Neurologists and the threat of bioterrorism.

Neurologists are most likely to become involved in primarily diagnosing those bioterrorist attacks utilising botulinum toxin. Oral ingestion, or possibly inhalation, are likely routes of delivery. The characteristic descending paralysis starts in the extraocular and bulbar muscles, with associated autonomic features. Repetitive nerve stimulation usually shows an incremental muscle response. Treatment is supportive. The differential diagnosis is from naturally occurring paralysing illnesses such as Guillain-Barré syndrome, myasthenic crisis or diphtheria, from paralysing seafood neurotoxins (tetrodotoxin, saxitoxin), snake envenomation, and from chemical warfare poisoning by organophosphates. Primary neurological infections are less feasible for use as bioweapons. There are theoretical possibilities of Venezuelan equine encephalitis transmission by inhalation and secondary zoonotic transmission cycles sustained by horses and mosquitoes. Severe haemorrhagic meningitis regularly occurs in anthrax, usually in the aftermath of severe systemic disease likely to have been transmitted by spore inhalation. Panic and psychologically determined 'me-too' symptomatology are likely to pose the biggest diagnostic and management burden on neurologists handling bioterrorist attack on an institution or a random civilian population. Indeed civilian panic and disablement of institutional operations are likely to be prominent intentions of any bioterrorist attack.

The Arjenyattah epidemic. A mass phenomenon: spread and triggering factors.

A massive epidemic of psychogenic aetiology occurred in three districts of the West Bank over two weeks in March-April, 1983. It affected 949 individuals, 727 (77%) of them adolescent females. The symptoms were not accompanied by positive physical signs or by laboratory findings. The epidemiological pattern was pathognomonic of that of a psychogenic disorder. The initial trigger was probably the odour of H2S escaping from a faulty latrine in the schoolyard of the first affected school. Subsequent spread of the disease was due to psychological and extra-medical factors, including publicity by the mass media. Spread was stopped immediately after closure of schools.

Quantitative measurements of the interaction between monosialoganglioside monolayers and wheat germ agglutinin (WGA) by a quartz-crystal microbalance.

Monosialogangliosides (GM1, GM2, GM3 and GM4) were reconstituted in lipid monolayers at the air-water interface. The binding amounts and the initial binding rates of wheat germ agglutinin (WGA) to the monosialoganglioside monolayers were quantitatively studied by use of a quartz-crystal microbalance (QCM). A QCM was horizontally attached to the monolayer from the air phase, and the binding behavior (mass increase) was followed by the frequency decrease of the QCM. WGA binding affinities for the ganglioside monolayers were influenced by hydrophilic head groups of lipid matrices, densities of gangliosides, and sequences of oligosaccharide in gangliosides. Binding of WGA to the gangliosides reconstituted in a phosphatidylcholine (sphingomyelin and distearoylphosphatidylcholine) matrix was strongly suppressed, but not in a neutral glycolipids (GlcCer, GalCer, and LacCer), dipalmitoylphosphatidylethanolamine, and dipalmitoylphosphatidylethanolamine matrix. WGA showed high affinity for monolayers containing 20 mol% gangliosides, but only low affinity for 100% ganglioside monolayers. WGA preferably binds to gangliosides in the following sequence: GM3 > GM4 >> GM2 = GM1. No affinities of WGA for GM2 and GM1 were observed. The combined techniques of monolayer and QCM have the advantages of investigating recognition properties of gangliosides.

Responding to community-identified suicide clusters: statistical verification of the cluster is not the primary issue.

