Parents' and teachers' praxes of and attitudes to the health and sex education of young people with mental handicaps: a study in Stockholm and Tokyo.

In 1989/1990, parents and teachers of young people with mental handicaps in Stockholm (41 parents and 20 teachers) and Tokyo (106 parents and 111 teachers) were given a questionnaire about their praxes of and attitudes towards health and sex education. Results from the study indicated that, compared to parents and teachers in Tokyo, parents and teachers in Stockholm gave more information about health and sex to their 15-16-year-old young people with mental handicaps. This was especially so regarding information about HIV/AIDS. More parents and teachers in Stockholm also had positive attitudes towards sex and interpersonal relationships--not only for young people in general but also for young people with mental handicaps--compared to parents and teachers in Tokyo. Some questions of an ethical nature were found difficult to answer in both Stockholm and Tokyo.

Differences in glottal opening pattern between Tokyo and Osaka dialect speakers: factors contributing to vowel devoicing.

In Japanese, high vowels /i/ and /u/ are often devoiced when they are preceded and followed by voiceless consonants. The phenomenon is called vowel devoicing. The frequency of its occurrence is high in dialects of eastern Japan including standard (Tokyo) Japanese and low in dialects of western Japan including Osaka dialect. It has been claimed that consonants, but not vowels, are pronounced more carefully in eastern dialects compared to western dialects, which results in higher frequency of vowel devoicing. This paper examines whether the glottal opening gesture of voiceless consonants differs between those Tokyo speakers who consistently show vowel devoicing and Osaka speakers who do not show vowel devoicing. The results indicated that the duration of glottal opening for /k/ in voiced CV syllables was significantly longer for Tokyo speakers than for Osaka speakers. For Tokyo speakers, duration and maximum size of the glottal opening in devoiced /kite/ are significantly larger than /k/ in voiced syllables. This difference cannot be explained by the simple addition of the glottal gestures of [k] and [t]. Furthermore, the difference in vowel length between /i/ and /e/ was related, at least in part, to the difference in the amount of the airflow passing through the vocal tract about the time the stop closure was released.

A comparative study of some pathologic features of mammary carcinoma in Tokyo, Japan and New York, USA.

Epidemiologic and clinical studies conducted in the past 15 years have demonstrated striking differences in the biology of mammary carcinoma among Japanese and American women living in their native countries. These variations have, in part, been related to some differences in the characteristics of the primary tumors between the two groups. As part of a collaborative study we have had an opportunity to compare the stage of disease and to examine and compare histological sections of patients with breast carcinoma treated in 1973-74 at the National Cancer Center Hospital (NCH) in Tokyo and in 1974 at the Memorial Hospital (MH) in New York. The former group consisted of 216 and the latter of 555 carcinomas. Fewer patients in each group had axillary metastases than reported in a prior study of patients treated at MSKCC and in Tokyo 20 to 30 years earlier. Negative axillary nodes were now found in 58% of the MH patients and in 63% of women treated at the NCH. The magnitude of improvement in stage relative to the prior report was similar in both groups. However, it would appear that the change occurred mainly from the mid-1950s to the 1960s in New York and approximately 10 years later in Tokyo. Results of this study confirming prior reports were: (1) higher frequency of colloid and of medullary carcinoma with lymphoid stroma and lesser frequency of lobular carcinoma in the Japanese patients; (2) more intense lymphoid infiltrate in and around primary tumors in Japanese women; (3) higher frequency of rounded or circumscribed tumors in Japanese women; and (4) the more frequent occurrence of intralymphatic tumor emboli within the breast in American women. The difference in the frequency of lobular carcinoma was less striking when comparison was limited to patients with unilateral carcinoma.

Crimes in New York and Tokyo: sociocultural perspectives.

Patterns of major crimes in Tokyo and New York City, based on official reports, are compared. Among other findings, the rate of infanticides among the total of murder cases was strikingly higher in Tokyo than in New York City. Intruders in New York City, on the other hand, were found to seek confrontation with victims more actively than their counterparts in Tokyo. Social and cultural factors underlying or contributing to the causes of major crimes are discussed.

Prevalence and persistence of certain serologic types of Streptococcus pyogenes in metropolitan Tokyo.

