Blood characteristics of the crab-eating monkeys (Macaca fascicularis) in Bali Island, Indonesia: implication of water deficiency in West Bali.

Environmental effects on blood characteristics of macaques were examined by comparing several blood indices of the free-ranging crab-eating monkeys (Macaca fascicularis) in two distinct regions of Bali Island, Indonesia. The total plasma protein level and concentrations of creatinine and sodium ion were higher with macaques of Teluk Terima in the west part of Bali Island (dry area) than those in the central Bali (wet area). Results indicated that water deficiency in the dry area significantly affects the blood characteristics of macaques.

Correlation of population indices of female Culex tritaeniorhynchus with Japanese encephalitis viral activity in Kapuk, Indonesia.

Nineteen consecutive monthly light trap collections of mosquitoes were made between October 1978 and April 1980 in Kapuk, Indonesia. Kapuk is a small suburb of Jakarta where pigs are raised in close proximity to rice paddies which are breeding sites for Culex tritaeniorhynchus. Japanese encephalitis (JE) virus is believed to be endemic and has been recovered from mosquitoes and pigs in the area on several occasions. A total of 18,435 female Cx. tritaeniorhynchus were allocated to 359 pools of approximately 50 per pool. Virus isolations were attempted in both Vero and BHK-21 cells and agents producing cytopathic effect were identified in a micro-neutralization test. Nineteen strains of JE were recovered from the 359 pools of Cx. tritaeniorhynchus tested. The light trap index of female Cx. tritaeniorhynchus (X) and the relative frequency of pools positive for JE (Y) for each month of the study were plotted and correlation coefficients (r) calculated after transforming the mosquito population data logarithmically and the relative frequencies of isolation by arcsine square root. The close fit of the data (p less than 0.001) to an inverse linear model (1/y = a + b log10X) suggests a close dependence of JE viral activity on the population dynamics of Cx. tritaeniorhynchus. Three additional strains of JE were recovered from other Culex spp. at the same study site. One strain each was isolated from individual pools of Cx. gelidus, Cx. vishnui and Cx. fuscocephala. Cx. tritaeniorhynchus was more frequently infected with JE than the other species tested.

Filariasis and microfilariasis in parrots in the eastern islands of Indonesia.

From 1978 to 1983, 411 bird submissions consisting of 911 birds were received in our laboratory for diagnosis. Filariasis occurred in four submissions (0.97% of total) consisting of four birds (0.44% of total), and microfilariasis was diagnosed in 34 submissions (8.27% of total) consisting of 123 birds (13.5% of total). Adult filariae were found in the abdominal air sacs of two cockatoos and two red lories and identified as Cardiofilaria sp. Although filariasis and microfilariasis in birds are found world-wide, there seems to have been no report made previously in Indonesia.

Isoenzyme analysis of stocks of trypanosomes isolated from cattle in Indonesia.

Three trypanosome stocks isolated from cattle in Indonesia were shown to differ markedly from an Indonesian stock of Trypanosoma evansi on the basis of the isoenzyme banding patterns of 12 soluble enzymes. The results obtained for these stocks were not consistent with those reported for typical forms of T evansi but were very similar to enzyme patterns obtained for rodent adapted stocks of T vivax.

[Anesthesiologic treatment of 3,665 patients in Red Cross hospitals in Thailand, Lebanon, Pakistan and Indonesia].

Between the end of 1979 and the beginning of 1984, the authors served as anaesthetists for Red Cross missions in surgical field hospitals on the Thai-Cambodian and the Afghan-Pakistan borders, in Lebanon, and in Indonesia. A total of 3665 civilian emergency patients were anaesthetised. 643 were operated on under local anaesthesia; 639, under regional anaesthesia, in most cases spinal anaesthesia. In spite of principal preference for local and regional anaesthetic techniques, 65% of the patients (2383 patients) were managed under general anaesthesia, which was maintained with halothane in 947 cases and ketamine in 1345 cases. 877 general anaesthesias with ketamine were performed with spontaneous breathing of ambient air; endotracheal anaesthesia was necessary in 1238 patients. In spite of the high risk for the patients and of the operative interventions and in the light of the simple and sometimes even primitive working conditions, no anaesthesia-related fatalities occurred. Postoperative lethality was 2%. In all cases, the duties of the anaesthesist involved more than performance of anaesthesia, which was sometimes assigned to paramedics. Other duties included preoperative examination, postoperative intensive care, consultation services in nonsurgical emergency cases, resuscitation, and training of local assistants.

