Factorial validity of the Maslach Burnout Inventory (MBI-HSS) among Spanish professionals.

OBJECTIVE: To assess the factorial validity and internal consistency of the Maslach Burnout Inventory (MBI-HSS). METHODS: In a sample consisting of 705 Spanish professionals from diverse occupational sectors (health, education, police and so one), seven plausible factorial models hypothesized were compared using LISREL 8. RESULTS: The four-factor oblique solution and the three-factor oblique solution showed the best and similar fit. Deletion of Item 12 and Item 16, taking into consideration the suggestions in the manual, improved the goodness of fit for both models. The four-factor oblique model suggests that, in addition to Emotional Exhaustion (EE) and Depersonalization (DP), Personal Accomplishment (PA) consists of two components labeled here Self-Competence (Items 4, 7, 17, and 21) and the Existential Component (Items 9, 12, 18, and 19). However, the alpha coefficient was relatively low for the Self-Competence component, suggesting that it is more suitable to estimate the syndrome as a three-dimensional construct. The Cronbach's alpha was satisfactory for PA (alpha =.71) and EE (alpha =.85), and moderate for DP (alpha =.58). CONCLUSIONS: The results show that the MBI-HSS offers factorial validity and its scales present internal consistency to evaluate the quality of working life for Spanish professionals.

[Factors associated with burnout's syndrome: an epidemiological study of teachers].

This article presents the results of an epidemiological study on burnout syndrome among private school teachers in Greater Metropolitan Porto Alegre, Rio Grande do Sul, Brazil, testing possible associations with demographic, work-related, and stress-related variables. The target population consisted of 217 teachers, of whom 190 participated (87.5%). The Maslach Burnout Inventory (MBI) was used to measure burnout, together with a questionnaire to record the other variables. The results showed that teachers have a low score in the three dimensions comprising burnout: emotional exhaustion, depersonalization, and decreased personal fulfillment at work. Demographic variables did not show any association with burnout, while occupational variables, workload, and number of students were associated with emotional exhaustion. Bad conduct by students, family expectations, and limited participation in institutional decisions were the stress factors associated with burnout.

[Incidence of the burnout syndrome among Brazilian cancer physicians].

BACKGROUND: The Burnout syndrome can be characterized by abnormalities in three domains: emotional exhaustion (EE), depersonalization (DP) and lack of personal realization (PR). In the medical profession, oncologists are especially prone to this syndrome. This study evaluates its prevalence among cancer physicians in Brazil correlating it to their demographic, work related variables and seeks possible solutions to prevent burnout. METHODS: We mailed three questionnaires (Maslach burnout inventory, general and opinion questionnaires) to all 645 members of the Brazilian Cancer Society and received 136 responses after 10 weeks. RESULTS: The response rate was of 21%. The burnout syndrome was present at moderate or severe levels in all three domains analyzed in 15.7% of the physicians. For each of these the frequency of moderate or severe dysfunction was analyzed and found to be present in 55.8% for EE, 96.1% for DP and 23.4% for RP. Practicing physical activity or having a hobby correlated significantly with lower levels of EE (p = 0.008) while working only for the private sector correlated with higher DP scores (p = 0.021). Cancer physicians pointed out that less paper work (73.5%) and a lower patient load (72.7%) were the most important factors for prevention of this syndrome. CONCLUSION: Burnout syndrome is frequent among Brazilian cancer physicians and further studies should be conducted to evaluate its prevalence and prevention among other sub specialists.

Kallmann's syndrome and schizophrenia.

OBJECTIVE: Kallmann's Syndrome is a heritable disorder characterized by the association of hypogonadotropic hypogonadism and anosmia or hyposmia. A common pathogenesis for KS and schizophrenia had been proposed based on shared pathologies of these two disorders, although no such clinical associations have ever been reported. METHOD: We report a 35 year old man with schizophrenia and Kallmann's Syndrome. The patient presented with signs and symptoms of hypogonadism, severe hyposmia and normal endocrine functions of the anterior pituitary. Hyposmia has been attributed to the absence of the olfactory bulbs and tracts and atrophy of the olfactory gyri, but normal olfactory mucosa. The patient presented with paranoid schizophrenia with persecutory delusions, auditory hallucinations, thought disorder, depersonalization, and gradual but marked global deterioration. RESULTS: Psychiatric evaluation revealed an entirely different psychopathological and personality profile between the patient and the six other Kallmann patients studied. Cycle sequencing analysis revealed a normal sequence of all 14 exons of the KAL gene. In conclusion, based on the presented case, Kallmann's Syndrome and schizophrenia represent a rare clinical association rather than a syndrome with a common pathogenesis, which if present should be confined to the olfactory dysfunction.

The impact of cognitively impaired patients and shift on nursing assistant stress.

