New bipolar diathermy forceps with automatic dripping and flushing--technical note.

A new bipolar diathermy forceps system was developed to solve the problems of constant, pressure-limited flow rate, and one-sided irrigation. A roller pump, activated synchronously by pressing a foot switch, feeds dripping and flushing solution to the target tissue via the tip at both ends of the forceps. This system is volume-limited. Continuous compression of the foot switch first activates the flushing function, which continues for less than 1 second, during which time bleeding spots can be detected. The flow then changes automatically to the dripping function to suppress tip burning and prevent damage to the surrounding tissues from heat and current leakage. Repeated pressing of the foot switch initiates the jet irrigation function (continuous high flow rates), allowing irrigation of hematomas and removal of excess debris.

Endocrine changes associated with a dietary-induced increase in ovulation rate (flushing) in gilts.

Effects of an increased level of dietary energy (flushing) on plasma concentrations of FSH, LH, insulin, progesterone and estradiol-17 beta and ovulation rate were studied in 16 gilts. Gilts received 5,400 kcal ME/d for one estrous cycle and the first 7 d of a second. On d 8 of the second estrous cycle, gilts received either 5,400 kcal ME/d (control [C], n = 8) or 11,000 kcal ME/d (flushed [F], n = 8) for the remainder of the estrous cycle. Blood was collected daily at 15-min intervals for 6 h from d 8 through estrus. Gilts were examined by laparotomy 6 d after estrus. Ovulation rate was greater (P less than .05) in F than C gilts (16.0 vs 9.4). Mean daily concentrations of FSH were greater (P less than .05) in F gilts at 5 d, 4 d and 3 d prior to estrus compared with C females. In both C and F gilts, FSH decreased (P less than .05) prior to estrus. Mean daily concentrations of LH and LH pulse amplitude were not different (P greater than .05) between treatments. Mean number of LH pulses/6 h at 4 d, 3 d and 2 d prior to estrus were greater (P less than .05) in F than in C gilts. In both treatments, LH pulse amplitude decreased (P less than .05) and pulse frequency increased (P less than .07) prior to estrus. Mean plasma concentrations of insulin tended to be higher (P less than .07) in F than in C females during the 7-d period before estrus.(ABSTRACT TRUNCATED AT 250 WORDS)

Organ flush out solutions and cold storage preservation solutions: effect on organ cooling and post ischemic erythrocyte trapping in kidney grafts. An experimental study in the rat.

Three flush out and cold storage solutions were tested for flow rate during flush out, ability to decrease organ temperature, degree of tissue edema and degree of preservation damage of the kidney as measured by the amount of post transplantation erythrocyte trapping in the renal medulla. The solutions tested were a modification of a new perfusion solution (University of Wisconsin; mUW), a standard preservation solution (Sacks') and an extracellular histidine solution (Frödin-Wolgast; FW). The flow rate was significantly higher for FW compared to mUW and Sacks' and consequently a more rapid decrease of organ temperature was achieved. The results also show that edema formation as well as the amount of erythrocyte trapping was lower in kidneys stored in mUW, indicating that mUW is better cold storage solution than Sacks' or FW.

Gendered personality dispositions, hormone values, and hot flushes during and after menopause.

The study aim was to test the relationship between gendered personality dispositions (GPD), sex hormones, and vasomotor complaints in the menopausal transition. Fifty-seven healthy women (mean age 51.1 years, standard deviation 2.0) were drawn from a population registry. At enrollment all women were menstruating regularly, and all women reached menopause in the course of the study. Questionnaire data and blood samples were collected once a year over a period of five years. GPD scores (based on the Bem Sex-Role Inventory, BSRI), frequencies of hot flushes and hormone data (estradiol, follicle-stimulating hormone, luteinizing hormone, testosterone, androstendione and dehydroepiandrostendione sulfate) were determined. Subjects were placed into four categories based on the BSRI: masculine, feminine, androgynous and undifferentiated. GPD did not change during the menopausal transition. Testosterone and androstendione were related to GPD in that testosterone and androstendione were higher in the sex typed categories (masculine and feminine) than in the non sex-typed categories (androgynous and undifferentiated). GPD are related both to androgens and to vasomotor complaints such as hot flushes during the menopausal transition, but the mechanisms of these relationships are not known.

