[Use of Betaxolol in patients with glaucoma and chronic nonspecific diseases of the respiratory system].

Betaxolol is a new antiglaucomatous drug which is blocking solely the beta 1 receptors and in contrast to Timolol may be applied in persons with spastic conditions of the bronchi. It has been given twice daily to 9 persons with glaucoma and long standing bronchial asthma. In the period of observation (1.5-6.5 months) neither of the examined patients showed any deterioration of the spirometric indexes nor bradycardia. In a part of the patients the intraocular pressure was normalized only by application of Betaxolol; over one half of them however needed additionally the use of miotics.

Posterior synechiae after laser iridectomy.

Eighty-six eyes of 46 patients were studied retrospectively to determine the incidence of posterior synechiae after laser iridectomy. All eyes were dilated and studied with a slit lamp. The incidence of permanent posterior synechiae in the eyes in this study (37%) is much higher than that previously reported. There was a strong positive correlation between the use of miotic agents postoperatively and the finding of posterior synechiae. The probable pathogenesis of this complication is discussed, and methods to decrease the incidence of this complication are given.

[Working ability of patients with various forms and stages of glaucoma].

Authors evaluated the questionnaries filled by patients with glaucoma of different type treated on the Prague Second Eye Clinic. These were completed by clinical data as state of visual functions and modes and course of the treatment. Different types of glaucoma were present: open-angle glaucoma (74), angular glaucoma (34), pigmentary glaucoma (12), exfoliative glaucoma (4), congenital glaucoma (13). Operations were performed in 26.5% of eyes, laser trabeculoplasty 18.8% and laser irodotomy 15.7%, and pharmacotherapy was applied in remaining patients. Ocular disease was the cause of the change of employment in 11.6%, the total disability pension and the partial one, were acknowledged in 3.6% and, 6.6%, resp. On contrary to the previous data, the number of patients with working disability was significantly decreased in comparison with the previous data. This fact could be explained by the use of the non-miotic antiglaucomatics, laser surgery, microsurgical operations and more precise diagnostics of the earlier stages of glaucoma.

[In vitro research on the interaction of natural zeolites with diploid cells from the human embryonic lung].

A study of the biological effect of the natural zeolites, clinoptilolite, mordenite and erionite on monostratum cellular culture of human embryonic lung is performed. The number of pyknotic cells, morphological changes in the cells, and monostratum, mitotic activity and changes in the cellular culture are reported during a continuous experiment. As positive control is used standard quartz dust DQ-12 and for negative dust of TiO2. It is already established that the dust particles of natural zeolites with dimensions under 5 micron display biological aggressiveness. With most strongly expressed effect is the clinoptilolite dust, which related to identical active surface, is comparable to that of quartz DQ-12. The mordenite is with moderate cytotoxic effect, proportional to the time and concentration, while erionite is with slight and very delayed effect. There are differences concerning the effect on the miotic activity of the cells. The biologic aggressiveness of the zeolites is discussed in relation to their strong active surface, their ionometabolic properties and their capacity to generate hydrogen at moistering.

Pharmacological effects of the antianxiety compound suriclone and its principal metabolites.

The pharmacodynamic effects of 4-methyl-1-piperazinecarboxylic acid ester with (+/-)-6-(7-chloro-1,8-naphthyridin-2-yl)-2,3,6,7-tetrahydro-7-h ydr oxy-5H-p- dithiino[2,3-c]pyrrol-5-one (suriclone, RP-31264) and its principal metabolites M1 and M2 on respiration, cardiovascular system, autonomic nervous system, smooth muscle and other physiological parameters were investigated in various animal species. Suriclone, 1 mg/kg i.v., increased the amplitude of respiratory movement, decreased the respiratory rate and blood pressure and increased the heart rate in conscious rabbits. The respiratory and depressor effects were more evident in pentobarbital anesthetized rabbits. In anesthetized dogs, suriclone, 0.05 or 0.5 mg/kg i.v., produced essentially the same effects as seen in the anesthetized rabbits. The ECG pattern was not significantly changed in any animal. Such effects on respiration and on the cardiovascular system of metabolites M1 and M2 in the rabbits were weak. In the isolated guinea-pig atria, suriclone, 10(-6) g/ml, had no effect but increased contractility and decreased heart rate at a high concentration of 10(-5) g/ml. Both M1 and M2 had weak effects. Suriclone had no action on flow rate of the perfusate through the blood vessels of the isolated rabbit ear. In anesthetized dogs, suriclone 0.5 mg/kg i.v., did not affect the responses to vagal stimulation or to pre- and postganglionic stimulation of cardiac ganglion. Suriclone instilled onto the eye or i.v. had no appreciable effect on pupillary diameter or the miotic response in rabbits, but an abnormal oculogyration was evoked when the drug was given i.v. at 1 mg/kg. M1 or M2 had no such effect. Suriclone did not exert analgesic effects in mice.(ABSTRACT TRUNCATED AT 250 WORDS)