This study compared the stress experienced by nursing assistants (NAs) under four work conditions: high or low proportion of cognitively impaired patients and day or other shift. Five standard measures of caregiver stress served as the dependent variables in this study; burden, reaction to patient behaviors, workload, and two measures of burnout. A 2x2 multivariate analysis of variance found an interaction effect of type and shift on the stress measures. Univariate tests found that Burden and Depersonalization accounted for this effect. A further multivariate analysis of simple main effects found significant differences for each independent variable within each level of the other independent variable. Univariate analysis found that NAs who care for cognitively impaired patients on the day shift show significantly higher scores on specific stress measures. The article concludes with a discussion of how institutions can respond to the stresses faced by NAs who care for cognitively impaired patients.

Preadolescent religious experience enhances temporal lobe signs in normal young adults.

The hypothesis was verified that an ontogenetically earlier onset of temporal lobe transients, as defined by a preadolescent religious experience, facilitates more frequent and varied temporal lobe signs in adults. Data were collected for 868 male and female university students in psychology over a 10-year period; the 19% of students who reported this early experience displayed more frequent depersonalization, beliefs in psi phenomena, sensations of presence, sensitivity to odors, widened affect, childhood imaginary playmates, and left (writing) handedness. Early experiments were more likely to display confusion of boundaries between themselves, other people, and cosmic forces.

Maslach Burnout Inventory: factor structures and norms for USA pharmacists.

This study was conducted to establish normative data as well as evaluate the construct validity and reliability of the three subscales of the Maslach Burnout Inventory when given to a national sample of USA licensed practicing pharmacists. Data were subjected to principal factors analysis with iteration and a varimax rotation to obtain a three-factor solution. Visual analysis and statistical comparison provided empirical support for the presence of the hypothesized (a priori) constructs of Emotional Exhaustion, Depersonalization, and Lack of Personal Accomplishment. Internal consistency of the derived subscales, as measured by Cronbach's alpha coefficient, were comparable with previous data. Pharmacists' subscale scores were significantly lower than those obtained in studies of the helping professions.

Tyranny of the obsessional character.

This essay relates many of the unwelcome features of modern life to the increasing influence of obsessional personality traits. This influence is becoming more pronounced as civilization advances and comes to be increasingly reliant upon depersonalized mechanical and commercial systems. The result has been a glut of unwanted information, an exaggerated reliance upon computing and numbers, an overgrowth of bureaucracy, a discarding of intuitive wisdom in favor of calculative reasoning, a loss of simplicity, a jargonized language, and ultimately, the degradation of human relationships and of human beings themselves.

Relationships among burnout, death anxiety, and social support in hospice and critical care nurses.

The present study was undertaken to compare the occupational stress, levels of burnout, death anxiety, and the social support of a national sample of 376 hospice and critical care nurses. t tests indicated that critical care nurses reported significantly more occupational stress, showed higher burnout, and experienced more death anxiety than hospice nurses. The two nursing groups differed significantly when the three components of the Maslach Burnout Inventory were compared: hospice nurses reported feeling less emotional exhaustion, utilized the technique of depersonalization less frequently, and experienced a greater sense of personal accomplishment. The two nursing groups did not differ in social support when both the quantity and quality of that construct were examined. Pearson coefficients indicated positive associations between burnout and occupational stress and between burnout and death anxiety, with a negative relationship between burnout and social support.

Maslach Burnout Inventory: factor structures for pharmacists in health maintenance organizations and comparison with normative data for USA pharmacists.

This study compared the factor structure and burnout scores obtained on the Maslach Burnout Inventory from 84 pharmacists in Health Maintenance Organizations (HMO) with the normative data for USA pharmacists. Results provided empirical support for the reliability and validity of the inventory to measure burnout within the profession of pharmacy. Values of Cronbach coefficient alpha for subscales of Emotional Exhaustion, Depersonalization, and Personal Accomplishment were similar to those obtained with the normative sample. Factor analysis was conducted to yield the best three-factor solution. Derived factor loadings matched the three hypothesized subscales. On Personal Accomplishment the mean subscale score for HMO pharmacists was significantly higher than the normative score. Given limitations of the small sample, research is indicated to substantiate use of the inventory among HMO pharmacists.

Organizational factors as predictors of teachers' burnout.

The purpose of this study was to examine the burnout experienced by a sample of Greek teachers and to explore the extent to which certain organizational factors predict teachers' scores on the Maslach Burnout Inventory. The sample consisted of 100 teachers, 28 to 59 years of age. Greek teachers' means were lower than those for burnout of U.S. teachers. Stepwise regression analysis identified satisfaction with the job itself was the only significant predictor for Depersonalization and Emotional Exhaustion subscales, while satisfaction with the job itself and satisfaction with promotion were significant predictors for the Personal Accomplishment subscale. These findings showed that stress, e.g., role conflict and role ambiguity, were not highly correlated with teachers' burnout.

Confirmatory factor analysis and reliability of the Chinese version of the Maslach Burnout Inventory among guidance teachers in Hong Kong.

