Location perception: the X-Files parable.

Three aspects of visual object location were investigated: (1) how the visual system integrates information for locating objects, (2) how attention operates to affect location perception, and (3) how the visual system deals with locating an object when multiple objects are present. The theories were described in terms of a parable (the X-Files parable). Then, computer simulations were developed. Finally, predictions derived from the simulations were tested. In the scenario described in the parable, we ask how a system of detectors might locate an alien spaceship, how attention might be implemented in such a spaceship detection system, and how the presence of one spaceship might influence the location perception of another alien spaceship. Experiment 1 demonstrated that location information is integrated with a spatial average rule. In Experiment 2, this rule was applied to a more-samples theory of attention. Experiment 3 demonstrated how the integration rule could account for various visual illusions.

Seroprevalence of varicella among children and adolescents in Valencia, Spain. Reliability of the parent's reported history and the medical file for identification of potential candidates for vaccination.

This study assessed the seroprevalence of varicella antibodies in children and adolescents in Spain and evaluated the reliability of two methods for detecting susceptible individuals: (1) parental-reported history of varicella and (2) medically-documented histories maintained by the pediatrician. A total of 186 children (6 to 15 years of age) were recruited in 13 pediatric offices of Valencia, Spain. A brief case report form was completed including previous history of varicella referred by the parents, and a 5 mL blood sample was obtained. The pediatrician medical file was reviewed for antecedent of varicella. The overall prevalence of varicella antibodies was 84% and 88% in the 6-9 years and 10-15 years age brackets, respectively. The predictive value of a negative history of varicella disease was 48% by parental recall (52% "false negative"), and only 26% by medical record (74% "false negative"). However, the positive predictive value of a positive parental reported history or a positive medically-documented history was 95%. The most effective strategy for varicella vaccination of older children and adolescents in Spain will be to immunize those individuals with a lack of positive (unknown or negative) history of disease.

A teaching file program for use in an ultrasound department.

A radiology teaching file program has been developed for use in an ultrasound department. Sonographic findings and diagnoses for each case are stored as groups of key words drawn from a vocabulary of up to 2000 key words created by the user. The program runs on a microcomputer and allows storage of 5000 cases per 360 kilobyte floppy disk. Data entry is accomplished entirely by ultrasound technologists based on the ultrasound report. Cases may be retrieved by selecting either individual key words or groups of key words, and the results may be printed out in a variety of formats. Experience in using the system over a 12-month period is discussed.

1. With technological advances, nurses in the workplace are challenged to continue the protection of confidential health records. 2. The security of health records can be viewed in three distinct categories: securing files from outside intrusion, maintaining the internal data security, and providing for recovery from disasterous events. 3. Common techniques for securing data include authenication, electronic firewalls, and data encryption.

Privacy Act of 1974; matching program--HCFA. Notice of a matching program--the Internal Revenue Service (IRS), the Social Security Administration (SSA), and HCFA--disclosure of IRS taxpayer identity and filing status information to be matched with SSA earned income information for Medicare beneficiaries and their spouses.

As required by Section 6202 of the Omnibus Budget Reconciliation Act of 1989 (OBRA 1989), Public Law 101-239, the Department of Health and Human Services is providing public notice that the IRS and the SSA will disclose certain information regarding the taxpayer identification and filing status and the earned income of Medicare beneficiaries and their spouses for HCFA's use in identifying Medicare secondary payer (MSP) situations. This will enable HCFA to seek recovery of identified mistaken payments that were the liability of another primary insurer or other type of payer. The matching report set forth below is in compliance with the Computer Matching and Privacy Protection Act of 1988 (Pub. L. No. 100-503).

Medicare program; criteria and standards for evaluating common working file hosts--HCFA. General notice with comment period.

This notice describes the criteria and standards to be used for evaluating the performance of Common Working File (CWF) hosts in the administration of the Medicare program, for the current evaluation period beginning June 1, 1991. The results of these evaluations will be considered whenever the Health Care Financing Administration enters into, renews/extends, or terminates a Medicare carrier contract which provides for the performance of CWF host services. Since these services are currently performed by Medicare carriers, this notice is published in accordance with section 1842(b)(2) of the Social Security Act which requires us to publish for public comment those criteria and standards against which we evaluate Medicare carriers.

Ferranti executives say they're talking to several potential buyers.

Civilian Health and Medical Program of the Uniformed Services (CHAMPUS); reimbursement of providers, claims filing, and participating provider program--Office of the Secretary, DoD. Final rule.