Factors affecting the normal perimetric profile derived by automated static threshold LED perimetry. I. Pupil size.

The relationship between pupil size and perimetric sensitivity was investigated in a group of clinically normal emmetropic subjects using the Dicon AP3000 computer-assisted perimeter at bowl luminances of 10 asb and 45 asb. Pupil size was modified with the miotic thymoxamine 0.5% and the mydriatic phenylephrine 10%; saline was used as the control. Perimetric sensitivity increased with an increase in pupil size. The effect was greatest for peripheral locations for both bowl luminances. The fluctuations in perimetric sensitivity increased with peripheral angle and with decrease in pupil size for both bowl luminances.

[Calmodulin and neuron: immunohistochemical studies].

Calmodulin is present in higher concentrations in brain tissues. The content rapidly increased during the 2nd postnatal week in rat brain. Although the protein is ubiquitous in all eukaryotic cells, immunohistochemical studies have revealed that calmodulin is mainly localized in the neurons, exhibiting a similar distribution to that of gamma-type neuron-specific enolase. In the mouse retina, both calmodulin and gamma-enolase were found to be localized in optic nerves, ganglion cells, and inner and outer plexiform layers. The development study showed that gamma-enolase increased in the 2nd postnatal week and that the levels of calmodulin did not significantly change in that stage. In the mouse retina with an inherited retinal dysplasia (C3H), in which all the photoreceptor cells degenerate during the 2nd and 3rd postnatal weeks, calmodulin-specific staining decreased in the residual layers. Calmodulin is also enriched in mammalian testes. In the mouse testis, levels of calmodulin were high in the spermatocytes and in the spermatids, as compared to the level in spermatozoa. This suggests that the large amount of calmodulin in the testis may be associated with miotic divisions and/or spermatogenesis. Immunocytochemical staining of calmodulin in C6 glioma cells and PC12 pheochromocytoma cells showed a high level of calmodulin to be localized on the half spindles between poles and chromosomes in mitotic cells. The protein was also shown to be localized on fibrous structures in the interphase of those cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of prostaglandin D2 and its analogues on intraocular pressure in rabbits.

Studies were carried out of the effects of topically applied prostaglandin (PG) D2 and its metabolites and analogues on the intraocular pressure (IOP) in rabbits, and the results were compared with those of studies using PGE2 and F2 alpha. The application of PGD2 (0.4-250 micrograms) reduced the IOP, in a dose-dependent manner without causing a hypertensive phase. The hypotensive effect was observed within 30 minutes after the application and lasted for over 7 hours. Higher doses of PGE2 (10, 50 micrograms) or PGF2 alpha (50 micrograms) caused initial IOP elevation followed by a prolonged hypotensive phase. Lower doses of PGF2 alpha (2, 10 micrograms) caused a prolonged (over 7 hours) reduction in the IOP following a latency of over 2 hours. The IOP reduction by 2 micrograms of PGE2 lasted for 5 hours. No miotic response followed the use of these PG's. Conjunctival and iridal hyperemia, aqueous flare, irritation (defined by lid-closing), and aqueous protein content were examined at equi-hypotensive doses of the three PG's (50 micrograms for PGD2, 2 micrograms for PGE2, and 10 micrograms for PGF2 alpha). PGE2 was the strongest in causing these side effects, followed by PGF2 alpha. PGD2 did not cause any of these responses except for some development of conjunctival hyperemia. All of the 4 PGD2 metabolites were ineffective in reducing IOP. Among 6 PGD2 analogues, BW245C, PGD3, and PGD2 methyl ester more effectively reduced IOP than did PGD2. PGD1 and 16,16-dimethyl PGD2 were not effective.