Establishing the presence of an epidemic is traditionally a first step in any outbreak investigation. For two reasons, however, this has not been a fruitful approach for suicide cluster investigations. First, the data necessary to statistically verify an excess number of suicidal incidents are often lacking or of poor quality. Second, and more important, when a community perceives that it is experiencing a suicide cluster, it is not immediately relevant whether the cluster is statistically significant. The perception of suicide clustering, and the highly charged emotional atmosphere associated with that perception, may dramatically heighten the potentially "contagious" effect of suicide. That the perception of clustering may itself be a risk factor for suicide distinguishes suicide clusters from all other clusters of fatal disease or illness. A community response plan should, therefore, be implemented to identify and refer persons who may be at high risk of suicide, regardless of whether the community-identified suicide cluster is statistically significant. Statistical techniques may be useful at several stages in the investigation and control of apparent suicide clusters, but statistical verification of a community-identified suicide cluster is not appropriate as a starting point for response to the cluster.

The nature of collective movements: the hidden second force.

While C. G. Jung postulated a nonpersonal, collective layer within psychic structure, it is often overlooked that he came to this conclusion from studying the effects of the content of this layer on human perception. Few other scientists have grasped the significance of Jung's point that energy from the collective layer has the power to influence, alter, or color human perception. This paper attempts to apply Jung's discovery to better understand the processes involved in collective movements when they affect considerable numbers of people. It is suggested that powerful collective movements have two sources of energy, one is the conscious aims of collective movements, while another, encompassing activated collective substance represents nonconscious contents. It is suggested that the nonconscious current acts remarkably similar to psychic forces that empower obsessions. The feminist movement is examined through these postulates as one of the powerful collective movements in today's world.

Between Marxism and psychoanalysis: antifascism and antihomosexuality in the Frankfurt School.

In their efforts to utilize individualist psychoanalysis as a tool for understanding mass behavior, the social theorists associated with the Frankfurt School increasingly came to rely on a static, essentializing construction of sexuality which ultimately led to an equation of fascism and homosexuality. Heretofore unexamined in studies of the Frankfurt School, this equation will here serve as the starting point for a fundamental critique of the concept of sexuality developed by this influential circle of Marxist thinkers. While directed at the concept of sexuality, such a critique more importantly opens up the underlying understanding of the social and psychological realms advanced by Critical Theory. Attending to the equation of homosexuality and fascism as the central point of concern, this essay will first trace the introduction of psychoanalysis into Critical Theory through Erich Fromm and then investigate the extent of Fromm's influence on the concept of sexuality propounded by his colleagues, especially Max Horkheimer and Theodor W. Adorno. Finally, it will take up a frequently overlooked essay by Herbert Marcuse which promoted a vision of sexuality radically different from that of his associates.

Mass psychogenic illness after vaccination.

When vaccines are administered to groups, the physical reactions of the recipients may be similar, causing a form of mass reaction, the mechanism for which is the same as that for mass reactions from other causes. These phenomena have been categorised as mass psychogenic illness (MPI), and have been defined as the collective occurrence of a constellation of symptoms suggestive of organic illness but without an identified cause in a group of people with shared beliefs about the cause of the symptom(s). A review of the literature shows that such outbreaks have been reported in differing cultural and environmental settings including developing and industrialised countries, in the work place, on public transport, in schools, and the military. The perceived threats have been against agents such as food poisoning, fire and toxic gases. Whatever the place or perceived threat, the response seems to be similar. The symptoms generally included headache, dizziness, weakness, and loss of consciousness. Once under way, MPIs are not easy to stop. Incidents reported in the literature show that they can quickly gather momentum and can be amplified by the press who disseminate information rapidly, escalating the events. Management of such mass events can be extremely difficult. Should the public health official in charge continue to try and determine the cause, or should this person call off the entire investigation? It is suggested here that once vaccines are identified as a probable cause of the phenomenon, a dismissive approach may actually be harmful. Unless the spokesperson has already earned a high level of trust, the public are not likely to be convinced easily that nothing was wrong with the vaccine until it has been tested. An increased awareness of MPIs on the part of organisers of future mass vaccination campaigns seems appropriate. Immunisation managers should be aware that mass immunisation campaigns could generate such mass reactions. It is therefore essential that surveillance/reporting systems for reporting adverse events be improved before such campaigns. A mass campaign using a smallpox vaccine should be accompanied by a surveillance system capable of distinguishing between multiple cases of conventionally understood vaccine reactions and outbreaks of mass psychogenic illness.

