The relationship of physical activity and cardiovascular fitness to absenteeism and medical care claims among law enforcement officers.

BACKGROUND: Previous research has shown an inverse relationship between cardiovascular fitness and both absenteeism and health care costs. The documentation of these relationships with law enforcement officers will support the usefulness of providing health promotion programs for this population. METHODS: Data were collected over a one-year period to examine the relationship of physical activity and cardiovascular fitness to absenteeism and medical care claims among law enforcement officers (N = 734). RESULTS: Analysis of covariance indicated that sedentary officers were absent significantly more often than active officers; also, female officers were absent significantly more than male officers. Although proportionally more female officers than males scored above the 50th percentile on fitness (based on Cooper's norms), increased fitness for females was not related to decreased absenteeism. Increased fitness for male officers was related to decreased absenteeism. Medical care claims for a sub-sample of male officers (N = 363) were lower for more fit and more active officers, although this relationship was nonsignificant. DISCUSSION: These observational data strengthen the hypothesis that absenteeism levels of physically active officers are lower than those of sedentary officers. For females, the relationship of absenteeism to family issues associated with child care and pregnancy leave in the workplace appears worthy of examination.

Relationship between physical activity and risk factors for cardiovascular disease among law enforcement officers.

This investigation examined the associations between exercise habits, measures of physical fitness, and 10-year cardiovascular disease risk (CVD10, expressed as %) among 470 law enforcement officers of differing ages (range = 21 to 63 y). Only 32% of this group exercised regularly (> or = 3 days/week, > or = 20 min/session, > or = preceding 4 weeks). Only exercising subjects > 48 years old exhibited a significantly (P < .01) lower 10-year risk of a CVD event than their inactive peers (12.2 +/- 5.6 vs 16.3 +/- 6.9%, mean +/- SD). At all ages, the peak oxygen consumption per unit time was higher (50.1 +/- 6.7 vs 44.8 +/- 6.1 mL.kg-1.min-1) in the exercising than in the nonexercising group. Exercising subjects < or = 36 years old were significantly (P < .05) leaner than nonexercisers (16.3 +/- 5.5 vs 19.6 +/- 5.5% body fat, respectively) and had greater muscular endurance (45 +/- 9 vs 40 +/- 9 60-s sit-ups, respectively). These data suggest that exercise reduces CVD risk by modifying major CVD risk factors only in law enforcement officers > 48 years old.

Critical appraisal of the law enforcement in abortion care in Ethiopia.

OBJECTIVE: To illustrate the disparity in the law enforcement in abortion care and the widely reported induced abortion rates. DESIGN: A descriptive study. SETTING: The computer-entered data from the Federal Democratic Republic of Ethiopia Police Information and Documentation Center. MATERIALS AND METHODS: Two hundred and sixteen case files handled by the police in the last two years. The data of 326 subjects accused of alleged abortion related legal wrong doings were analysed for age, marital status, educational level, occupation and the due process in the court of law. The magnitude of induced abortions is reviewed from the available institutional based studies. RESULTS: The majority of aborting mothers, the service providers and their accomplices are found to be young, unmarried, poorly educated and of low socio-economic profile. The files that are under investigation and pending court rulings are remarkably high. The published incidence of induced abortion ratio of 318 per 1,000 live births is disproportionately greater than those that actually come under the attention of the law. CONCLUSION: The 1957 law is not officially repealed and its restrictive nature is not influencing the prevalence of illicitly induced abortions since the legal instruments are not fully operational. This is the consequence of fewer complaints advanced to the police and/or lack of obligatory reporting system of the events by the service delivery points.

Interactions between mental health and law enforcement systems: problems and prospects for cooperation.

