The Angres Permalid-Liner method to enhance the result of cosmetic blepharoplasty.

The purpose of cosmetic blepharoplasty is the reduction of redundant soft tissue of the upper and lower eyelids, improvement due to wrinkling and relaxation of the lid skin, and eradication of the fullness and bulges in the lids. The Angres Permalid-Liner is a new microsurgical procedure that implants various earthtone color pigments to the base of the eyelashes. When done together with aesthetic blepharoplasty, it can dramatize the eyes by giving sharper definition to the eye shape, makes lashes look thicker, makes the eye look larger and amplifies the whites of the eyes. At the same time, it can also be used to eliminate dark circles around the eyes, blend in skin grafts, and mask unsightly blemishes.

Growth hormone investigation in patients with mental dysfunction.

Seventy-three children with disorders of physical growth and mental development were stimulated by insulin hypoglycemia and arginine infusion and their human growth hormone (HGH) responses were determined. Only four patients exhibited absent or impaired pituitary hormone secretion and one of them presented a unique syndrome of panhypopituitarism associated with intrauterine growth retardation, long eyelashes and eyebrows, sparse hair and degeneration of the retina. Statistical analysis indicated no significant HGH peak concentrations in groups of either tall males or stunted females who possessed various sex chromosome abnormalities, nor did these groups differ in a variety of clinical parameters including age, physical growth, birth weight and intelligence.

[The application of highly hydrophilic contact lenses in the Fuchs-Steven-Johnson syndrome (author's transl)].

In 3 patients with status after Fuchs-Steven-Johnson syndrome highly hydrophilic contact lenses (72% H2O-up take) were adapted and worn permanently. 2 patients had worn already for a long time usual HEMA-lenses. The lenses showed themselves to be favorable against mechanical irritation from cilia and accessory eyelashes and from scarring conjunctival conditions, and they affected favorably the signs of dessication of the cornea. Corneal defects healed, and the patients had subjectively no complaints. It is sometimes necessary to replace the tears. An increase in the corneal vascularisation already present could not be observed during the 18 months observation time.

Hereditary hypotrichosis. A previously undescribed syndrome.

A syndrome of hereditary hypotrichosis with an unusual natural history and clinical features was encountered in a Caucasian family of four generations with a total of twenty-four members of whom eleven were affected. The mode of inheritance was autosomal dominant with variable penetrance. The loss of hair involved the scalp, eyebrows, eyelashes and body hair, manifesting itself in the school years and progressing to almost complete baldness. There were no associated abnormalities and no sex limitation. Clinical, genetic, biochemical, mechanical, histological and immunological aspects were studied. Essential differences between this type of hereditary hypotrichosis and others previously recorded are stressed.

Hair changes due to zinc deficiency in a case of sucrose malabsorption.

A 7-year-old girl suffering from chronic diarrhoea due to sucrase deficiency was referred because of poor hair growth. Her scalp hair had a poor, colourless appearance and was much thinned in the occipital region. Her skin was dry, but otherwise normal. P-zinc was low (7.9 mumol/l), whereas P-albumin was normal. Oral zinc therapy, 40 mg daily, had a marked beneficial effect on her scalp hair, eyebrows and eyelashes, which became thicker and pigmented. Beau lines appeared on thumb-nails and 4th left finger-nail. A rise in P-zinc and S-alkaline phosphatase levels was observed during the zinc supplementation. Microscopic examination of her poor scalp hair, using polarized light, revealed well-defined abnormalities of the hair shafts, as reported by others in a case of acrodermatitis enteropathica: 1) a marked individual variation in diameter, 2) narrowing often associated with waving or sharp bending and broken ends, 3) striation with a tendency to trichonodosis. Such changes were absent in the pigmented hair appearing after the start of zinc therapy.

[Lucio-Latapí leprosy and the Lucio phenomenon].

The Lucio-Latapí's leprosy or diffuse lepromatous leprosy is a clinical variety of lepromatous leprosy first described by Lucio and Alvarado in 1852 and reidentified by Latapí in 1936. It is frequent in Mexico (23%) and in Costa Rica and very rare in other countries. It is characterized by a diffuse infiltration of all the skin which never is transformed into nodule, by a complete alopecia of eyebrows and eyelashes and body hair, by anhydrotic and dysesthesic zones of the skin and by a peculiar type of lepra reaction named Lucio's phenomenon or necrotic erythema which is a vascularitis of vessels especially of the dermohypodermic union and of the hypodermis. Clinically this vascularitis is represented by well-shaped erythematous spots, later becoming necrotic with scabs, ulcerations and scars. Three points of confusion are stressed: the differences between nodules and nudosities, Lucio's leprosy and Lucio's phenomenon and necrotic erythema and necrotic erythema nodosum leprosum. The differences between the pure and primitive form of Lucio's leprosy and the secondary one is also discussed such as the laboratory findings, histopathological data, pronostic and treatment. Lucio's leprosy is considered the most anergic one of the all immunological spectrum of leprosy.

