[Clinical effectiveness of prospidin in systemic lupus erythematosus (results of a 6-month follow-up)].

The efficacy of the new Soviet antirheumatoid drug prospidin (the long-term treatment dose 1.4-6.0 g) was studied in 22 patients with significant lupus erythematosus (SLE). As a result of prospidin therapy administered for a month, 3 patients considerably improved and 19 patients improved. For 6 months the maintenance therapy was given to 15 patients. Considerable improvement was recorded in 4 and improvement in 9 patients. No effect was marked in one patient and one female patient died. The drug was tolerated well. No side effects requiring prospidin withdrawal were recorded. Prospidin exerted the most powerful effect with respect to lupoid nephritis, the articulation syndrome, and cytopenias. In accordance with the clinical improvement, the drug was established to return the immunological shifts common to SLE to normal. The drug may be viewed as a new basic agent for the treatment of SLE.

[Indirect endolymphatic chemotherapy of patients with lymphoma].

Treated were 103 patients with malignant lymphomas (lymphogranulomatosis--87; non-Hodgkin lymphomas--16 patients). The treatment was complex including chemotherapy by means of endolymphatic administration of cyclophosphane, prospidin, methotrexate). The technique may be widely used and may be associated with other treatment methods. The efficacy of the method is substantiated and emphasized.

[Means of improving the chemoradiotherapy of small cell cancer of the lung].

The report deals with an analysis of the results of treatment of 84 patients suffering small cell lung cancer. Four regimens of combination chemotherapy as well as radiation were used. Complete plus partial regression rates ranged 18-81% (mean-42%) following chemotherapy and 73-87% after radiation treatment. Platidiam-containing schemes proved most effective. Prophylactic cranial irradiation was followed by increased survival. A correlation between extent of tumor and effectiveness of treatment was established.

[Toxicity of anti-tumor preparations after intracranial, subarachnoid and suboccipital administration (experimental study)].

Experiments were conducted on animals to study intrathecal injection of agents differing in the mechanism of action: variamycin, mitramycin, reumycin, proxyphein, and prospidin. Their acute toxicity in a single intracerebral injection and chronic toxicity in repeated suboccipital injection were studied. The brains of animals who died or were killed were studied pathomorphologically in different periods. Intracerebrally and suboccipitally injected variamycin and mitramycin induced marked toxic reactions, even the death of animals, and cannot be recommended for the neurooncological clinic. The antineoplastic reumycin, proxyphein, and prospidin cause no toxic reactions of the brain and meninges on injection by the above mentioned methods; they are recommended for approbation in the neurooncological clinic for the treatment of patients with malignant tumors of the brain by injection into the system of the cerebrospinal fluid.

[Attenuation of protein-nuclein interactions induced by chemical mutagens in a human fibroblast culture].

Using nucleoprotein celite chromatography, it has been shown that DNA of human cultured fibroblasts was strongly bound to the proteins of the nuclear matrix. N-methyl-N-nitrosourea and prospidin-active antitumor agents with marked mutagenic action--were shown to attenuate the interactions, which appeared to be partially sensitive in cultured fibroblasts from patients with Down's syndrome.

[Chemotherapy and combined chemo- and radiotherapy of small cell lung cancer].

Objective response of small cell lung cancer to different modalities of chemo-and radiation chemotherapy was recorded in 68-94% while complete regression of tumor--in 22-50% of a total of 166 cases. Complete regression was registered in approximately 33% of cases of localized tumor process and in 16% of cases of expanded process. The survival median for different treatment modalities was 13.7-22.5 months in cases of complete regression and 8.7-12.4 months for partial regression. The cyclophosphamide + adriamycin + methotrexate treatment scheme proved effective in 87% of localized tumor (complete regression--27%). As a result of treatment with the said drugs plus radiation, objective response was obtained in 95%, with complete regression being registered in 68% of the latter group. Therefore, radiation should be an indispensable component of treatment for small cell lung cancer. A two year relapse- and metastasis--free period was observed in 18% cases of localized process and a five year period--in 9%.

