[The properties of the epidemic influenza viruses A and B strains circulating in Russia in the 2004-2005 epidemic season].

The epidemic upsurge of influenza morbidity in Russia in 2004-2005 was caused by the active circulation of influenza A(H3N2) and B viruses. A hundred and sixty-six epidemic strains were studied. All the strains were isolated in the MCK cell culture. Influenza A(H3N2) viruses (n=77) were antigenic variants of the reference A/Fujian/411/ 2002 and A/California/7/2004 strains. Three influenza A(H1N1) viral strains that were antigenic variants of A/New Caledonia/20/99 strains were isolated in sporadic cases. Influenza B virus strains (n=83) were antigenic variants of the reference B/Shanghai/361/02--lineage B/Yamagata/l6/88. In addition, 3 antigenic variants of B/Hong Kong/ 330/2002 (lineage B/Victoria/2/87) strains were isolated. Nine (20%) strains resistant to rimantadine at a concentration of 5 microg/ml were identified. Chromatographic analysis of B/Shanghai/361/02 and BIHong Kong/330/01 viral protein M1 trypsin hydrolysates revealed differences in the profiles of chromatograms of influenza A virus proteins M1. Examination of 121 pair sera from patients revealed an increase in antibodies to influenza A(H3N2) viruses in 10-21% of cases and to influenza B viruses in 20-36% of cases.

[The use of Bayes' theorem in controlling the coming epidemic peak of epidemic meningitis in the 1980s].

To control the coming epidemic peak of epidemic cerebrospinal meningitis (ECM) in the 1980s in the regions with 100 million population in China, the mathematical models based upon Bayes' theorem (BT) were established and used respectively in provincial, regional and county's level. Reports of ECM from each ten-day's period or each month during the meningitis season were analysed to create forecast models. Records of ECM vaccinating rate in previous years were fully taken into account to modify the theoretical values. Calibration, split-sample, random-sample selection, as well as actual forecast tests, were used to check the efficiency of the models. The distribution of meningitis vaccine was planned according to the final predictive results. The incidence rates of ECM of above regions decreased obviously faster than other areas in China. Attributing the application of BT forecast research, it was estimated only in Henan province 79795 ECM cases; 4388 deaths and 21 million Yuan economic damage were avoided during the 4 years period, from 1985 to 1988.

[The epidemic process as a system. I. The structure of the epidemic process].

A new epidemiological concept (socio-ecological) has been formulated on the basis of the principles of the theory of systems and the theory of information. In accordance with this concept, the epidemic process is organized on the same principle as living matter, and the stability of this process at all levels of its organization is ensured by the processes of self-regulation. The conditions of the life of human society have been shown to be organically incorporated into the structure of the epidemic process as a regulating subsystem on the socio-ecological level.

[The epidemic process as a system. The social regulators of the epidemic process].

The article deals with the mechanisms of spontaneous (urbanization, the international migration of population, the anthropogenic transformation of nature) and goal-oriented regulatory action on the epidemic process. Social factors have been shown to transform into ecological ones with the subsequent transformation of information in the parasitic system and their reflection in the social subsystem as the indices of the risk of infection and its socio-economical importance. Using the processes of urbanization as an example, the present work demonstrates that the mechanism of transmission acts as a filter whose specific features are determined by a range of social factors playing the most important role in the regulation of the epidemic process on the socio-ecosystemic level of its organization.

The 1970 yellow fever epidemic in Okwoga District Benue Plateau State, Nigeria. 2. Immunity survey to determine geographic limits and origins of the epidemic.

Serological surveys undertaken to define the geographic limits of the 1970 rural yellow fever epidemic in Okwoga District, Nigeria, indicated that surrounding areas of Benue Plateau State and East Central State were not involved. However, the surveys uncovered a separate focus of unrecognized, recent epidemic yellow fever in Mbawsi, in southern East Central State. The highest proportions of yellow-fever-immune sera outside the Okwoga and Mbawsi foci were found in zones of Guinea savannah in the Benue River basin.

[The characteristics of the epidemic strains of the influenza B viruses that caused the 1990-1991 epidemic in the USSR].

Epidemics of influenza in towns of Russia were found to have been caused by influenza type B virus. Strains similar to B/Yamagata/16/88 were first isolated in the country. The study of antigenic and biological properties showed that the epidemic strains could be divided into 2 groups: similar to B/Victoria/2/87 and to B/Yamagata/16/88, that is, representing two branches of the evolutionary tree. In both groups the viruses were heterogeneous both in antigenic and biological properties.

