[Serum angiogenin concentration in women with adnexal tumors].

Angiogenesis is essential for most solid tumor growth and dissemination. Angiogenin (ANG), a potent new blood vessel formation stimulator may be a potential biomarker of ovarian malignancies. We have analyzed serum concentration of this polypeptide and CA-125 in 78 women with operated because of adnexal tumors. Sixty-five tumors were benign and 13 were malignant. Mean serum concentration of ANG in women with benign tumors and with ovarian cancer 410.8 pg/ml (range: 98.3-956.3 pg/ml) and 469.1 pg/ml (range: 65.3-1103.0 pg/ml), respectively. There were no significant differences in ANG concentrations between the groups, also we have not found any correlation with serum CA-125 levels assayed preoperatively. We conclude that serum angiogenin levels are not a useful predictor of ovarian malignant tumors.

[Adnexal defect identified with hysterosalpingogram and laparoscopy during infertility evaluation].

OBJECTIVE: To describe a previously unreported adnexal defect as a cause of an abnormal hysterosalpingogram (HSG), identified during infertility evaluation. MATERIAL AND METHOD: An abnormal hysterosalpingogram (HSG) consistent with partial left tubal obstruction or hydrosalpynx was identified during the evaluation of a 33 year-old nulligravida with no prior surgical history. At laparoscopy, the diagnosis was revised when a approximately 3x4cm regular, ovoid, physiologic aperture was noted in the central aspect of the left broad ligament in the region corresponding to the HSG defect. The lesion was explored but not repaired. Bilateral tubal patency was confirmed via indigo carmine chromopertubation. RESULTS: Ovulation induction and intrauterine insemination were undertaken based on findings at surgery and careful discussion of same with the couple. The patient successfully conceived and had a normal term delivery. CONCLUSION: While HSG abnormalities discovered during infertility assessment are not unusual, intrinsic or de novo peritoneal defects have not been described previously. Abnormal radiographic findings may be explained by this lesion in selected cases.

[Integral mathematical parameters of a gemogram as criteria for evaluation of severity of chronic adnexitis and of treatment efficacy in conservative therapy].

A number of hematological indices of the peripheral blood in patients with chronic adnexitis were mathematically calculated to evaluate a disease severity and an effect of recovery by using the traditional therapy method. It was established that the integral-and-mathematical indices of hemogram reflect clinical disease acuteness while receiving patients to hospital. A comprehensive evaluation of hematological indices makes it possible to determine the severity of inflammatory process in women, to detect the efficacy of a conducted therapy and to choose a strategy for further metabolic correction to ensure a maximum treatment effect.

Cavernous hemangioma presenting as a right adnexal mass in a child.

This is the case of an 11-year-old girl who presented with a right adnexal mass and vague abdominal symptoms since seven months prior to her hospital admission for surgery. CT-scan and sonographic images were those of a benign lesion, probably ovarian torsion or infarction. Serum tumoral markers were normal. A right salpingo-oophorectomy and appendectomy were performed. Pathology examination revealed a cavernous hemangioma of the ovary. The clinicopathologic presentation of this unusual benign ovarian tumor is discussed.

[Diagnosis and therapy of acute adnexitis using laparoscopy].

In cases of acute adnexitis laparoscopy is an excellent diagnostic method. By this way we found in 182 patients with clinical symptoms only 50,5% acute inflammatory diseases of adnexa. On the other hand there was a normal genital status in 18,1%, acute appendicitis in 9,9%, ectopic pregnancy in 7,7% etc. Clinical examinations and second-look-laparoscopy 6--8 weeks after ending the treatment cases of salpingitis alone showed the best results. Patients with more extensive inflammatory reactions had more adhesions too. An early diagnosis and treatment under hospital conditions is recommended. We have good experiences with immediately operation in cases of suppurative inflammation of the adnexa, which are only diagnosticate by laparoscopy.