In 1995 Chan and Hui examined the responses of a sample of Chinese teachers on the Maslach Burnout Inventory and recommended a possible 2-factor rather than the original 3-factor model for the assessment of burnout among Chinese teachers. In the present study, the factor structure of responses to the Chinese version of the Maslach Burnout Inventory in a sample of 1,398 Chinese secondary school guidance teachers was examined using the EQS approach to confirmatory factor analysis. The results showed that a 3-factor model (Emotional Exhaustion, Depersonalization, and Personal Accomplishment) provided the best fit, with the first two factors highly correlated (r = .80). Internal consistencies for the subscales ranged from .80 to .88.

Self-pluralism and dissociation.

The study examined the relationships among self-pluralism and dissociative experiences. 100 students were assessed using the Self-pluralism Scale and the Dissociative Experiences Scale. Scores on self-pluralism were positively correlated with others: overall Dissociation (r=.50), Absorption (r=.54), Depersonalization (r=.43) and Amnesia (r = .28).

Sick leave and social benefits during pregnancy--a Swedish-Norwegian comparison.

BACKGROUND: To analyze the correlation between sickness absence, working conditions, pregnancy outcomes and pregnancy associated social benefits in two urban pregnant populations in Sweden and Norway with different social benefit systems. METHODS: Relevant information on 1649 delivered women was manually extracted by the authors from the antenatal care and delivery records as well as from the personal social security files kept in the Värnamo and Hamar communities, and then computerized in a depersonalized form. RESULTS: The reproductive histories and the pregnancy outcomes appeared clinically similar in the two samples. Swedish pregnant women were significantly more often employed outside home (84 vs. 69 per cent). The types of occupations held were similar in Värnamo and Hamar. Swedish pregnant women were significantly more sick-listed during pregnancy than Norwegian women (64 vs. 32 per cent) and with a longer average duration of the sick-leave spells (61 days vs. 44 days). The sick-leave rate among Swedish employed pregnant women was 75 per cent as compared to 48 per cent in Norway. The differences appeared most evident in younger pregnant women (<25 years). The Swedish sick-leave rates were higher within all four occupational subgroups studied. During the observation period the pregnancy associated social benefits were significantly more generous in Sweden. CONCLUSIONS: Sickness absence during pregnancy does not seem to covariate in a simple way with ill health, working conditions or the amount of social benefits available. The increased sick-leave rates in Sweden may possibly be accounted for by a changing attitude towards pregnancy and its natural consequences, especially among younger women.

Substance abuse in borderline personality disorder.

The impact of substance abuse on patients with borderline personality disorder was investigated. Substance abuse was common. Female patients preferred alcohol and sedatives. Male patients preferred stimulants. Substance abuse was associated with poor school performance, unemployment, and promiscuity. Depersonalization-derealization was common in nonsubstance using and alcohol-sedative using patients, but was rarely found in stimulant users. Substance abuse appears to be a devastating complication in the patient with borderline personality disorder.

Nursing postvention for spousal survivors of suicide.

There is a scarcity of information available with respect to postbereavement outcomes for survivors of the suicide of a loved one. Few studies have focused on postvention therapies for the bereaved, particularly the bereaved survivors of suicide. The major aim of this study as to compare the effects of two theoretically derived nursing postventions, Bereavement Group Postvention (BGP) and Social Group Postvention (SGP), among the widowed whose spouses died of suicide. The findings suggest that both groups experienced an overall reduction in depression and distress. Although participants in the SGP generally showed significant improvement in social adjustment, they tended to be less well adjusted with respect to their parental roles at the end of the 8-week postvention sessions. Comparison of the psychoemotional correlates of grief varied such that there were no significant differences between the postvention groups for social isolation, loss of control, somatization, or death anxiety. The BGP participants experienced significantly reduced levels of anger/hostility and guilt; however, feelings of anger/hostility actually increased for those receiving the SGP. There was a significant reduction in feelings of despair, rumination, and depersonalization for both groups. Although social isolation was not significantly reduced for participants in either group, those receiving the BGP tended to experience a reduction in social isolation and those receiving the SGP showed no changes.

The concept of an organic dissociative syndrome: what is the evidence?

The question of whether organic conditions, such as complex partial seizures, can cause dissociative symptoms is controversial. Although a diagnostic category for organic dissociation is included in the tenth edition of International Classification of Disease, it has never been identified in the Diagnostic and Statistical Manual. Its inclusion in the upcoming DSM-IV is currently under debate. This article surveys representative literature regarding the role of organic factors in the causation of dissociative symptoms and considers the differential diagnosis of organic dissociation from current and historic perspectives. Dissociative symptoms and disorders (including amnesia, fugue, depersonalization, multiple personality, automatisms, and certain furors) can be induced by a variety of medications, drugs of abuse, and medical illnesses or conditions affecting cerebral function. Organic dissociation can be distinguished from intoxication, amnestic disorder, and delirium. Psychiatric nosology and our conceptualization of altered mental states and functions would benefit from use of the concept of an organic dissociative syndrome, which has clinical, neurophysiologic, and medicolegal significance. Such a category should be included in DSM-IV.
