[A study on chest radiographs in medical check-ups for over-staying foreigners].

During the past nine years from 1991 to 1999, Minatomachi Medical Center and other organizations have provided free medical check-ups for foreigners in Kanagawa, Tokyo and Chiba. Eighty-five percent of the total of the 2370 examined were over-staying foreigners who were not covered by national health insurance system. The ratio of male to female was two to one and the most of them were in the twenties and thirties. As for their ethnic origins from forty-three countries, the Filipinos were the largest numbers, followed by Koreans, Iranians, Bangladeshi, and Chinese/Taiwanese. As a result of chest radiographs, 82% were normal, 8.3% mostly normal, 9.6% tuberculosis shadow (TBS) including active, non active and previous, 0.09% suspected pneumonia, 0.05% suspected cardiac disease, and 0.05% suspected sarcoidosis. The TBS and active tuberculosis (ATB) rates of all examined were 9.6% and 0.69% respectively. There was no significant difference in each TBS and ATB rate between the two sexes. Seen from the age group, it was found that the more aged they were, the higher the TBS and the ATB rates were. The TBS and the ATB rates were 24.6% and 2.73% in the Koreans, 10.7% and 0% in the Chinese/Taiwanese, 10.2% and 0.65% in the Filipinos, 11.3% and 1.44% in the South-East Asians, 6.3% and 0.25% in the South Asians, 2.3% and 0% in the Iranians, 3.7% and 0% in the sub-Saharan Africans, 8.5% and 0.61% in the Latin Americans, and 0% and 0% in the Europeans/North Americans. Based on a presumed ATB rate of 0.75%, approximately two thousand cases with ATB could be calculated to exist among the estimated 270,000 over-staying foreigners. It is, therefore, important to give them more frequent medical check-ups for early detection of latent cases with ATB.

Antagonistic effect of chosen lactic acid bacteria strains on Yersinia enterocolitica species in model set-ups, meat and fermented sausages.

The present study was aimed at determining the influence of 15 strains of lactic acid bacteria on the growth of 8 Yersinia enterocolitica strains in model set-ups, and in meat and ageing fermented sausages. The investigations were performed within the framework of three alternate stages which differed in respect to the products studied, the number of Lactobacillus sp. strains and, partly, methodological approach. The ratio between lactic acid bacteria and Yersinia enterocolitica strains studied was, depending on the variant of experiment, 1:1, 1:2 and 2:1, respectively. The study also considered water activity (aw) and pH of the products investigated. The results suggest that all the lactic acid bacteria strains used within the framework of the model set-ups had antagonistic effect on all the Salmonella sp. strains. However, this ability was not observed with respect to of tested lactic acid bacteria strains in meat and fermented sausage. This ability was possessed by one of the strains investigated--Lactobacillus helveticus T 78. The temperature and time of the incubation of sausages, but not aw and pH, were found to have a distinct influence on the antagonistic interaction between the bacteria tested.

Midtreatment flare-ups.

It should be apparent that the prompt and effective treatment of midtreatment flare-ups of all types is an essential and integral part of the overall endodontic treatment procedure. The expeditious management of these flare-ups will do much to enhance a positive attitude among patients toward endodontic treatment and to ensure the well-being and comfort of these patients.

Flare-ups incidence and severity after using calcium hydroxide as intracanal dressing.

Acute pain and swelling following endodontic treatment are a challenge for both the patient and the dentist. According to previous studies, the incidence of flare-ups increases after endodontic treatment of teeth with necrotic pulps. Calcium hydroxide is currently used as a multi-purpose drug in root canal therapy. The aim of this study was to evaluate the incidence and severity of flare-ups after treatment of pulpless teeth using calcium hydroxide as an intracanal dressing. Sixty patients with single-root necrotic teeth participated in this study. These patients were randomly divided into three groups of 20. The patients were treated in Group A in a single-visit approach, in group B with a two-visit approach without any intracanal dressing and group C with a two-visit approach using calcium hydroxide as an intracanal dressing for one week. All of the patients were followed for 72 hours after each treatment session. The information about the incidence and severity of pain and swelling was recorded in tables, using a modified Visual Analogue Scale for pain severity measurement and a scale with four degrees for measuring the severity of swelling. The data were analyzed by chi-square test and GENMODE procedure.

Intracanal medications and antibiotics in the control of interappointment flare-ups.

