Cutaneous tuberculosis in Morocco.

OBJECTIVE: To describe the salient epidemiologic, clinical, histopathologic, and bacteriologic aspects of cutaneous tuberculosis in Morocco. METHODS: A retrospective review of all cases of cutaneous tuberculosis at our hospital from January 1981 through December 2004. RESULTS: Two-hundred and sixteen cases of cutaneous tuberculosis were identified and included. Men and women were equally affected. The mean patient age was 29 years. Major clinical types of cutaneous tuberculosis were scrofuloderma and gumma (72%), lupus vulgaris (12%), tuberculosis verrucosa cutis (7%), tuberculids (6%), orificial tuberculosis (1%), and tuberculous chancre (1%). Systemic involvement was seen in 35%. Where performed (66%), 81% of subjects had positive Mantoux skin tests. Lesion biopsy for histopathologic study was performed in 81% of patients and showed classical tuberculous findings in 57%. Mycobacterium tuberculosis was isolated in culture from 9% of patients. CONCLUSION: Cutaneous tuberculosis is still a common disease in Morocco, and scrofuloderma and gumma are the most common clinical presentations.

Characterization of lactic acid bacteria isolated from the one humped camel milk produced in Morocco.

One hundred and twenty (120) strains of lactic acid bacteria (LAB) were enumerated and isolated from raw dromedary milk in Morocco using various cultured media. Strains isolated were characterized by phenotypic, physiological and biochemical properties. Results showed that high counts of LAB were found. Presumptive lactobacilli counts ranged from 2.5x10(2) to 6x10(7)cfu/ml, presumptive lactococci levels varied from 5x10(2) to 6x10(7)cfu/ml, presumptive streptococci counts varied from 4.2x10(2) to 8x10(7)cfu/ml, presumptive leuconostoc levels ranged from 5.4x10(2) to 5.4x10(7)cfu/ml. Results showed also that Lactobacillus and Lactococcus were the predominant genera with 37.5% and 25.8%, respectively. The dominated species found were Lactococcus lactis subsp. lactis (17.5%), Lactobacillus helveticus (10%), Streptococcus salivarius subsp. thermophilus (9.20%), Lactobacillus casei subsp. casei (5.80%) and Lactobacillus plantarum (5%). This is the first report on the characterization of LAB strains isolated from the one humped camel milk produced in Morocco.

[ABO polymorphism in blood donors in Morocco].

Blood grouping were improved since the advent of monoclonal antibodies and the automation of the tests. No comparative study has been done before in Morocco concerning ABO regional genetic frequencies. Hence, the aim of this work is to actualize and to precise them. The ABO blood grouping has been carried out on 344 954 blood donors by micro method in several regional blood transfusion centers. Genetic frequencies were calculated according to the Bernstein's method. The national mean values are A: 0.2141, B: 0.105, O: 0.6777. The regional frequencies show gradients between the north and the south of Morocco.

Seroprevalence of T. gondii in sheep from Marrakech, Morocco.

Toxoplasma gondii is a ubiquitous intracellular protozoan parasite transmitted by food. Concerning this parasite, there are few studies done in Morocco. In this study, 261 sera from sheep intended for consumption in Marrakech were subjected to the Toxoplasma ELISA based serology test for the detection of anti-T. gondii specific IgG confirming a past infection. Of the total tested 72 (27.6%) sera were positive for IgG. This result shows that the seroprevalence approaches the world average and is similar to what is found in other cities of Morocco. This has prompted us to investigate other animal species in the region in order to evaluate the degree of contamination by this parasite as well as the potential risk incurred on consumption of their meat.

New challenges in schistosomiasis control in Morocco.

Cases of schistosomiasis were first detected in 1914. However, there is strong evidence that it was prevailing in the southern part of Morocco long before then. As reported from different African countries, over the last three decades, the development of irrigation has led to the spread of the disease to the north and centre of the country. Thereafter, a national programme of schistosomiasis control was launched by the Ministry of Health in 1983. The programme was based on case detection and treatment, snail control using chemicals, and health education. It has succeeded in reducing the prevalence and intensity of infection to a level low enough to allow an elimination programme to be launched in 1994. The aim is to reach a complete elimination of the disease by the year 2004 in all provinces affected. Though substantial progress was made since the programme started, there is a potential risk of outbreaks in many affected provinces. Therefore, an integrated approach including measures against the intermediate host is needed to reach the goal of elimination. The present paper sheds some light on the features of schistosomiasis in Morocco and presents an evaluation of environmental methods of control of Bulinus truncatus in Tessaout Amont and Akka oasis irrigation schemes. The role of community involvement in planning and implementing environmental measures against the snail intermediate host in modern and traditional irrigation schemes is also discussed.

Blood cadmium concentration in the population of the Rabat area, Morocco.

