A scoring system for the assessment of oral mucositis in daily nursing practice.

Nurses take care of patients around the clock, so they are in an ideal position to observe and record the signs and symptoms of oral mucositis. This requires a valid, reliable scoring instrument that is easy to use. The objectives of this study were to summarize the scoring instruments that are available, to develop a new Nijmegen Nursing Mucositis Scoring System (NNMSS) and to evaluate this new instrument. A systematic review was undertaken in which 21 scoring instruments were reviewed and compared. None of the instruments studied satisfied the criteria that were established beforehand. The six most common items from the systematic review were selected for the new instrument. To test the NNMSS, pairs of experienced nurses assessed the oral cavity of 26 patients independently. Inter-observer reliability (Kappa), correlation between items (Spearman's rank-order correlations) and internal consistency of the instrument (Cronbach's alpha) were calculated. The usability was evaluated with a questionnaire. Cohen's weighted Kappa was within an acceptable range. Almost all correlations were statistically significant and in the predicted direction. The Cronbach's alpha coefficient indicated sufficient internal consistency. All nurses found the NNMSS user-friendly and suitable for day-to-day care. The NNMSS can be used as a valid, reliable and usable instrument in daily nursing practice.

Parish nursing: advancing practice nursing. Model for healthier communities.

Advanced practice nurses (APNs) in the community are in a prime position to respond to the changing health care environment stimulated by health care reform. An emerging practice model for APNs is parish nursing, community-based nursing practice set in communities of faith. Parish nursing stimulates shared public-private partnerships and is a consumer-oriented delivery system. The article describes the models, context, and practice of parish nursing, emphasizing the problem of evaluating outcomes. It also discusses implications for research and education. Parish nursing is a significant practice model affording APNs the opportunity to contribute to healthier communities.

Models of nursing, nursing practice and nurse education.

Over the last few years, and concurrent with wider debates about the quality of patient care, increasing numbers of practising nurses and nurse educators have expressed interest in using conceptual models of nursing as a basis for planning and delivering nursing care. In the light of such interest, this paper identifies the distinctive features of three fundamental types of nursing model and explores the implications of these for contemporary approaches to nursing practice and nurse education.

Massage therapy as a nursing practice.

Nursing is a profession that can be practiced in many unique ways. Nursing care can be provided through conventional means in traditional settings or practiced in domains labeled as alternative or complementary. This article describes massotherapy as an alternative therapy of a holistic nursing practice. The elements of a business plan, including planning and marketing are addressed.

Promoting advanced nursing practice.

Nurses functioning in advanced practice roles, particularly those in private practice or in other entrepreneurial roles, need to develop strategies which inform potential clients about the professional services they offer. This article describes practice-promotion strategies used by psychiatric nurses in private practice. A survey by the authors suggests that many of these strategies are viewed as either professionally appropriate or business-producing, but usually not both. Nurse entrepreneurs, in seeking means to promote their practices, must determine which promotional strategies are useful in their own unique circumstances.

[Explanatory models for alcohol and drugs use and the nursing practice].

Nursing care practice, research and teaching have led to the accumulation of considerable experience in care for patients with problems related to alcohol abuse. This experience has shown that, among nursing professionals in general, there is a great need to acquire theoretical knowledge, to review the models or informal protocols that support practice in this area. This article aims at examining literature on the main theoretical models to explain alcohol and drugs abuse. It is concluded that, through an understanding of the explanatory models, nurses can seek means of facilitating their practice according to the health demand needs.

Shaping advanced nursing practice in the new millennium.

OBJECTIVES: To provide a review of converging themes and trends that are shaping advanced practice nursing roles in oncology nursing. DATA SOURCES: Review and research articles, text-books, and organization documents. CONCLUSIONS: The current managed care environment provides many opportunities and challenges for oncology advanced practice nurses. Advanced practice nurses have both clinical and organization competencies that enable them to mediate the clinical needs of patients and organization goals within the health care system. IMPLICATIONS FOR NURSING PRACTICE: Advanced practice nurses can help shape their roles and practice by active participation in the development of systems to support access to clinical and financial information for effective decision making, collaboration among disciplines, and incorporating evidence-based care in their clinical practices.

