BACKGROUND: Although millions of people with schizophrenia live in betel chewing regions, the effects of betel chewing on their symptoms are unknown. Betel nut alkaloids include potent muscarinic cholinomimetics: recent research suggests that these agents may be therapeutic in schizophrenia. AIMS: To compare the primary and extrapyramidal symptom profiles and substance-using habits of betel chewing v. non-chewing people with schizophrenia. METHOD: A cross-sectional study of 70 people with schizophrenia. Symptom ratings measured by the Positive and Negative Syndrome Scale (PANSS) and Extrapyramidal Symptom Rating Scale (ESRS), and demographic and substance-use data, were compared for 40 chewers and 30 non-chewers of betel nut. RESULTS: Betel chewers with schizophrenia scored significantly lower on the positive (P = 0.001) and negative (P = 0.002) sub-scales of the PANSS than did non-chewers. There were no significant differences in extrapyramidal symptoms or tardive dyskinesia. CONCLUSIONS: Betel chewing is associated with milder symptomatology and avoidance of more harmful recreational drugs. These initial results indicate that longitudinal research is merited.

A dengue fever/dengue hemorrhagic fever (DF/DHF) outbreak in Yap State caused by dengue-4 virus was confirmed serologically and by virus isolation from serum samples collected on each of three island groups. Most DF/DHF cases occurred during a three-month period between mid-May and early August 1995. Five fatal cases, three of which were in children between the ages of four and 11, occurred between June 20 and July 26. A serosurvey conducted in late August revealed anti-dengue IgM prevalence rates of 18% on Yap, 36% on Eauripik, and 6% on Woleai. The majority of residents (93-100%) on the three islands were positive for anti-dengue IgG antibodies, indicating widespread exposure to dengue viruses. The IgG titers indicative of secondary antibody response were noted on Eauripik (6.5%) and Woleai (17%), but were rare on Yap (0.7%). Entomologic investigations implicated the native mosquito species, Aedes hensilli, a member of the Scutellaris Group of Aedes (Stegomyia), as a previously unrecognized epidemic vector of dengue viruses. Aedes hensilli was the most abundant and widespread member of Ae. (Stegomyia) in Yap State, the only species of Ae. (Stegomyia) on Woleai, and the only mosquito species present on Eauripik. New distribution records for mosquito species are reported.

A case report of 6 years old female who sustained a difficult right subtronchanteric fracture of the femur is outlined. She was treated successfully, with local materials, over the internet with assistance of the orthopaedic surgeons at the Tripler Army Medical Center in Honolulu. Pohnpei State spends 10% of the health budget in referring patients off-island for tertiary treatments and this serves less than 1% of the total population. Before the use of the internet, approximately $US1,500 per month was spend on telephone bills, for outside consultations. After connection to the internet and the consult webpage, particularly at Tripler, this cost was reduced to below $US500 per month. To date, fifty consults, via the Internet, have been sent to the TAMC Consult Webpage. The introduction of this service has resulted in cost saving in terms of referral communication and perhaps avoided unnecessary off island evacuations. The difficulties so far have been limitation of on-line access and computer illiteracy amongst physicians.
Distance education courses have been offered by the University of Guam for the past ten years, however there were no programs developed for the students of the region. In 1994, the College of Nursing and Health Sciences began teaching courses over PEACESAT (Pan Pacific Education and Communications Experiment by Satellite) every semester. The first course was written to serve Palau, the Commonwealth of the Marianas, Pohnpei, and the Marshall Islands. These jurisdictions have large community colleges. An overview of this program is given. In 1997 the courses were extended to include Palau, Yap, Chuuk, Pohnpei, Kosrae, and the Marshall islands. The organisational structure and training of the site operators and training coordinators is described. Two courses were taught in the first year. The first was 'Nutrition for Health Professionals and the second Leadership/Management for nurses. These are described, evaluated and discussed.

Little is known about the impact of cancer and the extent of cancer-related services in Kosrae. The purpose of this study, funded by the National Cancer Institute, was to document the state of cancer awareness and services in Kosrae and to begin to identify cancer-care needs. Findings suggest that cancer is the eighth-leading cause of death in Kosrae, although a number of factors contribute to a possible undercount of cancer cases. Cancer-related services are limited. A number of needs were identified, and an action plan was developed based on three priority areas: 1) establishing a cancer registry; 2) increasing public awareness about cancer risk, prevention, and detection; and 3) expanding cancer screening and detection programs.
