Research implications of science-informed, value-based decision making.

In 'Hard' science, scientists correctly operate as the 'guardians of certainty', using hypothesis testing formulations and value judgements about error rates and time discounting that make classical inferential methods appropriate. But these methods can neither generate most of the inputs needed by decision makers in their time frame, nor generate them in a form that allows them to be integrated into the decision in an analytically coherent and transparent way. The need for transparent accountability in public decision making under uncertainty and value conflict means the analytical coherence provided by the stochastic Bayesian decision analytic approach, drawing on the outputs of Bayesian science, is needed. If scientific researchers are to play the role they should be playing in informing value-based decision making, they need to see themselves also as 'guardians of uncertainty', ensuring that the best possible current posterior distributions on relevant parameters are made available for decision making, irrespective of the state of the certainty-seeking research. The paper distinguishes the actors employing different technologies in terms of the focus of the technology (knowledge, values, choice); the 'home base' mode of their activity on the cognitive continuum of varying analysis-to-intuition ratios; and the underlying value judgements of the activity (especially error loss functions and time discount rates). Those who propose any principle of decision making other than the banal 'Best Principle', including the 'Precautionary Principle', are properly interpreted as advocates seeking to have their own value judgements and preferences regarding mode location apply. The task for accountable decision makers, and their supporting technologists, is to determine the best course of action under the universal conditions of uncertainty and value difference/conflict.

Position of information science in the medical and health studies curricula.

Innovation of both the content and forms of informatics teaching directed on exploitation of technical facilities and methods of information and communication technologies (ICT) in non-informatics universities study programmes and in life-long education is still actual today. Expanding knowledge base of the life and health sciences requires to form new skills to educate the end-users and facilitate an immediate information access. Theoretical and practical teaching in pregradual medical and health studies includes the basic contacts of students with the modern technique and special medical devices supported or completed by computers. The teaching of information science and corresponding subjects represents small but important part of medical and health studies curricula at all medical faculties in Slovakia (Fig. 2, Ref. 8).

Defining medical information sciences: a health care science for the future.

There exists a field; there are already researchers committed and busily at work; and there are meetings, journals, and organizations. Principal researchers in the field and those willing to lend their support should renew their efforts at crystallizing the identity of the field. In reviewing the nature of the field, there is a reasonable definition possible that is responsive to three fundamental constituents related to health care: the setting in which the care is provided, the patient to whom it is provided, and the health care professional by whom it is provided. Using these areas, we get a natural division of the field that is also historically and intuitively appealing. Using these divisions, research issues can be identified since information processing activities in the three categories become the areas to investigate. Instruction needs are similarly implied since one should have some understanding of each of these areas as part of a total health care science. In addition, understanding of the major information processing methodologies, combined with broad topics about the milieu appropriate for the professional, comprises a suitable graduate programme of study. By whatever title, the time is ripe to move forward on the enterprise to advance medical information science by self-identification and common support.
