Serological survey of salmonellosis in grey duiker (Sylvicapra grimmia) in Asejire, Irewole Local Government Area, Osun State, Nigeria.

A serological survey of salmonellosis in grey duiker was carried out in Asejire, Irewole Local Government Area (LGA), Osun State, between August and September, 1996. The sera of 50 animals were screened for antibodies to salmonella flagella (H) and/or somatic (O) antigens. Ten "bushmeat" hawkers/processors were interviewed on their state of health. Twenty-two (44%) and 3 (6%) had antibodies to the H and O antigens, respectively. Nine (18%) had antibodies to both the H and O antigens. Antibody titres of 1/320 were detected against Salmonella paratyphi serotypes B (12%) and C (2%) and S. typhi serotype D (8%). Four (40%) of the interviewed hawkers/processors had had 'fever' in the recent past, and 80% of them usually take traditional herbal medicines for most fever-related ailments. A human typhoid epidemiological surveillance carried out within the LGA between 15th March and 4th April, 1995 revealed 64 cases of tentatively diagnosed salmonellosis. Eleven (17.2%) cases were conclusively diagnosed as typhoid fever by laboratory examination. High antibody titres against H and O antigens of S. paratyphi serotype B and S. typhi serotype were detected. Two (3.1%) of the patients died. The public health importance of these findings is discussed.

Female circumcision and its health implications: a study of the Uruan Local Government Area of Akwa Ibom State, Nigeria.

A total of 400 subjects was randomly selected from 40 villages in the Uruan Local Government Area of Akwa Ibom State for the study. The purposes of the study were to: i. identify the 'established benefits' of female circumcision; ii. identify the health hazards that accompany the practice; and iii. create awareness among community members of the ill-effects of the practice. The study discovered a strong belief in the established benefits and poor appreciation of the health hazards of female circumcision by the participants. Recommendations were made for more efforts in public health education programmes on the ill-effects of the practice. Studies were also recommended to be conducted in other parts of the country to assess the level of awareness on the ill-effects of such an operation and the institution of educational programmes where applicable.

A survey of gastrointestinal parasites in a local government area of south-west Nigeria.

In the course of a survey of gastro-intestinal parasites among humans in Iwo Local Government area in South-Western Nigeria, eggs of helminths found were those of Ascaris lumbricoides, Necator americanus, Trichuria trichiura and Strongyloides stercoralis while protozoan cysts encountered were those of Entamoeba histolytica and Entamoeba coli. About 51% of the 283 faecal samples were positive for parasitic ova or cysts and 24% possessed mixed infection. Incidence of Ascaris was highest followed by Trichuris and Necator. Peak of Ascaris infection was among the 1-10 years age group while that of hookworm and Trichuris was in the 11-20 years age-group. Sixteen (16) combination of mixed infections were found of which that of hookworm and Trichuris was the commonest. Incidence of polyparasitism increases with age until it reached a peak in the 21-40 years age-group.

Private practice in the public sector: a unique relationship among local government, physicians, and health care consumers in Cincinnati.

Because of health care inequities that still persist under federal and state sponsorship plans, many local government agencies are starting to consider ways of directly providing medical services to the poor. Over 100 years' experience with direct provision programs in the City of Cincinnati, however, have been ultimately counterproductive. A unique three-way contract developed by the Board of Health, a private multispecialty physicians' group and a consumer advocate organization has recently allowed Cincinnati to implement a delivery system that integrates mechanisms for prepaid public health care into a fee-for-service private group practice, all within the context of a community-based neighborhood health care facility. While it is still too early to make a full evaluation of this public health care group practice model, there are already indications that is services will be superior to those offered in Cincinnati's traditional public clinics.

Onchocerciasis in the Benue State of Nigeria. II. Prevalence of the disease among the Tivs living in the Kwande Local Government area.

Between December 1979 and July 1980, a survey of the prevalence of onchocerciasis was carried out among the Tivs in the Kwande Local Government area of Benue State. Skin snips were taken from 372 people who were also inspected for the presence of skin nodules. Skin snips of 61, 72, and 71% of the people examined at Kuhe, Manor and Gube, respectively, showed Onchocerca volvulus microfilariae. The proportion of infected people increased with age, reaching a peak in the age group 31-40 years. Of the infected people 11.2% were blind; a higher proportion of infected males were blind compared with infected females. A greater proportion of infected males than females also had onchocercal nodules.