[Regional graft vs host reaction to H-Y antigen (immunological mechanisms)].

Structural changes in the regional popliteal lymph nodes have been studied in C57Bl/6 male mice at the peak of the reaction "graft-versus-host" to H-Y antigen. Morphological and morphometrical investigations have been carried out in three groups of males (10 animals in each group). The first group includes intact animals (the first control group). To the males of the second group (the second control group) lymphoid cells are inoculated from intact C57Bl/6 females. To the males of the third group (experimental group) lymphoid cells are inoculated from H-Y antigen immunized C57Bl/6 females (anti-H-Y effector lymphocytes). The popliteal lymph nodes of the male mice from the third group twice increase in their size comparing to those in the control (the first and the second groups). Miotic activity increases in them 4.5 times, amount of cellular blast forms in medullary cords--4 times and 10 times--in the light zone of the cortical substance. Ratio of macrophages and eosinophils in structural components of the lymph nodes studied changes; this is, evidently, connected with massive destructive progresses, that take place in the lymph nodes of the animals from the third group. The results of the morphological investigations are in agreement with the hypothesis suggested, explaining the mechanism of development of the regional reaction "graft-versus-host" to H-Y antigen, basing on idiotype-antiidiotype interaction (the idiotypic network in the immune system).

Effects of brotizolam on smooth muscle and other organs.

Effects of brotizolam (2-bromo-4-(2-chlorophenyl)-9-methyl-6H-thieno[3,2-f]-1,2,4-triazolo [4,3-a]-1,4-diazepine, We 941, Lendormin) on smooth muscle and other biological systems were examined. In isolated intestinal preparations, brotizolam in higher concentrations shifted the concentration-contractile response curve after addition of acetylcholine down to the right. However, brotizolam did not affect the intestinal transport. Therefore, even if brotizolam possesses a non-specific inhibitory action on smooth muscle, it would be quite weak. Brotizolam, nitrazepam and estazolam in high doses showed a miotic action. Brotizolam had no effect on the digestive system, that is, gastric secretion, bile secretion and intestinal propulsive activity were not influenced. A depression of salivary secretion, which may be due to an additive action of Eidelberg's mixture, was observed. Brotizolam, nitrazepam and estazolam enhanced the sleeping time induced by ethanol and increased locomotor activity induced by methamphetamine, but did not affect the chewing behavior. There was no indication that continuous administration of brotizolam affected significantly the levels of lipids and sugar in the blood serum.

[Central alveolar hypoventilation as a complication of massive intracerebellar hemorrhage: report of a case treated by diaphragm pacing].

The author reported a case of central alveolar hypoventilation (Ondine's curse) which was treated by diaphragm pacing. A 59-year-old man was admitted because of sudden deep coma and tetraparesis. Neurological examination on admission showed miotic pupils with absent light reaction, no oculocephalic reflex, no corneal reflex and tetraparesis. Glasgow scale was evaluated to be E-1 V-1, and M-3 with total of 5 points. Computed tomography confirmed a large hematoma in the cerebellum with ventricular rupture. Immediately, a suboccipital craniectomy was performed and the hematoma was evacuated. His neurological condition was improved gradually within a month after the operation. He responded to verbal orders and moved all his limbs against gravity. His respiration, however, was irregular with a pattern of so-called "cluster breathing" followed by prolonged apnea, that was more pronounced in night. He was suffered from hypoxic hypercapnea and recurrent pneumonia. Therefore, authors decided to employ diaphragm pacing for management of central alveolar hypoventilation. A diaphragm pacemaker (radiofrequency induction) was implanted. A cuff electrode was put around the right phrenic nerve in the right thorax, and the receiver installed subcutaneously in the right anterior chest. Postoperative respiratory study showed ventilation on pacing with satisfactory blood gas and he became able to move around using a wheel chair. In 1966 Glenn demonstrated a new technique to move the diaphragm paced by a receiver through the phrenic nerve, triggered by radio wave from external device.(ABSTRACT TRUNCATED AT 250 WORDS)

[Chromosome abnormalities in male infertility].