This article examines the challenges posed by system specialization, as illustrated by the difficulties of coordinating the roles of the mental health and law enforcement agencies working with people with severe mental illness. Dealing with the needs of clients in one system when they are most appropriately served by the other may make both law enforcement and mental health systems appear ineffective and inefficient. This could increase the incidence of disorderly or violent behavior, which forments the myth that the seriously mentally ill are inherently dangerous. Despite the evident need to manage these issues, conventional methods of coordinating services have failed. This article concludes by developing a contracting model that creates more appropriate incentives for the two systems and bridges the gap between them.

The injury pattern of a new law enforcement weapon: the police bean bag.

STUDY OBJECTIVE: This case series describes the injury pattern of the police bean bag, a new weapon adopted by US law enforcement agencies. METHODS: Retrospective chart review between 1996 and 2000 identified bean bag injuries. Autopsy data were gathered for 1 fatality. Circumstances of the shootings, toxicology results, and psychiatric diagnoses, if any, were recorded. RESULTS: Thirty-nine men and 1 woman between the ages of 16 and 77 years were shot by officers of either the Los Angeles Police Department or the Los Angeles Sheriff's Department. The fatality was caused by massive hemothorax after thoracic penetration. The locations of serious penetrating injuries included the thoracic cavity, eye, abdomen, arm, and leg. Blunt injuries included splenic rupture, pneumothorax, compartment syndrome, testicular fracture, subcapsular liver hematoma, and cardiac contusion. Complications included hemothorax, pneumopericardium, wound infection, compartment syndrome, and osteomyelitis. Psychiatric consultation was requested for 27 (69.2%) of 39 surviving patients. The Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) Axis I was used for diagnosis in all 27 patients. Psychosis was diagnosed in 16 (59.3%), suicidal ideation in 15 (55.6%), and major depression in 6 (22.2%) of the 27 cases. Schizophrenia had previously been diagnosed in 7 (25.9%) of the 27 cases. All but 1 of the patients had toxicology screenings performed, and the results of 29 (74.4%) of 39 were positive. Of these 29 positive results, 15 (51.7%) were positive for alcohol and 8 (27.6%) were positive for cocaine. CONCLUSION: Awareness of the broad scope of potential injuries mandates a thorough evaluation of both blunt and penetrating trauma in patients shot with police bean bags. The scope of these injuries raises significant public health considerations because use of this weapon has been adopted by law enforcement agencies in all 50 states and at least 10 countries.

RESEARCH: Triage Law Enforcement: Societal Impacts on National Forests in the West.

/ Uses of outdoor natural areas, like National Forests in the United States, have been undergoing change. Some changes, including criminal activities and domestic terrorism, may have quite important impacts to outdoor enthusiasts and managers. Activities include some relatively traditional management challenges, like arson and thefts, and newer challenges, such as domestic violence and murder. To better understand the prevalence and impacts of these activities, selected managers in the western United States were interviewed in summer and fall 1998. Results indicate criminal activities and domestic terrorism are found in both urban-interface and rural sites. The most prevalent challenges were arson, criminal damage, domestic violence, dumping of household trash and landscape materials, marijuana cultivation, satanic cult activity, and thefts. Not as prevalent but found in almost all study sites were body dumping, extremist group activity, gang activity, murders, and suicides. Managers felt that most outdoor recreationists were unaffected by the activities. Most managers noted the increasing prevalence and the costs to management in time, personnel, and in other ways. They noted that their job had ceased being one of natural resources law enforcement and became one of "city" law enforcement. The findings of the study raise serious issues that require additional research.

Development of a hearing performance standard for law enforcement officers.

A hearing performance standard for new police officer candidates in the State of Michigan was developed by a task force consisting of the authors and four members of the Michigan Law Enforcement Officers Training Council. Ratings of the importance of hearing sensitivity and speech intelligibility in performing each of 135 specific job tasks formed the basis for pass-fail criteria. The standard, described in this report, includes specifications for unaided and aided hearing performance, types of hearing assessment measures, hearing measurement procedures, and the appropriate examining professional. The authors believe that the standard provides an appropriate accommodation to hearing-impaired law enforcement officer candidates with respect to The Americans with Disabilities Act.

