Genetic mechanisms of knee osteoarthritis: a population-based longitudinal study.

To describe the differences in knee structure and non-knee structural factors between offspring having at least one parent with a total knee replacement for severe primary knee osteoarthritis and age- and sex-matched controls with no family history of knee osteoarthritis, a population-based longitudinal study of 163 matched pairs (mean age 45 years, range 26 to 61) was performed at baseline and about 2 years later. Knee cartilage defect score (0 to 4), cartilage volume and bone size were determined with T1-weighted fat saturation magnetic resonance imaging. Body mass index (BMI), lower-limb muscle strength, knee pain, physical work capacity at 170 beats/minute (PWC170) and radiographic osteoarthritis were measured by standard protocols. In comparison with controls, offspring had higher annual knee cartilage loss (-3.1% versus -2.0% at medial tibial site, -1.9% versus -1.1% at lateral tibial site and -4.7% versus -3.7% at patellar site, all P < 0.05), a greater increase in medial cartilage defect score (+0.15 versus -0.01, P < 0.05) and a greater decline in PWC170 (-0.7 watts/kg versus -0.4 watts/kg, P < 0.01). There were no significant differences in change in BMI, lower-limb muscle strength, knee pain or tibial bone area between these two groups; however, the differences in knee cartilage loss and cartilage defect change decreased in magnitude and became non-significant after adjustment for baseline cartilage volume, tibial bone area, BMI and knee pain. This longitudinal study suggests that knee cartilage loss, change in cartilage defects and decrease in physical fitness all have roles in the development of knee osteoarthritis, which is most probably polygenic but may reflect a shared environment. Importantly, the cartilage changes are largely dependent on baseline differences in cartilage volume, tibial bone area, BMI and knee pain, suggesting that these factors might have a role in their initiation.

Disorders of perceived auditory lateralization after lesions of the right hemisphere.

The disorders of perceived auditory lateralization after unilateral damage of the cerebral hemispheres were investigated by dichotic presentation of pure tones, lateralization being obtained by interaural intensity differences. Observations were made on 107 unilateral brain-damaged patients and 30 normal subjects who were required to localize the perceived position of fused sound images. The images were generated by dichotic stimuli delivered through headphones and the patients were asked to point the perceived position. A clear-cut hemispheric asymmetry was found. Whereas left brain-damaged patients and right brain-damaged patients without visual field defects were not impaired in this task in comparison with controls, the performance of right brain-damaged patients with visual field defects was defective, with a systematic directional error towards the right side, that is, the side ipsilateral to the damaged hemisphere. The defect of auditory lateralization involved both the half-space contralateral to the lesion and the ipsilateral one. Different hypotheses are discussed in order to explain the present results. It is argued that they cannot easily be interpreted in terms of defective processing of sensory information and the proposal is made that lesions of the right hemisphere may produce a distortion of the internal representation of egocentric space to the right, yielding a systematic error of auditory lateralization are discussed.

Impairment of BMIPP accumulation at junction of ventricular septum and left and right ventricular free walls in hypertrophic cardiomyopathy.

UNLABELLED: Myocardial scintigraphy using 123I-15-(p-iodophenyl)-3-(R,S)-methylpentadecanoic acid (BMIPP) reveals a high incidence of reduced accumulation of the tracer in patients with hypertrophic cardiomyopathy (HCM). This defect is thought to reflect impairment of myocardial fatty acid metabolism. In this study, the distribution of BMIPP defects was characterized and correlated with the clinical features of patients with HCM. METHODS: Thirty patients with asymmetric septal hypertrophy (ASH) were examined. Regional BMIPP accumulation was evaluated. Each region was normalized to the accumulation in the nonhypertrophic lateral region, which was represented as 100% on each bull's-eye map. The corresponding thallium accumulation for, each region was then used to correct for the partial-volume effect. RESULTS: BMIPP accumulation was significantly less in the septal portion of the anterior wall (As), the septal portion of the posterior wall (Ps) and the apex than in the lateral segments. BMIPP defects were significantly more frequent in the As, Ps and apical segments (20.0%, 20.0% and 33.3%, respectively) and were present in the As or Ps segments in 8 patients (27%). The patients with BMIPP defects in the As or Ps segments had a more frequent family history of HCM or sudden death and severe cardiac dysfunction. CONCLUSION: BMIPP defects occur predominantly in the As and Ps segments in some patients with ASH, which is often associated with severe cardiac dysfunction. The distribution of BMIPP defects may contribute to the classification of HCM and the assessment of its severity.

The subcutaneous pedicle flap in melolabial reconstruction.

