Molecular diagnosis and epidemiology of fungal infections.

A variety of methods are utilized for DNA strain subtyping of Candida spp. because no 'gold standard' exists. Random amplified polymorphic DNA (RAPD) or restriction enzyme analysis (REA) are useful to determine the source of an outbreak, but more reproducible and discriminatory methods such as Southern hybridization and pulsed field gel electrophoresis (PFGE) may be required. When applied to some nosocomial Candida infections, multiple strains and species have been identified. Microevolution of yeast species occurs and epidemiologically related isolates may show minor pattern differences, creating uncertainty as to whether they are distinct strains. Approximately 1000 isolates of Aspergillus fumigatus from environmental and clinical sources were typed by REA probed with an A. fumigatus-specific retrotransposon-like sequence. Patients with no symptom of aspergillosis may carry several strains, whereas patients with pulmonary aspergillosis may carry one or two strains; nocosomial transmission of aspergillosis was proven in 39% of the patients studied; any given environmental strain can be infectious; the environmental population of A. fumigatus is extremely diverse and no specific niche was found in the hospital. A PCR assay was designed to target conserved 18S-ribosomal DNA (rDNA) sequences shared by most fungi and a 687 bp product was amplified from 25 medically important fungal species. Studies with blood, cerebrospinal fluid and sputum specimens from patients with mycoses indicated that the PCR assay is more sensitive in diagnosing invasive fungal infections than blood culture methods. More specific identification is obtainable with genus/species-specif c probes designed from within the PCR-amplified sequences for C. albicans, C. krusei, C. lusitaniae, Pneumocystis carinii, Cryptococcus neoformans, Aspergillus/Penicillium spp. and C. glabrata/Saccharomyces cerevisiae. A. fumigatus and A. niger were differentiated by denaturing gradient gel electrophoresis. In situ hybridization (ISH) detected a 648 bp fragment of the 18S rDNA of C. neoformans and a 568 bp fragment of the alkaline proteinase gene of A. fumigatus in tissues from experimentally infected animals. In ISH, the entire process can be automated, making this procedure rapid and easy. The difficulty in establishing a diagnosis of invasive candidiasis has prompted the quest for a clinically useful PCR test for candidaemia. The universal fungal oligonucleotide primer pair, ITS3 and ITS4, amplifies portions of the 5.8S ad 28S rDNA subunits, and the ITS2 region. Although rRNA genes are highly conserved, the ITS regions are distinctive. DNA probes were designed from ITS2 that were specific for 16 different Candida species. Simple, rapid sample preparation was suitable for PCR analysis of BacT/Alert blood culture bottles. Sample preparation, PCR, and EIA detection of the amplicon from five different Candida species was accomplished in 7 h, 2.5 days sooner than by conventional culture methods. As well as saving time, minor yeast species among a major species, or among bacteria, were simultaneously detected. PCR-EIA using a microtitration plate format had sensitivity 10-times greater than that obtained with ethidium bromide-stained agarose gels. Taqman combines in one step PCR, probe hybridization, and fluorescent signal generation. Taqman PCR had sensitivity equivalent to PCR-EIA and required only 5 h, including sample preparation.

Changes in the FDP (fibrin and fibrinogen degradation products) value under mental and physical stress. II. Simultaneous observations of FDP and other indices of fatigue in persons under experimentally loaded stress.

We assumed the possibility of adopting FDP as an index of fatigue and carried out examinations upon various load conditions causing stress. For our observations other indices which are thought effective for fatigue evaluation, such as urine CA, flicker values, response time, Uchida's Kraepelin test, and examination of subjective symptoms recommended by the Japan Association of Industrial Health, were adopted at the same time. 1) Both mental and physical loads increased FDP values both in serum and in urine. 2) These changes in FDP values agreed comparatively well with the changes in CA by load. 3) The FDP value showed changes by load which was so light that it did not induce functional deterioration detectable by the response time or the flicker value which was considered to be one of effective physiological indices. 4) The extent or degree of subjective symptoms found by the survey agreed well with the changes in FDP values. Through above examinations, FDP values of serum and urine seem to be useful as an index of transient fatigue; especially they can well reflect mental fatigue which cannot be indicated well by other biochemical indices. Also FDP can well show individual characteristics, namely FDP indicates comparatively satisfactorily both individual sensitivity and specificity. Furthermore, this method does not require special techniques and examinations can be done simply.

The alpha and gamma subunit-dependent effects of local anesthetics on recombinant GABA(A) receptors.