[Comparative study of thiopental and midazolam for induction of anesthesia].

In 40 female ASA class I-II patients, scheduled for laparoscopy, midazolam 0.2 mg kg-1 i.v. was compared with thiopentone 5 mg kg-1 i.v. for induction of anaesthesia after parenteral premedication. Loss of the eyelash reflex occurred significantly earlier (p less than 0.005) in the thiopentone group (23.3 s) than in the midazolam group (37.5 s). During orotracheal intubation a more stable cardiovascular course was found with midazolam than with thiopentone. Awakening was markedly slower with midazolam than with thiopentone. Anterograde amnesia of more than 60 minutes after induction of anaesthesia occurred in 50% of midazolam patients compared with 10% in the thiopentone group. No significant differences in side effects could be found. Midazolam 0.2 mg kg-1 was sufficient to induce anaesthesia reliably after premedication with droperidol and fentanyl shortly before induction.

Microsurgery and the community-based plastic surgeon.

1. The majority of our residents who completed their training since 1974 use a microscope for nerve repairs. A majority of those doing microscopic nerve repairs also repair noncritical vessel injuries with the microscope. 2. Half of the respondents who completed training in 1974 or before do not use the microscope for any operative procedures. 3. Successful replantation or revascularization procedures are not uncommon early in the practice of a community-based plastic surgeon. The tendency is to do fewer of these procedures the longer the period of practice. 4. For most of the community-based plastic surgeons, the boundary line of community practice versus the University practice is at the level of replantation, revascularization, and free flaps. 5. This boundary was modified by the strategy of having a University microsurgeon (from the same respondent group) assist the community-based plastic surgeon on replants and free flaps in a community hospital. 6. All of the residents who completed training since 1978 used loupes on a frequent (daily or weekly) basis. 7. Only half of the residents who completed training before 1975 used loupes. 8. Other uses of the microscope in the community were vasectomy reversal, nailbed repair, individual transplantation of hairs for eyelashes, canaliculus repairs, and the prevention of traumatic tattoos. Otologic and laryngologic procedures were carried out under the microscope by two respondents who had had otorhinolaryngologic training prior to their plastic surgery residencies.

[Treatment of alopecia areata totalis and maligna with Solvezink (author's transl)].

Treatment with Solvezink has been performed in 284 patients: 42 with alopecia areata, 106 with alopecia totalis (preserved axillar and pubic air as well as eyelashes and brows) and 136 with alopecia maligna. The duration of the treatment including maintenance therapy was up to 6 years. In patients with alopecia areata a state without relapse of 1 to 4 years was achieved in 42,8% of cases, in alopecia totalis and maligna a maintenance therapy was necessary to ensure a permanent regrowth of the hair. When applied correctly, Solvezink was well tolerated. The dosages and the treatment intervals of the treatment should be adjusted individually.

Basal cell carcinoma. A case of longstanding neglect and a case of early detection.

The early detection and referral of a small basal cell carcinoma of the face will prevent extensive surgical excision and repair necessary for a neglected lesion. Since these tumors occur in the older population, careful examination of the skin should be performed on patients 50 years of age and older. This cancer has been estimated to constitute 90% of all malignant lid tumors and a suspicious lesion on the lid should be considered a basal cell carcinoma until proven otherwise. The lesion has a very characteristic appearance as described in the text; however, they can present with an atypical picture. An important sign in the evaluation of a possible malignant lid tumor is the loss of eyelashes within or adjacent to the lesion, which indicates the destructive nature of the tumor to the hair follicles. Basal cell carcinomas do not metastasize to distant locations in the body but rather extend locally and can produce deep tissue invasion. Differential diagnosis can present problems to the clinician but the definitive diagnosis is dependent upon the pathologist. The treatment usually requires surgical removal with wide excisional margins; however, other forms of treatment have been attempted. Recurrences of basal cell carcinomas are not uncommon, especially following an initial recurrence and it is mandatory that these patients be re-examined at least every six months for a two year follow-up period.

Burn injuries of the eyelids.

Burns of the eyelid and surrounding structures are relatively uncommon in spite of a high incidence of thermal injuries involving the face. The etiologic factors, circumstances that precipitated the injury, and the anatomical features unique to the eyelid and adnexal area may conceivably account for the low incidence. The basic aims in reconstructing deformities must include primarily measures of protecting corneal exposure and relieving epiphora. Early reconstruction of other deformities such as epicanthal folds, palpebral stenosis, and missing eyebrows and eyelashes should be discouraged because of frequent recurrence of deformities due to contracting scars and scar hypertrophy.

Studies on bovine demodecosis in northern Nigeria. Specification and host parasite relationships.