[A new basic drug prospidin in the treatment of rheumatoid arthritis. III. Comparative analysis of prospidin and cyclophosphamide].

A comparative study of the cytostatic drugs prospidin and cyclophosphamide used in equal doses for rheumatoid arthritis (RA) is reported. Clinical and immunologic effects were determined, and the nature and incidence of side effects and complications were compared. Prospidin showed a more pronounced antirheumatic effect, making for a smaller daily requirement of nonsteroid anti-inflammatory agents or hormones in hormone-dependent cases. Unlike cyclophosphamide, prospidin was not associated with severe side effects and complications precluding further use of the drug. Both drugs demonstrated a regulatory effect on RA-associated immunologic disorders.

[Efficacy of the conservative treatment of patients with inoperable esophageal cancer].

The results of gamma-beam therapy of esophageal cancer leave much to be desired. However betatron therapy improves them. To compare the results of esophageal cancer therapy 4 therapeutic modalities were employed: the 1st group--chemoradiotherapy (methotrexate at a weekly dose of 45 mg), the 2nd group--chemoradiotherapy (methotrexate at a dose of 10 mg alternating with prospidin at a dose of 200 mg), the 3rd group--radiotherapy (inhibitory radiation), the 4th group--radiotherapy (gamma-beam therapy). Higher efficacy of chemoradiotherapy as compared to radiotherapy only was established.

[Combined treatment of patients with metastases of skin melanoma using cryotherapy].

Cryotherapy or cryotherapy plus intratumoral injection of BCG vaccine were used in combination with either prospidin or imidazole-carboxamide and multidrug chemotherapy including vincristine, dactinomycin and CCNU for the treatment of 106 cases of cutaneous and subcutaneous metastases of skin malignant melanoma. Five-year survival for the entire group was 36 +/- 5.1%. The best results were obtained for prospidin (43.6 +/- 8.6%) (imidazole-carboxamide--31.3 +/- 9.4% and combined chemotherapy--30.5 +/- 5.8%). Imidazole-carboxamide should be used in conjunction with cryotherapy plus intratumoral BCG vaccine. Procedure of topical treatment did not significantly influence the effectiveness of prospidin or combination chemotherapy.

[Assessment of the effect of prospidin and cyclophosphane on the indices of the T and B immune responses in vitro].

Prospidin was shown to produce a decrease of receptors on T- and B-lymphocytes and T-subpopulations, to inhibit migration of leucocytes under the influence of the antigenic stimulus, to reduce the cytopathic activity of lymphocytes and the level of secreted immunoglobulins of the main classes. The degree of prospidin immunodepressive effect is compared with that of cyclophosphane.

[The new basic agent prospidin in the therapy of rheumatoid arthritis. An evaluation of the clinical effectiveness and the mechanism of action of prospidin].

A therapeutic effect of prospidine, a Soviet antitumor drug, was studied on the basis of clinicoimmunological correlations in 74 patients with proved rheumatoid arthritis (RA) who were resistant to well known basic drugs. It was administered parenterally and intraarticularly according to the methods developed by the authors, and its effect on the indices of T- and B-immune response was studied. Prospidine was shown to possess a true and high (91.8%) antirheumatic activity producing an analgesic, antiexudative and antiproliferative effect; it was capable of lessening hormone dependence and causing remission bringing about no serious side-effects which could require discontinuation of therapy. At the same time the use of the drug resulted in an elevation of the level of T-suppressors, a decrease in the amount of B-cells, T-helpers, titers of the rheumatoid factor, antisynovial antibodies, cytopathic and lymphokine synthesizing activity. Its mechanism of action and indications for use were discussed. Prospidine was considered as a drug of choice in drug therapy of rheumatoid arthritis.

[Immune status during the combined use of microwave hyperthermia and prospidin in animals with transplantable tumors].

Experiments were conducted to study the time course of changes of some immunological indices in animals after a combined use of local hyperthermia and course use of prospidin. Local microwave hyperthermia used at the beginning of a course of prospidin caused a decrease in some indices of immunity which was much lower than in case of the use of only prospidin or hyperthermia.