The impact of schistosomiasis mass treatment on hepato-splenomegaly in Napu Valley, Central Sulawesi, Indonesia.

The impact of mass treatment with praziquantel on 241 individuals, 131 males and 110 females in Napu Valley, Indonesia was studied. The pre-treatment overall stool positive cases ranged from 43% to 79%, the highest in the 20-29 age group. Post-treatment follow-up study showed that the cases dropped to 2.5%. Significant reduction was seen in liver rate from 68% to 51% and in the age groups 0-9, 10-19, there was a significant decrease in liver enlargement after treatment, but not significant differences were seen in spleen rate in all age groups. This might be due to the early stage of Schistosoma infection in younger age groups where complete resolution of fibrosis might occur.

Survey for soil-transmitted helminths in Asahan Regency, North Sumatra, Indonesia.

The survey on soil-transmitted helminthiasis was carried out in the three villages of Asahan Regency, North Sumatra, Indonesia, between October 1981 and December 1982. The prevalence rates of geohelminthiasis proved to be extremely high (average 97%) in the three villages and more than 70% harboured three or more helminths, especially Ascaris lumbricoides, Trichuris trichiura and hookworm. Necator americanus and Ancylostoma duodenale were detected in 61.1% and 52.2% of 835 fecal specimens, respectively. The effects of two anthelmintics, Combantrin and Trivexan, were essentially the same against A. lumbricoides and hookworm, although Trivexan was better than Combantrin against T. trichiura. The kinetic changes of parasitic infection in community after mass treatment were examined. Ten months after the first drug treatment, the incidence of Ascariasis or trichuriasis reverted nearly to pre-treatment level, while hookworm infection rate remained significantly low. Four months after the second mass treatment, the reinfection rate of A. lumbricoides was most prevalent followed by trichuriasis and hookworm infection. The reinfection rate (63.6%) of A. lumbricoides in children was about six times higher than that (10.4%) in adults at four month after the mass treatment. This study indicates that mass treatment with Trivexan at two month intervals for children and four month intervals for adults is necessary for the effective helminth control scheme in highly endemic areas of North Sumatra.

Virological surveillance for dengue haemorrhagic fever in Indonesia using the mosquito inoculation technique.

A dengue haemorrhagic fever surveillance system in Indonesia, based on virological and clinical observations, is described. The system uses the mosquito inoculation technique for virus isolation and is simple, economical, and well suited for endemic areas where support and facilities are limited. The data suggest that with good cooperation between the hospital and the virology laboratory, new serotypes and possibly even new strains of virus can be identified before the onset of epidemic activity. This type of virological surveillance may make it possible to prevent major epidemics in the future.

Epidemic dengue hemorrhagic fever in rural Indonesia. I. Virological and epidemiological studies.

Virological studies were carried out during an epidemic of dengue hemorrhagic fever in Central Java, Indonesia in 1976. Dengue virus was isolated from the acute sera of 45 of 69 patients (65%). The isolation rate was higher in primary than secondary cases. Dengue 3 was the predominant serotype being transmitted (27 isolates), but both dengue 1 (8 isolates) and dengue 4 (10 isolates) were also being transmitted. A composite picture of magnitude and duration of viremia showed that many patients were circulating over 10(8) MID50 per milliliter dengue 3 virus for the first 3 days of illness and that viremia persisted for 5-6 days in some persons. If all shock cases were considered, there was no relationship between dengue serotype and severity of disease. All three confirmed fatal cases, however, were associated with dengue type 3 infections.

Epidemic dengue hemorrhagic fever in rural Indonesia. II. Clinical studies.

