Light and electron microscopies were employed to examine biopsy specimens from 15 children suffering from the primary nephrotic syndrome with minimal alterations in renal bodies. The studies were carried out at the height of the disease and in the stage of a clinical remission after prednisolone therapy. The author describes the development of ultrastructural changes in the glomerular filter, primarily of those in podocytes, under the conditions of highly selective proteinuria as well as during hormonal therapy that favours reparative processes and discontinuation of protein "leakage". It is assumed that the anionic layer of the glycocalyx of podocyte pedicles and l. rara ext. heparan sulfates of the basal membrane may play a role in the origin of selective proteinuria.

In 16 children with steroid-responsive nephrotic syndrome (with minimal changes in the glomeruli) the plasma renin activity and aldosterone level were determined during recurrence of the nephrotic syndrome at the stage of oedema increase (FEN alpha 0.25%) and in early period of remission. Plasma renin activity was raised in all cases suggesting presence of hypovolaemia. Since it was not possible to establish a correlation between plasma renin activity and albumin level, as well as between this activity and aldosterone level, and aldosterone level and sodium excretion it may be surmised that the renin-angiotensin-aldosterone system has no decisive role in the pathogenesis of the nephrotic syndrome. The mechanism of oedema development is doubtlessly more complex.
A catamnestic study was made of 188 patients suffering from the nephrotic syndrome with minimum changes (NSMC) in order to examine the characteristic features of its course and outcome. The patients' age ranged from 3 to 26 years, with the disease standing being from 1 to 22 years. The authors explored the anamnestic and clinico-laboratory data together with the morphological findings (in 6.9% of cases). Estimated the results of the treatment. Described the NSMC outcome in 188 children.

Changes in serum protein and albumin levels following resolution of proteinuria were studied in 67 children with nephrosis. These two parameters returned to normal only after the 21st day, i.e. well after the natriuretic crisis and resolution of edema. Our results are consistent with recent studies showing that the fluid and electrolyte changes seen in the urine at resolution of the proteinuria (increased diuresis and natriuresis) are not due to a return to normal of serum protein and albumin levels. They probably reflect correction of an intrarenal abnormality whose mechanism is unclear.

Classically, the histological lesion observed in a drug-or heavy metal-induced nephrotic syndrome is membranous glomerulonephritis. We report two cases of "toxic" nephrotic syndrome with unusual histological features. One was secondary to mercury intoxication and the other, to D-penicillamine in a patient with rheumatoid arthritis. In both cases, renal biopsy revealed minimal glomerular changes. The proteinuria rapidly disappeared after exposure to the toxic agent was discontinued. Genetic factors and a disregulation of the immune system with lymphokine production may be responsible for these renal changes. This study demonstrates that renal biopsy is necessary in this clinical setting.
