[Evolution and adverse effects in patients with remittent-recurrent multiple sclerosis treated with interferon beta 1b. The influence of patient's weight and height].

BACKGROUND: The aim of the present study was to determine whether the adverse effects (AE) of interferon beta 1b (IFNb-1b) are related to patient weight, height and body surface area. Moreover, whether the basal incapacity and/or some of these AE may be related to the evolutive prognosis were also studied. METHODS: Twenty-nine females and 14 males with remittent-recurrent multiple sclerosis treated with IFNb-1b were studied. The clinical data and the AE, were compared with the number of outbreaks and the progression of incapacity over the first year of treatment in patients with high weight, height and body surface area with respect to those under the mean. RESULTS: Although the presence of fever was similar in the two groups during the first month (8/19 in the high weight patients and 11/24 in the low weight group), in the following months, overall, the low weight patients continued to present fever. Fever during the first month was associated with a lower number of outbreaks at one year of evolution (0.04 +/- 0.1, n = 19 vs 0.30 +/- 0.5, n = 24; p = 0.04). Furthermore, the patients with lesser incapacity presented a better evolution than those with more incapacity, particularly at 2 years (0.21 +/- 1.3 vs 1.36 +/- 0.9; p = 0.05). CONCLUSIONS: During the first month of treatment fever is very frequent in all the patients regardless of weight and fever was correlated with a lower number of outbreaks at one year of evolution. From the third month fever was less frequent in the lower weight patients group.

[Clinical and immunochemical characteristics of remittent multiple sclerosis].

Immunochemical monitoring was used to examine 37 patients with remittent multiple sclerosis (MS) for 3 years. There were changes in the blood concentrations of the supernatants IL-1 beta, IL-2, IL-6, alpha-TNF, in the serum levels of the adhesion molecule sVCAM, the neurospecific proteins GFAP and alpha 2GP. Only does active MS show a simultaneous increase in the serum concentrations of SVCAM and GFAP and alpha 2GP, which may be used as a test for the objectification of MS aggravation, the efficiency of therapy. The revealed higher secretion of antiinflammatory of cytokines at all disease stages, no recovery of blood-brain barrier responsiveness and at remission provide evidence for the likelihood of a permanent immunopathological process in MS with prevailing inflammatory and destructive changes during a clinical exacerbation and of the induction of active astrogliosis at its remission.

Quality of life in geriatric depression: a comparison of remitters, partial responders, and nonresponders.

The authors examined patterns of improvement in quality of life (QOL) in elderly patients with recurrent major depression (MDD) after acute treatment. One hundred elderly (age 60-88 years) patients with recurrent MDD were randomized to receive either bupropion sustained-release (100 mg-300 mg/day) or paroxetine (10 mg-40 mg/day) for 6 weeks. Treatment with both paroxetine and bupropion was associated with improvements in QOL. Lower perceived Physical- and Social-Functioning QOL ratings at baseline were associated with lower treatment response. Improvement in depression symptom ratings correlated significantly with improvement in QOL on many domains, but accounted for less than one-quarter of the total variance. Remitters showed significantly (P<0.001) greater improvement than both Partial Responders and Nonresponders on various measures. Findings support the importance of treating elderly depressed patients to full remission to maximize impact on both emotional and physical QOL domains.

The remittance advice, auditing for compliance.

Resubmitting claims can be costly in terms of both time and lost revenue. The remittance advice and return to provider reports received by laboratories from carriers/intermediaries are an excellent source for investigating compliance matters. The Department of Health and Human Services, Office of Inspector General, has estimated that laboratories account for 4.38% of the total improper payments paid by the Medicare program for fiscal year 1998 (1). This article will illustrate how the information in these reports can be used to assist your laboratory in capturing the reimbursement it is entitled to.

Migration, remittances and development: a study of Caribbean cane cutters in Florida.

The results of a 1981 survey of 302 Caribbean sugarcane cutters who were temporary immigrants in Florida are presented. The focus is on remittances to the islands of origin. The results provide "no evidence that seasonal stateside employment expands agricultural output, or enhances the productive capacity of small farmers in the Caribbean."

Hemorheologic changes in patients with a history of stroke or cerebral remittent ischemic attacks.

Hemorheologic parameters were measured in cerebrovascular patients with a history of stroke or recurrent ischemic attacks in a quiescent phase of the disease. All patients were submitted to aortic arch angiography and then classified according to the presence or absence of detectable arterial lesions. In comparison with a group of control subjects, cerebrovascular patients had significantly elevated levels of blood and plasma viscosity, fibrinogen, factors VIII:C and VIIIR:Ag and lowered values of erythrocyte filtration. Blood viscosity was increased and erythrocyte filtration reduced only in male patients. Elderly patients had higher blood viscosity and fibrinogen levels, and those with a history of stroke also showed the highest hematocrit and plasma viscosity values. Patients with a stroke history compared with those affected by remittent ischemic attacks, and patients with positive in comparison with those with negative angiograms were characterized only by higher fibrinogen (and sometimes factor VIII) levels, but not by other differences in hemorheologic values. It is concluded that changes in hemorheologic values consistent with hyperviscosity are a feature of cerebrovascular patients also in the quiescent phase of the disease and not only as a result of acute ischemic attacks. These changes are however most frequent in males and in elderly subjects, while they seem to be almost evenly distributed in patients with or without evidence of arterial disease.

[Interferon-beta 1B in the treatment of remittent-recurrent multiple sclerosis. Clinical experience of the Valencia group. Multiple sclerosis study group of the city of Valencia].

INTRODUCTION: Recombinant beta-1B interferon has been used in our country since the end of 1995 for treatment of remittent-recurrent multiple sclerosis (RRME). At the present time there are still gaps in the effectiveness, side-effects and management of patients on this treatment. OBJECTIVE: To determine the evolution, side-effects and practical difficulties involved in the management of patients with RRME given interferon beta-1B and prospectively followed up one year after starting treatment. MATERIAL AND METHODS: This paper gives a prospective description of 41 treated patients with an average follow-up of 254 days. RESULTS: The patients tolerated and followed their treatment well. Treatment was only withdrawn in two cases. We observed a 40% reduction in the annual incidence of exacerbations (with respect to previous years) and there was improvement on the amplified dysfunction scale of Kurtzke in the patients whose exacerbations were treated with megadoses of steroids. The side-effects were those we expected, apart from an increase in thyroid complications and the appearance of a depressive syndrome. Treatment did not have to be changed because of the side-effects. In two patients with serious side-effects (thyroiditis and a depressive syndrome) the symptoms disappeared when treatment was stopped. CONCLUSION: The results and side-effects were similar to those published. We had fewer patients who withdrew from the trial. There were no cases of patients not following the prescribed treatment.