Urinary schistosomiasis in rural communities of some local government areas in Plateau State, Nigeria: a preliminary parasitological and malacological survey.

A preliminary parasitological and malacological survey was effected in rural communities of some Local Government Areas (LGA) in Plateau State, Nigeria, to estimate the prevalence of urinary schistosomiasis and identify active transmission foci. Out of 2888 persons examined in six LGAs, 1381 (47.82%) were excreting eggs of Schistosoma haematobium in their urine. Prevalence rates did not vary significantly (P > 0.05) between the LGAs: Pankshin (62.4%), Shendam (40.2%), Qua'an-Pan (22.9%), Langtang South (45.4%), Langtang North (58.8%) and Wase (50.0%). Infection rates were significantly different (P < 0.001) between the sexes. Many water bodies in the study communities were colonized by infected Bulinus snails. Snail infection rates varied significantly (P < 0.001) between the dry and wet seasons. A positive correlation was observed between snail infection rates and the prevalence of S. haematobium.

Sexual behaviour, contraception and fertility among in-school adolescents in Ikenne Local Government, south-western Nigeria.

A cross- sectional study was conducted among in-school adolescents in six secondary schools in the health districts of Ikenne Local Government to assess the sexual behaviour, contraception and fertility experiences of the adolescents between the months of May and November 2002.Relevant information was collected from 1140 in-school adolescents with the aid of pre-tested, structured, self-administered questionnaires, selected by using multistage and stratified random sampling techniques Information sought from the questionnaires included socio-demographic characteristics of the respondents, parental background, their sexual behaviour, knowledge and use of contraception, human development, pregnancy and fertility experiences, information on sexually transmitted infections (STIs) and HIV/AIDS. During the same period, twelve (12) Focus Group Discussions (FGDs) sessions were conducted using an FGD Guide in the selected schools to highlight differences in opinions of students and also to highlight identification of the group consensus. The mean ages at first intercourse were 13.9 +/- 2.8 years and 14.8 +/- 2.4years for males and females respectively. Boys initiated sex earlier than girls. This difference was found to be statistically significant (p<0.05). Sexual intercourse had been experienced by 28.5% of the adolescent students, significantly more (37.6%) males than females (20.4%) The school adolescents that responded as being married were.4.5% (26males, 23 females) of the respondents. Knowledge on contraception was 36.9% and 22.1% for male and female students respectively, more males than females had knowledge of contraception in a significant proportion, apparently due to increase awareness of the male condom among males. Current use of contraception was equally low, and was found to be 10.9% and 6.0% for males and females respectively. The reasons for non-use were mainly that of non-availability (22.3%), cost (11.8%) negative attitude towards contraception due to societal disapproval (33.2%) and lack of knowledge of how to use them (21.3%). The proportion of adolescents that had ever experienced symptoms associated with Sexually Transmitted Infections (STIs) was 26.8%. Multiple factors were found to be responsible for the deplorable reproductive health situation of the adolescents in this community. The need for provision of sexuality and life planning education in schools including the provision of Youth friendly health services in the community were highlighted.

Physicochemical profile studies of soil along the banks of Ubu river in Ekwusigo and Nnewi local government areas of Anambra State of Nigeria.

