[Clinical and roentgenological characteristics of synostosis frontal plagiocephaly, induced by early synostosis of cranio-cerebral sutures].

In this study authors demonstrate description and analysis of clinical and roentgenological presentations of early synostosis of cranio-cerebral sutures in children. Roentgenological description of cranio-cerebral sutures in infants showed up their significant narrowing and the increase in shadow density, early appearance of ossifications zones in the peripheral parts of the sutures, diffuse increase of cranial bones thickness because of apposition osteogenesis disorder. Results of the study show that many alterations in the cerebral cranium are accompanied with malformations of facial skeleton.

[Roentgenologic characteristics of bronchopulmonary carcinoma].

The roentgenologic characteristics of bronchopulmonary carcinoma were studied in 578 patients in whom the disease had been diagnosed by bronchoscopy and confirmed by cytopathologic finding. Each of the pulmonary disease may imitate bronchopulmonary carcinoma and vice versa. Since there is no roentgenologic patognomonic sign for bronchopulmonary carcinoma the sorts and distribution of characteristic X-ray signs were examined. In our group of patients in 67% of cases the right side was involved. The leading point in X-ray was in 52.8% of cases the pathologically enlarged hilus unilaterally and bilaterally. In patients with periphery located bronchopulmonary carcinoma the most frequent X-ray sign was unclearly limited shadow in 41.3% of cases. In patients with centrally located bronchopulmonary carcinoma in 6.4% of cases the normal X-ray sign was found. However, in none of 177 patients with periphery bronchopulmonary carcinoma the normal X-ray was found.

[Histiocytosis X: roentgenological findings in 23 cases].

The authors describe the clinical, roentgenological findings, especially the oral manifestation and differential diagnosis of histiocytosis X in 23 cases. It is shown that the clinical and roentgenological differential diagnosis is problematically.

[Clinico-roentgenological and physiological evaluation of the spine in patients with juvenile kyphosis].

203 patients with juvenile kyphosis, aged 11-16 years, have been subjected to the complex examination, including ++clinico-roentgenologic and physiologic methods. Along with the cardinal signs, common for juvenile kyphosis diagnosis and characteristic of the dystrophic process (wedge distortion, osteoporosis of vertebra bodies and fragmentation of their apophyses, dedifferentiation of the bone structure, height reduction and pathologic restructuring of intervertebral disks following the pattern of fibrosis, presence of discal hernia), the signs of bone ++ dysplasia of spine and skeleton in general should be singled out of the roentgenologic characteristic of disease which determine the variants of pathologic process course and treatment tactics. The presence of dysontogenesis signs in patients with juvenile kyphosis is suggested.

[Roentgenological diagnosis of silicotuberculosis].

Possibilities of roentgenologic method of examination with the use of roentgenography and tomography in the diagnosis of current forms of silicotuberculosis and tuberculosis in workers of the industries highly exposed to silicosis are analysed. ++Clinico-roentgenological+ pictures of 305 patients were studied in their dynamics for the period of 3 to 10 years. Being characterized by a benign course, silicotuberculosis is most often diagnosed in persons who left their work. A follow-up at least at 6-month intervals is of great importance in assessing its activity.

[The relationship of clinical severity and roentgenologic findings in beta thalassemia].

There were altogether 68 patients suffered from beta-thalassemia in the Veterans General Hospital from 1979 to 1986. However only 18 patients had abnormal roentgenologic findings. They were 7 males and 11 females. Their ages ranged from 8 months to 47 years with an average of 13 years. Clinically beta-thalassemia was divided into 3 types: 1) thalassemia major, 2) thalassemia intermediate, 3) thalassemia minor. The osteoporosis, hepatosplenomegaly, and extramedullary hematopoiesis with pseudo-tumor formation. We concluded that the roentgenologic manifestation of the patient was more in patients with major or intermediate type.

[Roentgenological manifestations of metatuberculous coxarthrosis in adults].