Blood cadmium concentrations were determined in 377 adults, 297 men and 80 women, randomly selected from the Rabat Transfusion Center, Morocco. The mean blood cadmium level was 1.1+/-0.7 microg/l, which was higher than in French subjects, with an average of 0.7+/-0.6 microg/l. In Moroccan people, the mean blood cadmium concentration of men, 1.1+/-0.8 microg/l, was significantly higher than that of women, 0.8+/-0.4 microg/l, whereas in the French people tested, there was no statistically significant difference between men and women. In Morocco, employment of men and the smoking habits of men and women were linked to an increase of blood cadmium levels. The significantly higher level observed in men could be due to a higher percentage of men who were smokers and to professional activity leading to increased exposure to cadmium.

[Differences in the nutritional and health status among illegal immigrant adolescents from Morocco and Algeria].

BACKGROUND AND OBJECTIVE: We decided to determine if there were differences in the nutritional and health status among illegal immigrant adolescents from Morocco and Algeria. SUBJECTS AND METHOD: We performed a retrospective cross-sectional study (1997-2003); 120 male adolescents (86 Moroccans and 34 Algerians) admitted to protection centers of Zaragoza City were assessed. The nutritional status was assessed using the anthropometric method. The health status was assessed according to a standardized medical and laboratory protocol. RESULTS: The mean age (SDU) of the Moroccans was 15.9 (1.2) years and the mean age of the Algerians was 15.6 (1.5) years. Significantly lower values of weight (p = 0.027), upper arm circumference (p = 0.0005) and subscapular skinfold thickness (p = 0.023) along with a significantly higher incidence of ferritin isolated deficiency (p = 0.027; odds ratio = 7.54; 95% CI, 0.96-59.32) were observed in the Moroccan adolescents. CONCLUSIONS: Illegal immigrant adolescents from Morocco had a significantly lower nutritional and iron reserves status than that observed in illegal immigrant adolesents from Algeria.

Cutaneous leishmaniasis: an emerging epidemic focus of Leishmania tropica in north Morocco.

Cutaneous leishmaniasis in Morocco occurs mainly in the south and is caused by Leishmania major and L. tropica. In 1995, for the first time, 4 autochthonous cases were confirmed by smear and/or culture from the province of Taza in north Morocco. An active survey revealed 128 more cases. The number had increased gradually since 1994. Most of the cases (86%) came from the suburbs of the city of Taza. All cultured and typed parasites were characterized as L. tropica MON-102. A leishmanin skin test survey among a random sample of the exposed population showed an overall positivity rate of 19.9%, with no correlation with age or gender. The spatial distribution of the cases and skin test positivity, their occurrence in all age groups, the highly variable clinical picture, the severity and large size of lesions in older patients, the slow recovery of some treated patients, and the isoenzymic monomorphism of the parasite, all suggested that cutaneous leishmaniasis caused by L. tropica is an emerging disease in Taza.

Schistosoma haematobium in Morocco: moving from control to elimination.

In this article, Hammou Laamrani and colleagues summarize the Moroccan schistosomiasis control programme and discuss the challenges ahead for schistosomiasis elimination. In 1994, a programme was initiated by the Moroccan Ministry of Health to eliminate schistosomiasis from Morocco by the year 2004. In 1997, this objective had been achieved in three out of 20 affected provinces. This article discusses the background and strategies of this programme, as well as the achievements, the problems encountered and the challenges ahead, along with suggestions as to how to reach the goal of elimination of urinary schistosomiasis in Morocco, and possibly elsewhere in Africa.

Use of (137)Cs technique for soil erosion study in the agricultural region of Casablanca in Morocco.

Accelerated erosion and soil degradation currently cause serious problems to the Oued El Maleh basin (Morocco). Furthermore, there is still only limited information on rates of soil loss for optimising strategies for soil conservation. In the present study we have used the (137)Cs technique to assess the soil erosion rates on an agricultural land in Oued el Maleh basin near Casablanca (Morocco). A small representative agricultural field was selected to investigate the soil degradation required by soil managers in this region. The transect approach was applied for sampling to identify the spatial redistribution of (137)Cs. The spatial variability of (137)Cs inventory has provided evidence of the importance of tillage process and the human effects on the redistribution of (137)Cs. The mean (137)Cs inventory was found about 842 Bq m(-2), this value corresponds to an erosion rate of 82 tha(-1) yr(-1) by applying simplified mass balance model in a preliminary estimation. When data on site characteristics were available, the refined mass balance model was applied to highlight the contribution of tillage effect in soil redistribution. The erosion rate was estimated about 50 tha(-1) yr(-1). The aspects related to the sampling procedures and the models for calculation of erosion rates are discussed.

Sexually transmitted diseases in Morocco: gender influences on prevention and health care seeking behavior.

