A preliminary screening study on human cystic echinococcosis in Cairo slaughter house personnel.

A descriptive serological screening study was done on human cystic echinococcosis (CE) in high risk groups selected from Cairo slaughter house employees and the household nearby using indirect haemagglutination (IHA) and enzyme-linked immunoelectrotransfer blot (EITB) tests. Characterized camel hydatid cyst fluid (HCF) antigen by 12.5% sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS-PAGE) was used. Three asymptomatic clinical cases (1.3%) were seropositive using EITB assay (reacting with six HCF protein bands of molecular weights approximately 7, 21, 28, 35, 127 and 210 kilodaltons 'kDa') but one case was seronegative by the IHA test. All three cases had typical lesions of CE in liver and lung revealed by abdominal ultrasonography (US), computerized tomography (CT) scan and chest X-ray. The prevalence of CE in Cairo slaughter house workers was 1.6% versus 1.1% in the household nearby.

[Radical surgical treatment of hepatic echinococcosis].

The results of surgical treatment of 42 patients with hepatic echinococcosis were analyzed. Solitary cyst was revealed in 28 (66.7%) of patients, complicated forms of the disease--in 5 (11.9%). Radical surgical intervention included the hepatic resection (in 18 observations) and pericystectomy (in 24). Specific postoperative complications didn't occur, all the patients survived. In the late follow up period the recurrences were not noted.

Primary vascular echinococcosis: an uncommon cause of chronic iliofemoral arterial occlusion.

We report the case of a 33-year-old woman who was admitted to our clinic with the diagnosis of chronic arterial occlusion of the right lower extremity. Preoperative angiography suggested an iliofemoral atherosclerotic occlusion. During surgery multiple hydatid cysts causing iliofemoral occlusion were found and excised. Arterial reconstruction was performed using an iliofemoral bypass technique with a polytetrafluoroethylene vascular graft. This case emphasizes that, in sheep-raising countries or in patients from such countries, primary vascular echinococcosis should be considered in the differential diagnosis of chronic arterial occlusion.

[Alveolar echinococcosis: a dreadful orphan disease].

A WIDESPREAD DISEASE: Significant progress in screening for alveolar echinococcosis has reduced the number of new cases observed in Europe. Health education and serodetection campaigns have allowed earlier diagnosis and more effective treatment. The disease cannot however be totally eradicated due to the widespread wild reservoirs, sometimes even in the center of large cities. THERAPEUTICS: Early diagnosed and treatment can inhibit the inevitable progression observed after clinical manifestations appear. Drugs can block disease progression and surgical excision can be most effective. Inversely, the hopes raised by liver transplantation in patients with advanced stage disease have not been fulfilled due to the more or less late-onset metastasis favored by immunosuppressive treatments. PERSPECTIVES: There has been considerable progress in our knowledge of this parasite disease, particularly in improved diagnostic techniques. They have also demonstrated that humans are poor hosts for the parasite which is often spontaneously ejected. We are beginning to better understand the mechanisms of this spontaneous cure. Practical consequences would be a definition of receptive patient profiles or "vaccine" or immunotherapeutic procedures.

[Use of CO2-laser in the treatment of liver hydatid echinococcosis].

The results of laser scalpel use in 226 patients with hepatic echinococcosis are analyzed. Laser beam produces coagulation of the small vessels and intrahepatic bile ducts, destroys alive protoscolexes, suppresses microphlora. It ensures easier postoperative period, reduces postoperative complications rate 4 times, hospital stay 2 times.

[Echinococcosis of the pelvic bone: four cases].

We report four cases of echinococcosis of the pelvic bone in 3 women and a man aged 30 to 55 years. Disease spread was wide, involving the entire hemipelvis in one case and the sacrum in another making surgical excision difficult. In two cases, computed tomography was highly contributive to diagnosis and assessment of local extension. We used surgical excision and hydrogen peroxide sterilization for the first two case. For the two last cases, we used albendazole in four 4-week cures at a 2-week interval before and after surgery. The clinical course was favorable to these two cases at 3 and 4 years. The albendazole-surgery combination appears to be the best therapeutic option.

The role of the marabou stork (Leptoptilos crumeniferus (Lesson)) in Kenya in the spread of echinococcosis from the abattoir.

In experiments, adult marabou storks were fed with hydatid fluid of viable Echinococcus cysts obtained from sheep and goats. On microscopic examination of the birds' faeces no scolices were found, the only remaining undigested parts of the parasites fed being the chitinous hooks of the rostellum. The feeding trials have shown that the marabou does not play a role in the spread of echinococcosis from the abattoir.

[Ultrasound examination in the surgery of hepatic echinococcosis].

It is expedient to perform treatment of patients with the hydatic liver with special reference to the stage of the development of the parasite, the state of the fibrous capsule which can be established with the help of ultrasound examination both before and during the operation. The developed ultrasound classification of liver Echinococcosis allows taking into account all the stages and forms of the disease which is necessary for planning the optimum method of treatment. Using computed tomography and serological methods is justified in the situations more complex for diagnosis. In all the cases of exogenous development of echinococcal cysts the most radical method is thought to be maximum ablation of the fibrous capsule under control of intraoperative ultrasound investigation.

