[Teratoma in the middle ear and throat of an infant].

The authors present a case of an 8-moth-old female infant treated for prolonged otitis media. The inflammatory process was connected with teratoma of tympanum and throat. The authors discuss presumable connection of the both masses. The child was completely cured by surgical treatment.

Erosive-throat with rash (scarlet fever), an investigation on its brief history.

Being a kind of epidemic contagious disease in modern time, erosive-throat with rash was a fatal disease causing numerous victims. High attention was paid to by traditional medical professionals through various fields, theoretical and clinical. A correct conclusion was reached claiming that TCM did make contributions to its prevention and treatment before effective treatment in western medicine appeared in China.

Acute sore throat--diagnosis and treatment in general practice.

This study tested how general practitioners diagnose streptococcal infection on clinical grounds alone, in patients who presented with sore throats.Four hundred and fifty-two patients were admitted to the study. A clinical diagnosis, prognosis and follow-up was completed in each case and the clinical assessment was checked by throat swabbing at first contact and a week later.The doctors were inaccurate in predicting streptococcal infection, but better than might be expected if prediction were a matter of pure guesswork. Colds and influenza implied negative prediction, tonsillitis a positive prediction, and pharyngitis was doubtful.In this series negative prediction for pharyngitis was 85.2 per cent and positive prediction 31.5 per cent accurate. The equivalent figures for tonsillitis were 61.5 per cent and 38.9 per cent respectively. There was a general tendency to overpredict streptococcal infection which was most marked in acute follicular tonsillitis, but this led to few false negatives. The tendency to overpredict streptococci was most marked when the patient was an adolescent female.There were differences between the urban and rural patterns. During the same period, influenza (and similar illnesses) was recorded less often in the country, whereas urban practitioners were more likely to predict streptococcal infection. Rural practitioners were more accurate in prediction because they were less prone to implicate streptococcal infection than their urban colleagues; there was a higher proportion of cases with proven streptococcal infection in the town and there is a disproportionately high number of adolescent females among the urban patients.

Incidence of anaerobes in throat infections and their sensitivity pattern in Ludhiana.

Of 175 throat swabs processed, anaerobes were isolated from 16 (9.14%) patients. Isolation of anaerobes from healthy controls was 2 out of 25 (8%). Peptostreptococci and Bacteroides species were the commonest isolates followed by Peptococci and Propioni-bacterium. All of these isolates were sensitive to Metronidazole. Clindamycin, Erythromycin and Tetracycline also showed good response.

Tolerability of ibuprofen, aspirin and paracetamol for the treatment of cold and flu symptoms and sore throat pain.

This double-blind randomised study compared the tolerability of ibuprofen (up to 1.2 g daily), aspirin and paracetamol (both up to 3 g daily) for up to seven days, in patients with mild to moderate pain resulting from cold/flu symptoms or sore throat (CF/ST) (n = 2,815). The main outcome was the rate of significant adverse events (SGAE). Rates of SGAE for ibuprofen, aspirin and paracetamol were respectively 12.0%, 15.7% and 12.3%. Ibuprofen was significantly better tolerated than aspirin (p = 0.02) and had comparable tolerability with paracetamol. The latter was also true for total digestive system events and for abdominal pain and dyspepsia. In conclusion, in patients with CF/ST, ibuprofen used at over-the-counter doses is as well tolerated as paracetamol and much better tolerated than aspirin.

Beliefs and perceptions of ear, nose and throat-related conditions among residents of a traditional community in Ibadan, Nigeria.

ENT-related conditions constitute a major burden of disease in Nigeria. The bulk of the available studies on these conditions are derived from hospital-based studies that underestimate the extent of the problem. Few studies have explored the perceptions of community residents on the causes and treatment of these conditions. This article describes findings of a qualitative study that explored the perception of residents of Foko, a traditional community in Ibadan, Nigeria. Five traditional healers were interviewed in-depth while four focus group discussions were conducted for men and women to determine perception of types, causation and treatment modalities for ENT-related conditions. The healers mentioned seven ENT-related conditions including ear discharge, deafness, epistaxis, nasal catarrh, sore throat, neck swelling and hoarseness. Informants and discussants attributed mainly spiritual attack as the cause of many of these conditions. Many of the treatment remedies used are topical, including use of effirin leaf (Occimum gratissium) for nose bleeding and using cold red oil or honey to treat ear discharge. These procedures are dangerous and could aggravate the conditions. We discuss the implications of these findings for further research and clinical practice.