The study reported in the present paper discusses the clinical and histological picture of bovine demodecosis and the morphology of Demodex mites as seen in four cows suffering from generalized demodecosis. There were no clinical signs of other skin affections. Changes in both the number and the appearance of visible skin lesions were seen and related to the level of nutrition and the exposure to sunshine of the cattle. Histological sections of some skin nodules showed the presence of mite colonies in the hair follicles. Only adults were seen in the sebaceous glands. Microscopical study of the morphology of the mites revealed the presence of two types of demodicids in the skin lesions and three types from epilated eyelashes. Morphological criteria are presented to aid in identification of species and of life stages.

[The "Fantasy Island" syndrome. Identification of a new osteochondrodysplasia probably of autosomal dominant type].

Clinically, two unrelated patients, an adult male and a female child, coming from non-consanguineous parents, presented dwarfism, peculiar facies, with blepharophimosis, mongoloid slanted eyes, abundant eyebrows and eyelashes, harsh voice and short hands and feet. Radiologically, they presented brachymetacarpalia, brachymetatarsalia, and brachyphalangia of all fingers and toes, shortened and broadened long bones with normal morphology, hypoplastic pelvis and shape anomalies of the vertebral bodies. The clinical and radiological concordance and the differential diagnosis, mainly with acromicric dysplasia, allow the characterization of a distinctive osteochondrodysplasia probably due to an autosomal dominant mutation.

Clinical profile, comorbidity, and treatment history in 123 hair pullers: a survey study.

BACKGROUND: Trichotillomania, characterized by an irresistible urge to pull one's hair, may be more prevalent than previously believed. Despite increasing attention devoted to this topic in the recent literature, there are few studies based on large samples that are potentially generalizable to a community population. METHOD: Surveys addressing clinical profile, comorbidity, and treatment history were mailed to all responders to a nationally distributed magazine article on trichotillomania. Out of 772 surveys sent, 123 completed surveys were returned. RESULTS: While there was a predominance of females in the whole sample, female-to-male prevalence was lower in children than adults. Onset was predominantly in childhood (mean age = 11 years), most frequently in middle childhood and least frequently before age 6. Subjects pulled hair from a variety of sites, including scalp, eyelashes, eyebrows, pubic region, face, and body, but the highest incidence and severity involved scalp hair. Children under 6 were more likely than other age groups to pull scalp hair and possibly less likely to pull other hair. In adults, symptom profile was not associated with age at onset. While subjects reported high rates of comorbid conditions in both self and family, trichotillomania was reportedly formally diagnosed in only 40% of the subjects. Although subjects reported a range of treatments, the majority (58%) reported no treatment history. Finally, only minimal improvement was reported for all modalities, with no significant difference in response to psychotherapy, behavior therapy, clomipramine, or fluoxetine. CONCLUSION: Trichotillomania is a chronic illness that may be difficult to treat. Controlled studies on comorbidity, epidemiology, treatment-seeking patterns, and long-term treatment response are needed.

Peters'-Plus syndrome: report on an unusual case.

We report on a 10 1/2-years-old patient with clinical manifestations of Peters'-Plus syndrome. The patient had Peters' anomaly, slightly arched eyebrows, long eyelashes, a broad nasal tip; malformed and prominent ears, cleft lip and palate, brachydactyly, fingertip pads, severe growth deficiency, and mild mental retardation. Peters' anomaly is a defect of the anterior chamber of the eye, including central corneal opacity, thinning of the posterior aspect of the cornea and iridolenticulocorneal adhesions. Peters'-Plus syndrome is a disorder in which patients with Peters' anomaly are also found to have short stature, brachymedia, abnormal ears, cleft lip and palate, and mental retardation. Our patient showed Peters'-Plus syndrome associated with some clinical manifestations of the Kabuki make-up syndrome.

EyeScrub: simplifying the management of blepharitis.

1. The cause of blepharitis is still a mystery, and, consequently, a cure for this condition does not exist. Patients suffering from blepharitis must be made to understand this and that it is a manageable condition. 2. EyeScrub provides cleansing of the eyelashes and lid margins. Good lid hygiene is crucial to controlling blepharitis. Performed daily in tandem with other simple procedures, EyeScrub removes the oily buildup under the lashes and in the lid margins which causes the eyes to become irritated and encrusted. 3. The application of EyeScrub is simple and does not damage the ocular surface. Lint-free pads allow the patient to feel exactly where the pad is contacting the eye area so that the solution can be applied effectively and harmlessly.

[Biological function of morphologic peculiarities of the dromedary].

The dromedary is very well adapted to arid areas. It is able to survive for many days without drinking; the loss of water is negligible. Morphological characteristics hitherto interpreted as adaptations to dry areas now seem to have other reasons. In the natural habitat of the dromedary mostly thorny plants occur. Long eyelashes, eyebrows, lockable nostrils, caudal orifice of the prepuce and a relatively small vulva protect the animals against injury, especially during feeding.