Fine needle aspiration cytology of orbital and adnexal masses.

Fine needle aspiration (FNA) biopsy was performed on 35 patients with orbital and adnexal masses. Histopathologic study was performed on 15 of the cases. Examples of benign and malignant primary orbital neoplasms, as well as inflammatory lesions diagnosed this way, are described. The diagnostic accuracy of the technique in evaluating orbital masses was 97.1%; there was one false-negative case. The study indicated that this simple technique has considerable diagnostic value in palpable orbital masses.

Microcystic adnexal carcinoma. Electron microscopic and immunohistochemical study.

We present a patient with microcystic adnexal carcinoma. The lesion was an indurated plaque on the skin of the right upper lip of a 58-year-old woman which was slowly growing during 27 years. Carcinoembryonic antigen was immunoreactive in the luminal contents of the tumor ducts and in the cytoplasm of cells surrounding ducts. S-100 protein was positive in the cytoplasm of a few cells at the lower dermis. These observations suggested that this tumor was related to sweat glands. Furthermore, electron microscopy revealed that tumor cells had features of eccrine ductal cells. These observations confirm that this tumor appeared at least capable of eccrine duct differentiation.

Microcystic adnexal carcinoma.

Microcystic adnexal carcinoma is an unusual, locally aggressive tumor that recently has been recognized as a distinct clinicopathologic entity. It typically occurs on the face of young or middle-aged women and often requires extensive surgical excision to gain local control. A case involving the upper lip and columella of a young woman is described and the available literature reviewed. The difficult reconstructive challenge this lesion usually presents is illustrated in this patient who required upper lip and complete columellar reconstruction.

The postmenopausal adnexal mass: correlation between ultrasonic and pathologic findings.

One hundred two postmenopausal women underwent ultrasonographic evaluation of adnexal masses before surgery. Twenty-nine (28%) had malignant tumors and 73 (72%) had benign tumors. Two of 33 patients with a "simple cyst" smaller than 5 cm in diameter by ultrasound had malignant ovarian tumors. Twenty-two of the 52 women (42%) with "complex masses" by ultrasound and five of the 17 (29%) with solid tumors had ovarian cancer. For predicting malignancy in ovarian tumors, abdominal ultrasonography had a positive predictive value of 39% and a negative predictive value of 94%. If a negative sonogram had been relied upon, 6% of malignant ovarian tumors in postmenopausal women might have been missed.

Laparoscopic treatment of acute adnexitis; one step forward.

Supplementary laparoscopic method accompanying antibiotic treatment in 8 patients with acute PID (ALT Group) was compared to the group of 28 patients with acute PID, diagnosed laparoscopically and treated medically (MLT Group). Rapid subjective improvement of general status in which main symptoms of acute PID disappeared, the absence of early PID complications (p less than 0.01), shorter hospital treatment (p less than 0.05) and the absence of recurrence (p less than 0.01) in the first 6 months are the major advantages of the supplementary laparoscopic treatment of acute adnexitis found in preliminary results of the present study. It seems that supplementary laparoscopic procedures might be more efficient than medical treatment alone.

[Microcystic adnexal carcinoma. A case report].

Microcystic adnexal carcinoma (MAC) is a rare cutaneous neoplasm, locally aggressive, recently recognized as a clinicopathologic entity. Histologically, MAC shows both ductal and pilar differentiation. In this article we discuss a case of 52-year-old woman with MAC on the inferior lip, early recurred after initial excision.

[Electrophoretic evaluation of blood serum proteins during corticosteroid treatment of women with chronic adnexitis].