An ethic of the fitting: a conceptual framework for nursing practice.

Nurses are expected to act within an ethos of care cognisant of duty, the right, and the good. Concepts of virtue theory, utilitarianism and deontology are used to outline a conceptual ethical framework for nurses in practice. This 'Moebius' framework aims to locate the virtues in a symbiotic relationship with the principles of utilitarianism and deontology. Under this framework, fitting ethical responses are sought. Within an ethic of the fitting, rules and principles do not guarantee the most ethically justifiable outcomes. Within the Moebius approach, the nurse ought to respond with a sense of compassion that befits the moment rather than adhere to a rule at all costs.

Assessing clients' sexual health as a component of holistic nursing practice: senior nursing students share their experiences.

A phenomenological study was conducted to investigate baccalaureate nursing students' experiences with assessing the sexual health of their clients. The participants were 10 female nursing students. The participants were asked to respond to the statement: Tell me about your experiences with assessing your clients' sexual health and concerns. Data were analyzed using Van Manen's criteria for analysis. The common themes were (a) recognized sexual assessment as a component of holistic nursing, (b) experienced discomfort exploring clients' sexual health, (c) believed that sexual assessment was warranted only if related to the diagnosis, and (d) held that the assessment was less important than other assessments. Information from this study has potential to inform nurse educators engaged in clinical teaching and curriculum development that sexual health assessment must be valued as a study area if holistic nursing care is to be a part of professional nursing practice.

National agenda for advanced practice nursing: the practice doctorate.

The purpose of this article was to provide the background and rationale for the practice doctorate in nursing. The American Association of Colleges of Nursing's Position Statement on the Practice Doctorate in Nursing, approved in October 2004, will be discussed. Outlined are some of the changes that will be needed in education, regulation, and advanced practice. Common questions and concerns that advanced practice nurses have, including titling, salary, and transitioning to the doctor of nursing practice degree, will be addressed.

Clinical exemplars to recognize excellence in nursing practice.

Nursing is a dynamic, challenging, rewarding profession. It is important that we take opportunities to tell the world and each other about the richness of caring, which is the essence of our practice. Clinical exemplars are a way to capture this true nature of nursing. It is through sharing of clinical exemplars that we celebrate our own practice and share our uniqueness with others. This article defines clinical exemplars, describes their importance to nursing practice, and discusses their benefit to identify skill acquisition and recognition in nursing.

Parallels in practice: palliative nursing practice and Parse's theory of human becoming.

Nurses experienced in the art and science of palliative nursing will find many elements of congruence between the principles and philosophy of palliative practice and the theory of human becoming. In this brief exploration of parallels in practice between Parse's theory of human becoming and traditional palliative and hospice nursing practice, the author suggests that the theory of human becoming is consistent and consonant with the values that shape palliative nursing practice. The theory is briefly described, and four parallels of practice are identifed: whole person care; the presence of paradox in human experience; primacy of the person; and presence and dialogue, or "being with, " dying persons. The theory of human becoming holds relevance and promise in its capacity to provide palliative and hospice nurses with a theoretical framework with which to inform and guide nursing practice with dying persons.

Clinical expertise in advanced practice nursing: a Canadian perspective.

The need to articulate clearly the definition and description of advanced nursing practice is critical if the nursing profession is to move forward in the development of nursing practice. Concern arises when the core of advanced practice nursing is overshadowed with medical functions. Consensus on the educational preparation of advanced practice nurses, explication of the nature of clinical expertise in advanced practice, and clarity of its nature are issues that the nursing profession must achieve greater precision in understanding and explaining. Advanced practice nurses must be able to practice in a milieu that will allow them to function to their full scope of practice.

The International Classification for Nursing Practice (ICNP): nursing outcomes.

A classification of nursing phenomena (as diagnosed by nurses) and of nursing interventions is included in the Alpha Version of the International Classification for Nursing Practice (ICNP). The third dimension of the ICNP-nursing outcomes-is currently being developed. Below are some of the issues, initial thinking and possible directions for the classification of nursing outcomes. As in the case of the Alpha Version's classifications of nursing phenomena and nursing interventions, the active participation of nurses around the world is being sought for the development of an improved Beta Version.