M-typing was conducted for Streptococcus pyogenes strains isolated within 1 week of admission from patients admitted to the Tokyo Metropolitan Toshima Hospital for scarlet fever in the 23-year period between 1956 and 1978. M12 was detected in the largest number of patients, followed by M6, M4, M3, and M1. T-typing was also conducted for Streptococcus pyogenes strains isolated from healthy schoolchildren in three different parts of the 20-year period between 1977 and 1998. T12 was detected in the largest number of children, followed by T28, T1, T4, and T6. These results suggested that strains of similar types are prevalent in scarlet fever patients and healthy carriers, that these strains become prevalent at intervals of several years, and that they are detected every year, albeit in a small number of persons, indicating that they are endemic. Isolated strains were typed by M-typing and T-typing. The results of typing were identical in more than 90% of cases. Comparison of major Streptococcus pyogenes strains isolated in Tokyo and three other major cities abroad showed that M12 was detected in the largest number of persons in Tokyo and New York and that major types were similar in the four cities.

Prognostic implications of inhalation injury in burn patients in Tokyo.

Inhalation injury has recently emerged as the major cause of mortality in burn patients. However, the prognostic value of inhalation injury has not been thoroughly assessed in Japanese burn facilities. The aim of the present study was to evaluate the impact of inhalation injury on burn patients' mortality in Tokyo. Of 6416 patients admitted to 13 burn facilities of the Tokyo Burn Unit Association between 1984 and 2002; the 5560 eligible patients were included in this study (mean age, 40+/-20 years; male, 61.6%; mean partial- and full-thickness burn size, 10.7+/-13.0% and 9.6+/-20.5%). Of the 5560 patients, 1690 patients (30.4%) had experienced inhalation injury. The overall in-hospital mortality rate of the patients with inhalation injury was higher than that of those without inhalation injury (33.6% versus 8.1%, odds ratio, 5.72 [95% CI, 4.91-6.67]). The results of the multivariate analysis indicated that inhalation injury; full- and partial-thickness burn size, and age were independent predictors of outcome (relative risk, 2.58 [2.03-3.29], 1.10 [1.09-1.11], 1.06 [1.06-1.07], 1.05 [1.05-1.06], respectively). In conclusion, inhalation injury was the most important predictor of overall mortality among burned patients in Tokyo.

Noise control standards in the city of Tokyo.

The authors used the data on noise pollution processed by the Tokyo Metropolitan Institute for Environmental Protection, and discussed noise control standard in Tokyo. In the 23 wards, the noise level is between 52 and 69 dB, while in suburban city districts, the level is between 46 and 65 dB. When the level of noise is below 60 dB outdoors, the level is less than 50 dB indoors. When the level of noise exceeds 55 dB, more than 50% of the inhabitants feel uncomfortable. The standard outdoor noise level in residential districts is 60 dB in the daytime, and 50 dB in the nighttime. It must be 5 dB below the above in districts where quiet is particularly important, such as near hospitals. Regulation of noise control can only be dealt with through the cooperation of industries and inhabitants under the surveillance of the Tokyo Metropolitan Government.

Present characteristics of symptomatic amebiasis due to Entamoeba histolytica in the east-southeast area of Tokyo.

Admitted medical records, from January 1988 to December 1995, of 28 symptomatic amebic patients who lived in the east-southeast area of Tokyo were studied retrospectively, in order to find the present characteristics of symptomatic amebiasis due to Entamoeba histolytica in this area. Categorized by disease, there were 14 cases of colitis, 9 cases of liver abscess, 4 cases of colitis with liver abscess, and 1 case of liver abscess with brain abscess. Patients consisted of 26 Japanese males, 0 Japanese females, 1 non-Japanese male and 1 non-Japanese female. The mean age of colitis patients and liver abscess patients was 55.4 years old and 41.3 years old, respectively. The presumed place of contraction was Japan in 64% of the patients. Forty-eight percent of male patients indicated that they engaged in homosexual or bisexual practices, and 36% of male patients who denied such sexual practices or did not answer the question had no history of marriage. Positive rate of serum titre for Treponema pallidum hemagglutination test (TPHA) and human immunodeficiency virus antibody was 42.9% and 0%, respectively. Sixty-seven percent of TPHA-positive patients indicated that they engaged in male homosexual or bisexual practices. Zymodeme patterns of E. histolytica isolated from 4 colitis patients were XIV in 1 case and II in 3 cases. Symptomatic amebiasis in the east-southeast area of Tokyo is a disease which predominantly afflicts males, especially those in their middle age, and most patients contract the disease in Japan. The high rates of patients who engaged in male homosexual or bisexual practices and the high rates of patients with positive TPHA suggest that amebiasis is likely to be sexually transmitted disease in homosexual and bisexual men in the east-southeast area of Tokyo, and zymodeme II may be the predominant type in symptomatic amebic colitis in this area.