Homogeneity of yellow fever virus strains isolated during an epidemic and a post-epidemic period in West Africa.

Three strains of yellow fever virus (YFV) were isolated in 1982 in The Ivory Coast, one from a human case and two from Aedes luteocephalus, during and subsequent to an epidemic. The complete genomic sequence of the human strain was determined and compared to that of the 1927 Asibi strain of YFV. The divergence observed was on average of 8.3%, ranging from 5.5 to 11.7% in the coding region. The transitions to transversions ratio was 5.9. Most mutations (84.3%) occurred on the third position of the codons, with synonymous mutations representing 92.5%. However, when partial sequences representing 60% of each genome were compared, homology between the three Ivory Coast strains was greater than 99%. These results demonstrate the homogeneity of the virus strains circulating in different hosts and vectors in a limited geographical region and validate the concept of topotype in viral quasi-species.

A heroin epidemic at the intersection of histories: the 1960s epidemic among African Americans in Baltimore.

In the drug field the fundamental epidemiological question-why illicit drug use, here, now, among these people-has still not been adequately answered. Drawing on the work of colleagues in medical anthropology, we attempt to move closer to an answer by developing a "trend theory." In this article we analyze a single case: the increase in heroin use and addiction among African Americans in the City of Baltimore in the 1960s. We found that the two most important historical processes behind the epidemic were (1) a changing distribution/supply system and (2) the mix of hope and despair that was part of the early civil rights movement.

Morbillivirus infection among seals (Phoca vitulina) during the 1988 epidemic in the Bay of Heligoland. II. Serogological investigations reflecting a previous phocine distemper epidemic in a seal orphanage.

Of 57 seals hospitalized in the Norddeich Orphanage, 37 (65%) had died until 22nd of August, 1988, when the first collection of blood samples from the survivors commenced. All the sera including those obtained at a later date from the remaining animals had invariably higher neutralizing antibody titres against a phocine distemper virus (PDV) isolate than against canine distemper virus (CDV). The difference of mean titres was calculated to be more than 1.5 x log10 serum dilution. Peak titres demonstrated by a direct neutralization peroxidase-linked antibody (NPLA) assay reached 1/90,000. Attempts to isolate PDV in seal kidney cell cultures from heparinized blood samples collected from convalescent animals were not successful. From the increase in antibody titre following the last fatal case it was concluded that the devastating epidemic sweeping through the Norddeich Orphanage was primarily due to phocine distemper.

[Epidemic of infectious disease with echovirus type 16--epidemic in Tono area, Gifu Prefecture in 1984].

During the period from May to August, 1984, an epidemic of infectious disease with echovirus type 16 occurred in Tono area of southeast in Gifu prefecture. This virus caused children to have different clinical symptoms, one was a exanthem disease and another was aseptic meningitis. These cases confirmed by virological and serological methods were 48 cases, that is, patients with aseptic meningitis were 24 cases and patients with exanthem disease were 24 cases. By serological examination of antibody against echovirus type 16, it was confirmed that this virus type invaded Gifu Prefecture before 1984.

A single-season epidemic with respiratory syncytial virus subgroup B2 during 10 epidemic years, 1978 to 1988.

Respiratory syncytial virus strains isolated from infants and children hospitalized with acute respiratory tract disease in Huntington, W. Va., from July 1979 through June 1988 were categorized by subgroup by using two panels of monoclonal antibodies specific for subgroup A and subgroup B. Of 405 strains tested, 319 (78.8%) were subgroup A, 73 (18.0%) were subgroup B1, and 13 (3.2%) were subgroup B2. The subgroup B2 strains occurred as a single-season epidemic during the epidemiologic year July 1980 to June 1981, except for one strain isolated in the epidemiologic year 1978 to 1979. Subgroup B strains were associated with significantly fewer bronchiolitis illnesses than subgroup A strains. The severity of illnesses caused by subgroup B1 and B2 strains was the same.

Protein A and coagulase expression in epidemic and non-epidemic Staphylococcus aureus.