This final rule implements provisions of the Department of Defense Appropriations Act, 1993, section 9011, which limits increases in maximum allowable payments to physicians and other individual professional providers (including clinical laboratories), authorizes reductions in such amounts for overpriced procedures, provides special procedures to assure beneficiary access to care, and establishes limits on balance billing by providers. Also, the final rule implements a provision of the National Defense Authorization Act for Fiscal Year 1992 that requires providers to file claims on behalf of CHAMPUS beneficiaries, builds into the CHAMPUS Regulation provisions that have been in effect for several years regarding the Participating Provider Program, and implements a new approach for CHAMPUS reimbursement for ambulatory surgery.

Notice of attestations filed by facilities using nonimmigrant aliens as registered nurses--Employment and Training Administration, DOL. Notice.

The Department of Labor (DOL) is publishing, for public information, a list of the following health care facilities that have submitted attestations (Form ETA 9029 and explanatory statements) to one of four Regional Offices of DOL (Boston, Chicago, Dallas and Seattle) for the purpose of employing nonimmigrant alien nurses. A decision has been made on these organizations' attestations and they are on file with DOL.

Data file. Most Medicare plans to see only 2% rate hike in 1999.

Data File: You'll find HCFA's 1999 payment rates for 100 U.S. counties with the highest Medicare risk enrollment on page 108. Plus, is managed Medicaid losing its sizzle? A new study finds that declining payment rates and changes in state programs are driving commercial HMOs out of Medicaid risk. Although this may signal a market opportunity for Medicaid providers, researchers find that profit margins can be slim to none.

Benfluorex: what use? A second look. Poor assessment file.

The clinical file on the value of benfluorex for hypertriglyceridaemia is highly inadequate. No clinical trials have been done with morbidity or mortality as end points. Available placebo-controlled trials are small, methodologically weak in most cases, and their results regarding triglyceride levels are contradictory. None of the three trials comparing benfluorex with a fibrate is interpretable, because of methodological problems.

Filing for bankruptcy: a sign of failure or hope for the hospital?

Hospitals that file for bankruptcy are usually viewed as admitting failure or defeat. However, bankruptcy can become a constructive financial management tool if used appropriately. Under Chapter 11 of the Bankruptcy Code, hospital management can use bankruptcy to reorganize the failing financial situation of the hospital into one that is financially stable and able to continue operations indefinitely.

Drug-food interaction information: a file card for patient education.

In order to minimize hazards and maximize effectiveness of a medication, patients need to be informed of potential drug-food interactions. Patient education of drug-food interactions is often a perplexing task for health professionals. At Pocatello Regional Medical Center, representatives from the pharmacy and dietary services have developed a method using a 3 X 5 file card to aid in this teaching process. A review of responsibilities and procedures is described. Examples for warfarin and tetracycline are illustrated.

Employee and government access to personnel files: rights and requirements.

A maze of record-keeping, disclosure, and privacy statutes and regulations, with accompanying case law, have created a boom in privacy law. Employers find themselves caught in this rising tide, torn between the often conflicting requirements. The issue of access to employee files--by employees, government, and unions--is one of the most difficult issues in this complicated situation. This article discusses recent developments in the field and aims to help readers thread their way through the maze of contradictions and pitfalls confronting employers.

Time limits for states to file claims--Office of the Secretary, HHS. Final rule.

This rule clarifies the circumstances under which Federal Financial participation (FFP) is available to States in otherwise allowable State expenditures in the programs established under titles I, IV-A, IV-B, IV-D, IV-E, X, XIV, XVI(AABD), XIX, and XX of the Social Security Act, i.e. financial assistance, child welfare, child support enforcement, adoption assistance, and social services programs and the medicaid program. FFP is currently available with respect to otherwise allowable State expenditures for these programs made before September 30, 1978 only if the State filed a claim for FFP within one year of the expenditure.

Retiree medical benefit plans and the impact of the bankruptcy filing of LTV Corp.

In the past two decades, post-retirement medical benefit programs have become a significant feature in the benefits packages of many employers. Soaring medical costs and decreasing work forces have made it increasingly difficult for employers to pay for these programs, while at the same time, Congress has made it more difficult for employers to prefund them. The critical level of unfunded retiree medical liability faced by many companies was brought to prominence when LTV Corp., the nation's second largest steel producer, filed for bankruptcy reorganization and promptly terminated its retiree medical benefit plan.