Increased awareness of the medical and social costs of sexually transmitted diseases (STD) has resulted in greater attention to the control of these illnesses. STDs are responsible for a significant amount of morbidity in Morocco and have become a key target of the HIV control program. In 1996, the Ministry of Health conducted a qualitative study in order to enhance information, education and communication strategies in the national STD/HIV program. Data on the conceptualization and knowledge of STD, information sources and health-care-seeking behavior were gathered through 70 semidirected, in-depth interviews conducted with men and women in the general population and health care providers (HCPs). Two commonly applied health behavior theories in STD/HIV prevention, the Health Belief Model (HBM) and the Theory of Reasoned Action (TRA) served as a framework for data analysis. The most common name for STD is berd, which means "the cold" in Moroccan Arabic. Berd is caused either by cold striking the genital area or sexual intercourse and most often designates a syndrome of genital discharge. However, the term was also often used to indicate STD in general. The dual causality of berd maintains social stability by providing an honorable excuse for individuals who become infected, while warning against unsanctioned sexual behavior. Clear gender differences in understanding STDs and health-care-seeking behavior emerged through these interviews. STDs in Morocco are viewed as women's illnesses and men with STD often reported feeling victimized by women. Men appear to have more extensive informal information sources for STD than women. Consequences of STD, both physical and psychosocial, were viewed as more severe for women than men, and men had greater access to treatment, for both social and economic reasons.

Camel trypanosomosis in Morocco 1: results of a first epidemiological survey.

An epidemiological survey of camel trypanosomosis was conducted for the first time in Morocco in 1997 and 1998. Five provinces located in the south and southeast of the High Atlas mountain chain were included in this study. A total number of 1460 serum samples were collected and tested by a card agglutination test for trypanosomosis (CATT) and enzyme-linked immunosorbant assay (Ab-ELISA) to detect anti-Trypanosoma evansi antibodies. The overall seroprevalence was 14.1% by CATT and 18.2% by Ab-ELISA. Two very active foci were identified in Zagora and Merzouga with high Ab-ELISA prevalences of 43.3 and 35.4%, respectively, for each area. The concordance between the CATT and Ab-ELISA was 94.1%. The age-related distribution of seroprevalence showed a tendency for the infection rate to increase with age up to a maximum in the 7-10 year-old group. Clinical examination revealed that enlargement of lymph nodes was the most frequent sign in seropositive animals (79.7%). The comparison of haematocrit values of seropositive and seronegative animals showed a significant difference that indicated severe anaemia in infected animals. Isolation of the parasite by inoculation of mice allowed us to collect 15 T. evansi isolates from Zagora and three from Merzouga. The study showed that the Saharan provinces of Morocco were slightly affected by camel trypanosomosis but two hyperendemic foci, Zagora and Merzouga, were found to be sources of contamination for the surrounding regions.

Ethnopharmacological survey of medicinal plants used for the treatment of diabetes mellitus, hypertension and cardiac diseases in the south-east region of Morocco (Tafilalet).

In this study, the medicinal plants used in the treatment of diabetes mellitus, hypertension and cardiac diseases were inventoried based on the ethnopharmacological survey in south-eastern Morocco: Tafilalet region. Seven hundred persons including 320 diabetic patients and 380 patients with hypertension and cardiac disorders and 20 traditional herbal healers were interviewed in different areas of Tafilalet. The results indicated that 80% of patients interviewed used medicinal plants to treat diabetes, hypertension and cardiac diseases because they state that phytotherapy is cheaper (58%), more efficient (40%) and better (65%) than modern medicine. In this ethnobotanic enquiry, about 92 medicinal plants were cited. A lot of them are cited for the first time in Morocco. Many parameters have been evaluated such as knowledge of the toxic plants, doses, parts used, etc. Also, we have reported that 75% of type 2 diabetic patients used medicinal plants in association with modern drugs, while 10% of type 1 diabetic patients regularly used medicinal plants combined with insulin treatment. Some toxic plants have also been reported. In conclusion Tafilalet region disposes of a large phytotherapy knowledge which must be scientifically investigated especially in treating diabetes mellitus, hypertension and cardiac diseases.

Studies of the relationships between Schistosoma and their intermediate hosts. IV. The genus Bulinus and Schistosoma bovis from Morocco.

Schistosoma bovis from Morocco was used in infection experiments with several populations of Bulinus truncatus. The snails from Libya, Malawi, Morocco and Senegal were very compatible with the schistosome since the infection rates were approximately 90%, the mortality was low, and a very high production of cercariae, approximately 1.2 million per 100 exposed snails, was observed. Only a very few B. truncatus (2n = 36) from Rhodesia became infected. B. permembranaceus and B. forskalii were refractory.

A novel frameshift founder mutation in the cytochrome P450 1B1 (CYP1B1) gene is associated with primary congenital glaucoma in Morocco.