Influence of paint chips on lead concentration in the soil of public playgrounds in Tokyo.

Lead concentration in the surface soils from 31 playgrounds in a ward in Tokyo was measured to examine if paint chips, peeled off from playing equipment installed in the playgrounds, contribute to elevated Pb concentration in the soil of public playgrounds. Lead concentration in the paint chips sampled from playgrounds ranged from 0.003 to 8.9%. Lead concentration in the surface soil ranged from 15.2 to 237 mg kg(-1) (average, 55.5 mg kg(-1)) and higher Pb concentration was found in the soil near painted playing equipment indicating that paint chips from playing equipment contributed to increase soil Pb level of playgrounds in Tokyo. The degree of peeling-off of paint on the surface of playing equipment in the public playground (peeling-off index: POI) positively correlated with Pb concentration in the soil (Spearman rank-correlation coefficient, r = 0.366, p = 0.043). The stronger correlation between Pb concentration and isotope ratios (207Pb/206Pb and Pb conc., r = 0.536, p = 0.002, 208Pb/206Pb and Pb conc. r = 0.600, p < 0.001) than that between Pb and POI indicated that gasoline Pb contributed more to the playground-to-playground variation in soil Pb concentration. It was concluded that both gasoline Pb of the past and paint chips contributed to increased Pb concentration in the surface soil of playgrounds in Tokyo, though the contribution of paint chips is smaller than gasoline Pb.

Cutaneous lymphomas in Tokyo: analysis of 62 cases in a dermatology clinic.

BACKGROUND: The epidemiology of cutaneous lymphomas revealed that the incidence of lymphomas differed depending on various factors including area, race, and sex, among others. OBJECTIVE: This study was undertaken to analyse the incidence of cutaneous lymphomas in Tokyo. METHODS: The clinical records and histologic material from 50 patients with lymphomas of the skin and 12 patients with lymphomatoid papulosis seen during the last 10 years at the Department of Dermatology, The Jikei University School of Medicine, Tokyo, have been reviewed. RESULTS: T-cell lymphomas including mycosis fungoides (MF)-Sézary's syndrome (SS) complex and adult T-cell leukemia/lymphoma (ATL) were more frequent than B-cell lymphomas. The incidence of ATL is associated with the number of human T-cell lymphotropic virus type 1 (HTLV-1) carriers in the general population. Cutaneous B-cell lymphoma (CBCL) is not as rare as previously thought in Japan. CONCLUSIONS: Although the frequency of these cases depends on many unrelated factors, these values can provide a rough indication of the incidence of cutaneous lymphomas in Tokyo. The incidence of cutaneous lymphomas may be influenced by changes in environmental factors including viral infections.

Comparison of selenium status between Japanese living in Tokyo and Japanese brazilians in São Paulo, Brazil.

Selenium (Se) concentrations were determined in human serum, rice and wheat flour sampled in the city of São Paulo, Brazil, and compared with those sampled in Tokyo. Japan. Serum levels of Se were significantly lower in Japanese Brazilians than Japanese living in Tokyo. The samples of rice consumed by Japanese Brazilians in São Paulo contained 22.7 ng Se/g on average, which was about half the selenium level in rice consumed in Tokyo. Rice commonly consumed in São Paulo might be one of the factors to lower the serum level of Se.

[Epidemiological study on Chlamydia trachomatis infection in obstetrics and gynecology field in Tokyo].