On the basis of systematic cytogenetic studies carried out with men undergoing examination for infertility in the couple, it is now estimated that the incidence of chromosome anomalies is about 5 per cent. This rate is much higher than that observed in the general population (0.7 per cent). Anomalies in the sex chromosomes are the most frequently observed since they represent approximately 4 per cent of the cases, and the analysis of the various types of mutation shows a distinct predominance of the 47 chromosome composition, XXY, which is related to Klinefelter's syndrome. Autosome anomalies are also observed, notably Robertsonian and reciprocal translocations. They are more rare (about 1% of the cases), but the fact that they are linked with disturbances of spermatogenesis is an important element in understanding the control-mechanisms of this process. The occurrence in both man and animals of a non-random relationship between the forms of translocation and the X and Y chromosomes found in the seminal vesicle during the pachytene stage, suggests that this contact interferes with the normal process in which the X chromosome is inactivated. This interference could account for the observed deficits in spermatogenesis. The hypothesis is reinforced by the fact that female carriers of the same autosome translocations, whose two X chromosomes are normally active during the whole miotic prophase, have no fertility problems.

[Toxic and pharmacological characteristics of armin and nibufin homologs].

The experimental studies of alkylesters of phosphonic and phosphinic acids, close homologues of armine and nibufin, showed this series of compounds to have a strongly marked anticholinesterase action. All the substances produce an inhibitory effect on the central nervous system, raise the tone of the smooth muscles on an isolated strip of rabbit small intestine and induce a miotic pupil in the animals. Introduction of butyl radicals into molecules of phosphonic acid derivatives results in diminution of toxicity and a slight decrease in anticholinesterase activity. By their properties butyl paranitrophenyl esters of ethyl- and butylphosphonic acids are closely related to armine and nibufin.

[A case of spontaneous 3rd ventriculostomy (author's transl)].

We have reported a rare case of spontaneous 3rd ventriculostomy with spontaneous arrest of obstructive hydrocephalus. A 41 year old man, who had had an intermittent headache for about a year, was admitted to the department of neurosurgery Kitasato University with chief complaints of sudden onset of severe headache, vomiting and disturbance of consciousness. At the time of admission, 30 minutes after the onset of symptoms, the positive neurological findings were delirious state of consciousness, miotic pupils with sluggish reaction to light, mild hemiparesis on the left site and slight nucnal rigidity. He lapsed into coma after two hours, however he gradually relieved from these symptoms since the forth hospital day. Cerebrospinal fluid was bloody. Radiograms of the skull revealed decalcification of posterior clinoid process and postero-inferior displacement of pineal calcification. Brain scanning and vertebral angiography demonstrated tumor stain in the posterior portion of the 3rd ventricle. Dimer-X ventriculography revealed the obstruction of posterior portion of the 3rd ventricle and the leakage of Dimer-X through the floor of the 3rd ventricle into the intrasellar subarchnoid space. The patient died after about one year from the onset of symptoms. Any signs of increased intracranial pressure had not been noticed since the forth hospital day; At autopsy we confirmed the posterior portion of the 3rd ventricle was obstructed by tumor. In the floor of the 3rd ventricle there was a round opening which was patient and measured about 3 mm in diameter. Microscopic examination of the tumor showed an oligodendroglioma. Neoplastic cells partially infiltrated into the surface facing to the 3rd ventricle and slight gliosis was observed around the site of rupture. The surface along the subarachnoid space was lined with pia-aracnoid membrane except at the site of rupture. In the past literatures only 6 cases of spontaneous 3rd ventriculostomy have been reported. Three cases were observed spontaneous arrest of obstructive hydrocephalus. Our case is the first reported case of spontaneous 3rd ventriculostomy through the floor of the 3rd ventriculostomy through the floor of the 3rd ventricle. We suggested the pathogenesis of spontaneous 3rd ventriculostomy is a result of destruction at normally weak points of 3rd ventricle (ex. anterior, posterior wall and floor of 3rd ventricle), which has the reultant internal hydrocephalus caused by recurrent obstruction of C.S.F. pathway or long-standing obstructive hydrocephalus.