Law enforcement K-9 dog bites: injuries, complications, and trends.

STUDY OBJECTIVE: To quantify the number of individuals bitten, the number of bites per patient, and the types of injuries and complications caused by law enforcement K-9 dog bites treated in the Jail Ward Emergency Department of the Los Angeles County-University of Southern California Medical Center. These variables were compared before and after a change in K-9 police policy from the "bite-and-hold" to the "find-and-bark" technique or stricter controls were instituted over the K-9 teams. METHODS: A retrospective chart review of all patients in police custody with K-9 dog bites who presented to the Jail Ward ED between January 1, 1988, and December 31, 1995, was conducted. Demographic data of patients with K-9 dog bites, the number and location of bites, complications, procedures performed, and management of bites were recorded and compared between the periods 1988-1991 (before the policy changes) and 1992-1995 (after the changes). RESULTS: Between 1988 and 1995 790 in-custody patients were treated for K-9 dog bites in the Jail Ward ED; 705 charts were available for review. Nearly all the patients (98.6%) were male, with a mean age of 25; 85.0% were Hispanic or black. More than half (57.2%) sustained three or more bites, mainly to the extremities. Complications ensued in 19.3%: vascular in 7.0%, infection in 5.0%, fracture or cortical violation in 4.0%, nerve injury in 1.9%, and tendon injury in 1.1%. Half (49.9%) were hospitalized, with a median stay of 3 days. After the change in K-9 policy, the number of patients with K-9 dogs bites presenting to the Jail Ward ED decreased from 639 (1988-1991) to 66 (1992-1995). The proportion of patients who sustained three or more bites decreased from 58.4% to 45.5%. The rate of vascular complications decreased from 7.5% to 1.6%, the rate of fractures decreased from 2.4% to 0, and the rate of cortical violations increased from 1.4% to 6.3%. The proportion of patients hospitalized decreased from 52.0% to 33.8%. CONCLUSION: K-9 dog bites are associated with significant injuries and complications. In this study, changes in law enforcement K-9 policy contributed to a significant decrease in the overall number of individuals bitten, the number of injuries and complications, and the proportion of patients hospitalized.

Nonsanction seat belt law enforcement: a modern day tale of two cities.

This paper describes the implementation and evaluation of a nonsanction seat belt law enforcement program in two experimental communities of contrasting size in a state with a mandatory belt law. The main ingredients of the program were seat belt "salutes," public information and education, and limited use of inexpensive economic incentives. Driver shoulder belt use data collected before, during, and after the experimental programs, compared to similar data collected in a comparison community, showed the approach to be effective. While standard seat belt enforcement activities without incentives have been shown to be effective, many police departments, especially in smaller communities, are reluctant to make wholesale increases in seat belt citations. Although requiring some additional level of manpower and resources, a nonsanction approach to seat belt law enforcement can provide an alternate way of increasing belt use in these communities.

Tobacco policy and the role of law enforcement in prevention: the value of understanding context.

The Florida Office of Tobacco Control sponsors evaluations of the effect of its comprehensive youth tobacco prevention initiatives. As part of this evaluation, a quasi-experiment designed to assess the effect of law enforcement on youth tobacco use was conducted. An in-depth qualitative study was a critical component of this research. This study provided a rich description of the context in which law enforcement was implemented. Data collection involved interviews with judges, clerks of court, and selected officials in each of the study counties. Approximately 70 interviews were conducted. Extensive, ongoing computer-assisted analysis complemented the process. Several consistent themes emerged during the research that helped to give contextual meaning to the findings. These themes provided critical insights into the complexity of policies about youth tobacco possession, and the findings helped illuminate the varying contexts in which these interventions were applied.

The role of forensic medical services of Azerbaijan Republic in cases of natural calamities, terrorist actions and other urgent situations--organizational principles of joint activities of forensic medical services and the law-enforcement agencies.