The author has studied the clinical and roentgenologic findings of 68 patients (33 males and 35 females) aged 19 to 60 who had tuberculosis of the hip joint or of the greater trochanter and were cured by conservative means or with palliative and late surgical interventions. The disturbances in the trophics and in the formation of the joint acquired during the process of inflammation as well as the anatomic losses of destructive character became the basis for the development of metatuberculous coxarthroses at stages II and III in these patients, which occurred under the influence of statodynamic loads in pathologic conditions. The roentgenologic picture of metatuberculous coxarthroses is polymorphic; it depends to a certain extent on the age of the beginning of the tuberculous process and is accompanied by pronounced productive reactions of the bone tissue. In essence it corresponds to the manifestations of coxarthroses of other origins, but it also has some peculiarities consisting mainly in the presence of traces of trophic changes in the bone tissue and in the residual signs of the destruction that had taken place which are characteristic of tuberculosis of bones and joints.

[Evaluation of genetic risk of radiation during roentgenologic examination of women of reproductive age].

On the strength of the literary data an evaluation of genetic exposure-associated risk during roentgenologic examination of women of the reproductive age is presented. It is demonstrated that the degree of genetic risk depends on women's age at the moment of the roentgenologic examination and the amount of the gonadal dose. Identification of the high-risk exposed populations has been substantiated.

[Comparative clinico-roentgenological characteristics of degenerative-dystrophic changes in various segments of the spine].

A combined clinico-roentgenological investigation was performed in 306 patients with degenerative-dystrophic changes in the vertebral column. Roentgenological signs and peculiarities of degenerative-dystrophic changes of different vertebral segments are described with special reference to their earlier stages. Established were the causes of neurological signs connected with degenerative-dystrophic changes of the vertebral column which have clinical implications.

[Clinico-roentgenologic diagnosis of chronic venous insufficiency of the ovaries].

The paper is devoted to clinico-roentgenological correlations of venous reno-gonadal hemodynamics in 168 women of reproductive age (151 women with infertility, habitual abortion, disordered menstrual function and 17 patients without disorders of the reproductive tract). Clinico-laboratory investigation was followed by visceral and parietal phlebography for the detection of a pathological venous reflux into the ovarian pampiniform plexus. A retrograde blood flow along the internal ovarian vein with the development of venostasis and secondary pelvic varicocele was diagnosed in 67 women with disturbed reproductive function. In 17 women without disorders of reproductive function a reno-gonadal venous reflux was undetectable. Clinico-roentgenological semiotics of chronic venous ovarian insufficiency was analyzed. Infertility was shown to be the main clinical manifestation of ovarian varicocele.

[Correlations between various cardiometric and functional parameters in dilated cardiomyopathy and alcoholic lesion of the heart (data of roentgenological and echocardiographic studies)].

Volumetric changes of the heart and its left ventricle, as well as myocardial contractility were assessed in 50 patients with dilatation cardiomyopathy (DCMP) and 16 patients with acute alcoholic heart damage (AHD), on the basis of roentgenologic (cardiac shape and volume, left ventricular and aortic pulsation, respiratory volumetric variations, pulmonary circulation) and echocardiographic (end-diastolic size and volume, systolic increment of posterior wall thickness, ejection fraction and left-ventricular shortening) data. Both DCMP and acute AHD were shown to be associated with total enlargement of the heart, diffuse impairment of myocardial contractility, considerably depressed left ventricular pumping function, early congestion in the systemic circulation network coupled with minor roentgenologic signs of congestion in pulmonary circulation.

[Roentgenology in rheumatology].

The paper takes a retrospective view back to the historical beginning of roentgenology. The development of radiology of bone and joints is high-lighted with special respect of rheumatology. Completeness was not intended regarding the size of this paper. It is shown, especially to our younger colleagues, how hard it was in the past 90 years to achieve our today-knowledge in x-ray-diagnostic. Three factors determined Roentgenology to become an indispensable method. The continuing development and improving of efficient x-ray-machines. The development and improving of investigation methods. The scientific workup of radiological signs leading to radiomorphologic features. Some milestones of technical development of conventional radiology are listed up. Also, innovations in microelectronics. Computed tomography and Magnetic Resonance are listed up. Remarks are made about costs of "big machines" and about declining knowledge in conventional radiology in young american radiologists. Recommendations are made for efficient collaboration of rheumatologists and radiologists.

[Clinico-roentgenological characteristics of thromboembolism of the pulmonary arteries in patients with rheumatism].

The authors have analyzed 31 patients (10.2% of the total number of patients with rheumatic fever) to reveal the clinical and roentgenological signs of thromboembolism of the pulmonary arteries. Conditions for the development of pulmonary thromboembolism, peculiarities of its clinical picture and course have been described; attention has been drawn to the roentgenological signs of pulmonary occlusion. The authors believe that the definition of the 5 main pathogenetic variants of a course of thromboembolism of the pulmonary arteries in patients with rheumatic fever can serve as the basis for early diagnosis.