In the period 1980-1986 in the II Department of Surgical Gynaecology, Medical Academy in Lublin 61 patients were treated with corticosteroids for chronic adnexitis. They were given prednisone (Encorton POLFA) 10 mg twice daily for 10 days. The clinical effect of regression of infiltrations was achieved in over half the cases. In the analysis of the clinical state, ESR value, leucocytosis and electrophoretic separation of serum proteins it was found that the effectiveness of the treatment was highest in the group of patients with initial ESR values greater than 10 mm after one hour. The mean values of white blood cell count before and after the treatment were without essential prognostic importance. The best results were found in patients without clinical evidence of acute inflammation but with alpha 2globulin proportion in electrophorogram typical for acute phase of inflammation.

Evaluation of serum haptoglobin levels in patients with adnexal masses.

Over a four-year period, serum haptoglobin was measured in 152 patients with adnexal masses in an attempt to discriminate between benign and malignant disease. When serum haptoglobin exceeded 150 mg/dl, malignancy could be identified with a sensitivity of 83% and a specificity of 84%. While the predictive value of a positive test is only 66%, the predictive value of a negative test is 93%, and this inexpensive test may be clinically useful. Stage, grade, and size of tumor did not correlate with serum haptoglobin levels.

[Microcystic adnexal carcinoma--a light and electron microscopic studies].

Microcystic adnexal carcinoma (MAC) in a 48-year-old Japanese male was studied. A firm nodule had appeared on the right side of his upper lip at the age of 38 years and then had gradually enlarged. Light microscopic examinations revealed numerous strands and islands of basaloid cells and keratinous cysts with desmoplastic stroma in the dermis and subcutaneous tissue. Some keratinous cysts showed early calcification and some contained pilar structures. Clear cells resembling sebaceous gland cells were observed in the strands of tumor cells connecting the keratinous cysts. In the middle dermis, there were small ductal structures containing amorphous eosinophilic material. Immunoperoxidase staining for carcinoembryonic antigen was focally positive in the lumina of small ducts, on the ductal lining surface of tumor cells, and in the contents of keratinous cysts. Electron microscopic examinations revealed tumor cells resembling non-keratotic keratinocytes, and containing tonofilaments, mitochondria, and vacuoles. In some parts, tumors were differentiated to various types of sebaceous gland, sweat duct, or sweat secretary segments. Sebaceous gland cells contained lipid droplets and glycogen. Tumors differentiated to sweat duct had a marked similarity to embryo eccrine duct in the lower epidermis because of the presence of multivesicular dense bodies and periluminal filamentous zones and the absence of myoepithelial cells. Tumors differentiated to sweat secretary segments had a similarity to embryo eccrine secretary segments because of the absence of multivesicular dense bodies, periluminal filamentous zones and myoepithelial cells.

Prospective views in the treatment of eyelid and adnexal malignancies.

Some aspects of the surgical, radiotherapeutic, chemotherapeutic, cryosurgical, and immunological aspects of eyelid and adnexal malignancy have been discussed. Possibilities for future developments have been mentioned and even rationalized. Oncology is a constantly changing field. It is important for the oculoplastic surgeon to keep abreast of the changes. It is also important for him to be innovative and progressive in the area of possible improvements. It is not for those of us in the rank and file of ophthalmology to try all possibilities, but at least we should be aware of the new techniques and should support their trial.

[Adnexal inflammatory diseases and their influence on menstrual function].

Results are presented from a study on some changes in the menstrual function of women with inflammatory diseases of adnexae for two periods: of 1979-1984 and of 1985-1986. Healthy women at fertile age are used as control group for comparison. The meagre and abundant menstruation together consists of 41.17%--a considerable part. There are differences in the disturbed menstrual process cycle in accordance with the localization of inflammatory process. The most significant disturbance is in women with chronic bilateral adnexitis. Painful menstruation comes on the first place, analysing subjective complaints both for all patients and according to social groups. Algodysmenorrhea is encountered more frequently in women with inflammation of the uterine adnexae in comparison with healthy women. In the evolution of inflammatory diseases of adnexae algodysmenorrhea and disturbed menstrual rhythm deserves special attention to the clinical interpretation and to the formation of diagnostic hypothesis.
