Severe protein C deficiency and aseptic osteonecrosis of the hip joint: a case report.

UNLABELLED: Homozygous congenital protein C deficiency is accompanied by severe thrombophilia. Thrombotic events can be reduced in number and severity by lifelong oral anticoagulation therapy. A 4-year-old boy was first diagnosed as having severe congenital homozygous protein C deficiency during the neonatal period when purpura fulminans occurred as the first manifestation of thrombosis. From this time he had been treated with phenprocoumon (INR 3.5-4.5). During an infection of the upper respiratory tract he developed signs of a new episode of purpura fulminans (INR 2.6). Additional anticoagulation therapy with LMW heparin (Clexane) and protein C concentrate was given while the oral anticoagulation therapy was continued. On the third day of this episode the boy suffered from pain of unknown origin. MRI of the abdomen and of the pelvis revealed nontraumatic osteonecrosis of the hip joint. After a few weeks of immobilisation no special treatment was necessary. One year later he was able to walk with no problem. CONCLUSION: Aseptic osteonecrosis of the hip joint is associated with a variety of disorders including vascular thrombosis and haemorrhage. Especially young children and handicapped patients with thrombophilia and pain of unknown origin are suspected to have thrombosis in atypical regions.

Total replacement of the hip joint affected by Paget's diseases.

In seven patients with Paget's disease involving the bones about the hip joint, associated degenerative changes of the joint caused such severe pain and disability that total hip replacement was necessary. The results were uniformly satisfactory and no serious complications were encountered.

[Endoprostheses of the hip joint with plastic elements].

Presented are new constructions of hip joint endoprostheses, where the range of application of plastics was made broader, notably: full-range endoprosthesis with joining element made of plastic, and partial endoprosthesis with plastic head. We also describe the construction of an endoprosthesis with conical pin, meant for cementless implantation.

[Radiography assessment of the hip joint after the implantation of cementless ceramic cup and metal cup].

Radiographic assessment of the hip joint and clinical results after implantation of cementless Mittelmeier ceramic cup and metal Parchofer-Munch cup are presented. Material included 151 hips (56 ceramic cups and 95 metal cups). A-P radiograph and Engh criteria served for radiological assessment of the cup. Vertical, horizontal and angular migration as well as radiolucency in three De Lee zones were evaluated on postoperative and follow-up radiographs. Vertical migration and increased radiolucency within III De Lee zone were the most frequent findings. All types of migration and increased radiolucency in De Lee zones were observed more often after ceramic cup implantation.

Study of the lumbar curvature with various factors of pelvic inclination. Change of radiographic lumbar curvature according to hip joint flexion.

The objective of this study was to investigate the influence of the hip flexion upon parameters that are indicators of spinal mobility. These parameters include intervertebral disc angle and lumbar curvature by measurement of the radiography in twenty eight healthy men. The state of knowledge of how these parameters relate to lumbar mobility have resolved. The results of the parameters showed negligible changes in relation to the mobility of lumbar spine according to angle of hip flexion except at L4-L5 intervertebral disc angle and lumbar curvature (p < 0.05). This results of study confirms that hip joint movement therapy can be applied to these norms in patient management in supine position of for the stable spinal injuries in the upper lumbar region during early phase.

Prediction of the hip joint centre in adults, children, and patients with cerebral palsy based on magnetic resonance imaging.