In conjunction with the Tokyo Branch of Japan Association for Maternal Welfare, we have been carrying out examinations of Chlamydial infection on women, especially pregnant women, living in Tokyo. Specimens were collected from 212 gyneco-obstetric clinics in Tokyo during 4 years from January 1987-December 1990. A total of 13,925 swab specimens from patients who were suspected of sexually transmitted diseases from clinical findings and pregnant women who requested the examination were tested for the presence of antigen to C. trachomatis with EIA (Chlamydiazyme). Epidemiological analysis based on the results and case cards that were described sex, age, occupation and clinical findings were conducted. The results obtained are briefly summarized as follows. 1) C. trachomatis antigen was detected in 12.8% (1,237/9,657) of the female patients, 31.0% (124/400) of the male patients and 6.8% (10/168) of the infants. The detection rates of C. trachomatis antigen in the male patients was significantly higher than that in the female and the infants patients. C. trachomatis antigen was detected in 6.1% (226/3,683) of pregnant women. 2) The detection rates of C. trachomatis antigen were compared by occupation of female patients. Most high rate was 26.6% (53/199) in bar hostess , subsequently 20.1% (102/508) in students, 19.8% (59/298) in prostitutes, 13.6% (455/3,348) in office girls and 7.2% (471/6,573) in house-wives. It was noteworthy that the detection rates of C. trachomatis antigen in students was as similarly high as that in prostitutes. 3) The detection rates of C. trachomatis antigen was also compared by age groups. The detection rate had a tendency to be high in younger people.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparative study of the aerobic, heterotrophic bacterial flora of Chesapeake Bay and Tokyo Bay.

A comparative study of the bacterial flora of the water of Chesapeake Bay and Tokyo Bay was undertaken to assess similarities and differences between the autochthonous flora of the two geographical sites and to test the hypothesis that, given similarities in environmental parameters, similar bacterial populations will be found, despite extreme geographic distance between locations. A total of 195 aerobic, heterotrophic bacterial strains isolated from Chesapeake Bay and Tokyo Bay water were examined for 115 biochemical, cultural, morphological, nutritional, and physiological characters. The data were analyzed by the methods of numerical taxonomy. From sorted similarity matrices, 77% of the isolates could be grouped into 30 phena and presumptively identified as Acinetobacter-Moraxella, Caulobacter, coryneforms, Pseudomonas, and Vibrio spp. Vibrio and Acinetobacter species were found to be common in the estuarine waters of Chesapeake Bay, whereas Acinetobacter-Moraxella and Caulobacter predominated in Tokyo Bay waters, at the sites sampled in the study.

Temperature and air pollution as risk factors for heat stroke in Tokyo, July and August 1980-1995.

Heat stroke is associated with prolonged exposures to high air temperatures that usually occur in the summer months of July and August in Tokyo, Japan. Also during July and August, residents of Tokyo are often exposed simultaneously to high concentrations of air pollutants. To assess the impacts of these combined exposures, daily numbers of heat stroke emergency transport cases/million residents for Tokyo were stratified by gender and three groups: 0-14, 15-64; and > 65 years of age, for the months of July and August in 1980-1995. A regression model was constructed using daily maximum temperature (Tmax) and daily average concentrations of NO2 and O3 as model covariates. Classification indices were added to make it possible to compare the expected number of heat stroke cases by age and gender. Lag times of 1-4 days in Tmax and air quality covariates and terms to account for interactions between pairs of model covariates were also included as additional risk factors. Generalized linear models (GLMs), assuming a Poisson error structure for heat stroke emergency transport cases, were used to determine which covariates were significant risk factors for heat stroke for the three age groups of males and females. Same-day Tmax and concentrations of NO2 were the most significant risk factors for heat stroke in all age groups of males and females. The number of heat stroke emergency transport cases/million residents was greater in males than in females in the same age groups. The smallest number of heat stroke emergency transport cases/million residents occurred for females 0-14 years of age and the greatest number of heat stroke emergency transport cases/million residents occurred for males > 65 years of age.

[Analysis of clinical questionnaire of Tokyo Metropolitan Government for Sjogren's syndrome].