"Mystery illness" at Melbourne Airport: toxic poisoning or mass hysteria?

A government report concluded that the cause of the recent cluster of illness affecting 57 people at Melbourne Airport was a "mystery". On reviewing the evidence, I noted the appearance of a constellation of distinct psychogenic features (in the absence of an identifiable pathogenic agent or source), and non-specific symptoms not correlated with any particular illness, strongly suggesting a diagnosis of mass psychogenic illness. Given the time differential between the illness onset in the index case and the initiation of air sampling, and the added factor of the air-conditioning in the terminal being switched to exhaust mode, the possibility that a toxic agent was responsible for making some of the victims ill cannot be completely excluded. Future investigations of similar incidents should, in the absence of clinical or laboratory findings, consider the diagnosis of mass psychogenic illness. Failure to do so can engender avoidable confusion and un-ease among the Australian public. The issue of diagnosing collective psychogenic illness will continue to be a major public health challenge, exacerbated by widespread anxieties over the threat of chemical and biological weapons and fears of contamination.

Jonestown tragedy revisited: the role of dentistry.

The authors record the contribution of dentistry to the identification of victims of one of the most significant tragedies of the 20th century--the mass suicide of members of a religious cult, which initially resulted in 913 deaths and ultimately in 923. Though forensic dentistry played a significant part in identifying victims, the only report by members of the United States identification team did not examine dentistry's valuable role. Furthermore, the dental-investigation aspects have been mentioned merely in passing by the news media despite an infinite number of stories on the tragedy ranging from factual to expose. The dental team's organization, methodology, and significant contributions to forensic dentistry and a variety of remarkable problems that the team encountered and the lessons learned from them are documented in this paper.

[Hygienic standardization of tin level in the air].

Indices reflecting the functional state of the central nervous system, peripheral blood, enzymes and gonads were used in accordance with the toxico-dynamics of substances. Besides, body mass, behavior of animals, pathomorphological and histochemical indices were studied. 5 concentrations of tin chloride were studied: 400, 80, 15, 3 and 0.5 mg/m3. On the basis of an acute experiment (6-1440 h) it has been established, that the concentration-time relationship according to a number of biochemical and physiological parameters on the graph was expressed by direct lines on the double log scale with angles to abscissa from 124.9 degrees (according to blood transaminase activity reduction) up to 138.2 degrees, which allowed to determine the class of danger, safety and cumulation coefficients, threshold and no-effect concentrations of tin chloride under inhalational exposure to it. The lowest no-effect concentration 0.05 mg/m3 is recommended as an average 24-hour MAC and approved by the USSR Public Health Ministry.

Bile salts of the toad, Bufo marinus: characterization of a new unsaturated higher bile acid, 3 alpha,7 alpha,12 alpha,26-tetrahydroxy-5 beta-cholest-23-en-27-oic acid.

The bile salts present in gallbladder bile of the toad, Bufo marinus, were found to consist of a mixture of bile alcohol sulfates and unconjugated bile acids. The major bile alcohol was 5 beta-bufol; 5 alpha- and 5 beta-cholestane-3 alpha,7 alpha,12 alpha, 26-tetrols occurred as the minor bile alcohols. Bile acids of Bufo marinus were cholic acid, allocholic acid, 3 alpha,7 alpha,12 alpha-trihydroxy-5 alpha- and 5 beta-cholestan-26-oic acids, 3 alpha,7 alpha,12 alpha-trihydroxy-5 alpha- and 5 beta-cholest-23-en-26-oic acids, 3 alpha,7 alpha,12 alpha, 26-tetrahydroxy-5 beta-cholestan-27-oic acid, and a C27 bile acid which has not been previously described. By chromatographic behavior, mass spectral data, and identification of the products of catalytic hydrogenation and ozonolysis, the structure of the new higher bile acid was elucidated as 3 alpha,7 alpha,12 alpha,26-tetrahydroxy-5 beta-cholest-23-en-27-oic acid. The bile salt pattern of Bufo marinus closely resembles that of Bufo vulgaris formosus, except for the absence of 3 alpha,7 alpha,12 alpha-trihydroxy-5 beta-cholest-22-ene-24-carboxylic acid, the major bile acid of the latter toad.