[Alveolar echinococcosis in Belorussian polesie].

Results of long-time (1980-1998) investigations of wild animals, which are potential hosts of the pathogen of alveolar echinococcosis in Belorussian Polesie, have been already generalized. One species of carnivores, two species of insectivores and eight species of rodents take part in circulation of infection.

A survey of canine echinococcosis in Gobi Altai Province of Mongolia by coproantigen detection.

Few studies have been carried out for the prevalence of canine echinococcosis in Mongolia. This study was designed to elucidate a preliminary information of the prevalence from feces collected in the field. Sixty-seven fecal samples from dogs and 2 red foxes in Altai town were collected and examined for Echinococcus coproantigen and eggs. Coproantigen detection was performed by a sandwich ELISA using a monoclonal antibody EmA9 raised against Echinococcus multilocularis somatic antigen. Of the dog samples examined, 17 (25.4%) were positive by the ELISA. One out of two foxes was positive, too. Taeniid egg-positive feces were recognized in 12 dog feces. Only 6 samples were both coproantigen and egg positive. Eggs of Ancylostoma sp., Trichuris sp.; and Capillaria sp.; were also registered.

[Association of HLA-DRB1 allele and the susceptibility to alveolar echinococcosis in the west of China].

OBJECTIVE: To investigat the association of HLA-DRB1 alleles and the susceptibility to alveolar echinococcosis (AE). METHODS: Thirty-five patients with AE in the high prevalence areas in the west of China were investigated for HLA-DRB1 gene by PCR/SSP technique. The results were compared with 104 normal healthy peoples. RESULTS: Frequency of HLA-DRB1 * 040x was 26% in patient group, which was significantly higher than in control group (9.62%), with a relative risk (RR) of 4.45 (chi(2) = 13.67, P < 0.01), and with an etiological fraction of 0.20. Frequency of HLA-DRB1 * 0701 allele was decreased in patient group (patient 2.86%, control 13.94%; chi(2) = 6.67, P < 0.05), with preventable fraction of 0.30. While the frequencies of other DRB1 alleles were not significantly different. CONCLUSION: Susceptibility to AE is strongly associated with the HLA-DRB1 * 040x allele. The high prevalence of human AE in this region was associated with genetic factor. HLA-DRB1 * 0701 geners resistant to Echinococcus multilocularis metacestode infection.

[Differentiated treatment of pulmonary echinococcosis].

Results of operative treatment of 824 patients with pulmonary echinococcosis over the last 25 years were analyzed. In most often encountered oval shaped cysts cappitonage with vertical half-pursestring sutures was the most simple and effective method of residual cavity elimination with the least postoperative complications rate. In 43 patients with cysts of difficult configuration combined method of parasite bed closure with 2-3 surgical methods was used. Differentiated approach to residual cavity elimination has permitted to increase organ-saving surgeries rate from 79.2 to 92.4% that is very important for medical and social rehabilitation of patients.

[Osseous echinococcosis. Apropos of 6 anatomo-clinical observations].

Concerning these six cases, the authors recall the rarity of osseous localization of Hydatidiform disease, isolated as a rule, arising in the adult and preferentially involving the ilium, spinal column, femur, humerus and scapula. Being slowly progressive, the condition is revealed by pain, swelling and above all spontaneous fractures (long bones), or signs of root or medullary compression (spinal column), followed sometimes by fistula formation. Radiological diagnosis is often difficult with an osteolytic appearance without appreciable reactional osteogenesis. As for laboratory investigations, they are only of value in orientation; immunofluorescence being the weakest. The extent of osseous lysis frequently demands extensive surgery. From the anatomical standpoint, the hydatid variety of Echinococcosis has a specific intra-osseous development: beginning in the spongy bone, the architecture is intermediary between the hydatid appearance and the alveolar variety. Some cases however comprise a unilocular appearance with a suspicion of adventitial reaction. Extension into the soft parts frequently adopts a macroscopic appearance identical with that of visceral forms. Bony lysis is dependant on osteoclastic resorption or foci of necrosis while reactional osteogenesis is reduced. Medullary cellular reactions are determinant in the anatomical progress: osteitis or histiocyte and plasmodial reaction to the cuticle fragments.

Study on immunobiology in endoparasites of public health interest: echinococcosis-hydatidosis.

Cystic echinococcosis caused by Echinococcus granulosus is one of the most widespread zoonoses in the Mediterranean, endemic in some regions such as Sardinia. Some aspects of the research conducted in this area are briefly reviewed for an integrated analysis of both epidemiological and immunobiological knowledge, gained from field observations and experimental studies. Data on epidemiology in intermediate hosts, immunological assessment of exposure in humans, immune response and Th1/Th2 polarization in secondary experimental hydatidosis, kinetics of response in definitive host and cytokine production in experimental models are briefly reported. They confirm the usefulness of an immunobiological approach both in intermediate and definitive hosts and their potential in prevention, monitoring and control of E/H.