A physicochemical profile study on the soil along the banks of the Ubu river that runs across Ekwusigo and Nnewi Local Government Areas (LGA) of Anambra State of Nigeria was investigated for possible evidence of industrial waste disposal by the various large to medium scale industrial activities taking place within the two LGAs. When compared with soil samples from Uzoakwa Ihiala (pH 5.44) with little or no industrial activities, the soil along the banks of the course of the river were found to be more acidic with mean pH values that are lower than 4.7. Similarity in mean sulphate (7.01 mg/kg), phosphate (12.95 mg/kg) and chloride (242 mg/kg) contents was observed between the soils from the banks at Nnewi with those of the Uzoakwa Ihiala that was used as control. This was attributed to contributions from domestic sources rather than from industrial sources, and partly to the similarity of their soil chemistry that may be indigenous to both areas. Electrical conductivity values (55 -74 microS/cm) show that the ionic content of the soil is lower than those of the control (129 microS/cm). This may have been explained by the higher extractible Ca (175.73 mg/kg), K (34.32 mg/kg), and Mg (1.94 mg/kg) recorded for the control. The abundance of the metals extractible from the soils of the river banks follow the sequence Ca>K>Na>Fe. Results for the digested soil samples show that Fe is the most abundant metal in the soil with mean values that are close to 13,000 mg/kg, but is much lower than the mean value of 39,210 mg/kg recorded for the control. Both bank and control samples can be characterized as ferrous. The relative abundance of metal ions observed in the digested soil samples from the river banks follow the sequence Fe>>Al>Ca>Na>K>Zn>Cr. Values of 4,565 mg/kg and 300 mg/kg were recorded for total hydrocarbon content and lead respectively for soils around Cento Group, indicating that there were industrial waste disposal or spillages on soil around the area, which was not observed in other areas.

The prevalence, intensity and clinical manifestations of Onchocerca volvulus infection in Toro local government area of Bauchi State, Nigeria.

Between January and October 1994, a study of the prevalence, intensity and clinical manifestations of onchocerciasis in nine communities of Toro local government area of Bauchi State, Nigeria was undertaken using the skin-snip method. Of the 1117 inhabitants examined, 188 (16.8%) were positive for microfilariae of Onchocerca volvulus. The prevalence of onchocerciasis was significantly higher (P < 0.05) among males than females, in subjects 21 years of age and above than in those in the first two decades of life, in nomads, farmers, hunters and fishermen than smiths and traders. Intensity of infection was light, not exceeding a geometric mean of 5.3 microfilaria per 2 mm skin bite. Preponderance of positive cases below 20 years presented no chronic signs. Conversely, persons above 20 years had higher microfilaria counts which coincides with the period when most clinical signs manifest. Microfilarial-rate and -density in relation to age were closely associated (r = 0.75, P < 0.001). The need for a sustained mass distribution of Mectizan in these communities is highlighted.

Assessment of the impact of community-based medical education of the University of Maiduguri on communities in three local government areas of Borno State, Nigeria: community leaders' perspectives.

BACKGROUND: The community-based training of the University of Maiduguri Medical College was expanded in 1990 to cover three rural local government areas (LGAs) located between 25 and 145 km from the University. This study was conducted between March and May 2000 to assess the impact and level of awareness of the training in the three LGAs in promoting the ideals of Primary Health Care (PHC) and community-based medical education, as perceived by community leaders. METHOD: Focus group discussions (FGDs) were held with community leaders of each of the communities. RESULTS: Consistency in the recollection of what time in the year and for how long the students were in the various communities indicates awareness in all the community leaders. They also indicated that the programme has had a positive impact on their community's health, they would like the students to focus on specific endemic diseases, such as guinea-worm, hypertension, etc, and intensify community outreach programmes, such as home visits and health education. Most participants indicated that their communities participated in the programme by providing accommodation, feeding and other logistics on an ad hoc basis. Community leaders mobilized the communities for health education and other related activities carried out by the students. CONCLUSION: The community-based medical training of the University of Maiduguri has increased community awareness of preventive aspects for various communicable and non-communicable diseases. In addition it has helped to encourage the communities to participate actively in supporting PHC activities.

The role of ions in goitre prevalence in two local government areas of Plateau State, Nigeria.

This study tried to show the contributory role of ions (cations and anions) to great contrast in the goitre prevalence between Bassa and Jos Local Government Areas, both of Plateau State, Nigeria. In pursuance of this, the concentration of cations (Ca++, Fe++, K+, Mg++, Na+, & Zn++) and anions (C1-, F, I-, & NO-3) in soil and drinking water in the two LGAs were determined and their results compared and correlated with the goitre prevalence of these areas. It was observed that both Bassa and Jos LGAs have very low but similar amounts of iodide ions in their soils. Besides, Bassa LGA contained more ions in both soil and drinking water than Jos LGA. In conclusion, the results appear to suggest that the higher ion contents of both soil and drinking water in Bassa LGA exacerbated the coexisting low iodide condition of the LGA, thus resulting in the higher goitre prevalence found in the LGA.