Strains of Staphylococcus aureus were divided into groups on the basis of antimicrobial sensitivity and epidemiology and tested for protein A expression in a simple microtitre test, which detected the non-immunological binding of immunoglobulin to protein A on whole cells of S aureus. Isolates of the methicillin resistant strain prevalent in south east England (EMRSA) showed a low expression of protein A compared with the other strains of methicillin resistant S aureus (MRSA), other multiple resistant strains, and sensitive strains. Protein A and coagulase expression in 27 strains of MRSA from 15 countries associated with hospital outbreaks were compared with 27 strains of MRSA from 11 countries reported to be sporadic isolates. Twenty four of the 27 outbreak associated MRSA showed low expression of protein A and high expression of coagulase. Conversely, sporadic strains generally gave higher levels of protein A and a wide variety of coagulase reactions. The results suggest that many epidemic strains of MRSA may have phenotypic characteristics that distinguish them from sporadic strains.

Epidemiological approaches to 'epidemic neuromyasthenia': syndromes of unknown aetiology (epidemic myalgic encephalopathies).

The study of future outbreaks of 'epidemic neuromyasthenia' syndromes would be easier if there were a standing advisory arrangement for the co-ordination of basic standart studies, diagnostic tests and long term survey of patients. It is proposed that this might be achieved by establishing an 'EN Outbreaks Panel' at Colindale which, in addition to epidemiologists and virologists might include specialists in infectious diseases, neurology and psychiatry and other fields. The approach to such a panel would be through the Communicable Diseases Surveillance Centre (CDSC) when outbreak is suspected. Arrangements could then be made by the CDSC with the Epidemiological Research Laboratory and the Virus Reference Laboratory to assist immediate investigations and for a follow-up to ascertain the incidence of sequelae, as well as for long storage of sera and other specimens.

Syphilis epidemic in Slovenia influenced by syphilis epidemic in the Russian Federation and other newly independent states.

In 1994 an 18-fold increase in the annually reported incidence rate of early syphilis (to 1.81 cases per 100,000 total population) in comparison to the preceding year was observed in Slovenia. The reported incidence rates remained elevated for a period of five years. The possible link between the evolving syphilis epidemic in the Russian Federation and newly independent States (NIS) during the 1990s and the observed increases in the reported early syphilis incidence rates in Slovenia, was explored. Surveillance data obtained by mandatory reporting of all newly diagnosed syphilis cases between 1994 and 1999 were analysed according to the information on sexual partners, the country of probable source of infection, and occupation. More than half of all reported cases (62%) were directly or indirectly linked to a source of infection abroad. Among these, the majority of cases (73%) were linked to the Russian Federation or NIS. Of these, 68% occurred in males, a high proportion being long-distance lorry drivers.

The obesity epidemic as harbinger of a metabolic disorder epidemic: trends in overweight, hypercholesterolemia, and diabetes treatment in Geneva, Switzerland, 1993-2003.

Increases in obesity, hypercholesterolemia, and diabetes may be under way in Europe. We have reported the only data available from the 1990s for continuous monitoring of chronic disease risk factors in random samples of a general European population. In random surveys (1993-2003) of 6164 men and 6107 women in Geneva, overweight and obesity combined increased in both men and women; hypercholesterolemia prevalence also rose; diabetes treatment increased in men. Only population-based interventions can prevent the impending epidemic of obesity-related disorders.

Epidemic of dengue-4 virus in Yap State, Federated States of Micronesia, and implication of Aedes hensilli as an epidemic vector.

A dengue fever/dengue hemorrhagic fever (DF/DHF) outbreak in Yap State caused by dengue-4 virus was confirmed serologically and by virus isolation from serum samples collected on each of three island groups. Most DF/DHF cases occurred during a three-month period between mid-May and early August 1995. Five fatal cases, three of which were in children between the ages of four and 11, occurred between June 20 and July 26. A serosurvey conducted in late August revealed anti-dengue IgM prevalence rates of 18% on Yap, 36% on Eauripik, and 6% on Woleai. The majority of residents (93-100%) on the three islands were positive for anti-dengue IgG antibodies, indicating widespread exposure to dengue viruses. The IgG titers indicative of secondary antibody response were noted on Eauripik (6.5%) and Woleai (17%), but were rare on Yap (0.7%). Entomologic investigations implicated the native mosquito species, Aedes hensilli, a member of the Scutellaris Group of Aedes (Stegomyia), as a previously unrecognized epidemic vector of dengue viruses. Aedes hensilli was the most abundant and widespread member of Ae. (Stegomyia) in Yap State, the only species of Ae. (Stegomyia) on Woleai, and the only mosquito species present on Eauripik. New distribution records for mosquito species are reported.