Although convulsions due to local anesthetic systemic toxicity are thought to be due to inhibition of GABA(A) receptor-linked currents in the central nervous system, the mechanism of action remains unclear. We therefore examined the effects of local anesthetics on gamma-aminobutyric acid (GABA)-induced currents using recombinant GABA(A) receptors with specific combinations of subunits. Murine GABA(A) receptors were expressed by injection of cRNAs encoding each subunit into Xenopus oocytes. The effects of local anesthetics (lidocaine, bupivacaine, procaine and tetracaine) on GABA-induced currents of receptors expressing different subunit combinations (alpha1beta2, alpha1beta2gamma2s, alpha4beta2gamma2s and beta2) were examined via the two electrode voltage clamp method. At alpha1beta2, alpha1beta2gamma2s and alpha4beta2gamma2s GABA(A) receptors, all local anesthetics inhibited GABA-induced currents in a dose-dependent manner. The presence of the gamma2s subunit resulted in a greater inhibition by all local anesthetics, but the presence of the alpha4 subunit resulted in less inhibition. At beta2 homomeric receptors, local anesthetics directly induced an outward current similar to that of picrotoxin. These data indicated that (1) the alpha and gamma subunits of GABA(A) receptors modulated the inhibitory effects of local anesthetics on GABA(A) function, and (2) local anesthetics can activate the beta2 subunit and may block the GABA(A) receptor channel pore.

[Effects of open-heart surgery on renal function in patients with chronic renal failure--is hemodialysis during cardiopulmonary bypass really required?].

In 300 consecutive adult patients who underwent open-heart surgery in our department, 16 patients (ischemic heart disease in 8 patients, valvular heart disease in 7 and congenital heart disease in 1) were preoperatively complicated with chronic renal failure (CRF); creatinine clearance (Ccr) < 40 ml/min and serum creatinine (Scr) > 1.6 mg/dl. The effects of open-heart surgery on renal function were studied in these CRF patients who were divided into the following 3 groups according to their preoperative Ccr values: Group 1 (6 patients), 30 < Ccr < 40 ml/min; Group 2 (5 patients), 20 < Ccr < 30 ml/min; and Group 3 (5 patients, 4 of whom were on dialysis preoperatively), Ccr < 10 ml/min. In addition, Group C (38 patients, Ccr > 50 ml/min) was set up as normal controls. Instead of hemodialysis, the extracorporeal ultrafiltration method (ECUM) was employed for all patients during the cardiopulmonary bypass (CPB). The Ccr in Group 1 showed the lowest value of 24.2 +/- 12.0 ml/min on postoperative day (POD) 0 which then recovered to the preoperative level on POD 1. This quick recovery of the Ccr in Group 1 was similar to that in Group C. In contrast, the Ccr in Group 2 showed the lowest value of 13.0 +/- 6.0 ml/min on POD 1, followed by a delayed recovery that did not reach the preoperative level until POD 5. Improvement of T2-weighted images of central nervous system by motion artifact suppression technique (MAST).

Standard spin echo sequences of MRI with and without motion artifact suppression technique (MAST) were compared in 45 paired images of brain and 21 paired images of spine. The images were obtained on a Picker 1.5 T system with TR = 2 sec and TE = 100 or 120 msec. MAST is a method of refocusing transverse magnetization at echo time by modification of the gradient waveforms. Transverse, coronal, and sagittal planes of brain were compared in 10 paired images obtained from adults and five paired images from children. The spine was studied in sagittal images, 11 cervical images of adults, five lumbar images of adults, and five lumbar images of children. MAST was proven to be valuable not only for the detection of lesions but also for picturing anatomical details as in MR-cisternography and MR-myelography and the effects of iron ions. MAST improves significantly the quality of T2-weighted images, especially in children.

Percutaneous pulmonary angioscopy using a guiding balloon catheter.

The applicability of angioscopy for observation of the pulmonary arterial lumen was examined in 7 anesthetized dogs and in 10 patients with various heart diseases during cardiac catheterization. A novel guiding balloon catheter (#11F shaft diameter and 1.5 cm in balloon diameter) was introduced through the right femoral vein into the pulmonary artery by Seldinger's method. Then, a fiberscope (1.3 or 1.8 mm in diameter) was introduced through the guiding catheter into the desired portion of the artery. The balloon was inflated, saline was infused, and the pulmonary arterial lumen was photographed on 16-mm color cinefilms. In all dogs and patients examined, an increase in diameter of the pulmonary artery during systole and the backflow of blood from the pulmonary vein were clearly demonstrated. Also, pulmonary thrombi or emboli experimentally induced in dogs could be observed. No complications were observed in the patients. The results indicate the experimental and clinical applicability of angioscopy using a guiding balloon catheter for observation of the pulmonary arterial lumen.