Effect of estrogen replacement therapy on cardiac function in postmenopausal women with and without flushes.

Left ventricular heart function and its response to long-term estrogen replacement therapy was assessed in 30 postmenopausal women, 20 of whom had modest to severe hot flushes and 10 of whom had never had them. Continuous transdermal estradiol was given to women who had surgically induced menopause, and a combination of transdermal estradiol and sequential medroxyprogesterone acetate was given to those who had spontaneous menopause. Left ventricular systolic and diastolic function was evaluated by complete two-dimensional M-mode and pulsed Doppler echocardiography before and after 6 and 12 months of therapy. The parameters assessed were: systolic and diastolic blood pressure, heart rate, cardiac septal and posterior wall dimensions, left ventricular end-systolic and end-diastolic dimensions and volumes, ejection fraction (EF), ejection time, peak left ventricular outflow velocity (PFV), flow velocity integral (FVI), acceleration time (AT), mean acceleration of systolic flow (MA), duration of early and late filling phase, peak velocity of the early (E) and late (A) mitral flow, and A/E velocity ratio. Although no difference in chamber and wall dimensions between flushers and non-flushers was found, women with hot flushes had lower (not significantly) EF, PFV, FVI, MA, blood pressure and heart rate before therapy. Twelve-month estrogen replacement therapy significantly reduced cardiac wall dimensions and improved systolic function in both flushers and non-flushers. However, stroke volume, EF and MA were increased whereas systolic blood pressure and heart rate were decreased more in flushers. Also, the increase in E mitral flow and decrease in A/E were more pronounced in flushers. Thus, although estrogen replacement therapy significantly improves heart function in healthy postmenopausal women, there appears to be some minor differences in response between flushers and non-flushers.

A new intracellular flush solution improves renal transplant preservation.

A new renal preservation flush solution (PB-2) has been developed to minimize the ischemic injury processes that occur during hypothermic storage and reperfusion and that can decrease renal viability and survival. Development of the new formulation took into account the intracellular and biological interactions that occur pre- and post-transplantation. PB-2 was compared with the conventional standardized Collins-2 flush solution in the preservation of autografts in dogs and was found to provide significantly improved renal recovery, viability, and survival. Studies to test the new solution in human renal allograft transplant preservation are planned.

Flushing an i.v. line: a simple but potentially costly procedure for both patient and health unit.

This observational study investigated the intravenous (i.v.) medication administration technique of Registered Nurses (n = 47) using the injection port of a concurrent intravenous infusion. Study findings showed that the majority of nurses adhered to the most common although debatable recommended guideline in stopping the concurrent infusion and slowly administering the medication in the syringe over 3-5 minutes, prior to flushing the line quickly. Nurses failed to account for the dead space in the i.v. line when calculating the administration rate for pre and post medication flush. Similarly, the benefits of continuing the concurrent infusion when administering intravenous medication were not taken into consideration. Based upon these study findings, we recommend changes to procedural guidelines for i.v. medication.

A case of episodic flushing and organic psychosis: reversal by opiate antagonists.

A woman had episodic attacks of flushing associated with severe skin, bone, and abdominal pain, accompanied by mood alterations and anxiety, and followed by an organic psychosis. These symptoms and signs could be induced by small doses of clonidine, L-5-hydroxytryptophan, pentagastrin, insulin, epinephrine, compound 48/80, methacholine, morphine, histamine, or d-tubocurarine. Skin testing showed abnormally sensitive mast cells and an exaggerated axon flare. Cimetidine initially prevented the attacks but became less effective after 18 months. Thyrotropin-releasing hormone stopped the skin pain whereas neurotensin reproduced the burning sensation in the skin without inducing the attack. Both the flush and the organic psychosis were reversed entirely by naloxone or naltrexone, and the hallucinosis could be reversed by vasodilators.