The location of the hip joint centre (HJC) is required for calculations of hip moments, the location and orientation of the femur, and muscle lengths and lever arms. In clinical gait analysis, the HJC is normally estimated using regression equations based on normative data obtained from adult populations. There is limited relevant anthropometric data available for children, despite the fact that clinical gait analysis is predominantly used for the assessment of children with cerebral palsy. In this study, pelvic MRI scans were taken of eight adults (ages 23-40), 14 healthy children (ages 5-13) and 10 children with spastic diplegic cerebral palsy (ages 6-13). Relevant anatomical landmarks were located in the scans, and the HJC location in pelvic coordinates was found by fitting a sphere to points identified on the femoral head. The predictions of three common regression equations for HJC location were compared to those found directly from MRI. Maximum absolute errors of 31 mm were found in adults, 26 mm in children, and 31 mm in the cerebral palsy group. Results from regression analysis and leave-one-out cross-validation techniques on the MRI data suggested that the best predictors of HJC location were: pelvic depth for the antero-posterior direction; pelvic width and leg length for the supero-inferior direction; and pelvic depth and pelvic width for the medio-lateral direction. For single-variable regression, the exclusion of leg length and pelvic depth from the latter two regression equations is proposed. Regression equations could be generalised across adults, children and the cerebral palsy group.

[Early complication of the hip joint surface replacement according to Wagner (author's transl)].

Apart from infection and loosening of the implants, spontaneous fracture of the femoral neck is a specific early complication of the hip joint surface replacement according to Wagner. Based on our case material we demonstrate the problems involved, investigating not only the deficient operation technique but also the reasons for its deficiency which, in our opinion, are to be found in the method itself.

[Post-traumatic recurrent hip joint dislocation].

Recurrent post-traumatic dislocation of the hip joint without an accompanying fracture is very rare. Delayed reduction and a too short immobilization periods seems to favor redislocation. One case is presented together with a discussion of the operative therapy and the intraoperative pathological-anatomical findings with respect to the literature.

[Bilateral hip joint replacement as a one-stage or two-stage procedure for dysplastic coxarthritis: a comparative analysis of 30 patients].

AIM: Dysplastic coxarthritis usually occurs earlier than primary coxarthritis. As a rule dysplastic coxarthritis mostly affects young and active individuals. Bilateral coxarthritis may lead to severe impairment requiring surgical replacement of both hip joints, as the patients are frequently dissatisfied with the substitution of one joint alone. METHOD: In a retrospective study, 15 patients with bilateral hip dysplasia who had received total endoprostheses in both hip joints in a one-stage procedure (group A) were compared to 15 patients with bilateral dysplasia who had been operated on bilaterally in two stages (group B). Group B was subdivided into B1 (first side) and B2 (second side). All dysplasias were classified according to Hartofilakidis and Crowe. RESULTS: On the first postoperative day, group A experienced a mean drop of 33.9% in haemoglobin values, group B1 a mean drop of 25.1% and group B2 a mean drop of 26.3%. Patients of groups A/B1/B2 received on average 1.7/1.1/1.3 units of autologous blood and 1.2/0.1/0.3 units of foreign blood, respectively. Both groups experienced an improvement in their quality of life due to marked improvements in their Harris Hip Score and UCLA Score. Group A had no higher rate of complications. CONCLUSION: Although the single-stage procedure involved greater effort in terms of physiotherapy, patients preferred bilateral implantation of total hip endoprosthesis in a single surgical session because they needed to undergo the process of operation, mobilisation and rehabilitation only once.

[Special factors governing x-ray examination of the hip joint in children (author's transl)].

The special features governing roentgen examinations of the hip joint in children in respect of positioning, x-ray technique and radiation protection, are explained. Particular attention is paid to the criteria of an accurate focussing of the pelvis in infants. Furthermore, standard focussing procedures and x-ray symptoms are described for the following diseases and malpositionings: changes of the centre-collum-diaphysis angle (coxa vara, coxa valga), torsional hip, epiphysiolysis capitis femuris, aseptic necrosis of the femoral head (M. Perthes), and coxitis in babies, infants, and juveniles.

[Long-term observation after implantation of a carbon fiber-reinforced carbon hip joint endoprosthesis in foxhounds (author's transl)].

