Projects grants for community health services; proposed rulemaking--Public Health Service. Notice of proposed rulemaking.

The Assistant Secretary for Health with the approval of the Secretary of Health and Human Services proposes to revise the regulations governing the Community Health Centers grant program. The Health Services and Centers Amendments of 1978 made a number of changes in the statutory requirements governing the operation of the centers. The Amendments, among other things, change pharmacy services from supplemental to primary health services, establish priority for certain supplemental health services, provide an incentive for maximized collection of fees, permit conversion of certain centers from fee for service to prepaid operations, and change the governing board requirements or public centers. The proposed revisions are intended to revise the present regulations consistent with the revised statutory provisions.

Public health services use in a mountainous area, Vietnam: implications for health for policy.

AIMS: The aim of this study is to describe the use of public health services in different social and ethnic groups and to explore the implementation of user fee exemption in a mountainous area in Vietnam. METHODS: A cross-sectional household survey with a structured questionnaire and a four-week diary were used to collect information on illnesses, health seeking behaviour and socioeconomic factors. Three communes in a mountainous district in Northern Vietnam were selected and a random sample of 1,452 individuals in 300 households was drawn. RESULTS: Self-medication was most common (57%) while 30% used public health services when suffering from a health problem. Persons living far from health services attended public health services less frequently than the others (adjusted OR = 0.28; 95% CI 0.15-0.51). This was especially the case for ethnic minorities who were less likely to use public health services than the others were (adjusted OR = 0.47; 95% CI 0.25-0.87). Persons with mild conditions tended to use public health services less than those with more severe conditions (OR = 0.19; 95% CI 0.10-0.37). Health services use was similar among women and men, but the total expenditure per visit was higher for men. Almost no patients supposed to get free attendance had been exempted from user fees. CONCLUSIONS: It was found that there was a geographical inequity in use of public health services while there was relatively equal use of these services between social, gender, and ethnic groups. Long distance in combination with failure of the fee exemption may increase inequity in use of health services in remote and isolated areas. These observations contribute to the basis for implementation of the Vietnamese health policy, emphasizing efficiency and equity.

Linking high-risk, low-income, pregnant women to public health services.

Utilization of public health services by high-risk pregnant women is contingent on women's awareness of those services. This article reports on an evaluation of public health promotional methods and compares high-risk pregnant women users of public health services to eligible nonusers. For women who used public health services, the two most frequently reported sources of information were a public health nurse or a friend. Users of services in contrast with nonusers tended to have had fewer pregnancies, lower income, and use WIC. They were also less likely to attend church and to have more contact with their child's school. These findings can assist agencies in planning outreach activities.

[From a service charter to a public health service charter].

The official publication of the Charter of Public Health Service in Italy (19th may 1995) prompted a series of positive reactions, as it has been seen as an indicator of a new attention given to consumers' rights. The document is examined by underlining the aspects which could provide a specific opportunity for the nursing personnel to develop a closer role of ally for patients' rights.

Visual localisation of community health needs to rational decision-making in public health services.

The objectives were to visualise the locations of community health needs and to develop a community health needs assessment geographic information system (GIS) for rational decision-making in public health services. We compiled census data, digital data of basic planning maps, digital data of topographic maps, contents of registers of medical and welfare facilities, and statistics of establishments into a geographical database; visualised geographical distributions of specific community health needs by integrating sets of indicators to reflect individual needs; and quantified their clustering by the nearest neighbour method. The database aggregated 3400 items of demographic, life and environmental factors. Thematic maps and clustering values showed different patterns of geographical distribution of the individual community needs. Means to match needs with services in smaller geographical units were discussed. This GIS will support appropriate resource allocation, intersectoral collaboration and greater transparency in planning and implementing services, by visualising locations of community health needs.

Public health, epidemiology and health services.

Public health and the wider aspects of health administration are connected by their common dependence on the methods of epidemiology. Epidemiology is defined herein as the discipline of occurrence research in medicine. As such, it represents a body of methods rather than knowledge, and is as much applicable to study of the occurrence of phenomena relevant to health services as to phenomena relevant to disease aetiology and prevention. In consequence, much of the information base necessary to the development of health policy is obtained by the use of epidemiological methods. Health services planning and program development depend on an understanding of the relationship between services and the population that they serve, an understanding which is provided by the application of epidemiological concepts. The ongoing delivery of programs in a manner which optimises the use of health resources depends on the ability to respond to changes in the health services environment. Recognition of the changes demanding a response depends on continuing surveillance of the "environment" by use of epidemiological techniques. Finally, the evaluation of health programs against their objectives depends to a substantial degree on the use of epidemiological methods. The discipline of epidemiology arose in public health, but it now belongs to the whole of the health system.

The revitalization of the Public Health Service Commissioned Corps.

The Public Health Service (PHS) is the second oldest uniformed service of the United States; its tradition commenced with the establishment of the Marine Hospital Service in 1798. Congress, in 1889, established the United States Public Health Service Commissioned Corps under the aegis of the Treasury. The Corps was created as a uniformed nonmilitary service with a distinct uniform, insignia, and with titles, pay, and retirement protocols that corresponded to those of the uniformed military services (the Armed Forces). Initially the health care system of the country, and Commissioned Corps members, were concerned with infectious and vitamin-deficiency diseases; more recently the nation's medical community has focused on cardiovascular diseases, cancer, and AIDS. A comprehensive revitalization of the Commissioned Corps began in April 1987. The intent was to restore the Commissioned Corps to its traditional leadership role as a cadre of mobile, compassionate experts ensuring the nation's health. The revitalization activities have been successful. The Commissioned Corps has approximately 5,500 active duty officers. The Surgeon General directed the development of career tracks for 11 categories of commissioned officers to increase the opportunities for professional development within the PHS and thus increase retention and professional growth. The theme for the 1989 celebration of the centennial of the Commissioned Corps is "a century of service with distinction." A hundred years from now, at the bicentennial of the Commissioned Corps, the current Surgeon General would like it to be said that the Public Health Service has had "two centuries of service with distinction."