Housing mice in a caging system with automatic flushing.

Male Har:(ICR)BR mice were housed 8 wk in wire bottom cages over paper, in wire bottom cages over an automatic cascade flushing system, or in solid-bottom plastic cages with bedding material. There were no substantial differences in general health or weight gain. Pentobarbital LD50 values were lower for the mice housed in wire bottom cages than for the mice in solid bottom cages with bedding. This difference was probably related to gastrointestinal content. It appears that the automatic cascade flushing system is suitable for housing mice for periods up to 8 wk.

High concentrations of CA 125 in uterine flushings: influence of cause of infertility and menstrual cycle day.

Uterine flushings were obtained under transvaginal ultrasonographic control from 132 women presenting for investigation and treatment of infertility. Levels of CA 125 were measured by radioimmunoassay and results expressed in relation to the total protein concentration of the same flushings. CA 125 was detected in uterine fluid at levels higher than those previously reported in peripheral blood. Uterine fluid CA 125 concentrations varied throughout the menstrual cycle, being highest in the mid-follicular phase (days 6 to 10). Uterine fluid CA 125 concentrations may reflect endometrial secretion of this protein more directly than serum levels. CA 125 concentrations did not vary according to the cause of infertility but further work in larger numbers of women is required.

Hydraulics of a sewer flushing gate.

This paper presents field experiments carried out in a 1.8 m height egg-shaped sewer in Lyon, France in order to contribute to the knowledge and the simulation of Hydrass flushing gates. The main results are: i) definition of an empirical relationship giving the flow discharged by the gate as a function of the upstream water level and its use in modelling the gate behaviour, and ii) observation of the flush propagation along the sewer and evaluation of the potential cleansing efficiency.

Screening of chemical oxidation processes and other methods for decolorization of urine for its re-use as toilet-flush liquid in ecological sanitation systems.

Because of its potential use as fertilizer, urine ("yellow water") is a resource originating from sanitation. Its separate collection in no-mix toilets is a beneficial aspect of ecological (source control) sanitation. In order to avoid dilution of the fertilizing nutrients with toilet flush water, the utilization of yellow water as toilet flush liquid seems to be advantageous. To be accepted for this purpose, urine has to be decolorized (and also deodorized). In this study activated carbon adsorption, irradiation with UV light of different wavelengths, the advanced oxidation processes ultrasound, UV/H2O2, and photocatalytic oxidation have failed to decolorize urine. Biological treatment caused brown colour of the treated urine. Only ozonation was successful in colour removal, although it did not affect TOC. In spite of darkening of yellow water during biological treatment (generation of humic substances), smaller ozone doses were required for decolorizing the biologically pre-treated urine than for original urine. Photocatalytic oxidation of biologically treated urine also removed brown colour, but the original yellow colour remained. In ozonated urine, yellow colour was reconstituted unless hydrogen peroxide was added. In addition to colour removal, ozone contributed to deodorization as a consequence of ammonia stripping and probably of phenol oxidation.

Pancreatic islets isolation using different protocols with in situ flushing and intraductal collagenase injection.

Human islet transplantation seems to be a very promising clinical procedure for patients with type I diabetes mellitus. The aim of our study was to investigate the influence of in situ intravascular flushing with University of Wisconsin (UW) solution and intraductal collagenase injection at the time of pancreas procurement on the isolated islets and exocrine tissue injury. Our experiments indicated that in situ perfusion with the UW solution has a beneficial effect on pancreatic islets and intraductal distention results in an increase in the concentration of pancreatic enzymes released into the cold preservation solution during ischemic conditions. Cold ischemia reduced islet yield, but pancreas perfusion with the UW solution showed better ischemic tolerance of isolated islets during glucose static incubation. We conclude that intravascular pancreas flushing has a crucial effect on recovery and yield of pancreatic islets and protects against exocrine tissue injury.