[Clinical aspects and therapy of the posner-schlossmann-syndrom (author's transl)].

UNLABELLED: Difficulties in the early diagnosis of the Posner-Schlossmann syndrome can be avoided by looking for the following typical clinical signs: Very fine, unpigmented precipitates, which are often only signly present, and which are scattered over the entire cornea: bright white in incident light, translucent in reflected light, and in transmitted light dark in spots. Posterior synechiae are not present. Obvious differences in color or heterochromia are present in only 30-40% of the cases. In the remaining cases, there ist only a slight color difference or unilateral, diffusely trophic hypochromia of the iris, often only after several attacks. The chamber angle is and remains open during the attacks. There are similarities between glaucomatocyclitic crisis and heterochromic cyclitis: Unilaterality, the same specific precipitates, no synechiae, and practically the same percentage of color differences of the iris. Hypochromic dystrophy of the iris dependent on the magnitude and duration of the cyclitic process. In both cases, there is the same rate of physical changes, pointing to a congenital damage of the sympathetic nervous system (status dysraphicus Passow). Hence, the Posner-Schlossmann syndrome can be regarded as a special case of heterochromic cyclitis. THERAPY: Neither miotics nor mydriatics, nor operation during the crisis. Acetazolamide (Diamox) combined with local cortisone will stop the crisis.

[Sympathomimetic and sympatholytic drugs in the therapy of glaucoma (author's transl)].

The basis of sympathetic impulse transmission are summarised. Sympathomimetics and sympatholytics are discussed with regard to their mode of action, clinical use and contra-indications. We found that a 1% solution of adrenaline twice a day together with miotics is most suitable for long-term treatment. With the beta-blockers Bupranolol and Timolol tried by us can a massive pressure reduction be obtained, but which is of short duration with the concentrations used up till now. Adrenergic potentiators can not be recommended.

[Persistence of lymphocytic choriomeningitis virus in a transplantable line of Detroit-6 cells].

Defective LCM virus was found to persist in Detroit-6 cells. The persisting virus was not infectious for susceptible mice and some cell cultures. Its antigenic properties were enhanced to detectable levels in the CFT by cocultivation with LSV5 cells only. No reversion to its pathogenic properties, however, occurred either after 8 subpassages of the cocultivated cells or after blind passages of these cells in suckling mouse brains. The cytogenetic analysis, and the pattern of antigen fluorescence (IIF) revealed similarities between HeLa and D-6 cell lines persistently infected with LCM virus. The highest intensity of LCM virus antigen coincided with the period of maximum miotic activity of D-6-LCM cells, indicating a correlation between synthesis processes in the cells and reproduction of the persisting virus. Upon superinfection of D-6-LCM cells with homo- and isologous viruses no increase in the percentage of the antigen-containing cells was observed. No detectable interferon was found in the D-6-6CM system.

Effects of dapiprazole on pupillary size and intraocular pressure in rabbits.

The effects of 3-[2-[4-(2-methylphenyl)-1- piperazinyl]ethyl]-5,6,7,8-tetrahydro-1,2,4-triazolo[4,3-a]pyridine HCl (dapiprazole), a new drug with alpha-adrenergic blocking properties, on pupillary diameter and intraocular pressure have been studied in rabbits. Following i.v. administration, a reduction of intraocular pressure is observed at doses devoid of activity on pupillary diameter. Following topical application, a miotic and ocular hypertensive action was observed at the same doses; the hypotensive action is produced in both normal rabbits and in rabbits with water load- or corticosteroid-induced ocular hypertension. Contact lenses increase the duration of effects of dapiprazole. These results suggest a potential interest of dapiprazole in glaucoma, with particular reference to the topical treatment of angle-closure glaucoma.