[Clinico-roentgenologic characteristics of giant cell tumors of the bone].

The data obtained from 114 case histories of giant cell tumor were analysed on the EC-1010 type computer to identify clinico-roentgenological factors influencing the frequency of relapse and metastasis development. Such factors as tumor localization in tibia, local lesions in the abnormally thin cortical layer detectable by X-ray examination, the lytic form of tumor and tumor excochleation proved to be unfavorable. In the analysis of cases of metastatic spread, information could be acquired only from roentgenologic characteristics which permitted complete decay of the cortical layer, the presence of soft-tissue component in tumor and pathologic fracture to be identified as manifestations of caries.

[Roentgenological diagnosis of abdominal abscesses].

An analysis of clinico-roentgenological data of 74 patients has been made. The patients were operated upon for acute appendicitis (43) and cholecystitis (31) and their postoperative period was complicated by local peritonitis. The importance of indirect and roentgenological symptoms for diagnosis of abscesses in the abdominal cavity is stressed.

[Roentgenologic characteristics of Gaucher's disease (author's transl)].

Gaucher's disease is a rare metabolic disorder in which there is an abnormal accumulation of cerebrosides in the reticuloendothelial system due to a deficit of beta-glucuronidase. Three patients with this disease, 8, 14, and 23 years old, were studied. The purpose of this paper is to point out some of the roentgenologic findings, such as areas of femoral osteolysis, renal venous thrombosis, and in one patient, sclerosis of the sacroiliac joint. Splenomegaly and alteration of the tubular bony structure of the distal end of the femur were present in all three cases. The roentgenologic images of the disease are discussed. The osteoarticular system is the most often affected. Osteolysis is the basic lesion with expansion of the bone marrow and sclerosis. The earliest symptom and sometimes the only one is the Erlenmeyer flask deformity or widening of the distal portion of the femur. Sometimes there is a high degree of deossification without definite osteolytic lesions. Pathologic fractures and fractures caused by pressure of the vertebral bodies are common. Aseptic femoral necrosis are often present. Other less frequent sites of osteoarticular pathology are the skull, jaws, ribs, and sacroiliac joints. Splenomegaly is the most frequent visceral lesion. Other organs sometimes affected are the liver, kidneys, heart, lymphatic system, and lungs, with nodular or basal reticular infiltrations.

Small bowel transit time and roentgenological changes of the intestinal mucosa after jejunoileostomy in obese patients.

In ten patients, operated jejunoileostomy and ileocolostmy because of longstanding obesity resistant to conventional therapy, roentgenological examinations of the small bowel were performed before and after the operation. Seven patients were examined preoperatively, and all were investigated at least twice during a period from 1 to 38 months postoperatively. Preoperatively, the passage time pylorus-cecum was 80-270 min, and postoperatively, it varied between 1 and 30 min. There was no tendency to slowing of the gastrointestinal passage with time. Immediately postoperatively, the patients had 6 to 12 loose stools daily. At investigations performed 6-38 months after the operation, the stool frequency had diminished to 1-4 a day, and the patients had lost 21 to 98 kg in weight. Roentgenologically, an increased thickness of the mucosal folds of the small bowel could be demonstrated in 9 of the 10 patients. The results suggest that in patients with jejunoileostomy, the gastrointestinal passage time is decreased. The bowel function tends to normalize with time, but the passage time remains shorter than normal. Compensatory changes in the intestinal mucosa occur.

Roentgenologic results of total hip arthroplasty. A ten-year follow-up study.

A ten-year roentgenologic follow-up study was made of the results of 326 total hip arthroplasties in 256 patients. Seventy-two percent of the living patients are included in the study. Clinical results at ten years were good in 87% of the patients, fair in 7%, and poor in 6%. The incidence of femoral component loosening was 9% at ten years. There was no correlation between femoral component loosening and calcar resorption. The incidence of acetabular component loosening at ten years was 7.9%. Of six hips with calcar resorption greater than 5 mm, three had loose acetabula. Most of these components shown to be loose roentgenologically were clinically asymptomatic. One and one-half percent were revised because of loosening.