Detection of hypermethylated RIZ1 gene in primary tumor, mouth, and throat rinsing fluid, nasopharyngeal swab, and peripheral blood of nasopharyngeal carcinoma patient.

PURPOSE: The aims of this study are to examine the methylation status of the Fn2 RIZ1 gene in nasopharyngeal (NP) tumors and nasopharyngeal carcinoma (NPC) cell lines and to evaluate the clinical value of methylated RIZ1 gene in body fluid samples of NPC patients. EXPERIMENTAL DESIGN: Methylation status of RIZ1 was evaluated by MSP on CNE-2 and M1 cell lines, 30 tumor biopsies, and their matched body fluid samples, including mouth and throat (M & T) rinsing fluid, NP swabs, plasma, and buffy coat. Normal controls included 8 normal NP biopsies and body fluid samples from 29 healthy volunteers. Sequencing was performed on MSP products from one NP tumor and one M & T rinsing fluid. Transcription of the RIZ1 gene before and after 5-aza-2'-deoxycytidine treatment was examined on CNE-2. RESULTS: The methylated RIZ1 gene was detected in both CNE-2 and M1, 18 (60%) NP tumors, but not in any of the normal controls. Of 30 matched body fluid samples, methylated RIZ1 DNA was found in 11 (37%) NP swabs, 9 (30%) M & T rinsing fluid, 7 (23%) plasma, and 3 (10%) buffy coat samples. Sequencing analysis confirmed all cytosines to uracils conversion, excluding cytosines in CpG dinucleotides in methylated PCR products. Promoter methylation correlated with loss of RIZ1 mRNA expression, and 5-aza-2'-deoxycytidine treatment restored its expression in CNE-2. CONCLUSION: Our results suggest that promoter hypermethylation of the RIZ1 gene is commonly found in NPC. Its detection in body fluid samples of NPC patients but not in normal controls indicates that it is worth to further evaluate its clinical application in assisting screening of NPC and monitoring recurrence after treatment.

Nose and throat: occupational hazards.

Recent reports on the effects of occupational factors in disorders of the nose and throat are reviewed. Chemical irritation is usually responsible for immediate effects, which are transitory but unwelcome. Allergic responses may be more troublesome. The more insidious carcinogenic properties of some industrial irritants are now being recognized.

[A randomized controlled study on the efficacy of modified sclerotherapy in treating angioma of ear, nose and throat].

OBJECTIVE: To evaluate the efficacy and security of modified and traditional sclerotherapy in treating angioma in ENT. METHOD: For the Angioma of derma and hypodermis (naso face, auricles, neck) 56 cases and 82 cases of mucosa and hypomucosa (the cavities of nose, throat and mouth) were randomized divided into 2 groups each.: The 5% Sodium Morrhate + Triamcinolone Acetonide (The Modified Sclerotherapy n = 28, n = 41) and only 5% Sodium Morrhate (Traditional Sclerotherapy n = 28, n = 41) were respectively injected into bodies of Angioma, as one time a week for 2-6 weeks, on average 19 days. RESULT: The rates of cure and effective for Modified and Traditional Sclerotherapies were 84.1%, 65.2% and 91.3%, 85.5% respectively. There is a significant in statistics for effective rates and there is a notable difference in cured rates between the two therapies. It shows that Modified Sclerotherapy has an obvious advantage (P < 0.05). Although the efficacy is similar between the two therapies for angioma of mucosa and hypomucosa, the difference has a significant in efficacy for angioma of derma and hypoderma between the two therapies (P < 0.05) and the side-effects were very slight for Modified Sclerotherapy. CONCLUSION: There is a more prominent efficacy and security for Modified Sclerotherapy than Traditional Sclerotherapy.