Disastrous natural calamities, large-scale catastrophes and terrorist actions are characterized by a sudden and unexpected character and lead to numerous human victims. As a rule this fact seriously aggravates the work of local medical service and the law-enforcement agencies. Nowadays not a single society can totally guarantee itself from the danger of natural calamities, terrorist actions and emergency situations. Taking into consideration peculiarities of our country and also aware of the international experience a brand new set of preventive measures was elaborated by forensic experts of Azerbaijan in close cooperation with the law-enforcement bodies. The basic purpose of these sets of actions is to relieve the consequences of such kinds of unpredictable cases.

Large group crisis intervention for law enforcement in response to the September 11 World Trade Center mass disaster.

University Behavioral HealthCare, University of Medicine and Dentistry of New Jersey in partnership with the New Jersey Department of Personnel established a program entitled "Cop 2 Cop" in 1999 to assist law enforcement personnel within the state. The events of September 11, 2001, demanded an unprecedented response to address the behavioral health care needs of those individuals in New Jersey and New York. Although the Cop 2 Cop program was initiated as a crisis intervention hotline, the legislature which established the program also identified facilitating Critical Incident Stress Management services for New Jersey law enforcement and their families as needed to be within the scope of function. This paper describes the Cop 2 Cop program interventions with the Port Authority Police Department (PAPD) which involved service provision to over 1,200 PAPD officers and an intensified process entitled an "Acute Stress Management Reentry Program" created for over 200 officers with unprecedented exposure to traumatic events.

The role of law enforcement in minimizing the harm resulting from illicit drugs.

Illicit drugs have become a major global problem in recent decades following considerable recent political change, including the collapse of communism and the formation of international super-states to increase trade. Despite increasing collaboration between law enforcement authorities in different countries, illicit drug problems appear likely to increase in the future because of the vast profits available, continuing (and increasing) demand and more permissive attitudes concerning drugs among young people. While rejecting legalization or decriminalization, the search for more effective responses by law enforcement authorities and the community generally must be stepped up. Police services continue to play an important role restricting the availability of illicit drugs but increasing emphasis needs to be given to reducing demand, including more available and more effective preventive drug education in schools. Police also need to work with harm reduction approaches devised to reduce the negative consequences of drug use for those who continue to use illicit drugs. New measures proposed in Britain are outlined. These stress the importance of a multi-sectoral approach operating at both national and local levels with the objective of reducing drug-related crime, reducing the acceptability and availability of illicit drugs and reducing the harmful consequences of illicit drug use. Harm reduction requires a commitment for close collaboration between police and drug treatment services to maximize the effectiveness of needle-exchange schemes and other harm reduction approaches. Cautioning, now commonly used in Britain for selected minor drug offences, has a number of benefits including reducing criminal justice costs. Greater emphasis must be placed on diversion schemes involving close links between police and drug treatment services. Future progress requires firm commitments to providing adequate and effective drug treatment services, conducting research to develop and evaluate more effective diversion schemes, improving collaboration between sectors and effective leadership. In addition to the major costs of illicit drug use to the community, the huge cost to individuals must remain a major focus driving the search for more effective responses to the problems resulting from illicit drugs.

Need for law enforcement in cases of intimate partner violence in a university ED.

The purpose of this study was to quantify the proportion of men and women seen in a university emergency department (ED) for treatment of injuries resulting from intimate partner violence (IPV) that require reports to law enforcement authorities. A total of 1,516 adult ED patients were asked to complete a written survey instrument; 1,003 patients (66.2%) completed the survey. Two percent of patients reported they presented to the ED for treatment of injuries resulting from IPV. Three percent reported IPV within the last year, and 10% reported that they had ever been physically abused by a partner. Six percent of respondents reported that they had ever been threatened with a gun or knife by a partner, 2% within the past year. Only the lifetime prevalence of IPV was significantly greater among female patients, 15% versus 6% (P < .001). Approximately 2% of our ED patients require law enforcement intervention for IPV.

