The fate of autologous fascia lata grafts in dogs.

The fate of tricuspid fascia lata valves and intracardiac grafts was studied morphologically, histologically, and biochemically in dogs. Severe degenerative changes occurring by 21 days were largely correlated with graft thickening due to gross endothelialization. A primary event in graft alteration was an initial loss of the indigenous fibroblast population as detected both microscopically and by autoradiography after pulse labeling with 14C-Nethylmaleimide. Using different isotopic labeling procedures, both the in vivo and in vitro graft degradation patterns demonstrated that a second early eft degradation was correlated in vivo with endothelialization and collagenase secretion by invasive cells.

Biochemical modifications of fascia lata for autologous valve replacement.

The biochemistry of canine fascia lata was studied to assess the feasibility of tissue modification with a variety of cross-linking reagents and growth inhibitors in order to provide a more suitable material for heart valves. Chemical determinants for maximal reactivity were consequently established for several growth inhibitors, of which the hydrophobic ligands difluorodinitrobenzene, octadecylamine, and oleylamine were the most effective in preventing invasive cellular growth. This enhanced the longevity of atrial grafts in dogs as judged from morphological and histological examination of excised grafts.

Correction of intrinsic penile chordee with a ventral penile graft of fascia lata.

Chordee is an abnormal curvature of the penis detected on erection. Congenital or primary curvature of the penis without hypospadias is, however, rare. There are two kinds of primary curvatures; those associated with a normal corpus spongiosum and those with a hypoplastic spongiosum. Secondary curvatures associated with Peyronie's disease or periurethral fibrosis associated with urethral stricture are more common in adults. Four nonhuman primates had successful correction of artificially induced chordee with a graft of fascia lata.

Reconstruction of the anterior cruciate ligament in athletes, using a fascia lata graft: a review with preliminary results of a new concept.

This paper describes a new concept for the intra-articular reconstruction of the anterior cruciate ligament in competitive athletes. The concept includes careful selection of patients, the use of fascia lata as a graft, an operative technique which includes notchplasty and isometric placement of the graft, as well as aggressive rehabilitation according to Wolff's law. The majority of the patients were able to return to their preinjury activity level in their sport without major complications such as rerupture. A longer follow-up period is needed, however, to evaluate the long-term results of the concept.

The tensor fascia lata myocutaneous free flap.

The tensor fascia lata muscle and its overlying skin provide a reliable myocutaneous unit containing a large amount of soft tissue for a free flap transfer. Its anatomy is outlined, and the operative technique for transfer is detailed in a case of chronic stasis ulcer of the lower leg.

Tensor fascia lata myocutaneous free flaps.

Twelve tensor fascia lata myocutaneous free flaps were done in 11 patients. There were 2 complete failures and 2 partial failures. The complete failures were related to problems with the recipient vessels. The flap has a reliable pedicle and vascular territory and can be transferred with its sensory nerve. Excessive bulk is a common problem, especially on the heel and ankle.

Early and late results of fascia lata transplantation in high myopia.

Transplantation of a strip of fascia lata to strengthen the posterior sclera in high progressive myopia was performed on 184 eyes of 108 patients with myopia ranging from 7 to 39 D. Three weeks after surgery visual acuity with correction had improved by 0-1 to 0-5 in 151 eyes (82 per cent) and refractive power had decreased by 1 to 8 D in 169 eyes (92 per cent). In 105 eyes (67 patients) that were followed-up for periods of from 1 to 9 years myopia increased in six by 0-5 to 3 D. In 29 patients who had an operation on only one eye and were then followed-up for periods of from 1 to 7 years the degree of myopia in the fellow eye increased by 1-5 to 6 D in 24 (83 per cent), and of the eyes that were operated on myopia increased by 0-5 to 3 D in only 5 (17 per cent).

Reconstruction of the anterior cruciate ligament with freeze dried fascia lata allografts in dogs. A preliminary report.

