[The usefulness of virtual endoscopy and CT urography in the diagnosis of upper urinary tract tumors].

OBJECTIVES: Upper urinary tract (UUT) tumors are a relatively low incidence pathology which often represents a diagnostic challenge for the urologist, because several imaging tests are usually necessary to achieve the final diagnosis. Over the last years CT scan has suffered a notable development and nowadays, with the arrival of new image processing hardware and software, it is possible to detect small urothelial lesions and to perform 3-D urinary tract reconstruction and virtual endoscopic navigation. METHODS: Between January 2004 and June 2005 we performed a total of 15 examinations in 15 patients for the study of asymptomatic hematuria of probable upper urinary tract origin. The technique included three phases to obtain images: basal phase, vascular phase and excretory phase. Processing and image analysis was performed by one radiologist. RESULTS: A total of 10 tumors were diagnosed, with pathological confirmation in all cases. We did not register any complication secondary to the technique, and all patients tolerated well the exam. CONCLUSIONS: The CT urography is a safe, well-tolerated, highly sensitive and efficient imaging test, very useful in the diagnosis of renal and UUT pathology. It may become the only imaging test performed in the study of asymptomatic hematuria and the test of choice for the study of urological tumoral pathology.

[Initial clinical experiences with RARE-MR urography].

Potentials and limitations of a new, facultative non-tomographic Fast-Imaging sequence "RARE-MR-Urography" are presented. Its results in urological diseases are discussed on the basis of a pilot study of 35 patients. The images (4 s-32 s acquisition time) are extremely T2-weighted and display unbounded free water (urine). They reveal valuable information, not available with ultrasound or conventional radiology without application of contrast agents in the presence of urine retention. S/N problems reduce the accuracy of this method if the urinary tract contains only small amounts of urine.

Zonography: method of choice in urography.

Using zonograms in urography instead of the ordinary films and tomograms makes it possible to visualize the caliceal system as well as the renal contours with 1--2 exposures and limit the total number of exposures to 3--4 and thus markedly reduce the gonad radiation dosis.

[Diuretic isotopic urography. A method for its diagnostic use in nephro-urologic pathology in children].

In clinical pediatric urology distinction between obstructive and non obstructive dilatation of the upper urinary tract represents a difficult task for which usual procedures, both radioisotopes and conventional radiologic methods, have not discriminatory value. The aim of this investigation in to suggest a method of study that contributes to establish, in children, true diagnosis in clinical situations of hydronephrosis and to study evolution of renal function in surgical patients. Suggested technique, known an isotopic diuretic urography (UID), consists in intravenous administration of a diuretic (Furosemide, 0.4 mg/kg body weight) thirty minutes after injection of a radiotracer (I-123 Hippuran and or Tc-99-DTPA) into the vascular space and recording and analysis of variations of activity in parenchyma and renal pelvis during, at least, the following 60 m. Method described has been used in a series of 24 patients suffering from uni or bilateral hydronephrosis, with ages ranging from 2 months to 12 years. Results obtained, have shown that UID is notably superior to actually existing non-invasive methods of study of obstructive uropathy.

Diagnostic significance of excretory urography and ultrasonography in renal diseases.

A comparative study of intravenous urography (IVU) and ultrasonography (USG) was performed in 100 patients of all age groups, having symptoms and signs of various renal diseases. Sonography was found to be 100% sensitive and diagnostic in approximately 74% of renal pathologies included in this study, namely calculous disease (excluding midureteric calculi), obstructive lesions of pelvicalyceal system (PCS), cystic diseases, gross congenital lesions, traumatic lesions and neoplasms. It also provided information about the complications of long standing obstruction to PCS such as non-functional and atrophied kidneys, infection, etc. IVU proved better in midureteric calculi (100%), 88.2% of all congenital lesions including the major as well as minor variations. Both modalities proved to be of poor help in diffuse parenchymal diseases (IVU-40% and USG-50%) and renal tuberculosis (IVU-18.2% and USG-45.5%). Sonography was able to delineate the cause of obstruction to PCS in only 72.3% of cases as compared to 88% by IVU.

[Ultrasonography and intravenous urography in the diagnosis of blunt renal trauma].

OBJECTIVE: To analyze the sensitivity of renal ultrasonography and intravenous urography in renal trauma. METHODS: An extensive analysis on the sensitivity of IVP and US in the assessment of renal trauma was conducted using CT, renal selective arteriography or surgical exploration as the gold standard tests of reference to validate the initial diagnosis. RESULTS: IVP and US are useful techniques because of their high sensitivity in the diagnosis of renal trauma. The sensitivity of US decreases as the severity of the trauma increases (75% for grade I, 88.9% for grade II, 73.3% for grade III and 0% for grade IV), while that of IVP remains high for all degrees of severity (100% for grade I, 92% for grade II, 100% for grade III and 100% for grade IV). CONCLUSIONS: IVP and US were found to be useful techniques because of their high sensitivity in the diagnosis of blunt renal trauma. The sensitivity of US decreases as the severity of the trauma increases, while that of IVP remains high for all degrees of severity.