Acute exacerbation of symptoms, or "flare-up," after the debridement of the root canals and provisional restoration is a well-known postoperative complication of endodontic treatment. In this clinical study, various types of intracanal medications were compared for their ability to decrease interappointment flare-ups. The result showed that use of intracanal medicaments containing anti-inflammatory agents in combination with the administration of prophylactic systematic antibiotics was the most effective method of controlling interappointment flare-ups.

[The value of routine check-ups of patients after radical surgery of colonic cancer or rectal cancer].

UNLABELLED: In this retrospective study, the efficiency of the postoperative follow-up regimen was evaluated in patients, radically operated for cancer of the colon or cancer of the rectum. The follow-up regimen consisted of clinical examination, including sigmoidoscopy every six months, supplemented by an X-ray of the colon every year. Two-hundred and eighty-nine patients were included in the study. Twenty-three local recurrences and 19 new malignant tumors were diagnosed 20 and 13 months (mean) after the primary operation, respectively. About one third of the local recurrences and half of the new malignant tumors were diagnosed at the planned follow-ups. Among these patients, a significantly higher success rate as regards radical operation was observed at the second operation. IN CONCLUSION: more frequent follow-ups during the first two years after the primary operation are recommended in order to improve early diagnosis.

Progression of chronic renal failure in man is retarded with more frequent clinical follow-ups and better blood pressure control.

In 17 patients with chronic renal failure (creatinine clearance 12-66 ml/min) who entered a prospective study, clinical and laboratory check-ups were made once a month. This was much more frequent than during the preceding 2-year period. The progression rate of renal failure evaluated from the change with time in the reciprocal of serum creatinine and creatinine clearance, respectively, was significantly slower after entering the study than before, without any dietary intervention having been instituted. The retardation of progression was associated with improved blood pressure control. We conclude from this study that the frequency and presumably the quality of the clinical check-ups, especially with regard to control of hypertension, may favorably influence the progression of chronic renal failure, independently of the protein intake. This may have important implications for the design of randomized studies, aimed at assessing whether a lowering of protein intake may retard progression.

[Pacemakers: concerning check-ups (author's transl)].

The Authors have re-examined their case histories of 1503 patients who have had 2459 pacemakers implanted and who have undergone check-ups as outpatients. All the complications which have arisen from 1967 to the present have been taken into consideration. The total number of complications has been found in 355 patients with 14% of the total number of pacemakers and they have been subdivided into 3 groups: 1st group: complications arising during the stay in hospital (110); 2nd group: complications arising after discharge from hospital and discovered by the patient (239); 3rd group: complications found casually during outpatients' check-ups (6). Of the complications found after discharge, 97% belong to the second group and only 3% to the third group. 657 substituted pacemakers have also been taken into consideration with the purpose of finding out the parameters which showed up as the battery was running down. Out of 145 pacemakers substituted because of flat batteries, 130 (90%) have shown brusque variations in the frequency of stimulation, while only 15 (10%) have shown variations in other parameters (width, length of impulse, etc). The Authors therefore retain that a periodic outpatients' check-up of the pacemakers is of little use as regards the patients' safety and the complete utilization of the device, while they advise that the patient be educated so as to be able to carry out his own daily control of the pacemaker.

Standard-deviation-index-pattern analysis of blood chemical data in health check-ups for chronic liver disease. A clinico-pathological study.

Eighty-nine patients with chronic liver disease were selected as the subject for this study. These patients were found to have some abnormalities in health check-ups and confirmed finally by biopsy or laparoscopy findings. As healthy controls, 1633 subjects who had no abnormalities in health checkups, were selected. Serum specimens in the fasting state were collected from all of these subjects, and twenty chemical items were assayed by an automatic analyzer. Data of the control group were transformed for each item and both sexes into the normal distribution by adopting the most suitable mathematical processing, and the standard deviation indices, SDI = (Xp - Xn)/SDn, were calculated. The items specific to each disease were decided by selecting those showing significant differences from the control group both in mean values and in distribution of SDI. Using these items, the specific SDI-patterns were obtained. By such processing it has been possible, to show every item on the same yardstick, to find significant deviations even within the normal range more quantitatively, and to find overall abnormalities of a subject at a glance. On account of these characteristic advantages, it is clear that this pattern presentation is efficient for the early recognition of pathophysiological states of subjects in health check-ups.

Factors to consider for predictable post and core build-ups of endodontically treated teeth. Part II: Clinical application of basic concepts.

Post and core build-ups represent an important pre-prosthetic procedure prior to the restoration of an endodontically treated tooth. The dental practitioner is presented with the dilemma of selecting from an ever increasing variety of materials, techniques and designs related to this procedure, many of which are harmful and mired in controversy. Part II of this paper will review some basic theoretical concepts related to post and core build-ups with the objective of applying these concepts to address a number of pertinent clinical questions related to the above selection process. This review should provide the dentist with a reference to produce a more predictable post and core build-up.