Clinical observations were made on 95 serologically or virologically confirmed dengue fever cases during an epidemic in a rural area of Indonesia in December 1976. The age distribution was similar to that observed in patients with dengue hemorrhagic fever in Jakarta, a highly endemic urban area. The observed disease ranged in severity from undifferentiated fever to shock and death. The majority of patients had acute onset of fever with nausea, vomiting, headache, and abdominal pain. Hepatomegaly was observed in only 19% of the patients. A positive tourniquet test was the most frequently observed hemorrhagic manifestation, but epistaxis was observed in 20% and hematemesis in 6% of the patients. Dengue shock syndrome was observed in 37% of the patients. There were four deaths, three of which were confirmed as due to dengue infection by virus isolation. The data suggest that one, and possibly two, of the fatal cases with virus isolation were primary infections, based on the results of hemagglutination-inhibition test using all four dengue antigens.

Diarrhoea among infants in a crowded area of Djakarta, Indonesia. A longitudinal study from birth to two years.

Diarrhoeal diseases are common in Djakarta, Indonesia, especially among infants and young children. A study has been made of possible bacterial and parasitic causes of outbreaks in a group of 156 infants in a crowded area of the city. Before the study was complete, 60 infants had left the area and 30 had died; diarrhoea was probably the direct or indirect cause of 13 of the deaths.Diarrhoea was associated with pathogenic Escherichia coli in about 20% of the cases studied; other causes of diarrhoea were Shigella, less frequent, and rare among infants below the age of 6 months; Salmonella, insignificant; Giardia lamblia, common, but not usually associated with diarrhoea; Entamoeba histolytica and Isospora belli, relatively rare. The role of Trichuris trichiura was probably important, but was difficult to assess. Many diarrhoea cases were not associated with either pathogenic bacteria or parasites. Other potential causes, not considered in this study, include enteropathogenic virus infection, parenteral infections, faulty diet and malnutrition. Further investigation is considered desirable.

Intestinal and blood parasites in the Banggai Kabupaten, Central Sulawesi, Indonesia.

A survey for blood and intestinal parasites was carried out in the Banggai Kabupaten Central Sulawesi, Indonesia. A total of 336, stool specimens were obtained from 182 males and 154 females ranging in age from 2 to 90 years. Hookworm, Trichuris trichiura and Ascaris lumbricoides were the most common helminths encountered. Schistosoma japonicum was noticeably absent in the population sampled. The following intestingl protozoa were detected: Entamoeba coli, E.histolytica, Iodamoeba bütschlii, Giardia lamblia, Chilomostix mesnili and Endolimax nana. A total of 650 blood smears were examined and both Plasmodium falciparum and P. vivax were found in 5 % of the populations sampled. In addition, Brugia malayi microfilariae were detected in 3% of the blood smears examined.

Parasites of man in remote areas of Central and South Sulawesi, Indonesia.

A biomedical survey was conducted in 10 villages in remote, high mountain valleys of Central and South Sulawesi, Indonesia to learn whether Oriental schistosomiasis was endemic and to determine the prevalences of other intestinal parasites, malaria and filariasis in those areas. Although persons with Oriental schistosomiasis were found in three villages of South Sulawesi, follow-up inquiries revealed that these had recently migrated from a known schistosomiasis area in Central Sulawesi. Other intestinal parasites diagnosed were Ascaris lumbricoides (14%), Trichuris trichiura (7%), hookworm (68%), Entamoeba histolytica (3%), Entamoeba coli (17%), and Giardia lamblia (5%). Enterobius vermicularis, Strongyloides stercoralis, Trichostrongylus sp., Physaloptera sp., Diphyllobothrium sp. heterophyid, echinostome and dicrocoelid-like termatodes, Endolimax nana and Chilomastix mesnili were detected infrequently. Malaria parasitemias due to Plasmodium falciparum, P. vivax and P. malariae were detected in 4% of the sampled populations, Malayan filariasis was diagnosed in 21% of the subjects examined.

A study of malayan filariasis in Lubuk Mumpo and Datar Lebar villages in Lais Regency, North Bengkulu, Sumatera, Indonesia.

Filariasis surveys at Lubuk Mumpo and Datar Lebar, Bengkulu, Sumatera, showed microfilarial rates of 23.9% and 30% respectively. Periodicity studies showed the parasite to be nocturnally periodic B. malayi with a periodicity index of 82.17. However, unlike the Malaysian form of periodic B. malayi, 44-100% of the microfilariae in thick blood smears were sheathed. Mansonia annulata, M. bonneae, M. dives, M. uniformis and Anopheles nigerrimus are probably involved in the transmission of the disease. B. malayi infections were seen in 7.1% of domestic cats and 20% of Macaca fascicularis examined. In addition Edesonfilaria malayensis is reported for the first time in Indonesia, 60% of 5 M. fascicularis being infected.

