80 patients with recurrent carcinomas of the oropharynx, hypopharynx and supraglottis were examined. Average recurrency occured after 15,1 months. 10 patients showed first and 3 patients second recurrent carcinomas on the mucous membranes of the upper respiratory tract. All the recurrencies were located distant from the primary tumor site, within the irradiation field and most of the times contralaterally. Lymphatic vessel damage and inversion of the regular lymphatic drainage are discussed for their etiological role.

The factors which influenced the prognosis of 306 supraglotic carcinomas were studied. The determinate survival at 3 and 5 years was 7.8% and 57.5% respectively. The lymph node metastases and the presence of recurrence affected significantly the survival. Also showed a worse prognosis those with an advanced local stage, and the patients with excessive alcoholic consumption. Without significative prognostic influence were the grade of tumor differentiation, involvement of surgical margins, previous tracheostomy, and the existence of a second primary tumour.

Glottic problems complicating a subjacent tracheal or subglottic stenosis may result from various causes, such as glottic closure due to fixation or paralysis of vocal cords, fibrous stenosing scars of the glottis, or instability of arytenoid cartilages. Resecting scar tissues, molding the closed glottis, and stenting a patent laryngeal lumen until consolidation are the basic means which, when associated in selected indications, allow recovery of respiration and speech through the normal routes.
