Composition of myofiber types in the vocalis muscles involved in rapid closure of the glottis in Japanese macaques.

Myofiber types of the medial thyroarytenoid (vocalis) muscle, lateral thyroarytenoid muscle, and cricothyroid muscle of the Japanese macaque were examined with enzyme-histochemical methods. For comparison, the semitendinosus muscle of the Japanese macaque and the thyroarytenoid (vocalis) muscle of cattle, sheep, and pig were examined with the same methods. The vocalis muscle of the Japanese macaque was composed exclusively of fast-twitch/oxidative/glycolytic (FOG) myofibers; it differed from the lateral thyroarytenoid, cricothyroid, and semitendinosus muscles of the Japanese macaque and from the vocalis muscles of the other animals, which consisted of slow-twitch/oxidative, FOG-, and fast-twitch/glycolytic myofibers or type IIC myofibers. The histochemical properties of the vocalis muscle of the Japanese macaque show that the vocalis muscle has a capacity to close the glottis rapidly and completely.

[A contrast analysis of four phonation methods frequency between patients with glottis closure disturbance and normal human].

OBJECTIVE: To find out frequency change parameters of four phonation methods (minimum, natural, maximum and falsetto maximum voices) and different maximum phonation persistent time (MPPT) of them in patients with glottis closure disturbance (GCD). METHOD: With frequency spectrum analysis, the frequencies of four phonation methods were studied in functional GCD patients (150 cases) and normal human (60 cases). RESULT: There were marked differences of voice frequencies between them not only in male but also in 5 female. And with the prolongation of MPPT, the voice frequencies also changed. CONCLUSION: The functions of both low and high voices decline in GCD patients. With the prolongation of MPPT, the functions of low and high voices strengthen in GCD patients.

[The own experience with complications narrowing the glottis as a result of microsurgical procedures].

Proceeding rules in the larynx narrowing scars were presented. The clinical material consisted of patients with bilateral vocal cord paralysis, oedema Reincke and early stages of laryngeal carcinoma who underwent laser microsurgery of the larynx. The attention was paid to prophylaxis relating to the surgical procedure and perioperative care. The possibility of the silastic sheet insertion into the glottis was introduced as a way of treatment in recurrent scars.

Surgery for benign lesions of the glottis.

This article examines benign lesions of the glottis and adjacent structures commonly encountered in clinical practice. The goals of management are to understand the cause and the characteristics of these lesions and to provide the most conservative management possible. Nonsurgical management is selected, if possible, with surgical intervention reserved for refractory conditions. Traditional cold steel operative techniques as well as carbon dioxide laser techniques are discussed in this article.

[Correction of a glottis gap by injection of collagen. Apropos of 25 cases].

Results of treatment of patulous glottis by injection of collagen are discussed. The technique used differs little from that in which Teflon is employed, the difference being that the collagen paste is injected into Reinke's space. The quantity deposited varies between 1 and 1.5 cm3. Twenty-five patients had been treated in this way by April 1986, the longest follow up being for 19 months (mean: 9.5 months). The compound presents remarkable tolerance and no complication has been noted up to the present. Results were at least equal to those obtained using Teflon. Vocal quality was evaluated, apart from by vocal recordings, by analysis of vocal spectrum and objective measurements such as phonatory quotient and output. Results appear to be stable and although some reduction in quantity injected was noted most of the collagen becomes living tissue by colonization with host tissue. Availability of new generations of collagen such as that crosslinked at 35 mg/ml will probably resolve definitely the problem of persistence of results.

[Modification of surgical reconstruction of the glottis].

A personal modification to Staffieri's technique is presented. This change respects the principles of the operation but is different on the practical performance in order to avoid the incontinence of the neo-glottis. A partial decartilagination of the tracheal or crico-tracheal stump over the definitive tracheostome is performed, so that it can be funnel shaped and idoneus to the end to lateral anastomosis with the mucous membrane of the ipopharinx.

[Partial surgery of carcinomas of the glottis. Results and prognosis (author's transl)].

The authors present a computerized study of the prognosis of 132 cases of carcinoma of the glottis treated by vertical, partial surgery and seen over a period of 1 to 15 years at the Laennec Hospital. After defining the percentage survival at 3.5 in 10 years, the authors studied the causes of death and showed that, after 5 years, it is still deaths due to malignant recurrence which influenced the vital prognosis. These local, lymphnode and metastatic oncological failures are subsequently analyzed on the basis of clinical, surgical and histological parameters as well as in terms of the course of the disease. Such analysis makes it possible to draw the distinction between local recurrence and a second local tumor localization, and pulmonary metastases from a second pulmonary localization. Finally, the prevention of rare lymphnode recurrences is considered in terms of specific surgery for this purpose.

[Granular cell tumor localized in the glottis in children. Apropos of a case].

We report the first case of adult granular cell tumor form localized in the glottis in a 8 year-old girl. The first symptom was a chronic dysphonia. Examination with rigid optic demonstrated an irregular yellow hypertrophic lesion in the posterior third of the left vocal fold. The lesion was removed by CO2 laser micro-spot under general anesthesia. There has been no recurrence after 15 months of follow-up and the postoperative voice quality is satisfactory regarded both objectively as subjectively. Granular cell tumors are rare. The histogenesis is still badly known. Enzymological and electron microscopy studies seem to indicate a nervous origin (Schwann cells). Treatment is surgical. Wide local excision is the general rule for limited lesions with easy access. When bigger lesions are present in more vital structures such as trachea or oesophagus, surgery has to be more cautious.
