficient supplementary measure for differentiating between malignant and benign lung lesions.