Angioscopy-guided semiclosed technique for in situ bypass with a novel flushing valvulotome: early results.

To allow and facilitate endoluminal vein preparation under angioscopic guidance for in situ bypass grafting, a Mill's valvulotome was equipped with a flushing channel. The advantages of the novel valvulotome are obvious: it allows perfect angioscopic view within a saphenous vein that still contained blood by adjusting the jet of lactated Ringer's solution directly to the lens of the angioscope; it permits the use of very small angioscopes without integrated flushing channels, preventing endothelial damage. The described technique was tested in 27 peripheral reconstructions in 26 patients. The secondary cumulative patency rate within a follow-up time of 4 to 30 months (mean, 14 months) was 89%. The described technique substantially facilitates in situ bypass grafting and minimizes early and late complications and further improves surgical outcome.

A new modeling approach for estimating first flush metal mass loading.

The purpose of this study was to investigate pollutant mass loading from major highways in Southern California, with emphasis on interpretation of event mean concentrations and first flush effects. The results of monitoring eight sites during the 1999-2002 storm seasons found that metal contaminants had higher concentrations at the early stages of storm events compared with other stages of rain storms. A new washoff model was developed to predict the event mean concentrations of metal contaminants taking first flush effect into account. Model variable parameters included average daily traffic, antecedent dry period, rain intensity, total runoff volume, and runoff coefficient. The results obtained using the washoff model were compared with measured values and found to fit well for heavy metals with R2 ranging from 0.8 to 0.95.

Modelling of flushing waves for optimising cleaning operations.

In the framework of a German field and numerical study the possibilities of an optimal cleaning by flushing of a combined sewer with storage capacity and bottom-end overflow were investigated. The objective of this combined investigation was to find out the boundaries for the flushing operation to prevent wave reflections and impoundages at the throttle by guaranteeing the necessary shear stress for remobilization.

[Seasonal influence on the microbiological colonization on surfaces of milk liners and flushing adapters in a newly installed milking parlor].

A field trial was conducted to evaluate long term and seasonal effects on the development of mictobiological colonization in a newly installed milking parlor. The milking equipment was a 2 x 34 side-by-side milking parlor in a commercial dairy herd in Germany. The 1st batch of samples was taken before beginning of milking and in weekly intervals for a period of two months in winter 2002. The 2nd batch of samples was collected in summer 2003. After sampling the diagnostic material was kept in Amies medium at 4 degrees C until analysis. Samples were analyzed from the mouthpiece, the shank of the liner and the flushing adapter. The samples were collected immediately after disinfection of the milking equipment. They were analyzed qualitatively and quantitatively according to the official German guidelines (Amtliche Sammlung von Untersuchungsverfahren, paragraph 35, LMBG). Mesophil aerobe total plate count, S. aureus, E. coli, Coliforms, Streptococci, Yeast and Lactobacilli were considered. Results indicate a relationship of microbiological colonization of the surface of the milking technique with season and location of sampling. The microbiological colonization was higher in summer than in winter. There were more mesophil aerobe pathogens on the flushing adapters than on mouthpiece and shank of the liner.

Idiopathic orthostatic cyclic edema as a unique etiology for vasomotor flushing in a normal estrogenic woman with normal day 3 follicle stimulating hormone--case report.

PURPOSE: To describe a unique cause and therapy for vasomotor flushing. METHODS: Serum follicle stimulating hormone (FSH) and estradiol, renal and liver function studies and urinalysis were performed as well as a water load test. RESULTS: All laboratory tests were normal but the water load test showed inadequate four-hour urine excretion when she was erect but normal when supine. Complete relief from vasomotor symptoms occurred shortly after treatment with sympathomimetic amines. CONCLUSIONS: Idiopathic orthostatic cyclic edema can cause vasomotor flushing even with normal estrogen and serum FSH, and treatment of the edema state with the drug of choice dextroamphetamine sulfate can effectively control these symptoms.