[A "hairy polyp" of the throat causes larynx occlusion in a newborn (author's transl)].

A case of "polyp with hair" in the throat is reported. It's occurence is extremely seldom. It is the simplest form of an epignathus. The birth defect is considered as an uncomplete dual formation, it is more often found in female than in male newborns. The left palate is the preferred place of occurence. Because it is anchored with a pedicle, the tumor may change it's position resulting in an occlusion of the larynx, as it was in this case. Recurrence after surgery is not known.

Acute epiglottitis in adults: a potentially lethal cause of sore throat.

The diagnosis, management and outcome in 12 adults with acute epiglottitis was reviewed. Painful dysphagia was a universal symptom and respiratory distress affected eight patients, six of whom required urgent airway intervention. All patients received parenteral antibiotics, ten received steroids and four received adrenaline. Respiratory distress resolved in two patients given adrenaline and airway intervention was avoided. Indirect laryngoscopy is the investigation of choice and this is preferable to neck radiology. Two patients died and it is stressed that this condition must be distinguished from other more common causes of a severe sore throat. The patient should be managed in a unit with the facilities and expertise to effect acute airway intervention.

Incidence of poliomyelitis; the effect of tonsillectomy and other operations on the nose and throat.

A statistical survey was made of all the cases of poliomyelitis occurring in all of Los Angeles County during the three years of 1949, 1950 and 1951 in an attempt to determine the effect of operations on the nose and throat on the incidence of poliomyelitis. Tonsillectomy and adenoidectomy were the only operations noted with any degree of frequency. Yet, in the total of 3,601 cases of poliomyelitis that occurred in this three-year period there were only 20 (0.55 per cent) in which the patient had had recent tonsillectomy and adenoidectomy. The incidence of this disease in patients who had had tonsillectomy and adenoidectomy was compared with the "expected" incidence as determined from the incidence in other patients, in the same age group. There was no significant difference between actual and expected incidence even during the summer months when most cases of poliomyelitis occurred. The same was true with regard to recently tonsillectomized patients in the epidemic months of July through October. In a separate survey of 675 patients with poliomyelitis, it was noted that only 30 per cent ever had had tonsillectomy or adenoidectomy. Inasmuch as it is estimated that one of every three persons in the general young population nowadays has had tonsillectomy and adenoidectomy, this figure is no more or less than could be expected.

Streptococcal throat carriage in school children with special reference to seasonal incidence.

A number of studies on throat carriage of beta hemolytic streptococci (BHS) carried out during the years 1972-90 in urban and rural school children from low socioeconomic groups in the age group of 5-15 years in and around Delhi showed an overall carriage rate of BHS varying from 12.2%-64.3% depending upon the season and number of swabs taken. Group A was found to be the most predominant serological group (31.1%-62.6%). The T-typability was found to be 98.2%. The most prevalent T-patterns observed during 1972-78 study were 3/13/B3264 followed by 5/11/12/27/44. A significant difference was observed in the prevalence of T-patterns during the study of 2,034 children from 1979-83 and 3,094 children from 1984-90. When the most prevalent T-patterns were found to be 5/11/12/27/44 followed by 3/13/B3264. The study of the school children from 1972-90 showed the isolation of BHS as well as significant predominance of GAS (p < 0.001) in winter months than summer months. There was no difference in the distribution of carriage of BHS and GAS amongst rural or urban school children. Since RF/RHD are illnesses which were often encountered in school children among socially and economically disadvantaged populations stronger support for streptococcal surveillance programs should be encouraged.

[Mineral oil pneumonia caused by throat gargles containing lipoid paraffin. Diagnosed by biochemical examination of the sputum (author's transl)].

A 20-year-old man developed a pneumonia after using throat gargles containing lipoid paraffin over 5 years. The lung biopsy showed a lipoid pneumonia and the bodyplethysmographic examination a ventilation disorder indicating a starting lung fibrosis. By the means of gaschromatography the identity of the lipoid in the sputum and in the gargle sample was established and the diagnosis continued. The disease, its pathophysiology and iatrogenic etiology are discussed and the consequencies concerning the ENT-specialist are stressed.