Backyard swimming pool safety inspections: a comparison of management approaches and compliance levels in three local government areas in NSW.

ISSUE ADDRESSED: Since 1992, swimming pool fencing has been a legislative requirement in New South Wales (NSW), yet compliance with the NSW Swimming Pool Act is mixed. Local councils are responsible for the enforcement of the act. However, their approach to enforcement and the management of backyard swimming pool safety inspections is varied. METHODS: A random sample of backyard swimming pools was inspected in Council A and existing compliance data for pools in two other councils (B and C) were obtained. Pool owners in Council A were surveyed regarding their attitudes to pool fencing and inspections. Semi-structured interviews were conducted with council employees. RESULTS: Pool compliance rates across the three council areas varied. In Councils A and C, 51% and 54% of pools, respectively, were found to be non-compliant at the first inspection. Following re-inspection in Council A of 227 pools, a further 125 pools reached compliance. In Council B, 97% of the inspected pools were compliant. CONCLUSIONS: This study provides evidence of poor backyard swimming pool safety compliance where local government inspection activity is minimal or non-existent.

Reorganizations of health agencies by local government in American urban centers: what do they portend for "public health"?

The major health agencies of local urban governments--public health department and public hospital--are often dismissed as "irrelevant" or attacked as "unresponsive". The rush to reform through reorganization hasn't often addressed the underlying problems of an eroded tax base, an indifferent federal policy, and intra-professional disputes. Newer corrective steps are called for if complex demands for public and personal service are to be met.

Managing local government opposition to community-based residential facilities for the mentally disabled.

"Prior Notification" policies have elicited fears of community opposition from agencies who have adopted a "low profile" approach in locating community-based residential facilities. Nevertheless, data from a survey of New Jersey community care providers indicate that local government officials express less opposition when informed of the proposed location and invited to meet with the provider. Arranging to meet with local officials is most important when neighbors have leadership, since officials express more intense opposition when neighbors invite them to a meeting. The literature has already established the fact that meeting with neighbors can facilitate mobilization and thereby engender more intense opposition, both from neighbors and from local officials. Hence, the available evidence suggests that providers should meet with local officials to discuss the community care program but should attempt to deal with neighbors individually. This strategy is compatible with the policy of "prior notification" adopted in New Jersey.

Endemic goitre prevalence in Ifedapo Local Government Area of Oyo State, Nigeria.

As part of efforts aimed at providing an adequate data base for the control of iodine deficiency disorders (IDD) in Nigeria, we undertook an extensive goitre survey of Ifedapo Local Government Area (LGA) of Oyo State, which lies within the goitre-belt of South-western Nigeria. Primary school pupils aged between six and twelve years were the subjects of the survey. From each of ten health districts of the LGA, one primary school was included in the survey. For each school, the entire school population was assessed for goitre by palpation and goitre graded according to WHO/ICCIDD criteria. The survey included 3599 pupils (1889 males and 1710 females). The total goitre rate (TGR) for the LGA was 23.4% while the visible goitre rate (VGR) was 6.4%. The highest goitre rate was recorded in District I (Sango, Saki) TGR = 39.5%; VGR = 17.3%), while the lowest rate was seen in District 4 (Wasengare) TGR = 5.2%; VGR = 0.7%). Females had higher goitre rates than males (TGR: 25.7% vs 21.3%; VGR: 7.5% vs 5.5%). Most of the goitres were Grades IA (36.7%) or IB (35.7%). Goitre grades 2 and 3 accounted for only 27.3% and 0.2%, respectively. These results show that there is a mild to moderate IDD problem in Ifedapo LGA. The implications of this finding, in view of the known deleterious consequences of IDD, are discussed and the need for correction highlighted.

Grant programs for schools and hospitals and for buildings owned by units of local government and public care institutions, amendment of regulations: Department of Energy. Final rule.

The Department of Energy (DOE) today issues a final rulemaking amending existing regulations for the administration of the grant programs providing schools, hospitals, units of local government, and public care institutions with financial assistance for the purpose of reducing energy consumption. The revisions are made as a result of experience obtained during the operation of two grant cycles, and are made for the purpose of reducing the administrative burdens on institutions and States participating in these programs and providing greater flexibility to States and institutions in their implementation of the program.