Harris-Lingoes MMPI-2 Pd subscales and the assessment of law enforcement candidates.

This study analyzed the performance of 111 male law enforcement applicants on MMPI-2 Harris-Lingoes Psychopathic Deviate (Pd) subscales. Applicants' self-descriptions derived from their scores on these subscales showed greater confidence and resistance to authority and less social alienation and personal unhappiness than the MMPI-2 national normative sample. A multiple regression analysis indicated that Pd2 (Authority Problems) was the best predictor of K-corrected Pd scores for the sample of applicants.

Injury pattern of the Flash-Ball, a less-lethal weapon used for law enforcement: report of two cases and review of the literature.

Less-lethal weapons are used in law enforcement to neutralize combative individuals and to disperse riot crowds. Local police recently used such an impact weapon, the Flash-Ball, in two different situations. This gun fires large rubber bullets with kinetic energies around 200 J. Although it is designed to avoid skin penetration, impacts at such energies may still create major trauma with associated severe injuries to internal organs. This is a report of 2 patients shot with the Flash-Ball who required medical attention. One could be discharged quickly, but the other required hospitalization for heart and lung contusion. Both patients required advanced investigations including computed tomography (CT) scan. The medical literature on injuries induced by less-lethal impact weapons is reviewed. Impacts from the Flash-Ball can cause significant injury to internal organs, even without penetration. Investigations as for other high-energy blunt traumas are called for in these cases.

Alcohol use among pedestrians and the odds of surviving an injury: evidence from Florida law enforcement data.

Alcohol consumption by pedestrians is widely recognized as a factor influencing the risk of being hit by a motor vehicle, but its effect on the likelihood of dying, given that a collision has occurred, is more uncertain. Studies of drivers find that alcohol increases the risk. Unlike previous studies based on clinical data, the present study is population-based and takes into account indicators of crash severity. Using Florida law enforcement data, logistic regression models yielded a four to fivefold increase in the odds ratios associated with alcohol use, depending on the model. Findings from a model investigating the magnitude of the potential bias due to differential investigative behavior indicated bias may account for some but not all of the increase in the odds of dying. Recommendations include improving law enforcement procedures for identifying alcohol impairment and increasing public awareness of the risk involved in mixing heavy drinking and walking.

Law Enforcement Agency Defibrillation (LEA-D): proceedings of the National Center for Early Defibrillation Police AED Issues Forum.

Why does LEA-D intervention seem to work in some systems but not others? Panelists agreed that some factors that delay rapid access to treatment, such as long travel distances in rural areas, may represent insurmountable barriers. Other factors, however, may be addressed more readily. These include: absence of a medical response culture, discomfort with the role of medical intervention, insecurity with the use of medical devices, a lack of proactive medical direction, infrequent refresher training, and dependence on EMS intervention. Panelists agreed that successful LEA-D programs possess ten key attributes (Table 6). In the end, the goal remains "early" defibrillation, not "police" defibrillation. It does not matter whether the rescuer wears a blue uniform--or any uniform, for that matter--so long as the defibrillator reaches the victim quickly. If LEA personnel routinely arrive at medical emergencies after other emergency responders or after 8 minutes have elapsed from the time of collapse, an LEA-D program will be unlikely to provide added value. Similarly, if police frequently arrive first, but the department is unwilling or unable to cultivate the attributes of successful LEA-D programs, efforts to improve survival may not be realized. In most communities, however, LEA-D programs have tremendous lifesaving potential and are well worth the investment of time and resources. Law enforcement agencies considering adoption of AED programs should review the frequency with which police arrive first at medical emergencies and LEA response intervals to determine whether AED programs might help improve survival in their communities. It is time for law enforcement agency defibrillation to become the rule, not the exception.