Capillary column high-performance liquid chromatographic-electrospray ionization triple-stage quadrupole mass spectrometric analysis of proteins separated by two-dimensional polyacrylamide gel electrophoresis. Application to cerebellar protein mapping.

A method is presented for the structural characterization of proteins separated by two-dimensional polyacrylamide gel electrophoresis (2D-PAGE). The method includes separation of a protein mixture by 2D-PAGE, recovery of proteins from the gel spots revealed by copper staining and analysis of the proteins by triple-stage quadrupole mass spectrometry using an electrospray ionization interface (ESI-TSQMS). Prior to the mass spectrometric analysis, the extracted proteins were passed through a small reversed-phase column (10 x 4.0 mm I.D.) to remove salts and gel-derived contaminants and then introduced into the mass spectrometer through a reversed-phase capillary column with 0.25 mm I.D. Application of the method to the analysis of rat cerebellar proteins suggests that the molecular mass could be accurately determined with sub-picomole amounts of protein samples derived from one or two 2D gels. The method was also useful for peptide mapping and determination of amino acid sequences of proteins micro-prepared from the 2D gel. Because 2D-PAGE has an excellent resolving power in protein separation and because capillary LC-ESI-TSQMS provides structural information with very small amounts of samples, the combined system of 2D-PAGE and capillary LC-ESI-TSQMS described here should allow wide applications to molecular studies of genes and proteins, such as identifications of protein spots on 2D gels, confirmation of gene/protein sequences and analysis of post-translational modification of proteins present naturally in tissue/cell extracts or expressed by recombinant DNA techniques.

The incidence of deep vein thrombosis after hip and knee arthroplasties in Japanese patients: a prospective study.

PURPOSE: To document the incidence of proximal deep vein thrombosis and pulmonary embolism in 58 consecutive Japanese patients undergoing total hip arthroplasty or total knee arthroplasty. METHODS: Patients were routinely examined for proximal deep vein thrombosis by B-mode ultrasonography before and after surgery. Those patients who had ultrasonographic findings of deep vein thrombosis were also investigated for pulmonary embolism by ventilation-perfusion lung scan. RESULTS: The incidence of deep vein thrombosis after total hip arthroplasty and total knee arthroplasty were 9.1% and 4.0% respectively, and the incidence of pulmonary embolism were 3.0% and 0%, respectively. There were no cases of fatal pulmonary embolism. CONCLUSION: The incidence of deep vein thrombosis and pulmonary embolism in Japanese patients may have increased over the last few decades.

Rapid detection of male-specific DNA sequence in bovine embryos using fluorescence in situ hybridization.

An accurate, reliable, and quick (less than an hour) method for determining the sex of bovine embryos was developed using a fluorescence in situ hybridization (FISH), with a probe designed from a bovine Y chromosome specific DNA (BC1.2). First, to improve a protocol of FISH and evaluate an accuracy of the method, lymphocyte nuclei prepared from three bulls, two cows, and one freemartin were tested. We found that 5 min was enough for hybridization. The washing solution adequate for posthybridization was 0.5x SSC at 72 degrees C for 5 min. The whole procedure for FISH can be accomplished in less than an hour. A male-specific signal was detected, on average, as 97, 0.5, and 83%, respectively, of lymphocytes in males, females, and a freemartin. Using the rapid FISH protocol developed, 28 embryos were divided. According to the presence of the digoxigenin signal, 16 embryos (57.1%) were predicted as male, and 12 embryos (42.9%), predicted as female.

Chronopharmacology for deoxyspergualin: toxicity and efficacy in the rat.

BACKGROUND: Deoxyspergualin (DSG), a potent immunosuppressive drug, has been clinically used as a rescue therapy for acute graft rejection. However, DSG has severe adverse effects that limits its use. Here, we examined the optimized therapeutic protocol for DSG using chronopharmacological profiles of it. METHODS: First, a dosing time-dependent difference in the toxicity and efficacy of DSG was determined using a heterotopic heart transplantation model. Second, chronokinetics of DSG were examined to reveal the mechanism for dosing time-dependent differences in the effect. RESULTS: In rats treated with DSG at the active period, bone marrow suppression and damage of small intestine were significantly severe. However, no significant difference was found in cardiac allograft survival and pharmacokinetics according to the timing of drug administration. CONCLUSIONS: The toxicity of DSG varied with the dosing time, whereas its efficacy did not. The chronopharmacological approach may provide merits for immunosuppressive therapy with DSG in clinical organ transplantation.