OBJECTIVE: To review and report our experience with the subcutaneous pedicle flap in the reconstruction of defects adjacent to the melolabial crease. DESIGN: Retrospective review. SETTING: Cutaneous surgery unit of an academic tertiary referral center. PATIENTS: One hundred nine patients with defects of the lateral aspect of the upper lip, the medial aspect of the cheek, and the melolabial crease after Moh excision of cutaneous lesions. CONCLUSIONS: In our experience, the subcutaneous pedicle advancement flap is often ideally suited to the reconstruction of cutaneous defects adjacent to the melolabial crease. We have been particularly satisfied with the results of this reconstructive technique when addressing deep defects adjoining the alar facial sulcus in patients with full cheeks.

Midaxillary lateral thoracotomy for closure of atrial septal defects in pre-pubescent female children: reappraisal of an "old technique".

Our long-term follow-up has revealed that symmetrical development of the breasts is significantly impaired in pre-pubescent female patients subsequent to an anterolateral thoracotomy. Although standard posterolateral and anterolateral right-sided thoracotomies are used for correction of "simple" cardiac lesions such as patency of the arterial duct, coarctation of the aorta, or atrial septal defect, the required partial transection of large muscle groups, and injury to the developing tissues of the breast, may contribute to an unfavourable cosmetic outcome. Over the years, many surgeons, mostly specialising in thoracic procedures, have advocated mini- or axillary thoracotomies. In an attempt to improve surgical and cosmetic outcome, we have now adopted such a muscle-sparing approach, using a small horizontal midaxillary incision. We have now successfully employed the technique to close atrial septal defects in the oval fossa in 17 pre-pubescent females.

How much value is muscle charting? A study of the relation between neonatal assessment of muscle power and later mobility in children with spina bifida defects.

95 syrviving children with spina bifida were examined between the age of 3 to 8 years and their mobility assessed in relation to the charting of muscle power in the neonatal period. There was no good correlation between these two methods of assessment and the value of repeated muscle charting by the physiotherapists is questioned.

[Comparative analysis of the regenerative activity o the muscular and neural tissues after trauma and after transplantation of minced muscle].

Regenerative processes in the body muscle of fish Variocorhinus capoeta sevangi were studied both after the excision of a portion of three muscle segments and following autografting minced muscle into the defect formed. Muscle trauma resulted in a damage field, including the defect and the adjacent degeneration zone. The inflammation, necrotic mass resolution and regeneration of the nerve and muscle tissues were lengthy. Formation of myogenic elements started during the 3rd week only. Individual regenerating nerve fibers in the damage zone appeared 2 months later. Filling the defect with minced muscle accelerated the regeneration. The grafted muscle fiber fragments were not only involved in the regenerative process, but also stimulated the plastic activity of the muscle and nerve tissues of the grafted bed.

Role of cell adhesion molecule nectin-3 in spermatid development.

Seminiferous epithelia of the testes contain two types of intercellular junctions: Sertoli-Sertoli junctions and Sertoli-spermatid junctions. The former junctions are equipped with tight and adherens junctions while the latter junctions are not. Ca2+ -independent immunoglobulin-like cell-cell adhesion molecules, nectin-2 and nectin-3, asymmetrically localize at the Sertoli cell side and at the spermatid side of Sertoli-spermatid junctions, respectively. They heterophilically trans-interact to make contact between the two cells. Nectin-2(-/-) mice have shown male-specific infertility, disrupted Sertoli-spermatid junctions and morphologically impaired spermatid development. Here we report testicular phenotypes of nectin-3(-/-) mice exhibiting male-specific infertility. Nectin-3(-/-) mice had defects in the later steps of sperm morphogenesis including distorted nuclei and abnormal distribution of mitochondria, as well as in localization of nectin-2 at the Sertoli-spermatid junctions. Transplantation of wild-type spermatogenic stem cells into the nectin-3(-/-) testes partially rescued these defects in sperm morphogenesis. These results indicate that the heterophilic trans-interaction between nectin-2 and nectin-3 is essential for the formation and maintenance of Sertoli-spermatid junctions that plays a critical role in spermatid development.

Free autogenous grafts in eyelid tarsoconjunctival reconstruction.

Classical techniques of reconstruction of large eyelid defects require temporary occlusion of the eye. Use of a free autogenous tarsoconjunctival graft with a skin-muscle flap is described. Twelve patients were operated with ten good, two fair, and no poor results. This method can be used to repair medial, lateral, and central defects of the upper and lower eyelids.

Compound pharyngeal myocutaneous flap.

Rehabilitation of major palatal ablative defects associated with lateral composite resection of the tonsil, base of tongue, and/or hemimandible is a difficult problem. Palatal reconstruction is described with emphasis on the compound medially based posterior pharyngeal, pectoralis major myocutaneous flap, which we have found to be an effective and reliable means of reconstructing these defects.

Gradual changes of influenza virions during passage of undiluted material.