Primary congenital glaucoma (PCG) is a heterogeneous autosomal recessive disorder caused by unknown developmental defect(s) of the anterior chamber of the eye. A member of the cytochrome P450 gene family, CYP1B1, was found to be mutated in PCG patients in different populations, albeit to a variable extent. In this study, CYP1B1 mutations were searched for in 32 unrelated PCG patients from Morocco. Two mutations were detected in 11 (34%) patients. One, 4339delG, is novel and causes a frameshift at residue 179. The other, G61E, was previously found in patients from Turkey and Saudi Arabia. Seven patients were homozygous for 4339delG and two other patients for G61E, whereas the two remaining patients were compound heterozygotes. The close association of 4339delG with a rare allele of D2S177, a microsatellite marker located 270 kb upstream of CYP1B1, strongly suggested a founder effect for 4339delG. The occurrence of this mutation was tentatively dated at between 900 and 1700 years ago. Typing 4339delG and G61E mutations should help to prevent blindness resulting from a delayed diagnosis of PCG in Morocco.

Phenotypic and molecular characterization of chickpea rhizobia isolated from different areas of Morocco.

AIMS: To determine the biodiversity of rhizobial strains nodulating Cicer arietinum L. in representative soils from various areas of Morocco. METHODS AND RESULTS: Symbiotic traits, utilization of 49 carbohydrate sources, resistance to antibiotics and heavy metals, tolerance to salinity, to extreme temperatures and pH were studied as phenotypic markers. In addition, restriction fragment length polymorphism (RFLP) of PCR-amplified 16S rDNAs were compared with those of reference strains. Numerical analysis of the phenotypic characteristics showed that the 48 strains studied fell into three distinct groups. RFLP analysis of 16S rRNA genes revealed an additional heterogeneity and four ribotypes were identified. CONCLUSIONS: Chickpea rhizobia isolated from Moroccan soils are both phenotypically and genetically diverse. Most of these rhizobia belong to the Mesorhizobium genus. However, some strains originating from a particular soil appeared to have 16S rRNA genes similar to Sinorhizobium as well as very distinct auxanographic characteristics compared with Mesorhizo- bium isolates. SIGNIFICANCE AND IMPACT OF THE STUDY: A well characterized collection of chickpea-nodulating rhizobia in representative soils of Morocco has been generated, which can be used to develop efficient inoculants for this crop. This is the first report evidencing that chickpeas may be nodulated by bacteria from the Sinorhizobium genus.

Use of climatic data and satellite imagery to model the abundance of Culicoides imicola, the vector of African horse sickness virus, in Morocco.

African horse sickness (AHS) is a vector-borne, infectious disease of equids caused by African horse sickness virus (AHSV). The only proven field vector of the virus is the biting midge Culicoides imicola. Following a recent epizootic (1989-91) of AHS in Morocco, light traps and automatic weather stations were operated for 2 years at twenty-two sites distributed over much of the country. The annually-averaged mean daily trap catch of C. imicola at these sites was negatively correlated with wind speed, and positively correlated with the average and mean annual minimum NDVI (Normalized Difference Vegetation Index, a remotely sensed measure of vegetation activity). There were no significant correlations between the mean daily trap catch and air temperature, soil temperature, relative humidity, saturation deficit, rainfall, altitude or the mean annual maximum or range of NDVI. The best two-variable model, which combined WindspeedMnAvMn (the average daily minimum wind speed of the least windy month) and NDVImin (the average annual minimum NDVI) as predictors, explained over 50% of the variance in the annually-averaged mean daily trap catch of C. imicola. There was a significant, positive correlation between minimum wind speed at night and the daily mortality rate of adult female C. imicola and it is suggested that the relationship between wind speed and the abundance of C. imicola arises from effects on adult mortality or dispersal. Considering several climatic variables, in North Africa NDVImin was most significantly correlated with total annual rainfall. It is suggested that the relationship between NDVImin and the abundance of C. imicola arises from the impact of soil moisture on both. It is proposed that areas of Morocco with higher levels of soil moisture in late summer or autumn provide more, larger and/or more enduring breeding sites for C. imicola, as well as supporting more photosynthetically active vegetation and hence having higher NDVI.

Human papillomavirus DNA in cervical lesions from Morocco and its implications for cancer control.

To determine the types of human papillomaviruses (HPV) in northern Morocco, information which is needed for the design and use of HPV vaccines, we have analysed 129 cervical biopsies from this region. In our study, 91 cases were HPV positive, 45 cases had HPV-16 DNA, and 20 cases had HPV-18 DNA. This distribution of virus type was similar in inflammatory cervical lesions and in invasive cervical carcinomas. In conclusion, the HPV type distribution in Morocco is similar to that in other African Mediterranean countries, where the proportion of HPV-18 cases is significantly higher than in Europe. Determination of virus-type distribution is essential for vaccination programs.
