The authors report the anatomoclinical observation of an 82-year old patient presenting a mixed malignant mesodermal tumour (MMMT) revealed by a uterine inversion. The histological examination revealed sarcomatous and carcinomatous lesions with zones of chondroid metaplasia infiltrating largely the myometrium. A total vaginal hysterectomy, accompanied by radiotherapy, got the better of an early local relapse. The histogenetic hypotheses of MMMTs, the particular circumstances of the tumour revelation, as well as the methods of treatment are reviewed.
Six cases of acute puerperal inversion of the uterus are reported. In four of these, tocolytic agents were used to facilitate replacement of the inverted uterus; and in five, Prostin-15M was given after replacement. The use of tocolytics may obviate the need for general anesthesia for uterine replacement in some patients with uterine inversion. Prostin-15M serves to minimize bleeding and to maintain uterine position after replacement.
Sequential administration of a beta 2-mimetic and a prostaglandin, the latter preferably injected into the uterus, is a simple and highly effective method for managing acute puerperal uterine inversion.
The potential for a poor reproductive outcome is particularly pronounced in women with cervicovaginal changes related to diethylstilbestrol (DES). A women with documented DES exposure and cervicovaginal abnormalities developed an acute postpartum uterine inversion requiring a hysterectomy.