[Extended left atriotomy by temporary division of the superior vena cava].

The extended left atriotomy which combine the standard approach with the superior approach by concomitant temporary division of the superior vena cava is a safe and useful method especially in the case of small atrium or in the case of having anatomically abnormal situation of the left atrium. The technical detail was introduced here, and it is a modification of that reported by Selle and Kyger. We used this technique in three cases. The first was corrected transposition of the great arteries which required prosthetic replacement of the left sided atrioventricular valve. The second and third cases were mitral valve insufficiency with small left atrium which were performed mitral annuloplasty and caspal repair. All three patients have done well without SVC stenosis and perioperative morbidity like arrhythmia or bleeding.

Preventing ultraviolet light lip injury: beachgoer awareness about lip cancer risk factors and lip protection behavior.

BACKGROUND: The damaging effects of ultraviolet light (UVL) on the skin and the importance of using photoprotective sunscreen and other sun-protective measures are well recognized by beachgoers. However, it is currently unknown if UVL is well recognized as a modifiable risk factor for lip cancer and if photoprotective lipblock and sunscreen are used to the same extent in this setting. OBJECTIVE: Evaluation of UVL lip protection and knowledge about the role of UVL as a risk factor for lip cancer in a high-risk outdoor environment. METHOD: Anonymous questionnaire given to a convenience sample of beachgoers on Galveston Island, Texas, beaches during the summer. RESULTS: There were 299 survey responses. Subjects using sunscreen to protect their skin were also more likely to use UVL lip protection (p = .0002). Excluding the use of cosmetic lipsticks, women were still more likely to use UVL lip protection than men (p = .0008). Tobacco users were less likely to use lip protection than nontobacco users (p = .03). Survey respondents demonstrated much greater knowledge about UVL damage to skin than about the risk factors for lip cancer (p < .0001). CONCLUSION: More public education focused on UVL lip protection from UVL is needed.

Satigrel, a new antiplatelet agent, inhibits platelet accumulation in prosthetic arterial grafts.

BACKGROUND: Early and late vascular obstruction are both related to platelet adhesion and aggregation in the grafts. We assessed the effect of satigrel, a new oral antiplatelet agent, on the accumulation of indium-labeled platelets in knitted Dacron grafts inserted proximal to the femoral artery. METHODS: Nine patients with arteriosclerosis obliterans receiving grafts were treated with satigrel, and 10 others were enrolled as untreated controls. Scintigraphy was performed in postoperative weeks 2 and 4, and the ratio of the scintillation count of the graft to that of the native artery was calculated to assess platelet accumulation. RESULTS: In both weeks 2 and 4, the ratio was significantly smaller in the satigrel group than in the control group for the whole graft, the proximal anastomosis, and the distal anastomosis. CONCLUSIONS: Satigrel significantly inhibited platelet accumulation in vascular grafts and thus may be useful for preventing postoperative graft occlusion.

Identification of Warthin tumor: magnetic resonance imaging versus salivary scintigraphy with technetium-99m pertechnetate.

OBJECTIVE: The aim of this study was to evaluate the accuracy of technetium-99m (Tc-99m) pertechnetate scintigraphy and magnetic resonance (MR) imaging in the diagnosis of Warthin tumor. METHODS: Sixteen cases of Warthin tumor and 17 cases of non-Warthin tumor were examined by Tc-99m pertechnetate scintigraphy with lemon juice stimulation and MR imaging, including T1-weighted, T2-weighted, short inversion time inversion recovery, diffusion-weighted, and contrast-enhanced dynamic images. We used the receiver operating characteristic (ROC) curve to evaluate diagnostic accuracy. RESULTS: The mean area under the ROC curves of MR imaging in the diagnosis of Warthin tumor (0.97) was higher than that of Tc-99m pertechnetate scintigraphy (0.88). CONCLUSION: Magnetic resonance imaging is more useful in the evaluation of Warthin tumor than Tc-99m pertechnetate scintigraphy.

Preparation and characterization of enteric microspheres containing bovine insulin by a w/o/w emulsion solvent evaporation method.