A telling tale: a case of vigilantism and its aftermath in an English town.

This paper considers one 'vigilante' episode in an English town in 1993 and its subsequent appearances in the press and in local 'crime-talk'. In so doing it a) proposes as an alternative to most current constructions of 'fear of crime' an interpretive approach grounded in place; b) considers the intersections between the generic 'law and order' preoccupations of the national press and the salience in local knowledge of a particular sequence of events (and their consequences for their dramatis personae); c) raises conjecturally some preconditions favouring the adoption of the 'vigilante' option amongst available styles of security-seeking action. Theoretically, the paper demonstrates the relevance of locally-circulating stories of crime and low-level street disorder to the contemporary understanding of crime, place and community.

Associations of functional ability with health-related behavior and body mass index among the elderly.

The main purpose of this study was to determine whether functional ability among the elderly associates with body mass index (BMI) and health-related behavior. The secondary aim was to examine whether health behavior and BMI can be seen as mechanisms explaining sociodemographic disparities in functional ability. Cross-sectional biennial surveys from 1985 to 2001 were used to study 11,793 Finnish people aged 65-79 years. Associations of activities of daily living (ADL) with BMI, health behaviors (smoking, alcohol consumption, diet, physical activity), time period, previous occupation, marital status and certain diseases were tested using an ordinal regression model. Current and ex-smoking, heavy and non-alcohol use, unhealthy diet, physical inactivity and obesity were associated with inferior ADL. Alcohol consumption among men showed a U-shaped relation to functional ability. Most of the differences in ADL by occupation and marital status vanished after adjustment of multiple factors. The results showed clear associations of ADL with health-related behaviors and BMI when adjusted for multiple factors. The findings suggesting a U-shaped relation between ADL and alcohol consumption among men and the association between diet and ADL add to our previous knowledge of factors related to functional ability.

The relationship of childhood adiposity to parent body mass index and eating behavior.

OBJECTIVE: To better understand risk factors for the development of obesity in early childhood, we examined the association between children's adiposity and their parents' eating behavior and body mass index (BMI). RESEARCH METHODS AND PROCEDURES: Parents of 85 white children 36 months of age (49 boys and 36 girls) completed the Three-Factor Eating Questionnaire measuring three dimensions of parent eating behavior: disinhibited eating, cognitive restraint of eating, and susceptibility to hunger. Parent BMI (kg/m2) was calculated using self-reported height and weight. The children's percentage body fat was assessed by dual energy X-ray absorptiometry analysis. RESULTS: Twenty-six percent of parents were obese (BMI > or = 30 kg/m2). Both maternal and paternal BMI were associated with higher scores for disinhibition (r = 0.69 and r = 0.68, p < 0.001), and maternal BMI was also associated with higher scores for hunger (r = 0.51, p < 0.001). There were no significant relationships between children's percentage body fat and parent eating scores, and the correlation between children's percentage body fat and parent BMI was significant only between mothers and daughters (r = 0.35, p = 0.04). Obese parents were no more likely to have a child who was fatter (upper quintile of percentage body fat for gender). DISCUSSION: Among 36 month-old white children, parent eating behavior was related to parent BMI, but not to children's adiposity. There was only a weak relationship between parent BMI and child adiposity. Despite the aggregation of adiposity within families due to shared genes and environments, children may not express differences in susceptibility to obesity by 3 years of age.