A hip-joint endoprosthesis, comprising a carbon fiber-reinforced stem, aluminum oxide head, and polyethylene acetabular cup, was studied in vivo trials in ten foxhounds after implantation with bone cement. Observations over 6 months and 1 year demonstrated that carbon fiber-reinforced carbon exhibits adequate strength for use as a stem material under the observed conditions.

Transient monoarticular synovitis of the hip joint in adults.

Clinical, laboratory, and radiological investigation of 6 cases of idiopathic monoarticular adult hip synovitis demonstrates that unlike the syndrome in children, in the adult the onset is very acute, short in duration, and followed by a short tapering off period with no sequelae to date. The ESR is the best index of the severity and course of the illness. Hip aspiration is of diagnostic value. Viral etiology was suspected, but not proven. Recognition of this syndrome in the adult will prevent an unnecessary arthrotomy for "septic arthritis" of the hip joint.

Idiopathic chondrolysis of the hip joint in adolescents. Case reports.

A group of patients with idiopathic chondrolysis of the hip joint reported on in 1971 have been reviewed and the findings reveal no significant change in their condition. since then a further 9 patients have been encountered and the findings are included in this article. This, we feel, confirms the existence of this strange condition, which almost inevitably results in the development of a fibrous ankylosis.

Activation of plasma coagulation by retransfusion of unwashed drainage blood after hip joint arthroplasty: a prospective study.

Twelve patients undergoing cementless hip joint arthroplasty were retransfused with unwashed drainage blood collected postoperatively. Global coagulation parameters, coagulation factors (factor V:C, factor VIII:C, activated factor XII, and factor XIII) and markers of thrombin generation (F1+2 Fibrin split products, thrombin-antithrombin complexes), fibrin generation (fibrinogen and fibrin degradation products), and fibrinolysis (D-dimers, thrombin degradation products, plasminogen) were determined. High levels of factor XIIa, thrombin and fibrin generation markers, and markers of fibrinolysis were present in the shed blood. After retransfusion (mean, 433 mL), increased levels of these markers together with decreased values for factor XIII and plasminogen were indicative of renewed clot formation and fibrinolysis in the circulation. These changes were highly significant compared with preretransfusion values. The unwashed drainage blood contained high levels of procoagulation material and induced an activation of the plasma coagulation pathway with renewed clot formation and fibrinolysis in the patients.

[Embryonic development of the hip joint].

Articular elements, joint cavity and intra-articular structures of the human hip joint are completely developed at the end of the embryonic period (8. week). The fetal period is characterized by growth of articular elements, by vascularization of the primordial skeleton and by the ossification of the acetabulum.

Effects of experimental excision arthroplasty of the hip joint.

The experimental results are reported on a series of sheep in which unilateral hip excision arthroplasty was performed. These sheep were slaughtered at regular postoperative intervals. Radiography and histology were performed on the normal and excised hip joints of the experimental sheep to provide information of the formation of the pseudarthrosis that develops following excision arthroplasty. The efficiency of the false joint that develops is good, judged on a clinical basis. Continuous remodelling of the acetabulum and resected face was noted throughout the experimental period. A sequence of degenerative changes in the acetabular cartilage was observed followed by filling in of the acetabular favity with new bone. New bone also developed on the cemoral component of the pseudarthrosis in the form of osteophytes situated around the perimeter of the resected surface. Growth deformities of the contralateral, nonoperated limb were induced.

Ultrasound examination of neonate's hip joints.

In a mass screening project, 872 neonates were examined clinically and by ultrasound. Hip joint stability was assessed in the ultrasound stress test showing a normal or physiologic instability, in the range of 0 to 2 mm. Along with the dynamic examination, a modified Graf's method was used. All obtained sonograms were submitted to careful assessment by one of authors, including regular measurement of alpha and beta angles. This relatively small group in the low-risk population served as a model of normality. White newborns compared with an black Caribbean group did not show the presence of apparent primary acetabular dysplasia. Both methods--dynamic ultrasound test and Graf's scanning--seemed to us to be valuable.