The objective of this study was to produce enteric microspheres containing bovine insulin as a model drug using a water-in-oil-in-water (w/o/w) emulsion solvent evaporation method, and the preparative conditions were optimized. When hydroxypropylmethylcellulose acetate succinate (AS-HG type; high content of succinyl group) was employed as an enteric wall material, optimized microspheres showed almost 90% of the loading efficiency of insulin and 30.8 microns of mean volume diameter. The mixture of methylene chloride and acetone (4:1) as an oleaginous phase, 400 microliters of bovine insulin solution (dissolved in 30% of acetic acid) as an internal aqueous phase, and 1.0% of polyvinylalcohol dissolved in pH 3.0 citrate buffer as an external aqueous phase, were employed in the experiment. In relation to other enteric cellulose derivatives (AS-MG type, AS-LG type; medium and low content of succinyl group, respectively), the microencapsulation using a simultaneous preparation method also resulted in quite high loading efficiencies, whereas the choice of poly(methyl methacrylate) as a wall material caused aggregation or flocculation in the preparative process of every batch. The AS-HG microspheres showed very fast release profile in pH 6.8 buffer, but no released fraction was observed in pH 1.2 buffer. This phenomenon suggested enteric characteristics of prepared microspheres. Finally AS-HG microspheres containing 4% lauric acid and 9% insulin were prepared, suspended in 0.1% of carboxymethyl cellulose solution, and administered to the rat rectum (corresponding to 50 I.U./kg insulin). The plasma glucose level reached minimum level at 0.5 h after administration then gradually rose to normal.

Seprafilm does not aggravate intraperitoneal septic conditions or evoke systemic inflammatory response.

PURPOSE: To determine whether Seprafilm aggravates the systemic inflammatory response and adversely affects the outcome of postoperative intraperitoneal septic conditions. METHODS: We retrospectively analyzed the relationship between the intraperitoneal placement of Seprafilm and the rate of intraperitoneal septic complications in 278 consecutive patients. Experimentally, 40 rats were subjected to laparotomy with or without the intraperitoneal placement of Seprafilm. Bacterial peritonitis was induced by cecal ligation and puncture (CLP) and the serum cytokine levels were measured. RESULTS: Seprafilm did not increase the rate of septic complications or aggravate inflammatory responses in patients with or without postoperative intraperitoneal inflammatory complications. Experimentally, there was no significant difference between the serum inflammatory cytokine levels after CLP with or without Seprafilm. CONCLUSIONS: Seprafilm did not adversely affect postoperative inflammatory response or clinical outcomes, even in patients with intraperitoneal septic complications.

Direct measurement of phagosomal reactive oxygen by luminol-binding microspheres.

A new method, utilizing microsphere-bound luminol, which makes possible the direct measurement of highly reactive oxygen within phagosomes, was studied. When Freund's complete adjuvant-elicited mouse peritoneal macrophages and luminol-binding microspheres were mixed, the microspheres were engulfed in macrophages and enclosed in phagosomes, where chemiluminescence (CL) was generated, showing the generation of highly reactive oxygen. The reactive oxygen could be quantitatively assayed by measuring the intensity of CL. The addition of cytochalasin B inhibited the CL. CL production by the thioglycollate-elicited macrophages was found to be only a ninth of that by Freund's complete adjuvant-elicited macrophages, though the phagocytic activities were almost equivalent in both cases.

Changes of hepatic microsomal oxidative drug metabolizing enzymes in chronic renal failure (CRF) rats by partial nephrectomy.

Male SD rats, 7-weeks-old, were used to investigate the changes in the hepatic drug metabolizing system of chronic renal failure (CRF) model rats. Partial nephrectomy (5/6) was performed in a two-stage surgical procedure. After nephrectomy, the rats were housed under regular conditions at least 21 days. After confirming the CRF states, trimethadione (TMO, 100 mg/kg, i.p.) was administered for evaluation of the hepatic drug metabolizing capacity; the ratio of dimethadione (DMO: the only metabolite of TMO) to TMO (DMO/TMO) in the serum and the dialysate from the blood microdialysis method were ascertained. The hepatic drug metabolizing enzyme contents and activities were also determined. In the CRF rats, the DMO/TMO ratios decreased significantly; total cytochrome P450 (CYP) contents, aminopyrine N-demethylase activity and delta-aminolevulinic acid synthetase activity also decreased significantly in the CRF rats. The extent of the alterations of these enzyme contents and activities correlated well with the severity of the CRF states evaluated by the serum blood urea nitrogen and creatinine concentrations. With Western blot analysis, the levels of CYP2C6, CYP2C11 and CYP3A2 decreased considerably in the CRF rats. These results suggest that CRF states induce not only a reduction of renal function but also an alteration of hepatic metabolism.