Susceptibility of Neisseria gonorrhoeae isolates from Jakarta, Indonesia, to antibiotics.

The minimum inhibitory concentration (MIC) of penicillin, tetracycline and spectinomycin was determined for 6 beta-lactamase (PPNG) and 73 non beta-lactamase producing Neisseria gonorrhoeae isolates from Jakarta, Indonesia. All PPNG were resistant to greater than or equal to 128 micrograms/ml penicillin. Forty-six percent of the non-PPNG strains were inhibited by less than or equal to 1.0 microgram/ml of penicillin and 97% by less than or equal to 4 micrograms/ml. Most of the PPNG and non-PPNG isolates (90%) were inhibited by less than or equal to 4 micrograms/ml tetracycline and less than or equal to 16 micrograms/ml spectinomycin. Two non-PPNG strains were resistant to 64 micrograms/ml of tetracycline, 8 micrograms/ml penicillin and 32 micrograms/ml spectinomycin. The non-PPNG strains generally were more resistant to penicillin and tetracycline compared to strains tested from other parts of the world in previously reported studies.

Malaria control with residual fenitrothion in Central Java, Indonesia: an operational-scale trial using both full and selective coverage treatments.

An operational-scale trial, using residual fenitrothion, for control of malaria was carried out in Central Java, Indonesia, from 1980 to 1982. Two areas, each comprising about 70 km(2) and a population of about 50 000, were treated with fenitrothion (40% water dispersible powder) at a target dosage of 2 g/m(2) for 3 cycles at 6-monthly intervals. One area was treated with full coverage (i.e., the interiors of houses and cattle shelters were sprayed to a height of 3 m) for 2 cycles, followed by a third cycle with selective coverage (i.e., the interiors of houses were sprayed with one 75 cm horizontal swath between 10 cm and 85 cm from the floor while the cattle shelters were sprayed to a height of 3 m). The other area was treated for 3 cycles with only selective coverage. While both treatment methods reduced malaria rates and vector populations to very low levels, the full coverage treatment was more rapidly effective and also reduced the Plasmodium falciparum index. However, the selective coverage treatment was 68% less expensive than full coverage and greatly reduced the degree of cholinesterase depressions among the spraymen. The trial also showed that a dosage of 1 g/m(2) with full coverage was nearly as effective as the 2 g/m(2) dosage.

Hybridization between Brugia malayi and Brugia pahangi from south Kalimantan, Indonesia.

Hybridization experiments were carried out between sub-periodic Brugia malayi and B. pahangi recovered from cats originating from the same area in South Kalimantan (Borneo), Indonesia. Five to six week-old immature males and females of both species from Mongolian gerbil (Meriones unguiculatus) intraperitoneal infections were inoculated into the peritoneal cavity of other gerbils. Mating occurred between the species and microfilariae developed into infective third-stage larvae in mosquitoes. Larvae from the mosquitoes developed into adults in the peritoneal cavity of gerbils but microfilariae were not produced since the males were sterile. Back-crossing between hybrids and males and females of the parental species were also done and female hybrids produced microfilariae when crossed with parental males but females from the parental species did not produce microfilariae when mated with the hybrid males. This confirmed that the males were sterile but the females were not. If these species are hybridized in nature and humans are exposed to hybrid third-stage larvae, could this lead to occult filarial infection? These results confirmed previous hybridization experiments of these species orginating from other geographic areas.

Ecology of schistosomiasis in Indonesia with certain aspects of control.

Intensive studies on schistosomiasis in Indonesia were carried out since 1971. During this period studies on epidemiology of the disease, ecology of the intermediate host and pilot control trials were conducted. In 1975 a schistosomiasis pilot control measures in two endemic villages of Lindu Valley to develop rational and effective methods for schistosomiasis control under local conditions was initiated. The results showed some improvements of sanitation, snail habitats, decreased infection rates among the human population as well as among the rats and the snails. The increase in travel and better transportation, and development of transmigration programmes in recent years in the endemic areas cause some concern, and prevention of the spread of the disease was discussed.