In 16 adult dogs the anterior cruciate ligament (ACL) was transected and then reconstructed using freeze dried fascia lata allografts. Eight of the dogs had the intraarticular graft passed through drill holes in the tibia and femur, while in the other eight dogs the graft was passed through a drill hole in the tibia and routed "over-the-top" of the lateral femoral condyle. In all dogs the grafts were then passed deep to the lateral collateral ligament and secondarily secured to the tibia as an extraarticular reconstruction. Specimens from each group underwent histologic study and biomechanical testing on a mechanical testing system at 3, 6, 12, and 24 week intervals. All grafts were found to be intact at sacrifice with no overt evidence of biologic incompatibility. The knees displayed only mild instability to clinical testing without evidence of arthrosis. Histologically, the grafts appeared to function as a collagenous scaffolding for revascularization and fibrovascular creeping substitution. The mean maximum load at failure in the 24 week specimens reached 536.44 N, compared to values of 801.85 N in contralateral knees with an intact ACL.

[Morphological study of the heart valves formed from the fascia lata femoris after their implantation because of a rheumatic defect].

Pathological anatomy of grafting artificial heart valves formed of the broad fascia of the hip in 5 patients was studied. The fate of a transplant depended upon conditions of its adaptation on a new place. In the case of an unfavourable outcome of transplantation there were observed decomposition of the connective-tissue fibres with degradation of collagen, their decomposition with formation of lacunes, inflammatory reactions, vascularization of the transplant and sclerosis thereof, which, all together, brought about deformation of a new valve. It was suggested that dystrophic and autoimmune mechanisms of destruction of the transplanted tissue were involved.

Some applications of the tensor fascia lata myocutaneous flap.

Two cases of reconstruction using the Tensor Fascia (TFL) myocutaneous flap are presented. In the first case the T F L flap was used to resurface a complex soft tissue defect on the extensor surface of the left forearm. The T F L flap was used in the second case to resurface a left trochanteric bedsore in a patient recovering from paraparesis sustained from an attack of Transverse Myelitis. The flap was successfully used in both cases.

The functional anatomy of tensor fasciae latae and gluteus medius and minimus.

The more accurate description of the anatomy of the glutei and the new biomechanical theory that has been presented describe the abductor mechanism as a system in which the tensor fasciae latae has the primary function of balancing the weight of the body and the non-weight-bearing leg during walking. Gluteus medius with its three parts and phasic functions is responsible for the stabilisation of the hip joint in the initial phase of the gait cycle. It is important also in initiating the major gait determinant of pelvic rotation. Gluteus minimus functions as a primary hip stabiliser during the mid- and late phase of the gait cycle.

Modified suture technique for six-tailed fascia lata graft to slim palmaris longus tendon. A preliminary case report.

A modified suture technique is described, which allows to join a very slim tendon palmaris longus (below 3 mm wide) with a many-tailed fascia lata graft (15 mm wide). This technique is useful in case of ulnar palsy in children, where palmaris longus many-tailed graft is desired as lumbrical replacement and where Brand's anastomosis is considered too difficult. One successful case is also shown.

A new technique of threading fascia lata strip.

By using disposable needles, we have developed a new technique of threading the fascia lata strip through the stripper or through the eye of a needle. The technique is simple, effective, and needs no special instruments.

Is arthroplasty of the knee with fascia lata still valid?

A case of arthroplasty of the knee with fascia lata is presented, in a young patient who was followed up after more than ten years, with a very satisfactory functional result. This operation is certainly not new and has been abandoned by most surgeons. In our case, however, it was shown to be still effective.

Vascular pedicle of the tensor fascia lata myocutaneous flap.

We analyzed 122 lateral femoral circumflex arteries, using arteriograms and cadavers, to establish more precisely the course of the dominant vascular pedicle of the tensor fascia lata myocutaneous flap. Measurements were also done for preoperative planning.

Case report. Cardiobacterium hominis: review of prior infections and report of endocarditis on a fascia lata prosthetic heart valve.

The clinical spectrum of disease caused by Cardiobacterium hominis, a small, gram-negative coccobacillus, is little known to physicians. From analysis of a new case of C hominis endocarditis on a fascia lata aortic valve, and from review of all previous reports of infection due to this microorganism, a remarkably consistent clinical picture emerged. C hominis only causes endocarditis, affects middle-aged individuals with structurally abnormal, left-sided cardiac valves, and follows a subacute course associated with the frequent occurrence of mycotic aneurysms and embolic phenomena. The methodology and therapeutic importance of identifying this organism and separating it from the closely related Actinobacillus actinomycetemcomitans, Eikenella corrodens, and Haemophilus aphrophilus is emphasized. Either penicillin alone or penicillin plus an aminoglycoside is recommended for therapy.