Defective influenza virions undergo gradual changes during passage of undiluted material: the amount of RNA in virions diminishes, the equimolarity of the RNA segments is lost and the interfering activity of defective virions decreases. Defective virions of early passages have a small deficiency in genetic material and at a high m.o.i. induce the synthesis of the complete set of virus-specific proteins. Defective virions from later passages are characterized by a considerable deficiency in genetic material and a decrease in the amount of high mol. wt. RNA segments. They do not demonstrate complementation and at a high m.o.i. do not induce the synthesis of virus-specific proteins, thus revealing analogous defects in the genome of the majority of defective virions. The characteristics of protein synthesis in cells infected with these virions are similar to those in uninfected cells. During undiluted passage of influenza virus the cyclic production of defective and infectious virions takes place.

[Repair of soft tissue defect at finger-tips by cross-arm skin flap with lateral ante-branchial cutaneous nerve].

OBJECTIVE: To summarize the application of cross-arm skin flaps with lateral antebranchial cutaneous nerve in repair of soft tissue defect. METHODS: From March 1996 to March 2001, 37 cases of soft tissue defect at fingertips were repaired by cross-arm skin flaps with lateral antebranchial cutaneous nerve, 1.5 cm x 1.5 cm to 3.5 cm x 4.0 cm in size. All of the cases were followed up for 3-48 months with routine evaluation of the wound and the function of hands. RESULTS: All of the flaps survived and the wound achieved primary healing. The sensation and shape of hands recovered well. CONCLUSION: It's a good choice to repair soft tissue defect at fingertips by cross-arm skin flaps with lateral antebranchial cutaneous nerve.

[Interrupted aortic arch--report of two cases].

UNLABELLED: Interrupted aortic arch is a rare congenital heart malformation frequently associated with other extra or intracardiac congenital defects. These anomalies are divided into three types, depending on the site of aortic interruption. The diagnosis may be suspected clinically in patients with diminished femoral pulses and when the partial pressure and saturation of oxygen in the blood from arteries proximal to the interruption are higher than those from arteries distal to the interruption. The characters of heart murmurs are not of help in the diagnosis, which can be confirmed only by an ascending aortogram. SURGICAL TREATMENT: A two-stage approach has been routinely used in newborns. This consists of initial repair of the aortic interruption with left subclavian arteries (type A) or with a prosthetic tube (type B, C) and concomitant closure of the proximal end of the ductus by ligation or suturing, to be followed by closure of the intracardiac defect at a later date. Correction of the interrupted aortic arch and intracardiac malformation in elder children can be done at one sitting.

Lupus carditis: evaluation with technetium-99m MIBI myocardial SPECT and heart rate variability.

The objective of this paper was to investigate the incidence of myocardial perfusion defects in patients with systemic lupus erythematosus (SLE) associated with dysautonomic alterations. Twenty patients without any sign or symptoms of heart disease, selected from a larger population of patients with SLE, underwent technetium-99m sestamibi (Tc-99m MIBI) single photon emission computed tomography (SPECT), at rest and after dipyridamole infusion; they also underwent heart rate variability (HRV) examination by a 24 hour ambulatory electrocardiography, analyzing in the time domain the standard deviation of the R-R intervals average (SDNN) and the percentage of R-R adjacent intervals differing from each other more than 50 msec (pNN50); in the frequency domain the low (LF) and high frequencies (HF) were analyzed. Twenty healthy volunteers served as control group for heart rate variability. At MIBI-SPECT examination, the scan was found abnormal in 15 patients and normal in five: three patients demonstrated reversible defects in the anteroseptal region, four had irreversible defects in a region (two in the anteroseptal region and two in the lateral region), two had rest defects in two different regions (lateral and inferior, lateral and anteroseptal) that improved during dipyridamole scan, and six had both reversible and irreversible defects: four in a single segment (three anteroseptal and one lateral, and two in two different regions, particularly anteroseptal and lateral, lateral and inferior). All 20 patients showed significantly lower HRV parameters in comparison with the control group, except for pNN50, which indicates decreased physiologic periodic fluctuations of the autonomic nervous system. In six patients who underwent coronary angiography, the epicardial vessels were found completely normal. In view of the high incidence of myocardial hypoperfusion in patients with HRV alterations, the authors hypothesize that autonomic dysfunction may be associated with microvascular disease or metabolic alteration. They also believe that MIBI scintigraphy is a suitable technique in detecting myocardial damage in SLE patients free of clinical manifestation.

The lateral thoracic fasciocutaneous island flap for treatment of recurrent hidradenitis axillaris suppurativa and other axillary skin defects.

A series of eight axillary skin defect reconstructions in seven patients using the lateral thoracic island fasciocutaneous flap is presented. The defects originated from wide excision of recurrent hidradenitis axillaris suppurativa and in one case from radical melanoma resection with axillary lymph node clearance. The technique used to cover the resulting large defects is advancement or transposition of an island flap from the lateral thoracic wall, pedicled on two or three nourishing vessels arising from the lateral thoracic or thoracodorsal vessels. No flap complications occurred and the results with respect to donor site morbidity, functional and aesthetic outcome were very satisfactory.