Unity of analysis: similarities and differences in the analysis of children and grown-ups.

When comparing child analysis with that of grown-ups, we are confronted with a substantial unity of psychoanalytic method, beyond the apparent differences. The more we consider the analysis as a here-and-now interaction between analyst and patient with transformative potential, the more age-dependent differences become blurred, and the specificity of a particular analytic couple acquires more significance. At the same time, what seems to be all the same--the actual patient before us--is quite different, with several components (the child, adolescent, and adult parts), implying a need to recognize the part we should address at any given point.

Evaluating refractory cast wax-ups for removable partial dentures.

A review of the waxed refractory cast and blocked-out master cast by the dentist is an effective method for controlling the quality of framework castings. Major errors in wax-ups have been shown to occur frequently, but they are corrected easily if detected prior to casting. This critique can be incorporated readily into the private practice of dentistry.

UPS shipping and handling.

It is now clear that the two broad sets of reactions in the ubiquitin-proteasome system (UPS), the conjugative and degradative steps, are intricately coupled in vivo and that there are numerous factors that aid in the handling and delivery of ubiquitylated proteins to the proteasome. The report from Richly et al (2005)[this issue of Cell]) suggests how several of the known players in this process might work together in guiding and delivering some substrates to their final destinations.

Poor in vitro fertilization outcome with semen yielding low sperm density "swim-ups" is not because of altered sperm motion parameters.

OBJECTIVE: To examine fertilization, cleavage, and pregnancy rates (PRs) with semen samples yielding numbers of total motile sperm per swim-up ranging from < 1 to > 20 x 10(6) and to correlate the findings with changes, if any, in the sperm motion parameters. DESIGN: Fertilization, cleavage, and pregnancy outcomes in 439 in vitro fertilization (IVF) cycles were correlated with the total number of motile sperm per swim-up and the sperm motion parameters as determined with an automated semen analyzer. SETTING: A university-based tertiary referral hospital center. PATIENTS: Patients undergoing IVF or intrauterine insemination treatments for multiple etiologies. RESULTS: Higher numbers of motile sperm per swim-up, most notably above the value of 3 x 10(6) motile sperm, were associated with improved fertilization rates and viable PRs. Sperm velocity, linearity, amplitude of lateral head displacement, and flagellar beat per cross frequency for sperm from swim-ups with poor or good pregnancy outcome, however, showed no significant differences. CONCLUSIONS: As a group, semen samples that yield < or = 3 x 10(6) motile sperm per swim-up are associated with poor fertilization rates, cleavage rates, and PRs. This relationship can not be attributed to differences in sperm motion parameters.

Practice write-ups: an assessment instrument that contributes to bridging the differences between theory and practice for student nurses through the development of core skills.

Nurse education currently faces two major challenges. The first involves deciding the best way to increase the skill level of practitioners and the second involves determining how this can be achieved within a culture of lifetime learning. Each of these challenges needs to be confronted as early as possible and preferably at undergraduate level. Practice write-ups (PWUs) enable students to demonstrate competence in a range of core nursing skills. Although PWUs are essentially an assessment instrument, they are underlined by an educational commitment to encourage students to be reflective in their practice. In doing so students should be able to question existing strategies for implementing care and decide whether care is appropriate and contributing to the well-being of the patient.

Pain management program follow-ups. a national survey of current practice in the United Kingdom.

In the United Kingdom (UK), the number of multidisciplinary pain management programs (PMP) has sharply increased in the last decade. Treatment aims to effect long-term changes, yet arrangements for evaluating long-term treatment outcome in routine practice remain unknown. All PMPs identified in the UK were invited to provide information by mailed questionnaire on aspects of follow-up evaluation. Sixty-six of 70 PMPs identified were successfully contacted (94% response rate). Ten of 48 PMPs (21%) provided complete data and met suggested criteria for adequate length and rates of follow-up, and outcome measurement. Levels of staffing and content explained only a proportion of the variability seen. A general commitment to follow-up outcome evaluation by PMPs was identified but there was substantial variability among programs in length of follow-up, attendance rates, and outcome measurement. These findings raise issues about the consistency of follow-up evaluation across the UK and the ability of current arrangements in some PMPs to accurately determine the longer-term outcome of chronic pain patients. Incorporating follow-ups into existing desirable criteria and agreeing on core outcome measures are two options available to service providers.