Sjögren's syndrome is not one of the forty-five diseases included in the incurable disease treatment research project funded by the government. However, it is approved as a designated incurable disease for government funded treatment in Tokyo. The criteria of approval are based on diagnostic criteria revised in 1999. Complications and other factors are taken into consideration in the recognition standard established independently in Tokyo. In this study we examined the recognition standard of approval and their significance by analyzing the clinical questionnaires of patients with Sjögren's syndrome in Tokyo through 2003. The major objective of the recognition standard (2003) appears to have been the approval of subjects with severe Sjögren's syndrome with such conditions as advanced glandular involvement xerosis affecting their daily lives and visceral complications. However, examinations using biopsy, salivary gland scintigraphy, and contrast sialography can only be performed at an institution with appropriate facilities. For an approval process that is impartial and accomplished with ease, we believe that combinations of simple multiple tests should be considered as auxiliary items which could be substituted for the aforementioned examinations. In addition, future investigation is necessary to examine the high cost incurred in requiring findings within six months for an approval application, performing of annual invasive tests, and the validity of the process for determining the approved or unapproved status using the present clinical questionnaire profiles.

[Estimation of patient visit rate and incidence of keratoconus in the 23 wards of Tokyo].

PURPOSE: To estimate the patient visit rate and the incidence of keratoconus in the 23 Wards of Tokyo. SUBJECTS AND METHODS: We searched for keratoconus patients with questionnaires sent to 141 hospitals in the 23 Wards of Tokyo. The patient visit rate was estimated using the number of patients from 15 to 44 years old. The incidence was estimated from the number of patients in whom the onset of the disease was at the age 15 to 34. RESULTS: Three hundred and twenty-five patients (205 males and 120 females) were found during a survey of three months. The return rate of the questionnaires was 49.6%. Male/female ratio was 1.7/1.0. The patient visit rate of keratoconus during three months was estimated to be 12.4 x 10(-5) (1/8,100) in males and 6.7 x 10(-5) (1/15,000) in females. The annual incidence was estimated to be 12.1 x 10(-5) (1/8,300) in males and 5.6 x 10(-5) (1/17,900) in females. 6.5% of the patients were given corneal transplants. CONCLUSION: We estimated the patient visit rate and the incidence of keratoconus on the basis of the survey of hospitals of the 23 Wards of Tokyo during a 3-month period. Compared with the previous reports, our data suggests that the proportion of women is increasing in patients with keratoconus, and that the ratio of patients who have to receive corneal transplant therapy are decreasing. A large-scale epidemiological study is required.

[Geospatial analysis of tuberculosis in Tokyo].

PURPOSE: To assess geographic variations in the incidence of tuberculosis in Tokyo. METHODS: Using information on tuberculosis incidence, 2000 to 2002, from the annual notification report, patients were categorized into 8 groups by sex and age. We then calculated the Standardized Incidence Ratio (SIR) for each of the 23 wards in Tokyo. The SIR map was described by spatial interpolation and evaluated by cross validation. Spatial scan statistics were used to detect the significance of high-risk areas across the region. We compare this with the proportional distribution of those receiving public assistance and according to the SIR. RESULTS: The geographic variations of SIR did not show a uniform pattern for each group. Spatial scan statistics clearly identified locations, that were significantly high for male groups over 20 years old. Groups under 20 years old and all female groups did not produce high incidence cluster, which are likely to demonstrate spatial features of the proportion of those receiving public assistance. CONCLUSIONS: The geographic distribution of the proportion of those receiving public assistance should impact upon the geographic distribution of the high incidence clusters. However, considering the results of the young age group and each female group, we suggest that recent infection risk among local habitants was almost uniform, with a slightly higher tendency in urban locations of Tokyo.

Recovery of circadian rhythm of plasma cortisol levels after a 3-day trip between Tokyo and San Francisco.

The objective of the present study is to investigate the change in the circadian rhythm of plasma cortisol levels following a 3-d trip and a westbound return flight from San Francisco (SFO) to Tokyo. Six healthy male students volunteered for the project. Plasma cortisol levels were monitored 4 times daily for 13 d during the experimental period. In Tokyo, the baseline pattern of cortisol concentrations showed the classical diurnal profile. During the 50-h stay in SFO, phase advancement of 8 h was observed for the initial 32 h. This was followed by a disturbance in the profile for the remaining 18-h period. After returning to Tokyo, the subjects exhibited the original diurnal pattern beginning Day 1. However, complete resynchronization was not apparent until Day 4 after the return.
