Additive genetic groups for animals evaluated in more than one breed association national cattle evaluation.

Additive genetic groups were included in the 1993 Red Angus Association of America national cattle evaluation for phantom parents of individuals who were registered with the American Angus Association (AAA). Genetic groups were formed for each component in two multiple-trait evaluations in which all animal effects were fit. Additive direct effects were included for birth weight, weaning weight (WW), and milk (MILK). In a second analysis the additive direct effect of 160-d postweaning gain was analyzed with WW and MILK. Of the 387,665 animals, 50,838 had at least one phantom parent assigned to one of five genetic groups fit as fixed effects for each additive component. Of these 50,838 animals, 1,324 were identified as registered with the AAA. An average of 906 animals per component had an AAA EPD available. Animals with a known AAA EPD were designated into one of three groups of equal numbers based on AAA EPD for each component (1 = low, 2 = medium, 3 = high). Animals in the fourth genetic group were those registered with the AAA but with no EPD available for the component. The fifth genetic group included all other animals with phantom parents. Grouping on AAA EPD allowed for EPD on animals out of parent(s) registered with the AAA to be more closely aligned to the AAA EPD because they were regressed from the group solution instead of zero. Grouping based on EPD from another NCE should be considered in the production of multibreed EPD.

Effects of enzyme-potentiated desensitization in the treatment of pollinosis: a double-blind placebo-controlled trial.

Several controlled clinical trials have shown that specific immunotherapy (SIT) using incremental injections of allergens can be effective in the treatment of allergic rhinitis and asthma. Nevertheless, the risk of side effects have led to some recommended limitations of SIT. Enzyme-potentiated desensitization (EPD) is a proposed method for immunotherapy with very low doses of mixed allergens plus beta-glucuronidase enzyme, for which irrelevant or no side effects have been claimed. The aim of this study was to determine the clinical efficacy of EPD in the treatment of pollinosis. A double-blind placebo-controlled trial of EPD among 20 patients sensitive to Parietaria and grass pollen was performed. All patients recorded daily symptom scores for nine months following a single intradermal injection of EPD or buffered saline received in February. Symptoms recorded were nasal itching and obstruction, sneezing, rhinorrhea, itchy eyes and excessive tear production. Moreover, total and specific lgE were measured and CD3+, CD4+ and CD8+ peripheral blood lymphocytes were counted at different times. In the same period, ten additional subjects, with an allergic clinical profile similar to the subjects admitted to the double-blind trial, were studied in an open clinical trial in order to evaluate the effects of EPD without enzyme using a mixture of allergens. Symptom scores were higher in the placebo group (p < 0.001), with a similar level of significance for both global symptom score and for each individual symptom. Active-treated patients had a significant post-treatment increase in the mean percentage of T-CD8+ peripheral blood cells and a significant post-seasonal decrease in the mean percentage of Parietaria specific lgE. On the contrary, placebo-treated patients had a borderline significant post-seasonal decrease in the mean percentage of CD8+ circulating cells and a significant seasonal increase in the mean percentage of Parietaria specific lgE with no significant post-seasonal decrease. Finally, clinical results of the mixture of allergens injection were similar to those of the placebo in the double-blind trial. EPD injection caused only an asymptomatic, local wheal and flare lasting about two hours. Two patients (20%) in the active-treated group experienced a delayed, mild, unusual headache lasting about two days. In conclusion, EPD is clinically effective in the treatment of pollinosis. Some immunological modifications observed in the EPD-treated patients suggest an EPD-induced enhancement of tolerogenic mechanisms like "immune deviation."

Results of pancreaticoduodenectomy for pancreatic cancer: extended versus standard procedure.

In Western experience, the long-term survival benefit after extended pancreaticoduodenectomy (EPD) in patients with pancreatic ductal adenocarcinoma is still controversial. The aim of this work was to evaluate weather EPD for pancreatic ductal adenocarcinoma prolongs long-term survival compared to standard pancreaticoduodenectomy (SPD). From November 1992 to September 1996, we performed pancreatic resections in 30 patients affected by stage I-III pancreatic ductal adenocarcinoma: 13 patients underwent SPD and 17 patients underwent EPD, consecutively. The two groups of patients were similar for all the demographic, clinical, and pathological characteristics, and all the intraoperative factors considered except the number of resected lymph nodes (mean number per case = 34.2 +/- 15.5 in the EPD group versus 12.8 +/- 3.6 in the SPD group, p <0.001) and the operative time (median time per case = 375 minutes in the EPD group versus 270 minutes in the SPD group, p = 0.009). Patients in the two groups experienced a similar postoperative course. The estimated survival probability at 1 and 3 years after operation was 0.76 (95% confidence interval [CI]: 0.49 to 0.90) and 0.24 (95% CI: 0.07 to 0.45) in the EPD group; 0.31 (95% CI: 0.09 to 0.55) and 0.08 (95% CI: 0.00 to 0.29) in the SPD group (p = 0.014). According to a Cox model, the treatment was associated with R0 patients' long-term survival (SPD versus EPD: hazard ratio (HR) = 4.82, 95% CI: 1.66 to 14.00, p = 0.004). Grading of tumor differentiation was confirmed to be a relevant prognostic factor (poor versus moderate: HR = 4.33, 95% CI: 1.49 to 12.61, p = 0.007), whereas type of resection had no significant effect (pylorus-preserving versus hemigastrectomy: HR = 1.49, 95% CI: 0.56 to 3.95, p = 0.42). The proportion of R0 patients with local recurrence was lower in the EPD group (20.0% versus 70.0%, p = 0.034).

Ependymins from the cerebrospinal fluid of salmonid fish: gene structure and molecular characterization.

So far, ependymins (Epds) have been sequenced only from cypriniform fish, and in the past all attempts have failed to characterize, on a molecular level, homologous Epd proteins in higher vertebrates. Therefore, rainbow trout (Oncorhynchus mykiss) Epds, which represent the predominant proteins of the cerebrospinal fluid, have been N-terminally sequenced and the encoding cDNA subsequently cloned using the polymerase chain reaction. Surprisingly, only 40-42% of the amino acids are identical with the corresponding sequences from goldfish (Carassius auratus), and no convincing immunological cross-reactivity is observed with an antiserum raised against purified Epds from C. auratus. O. mykiss possesses two highly homologous genes encoding Epds (Om-I, Om-II), a feature typical of a quasi-tetraploid species. Western analysis, using two specific antibodies against Epds from O. mykiss, revealed a variety of different glycosylation variants. In contrast to C. auratus, Epds from O. mykiss probably do not form disulfide-linked dimers. The structure of one Epd gene and its flanking regions have been determined for the Atlantic salmon (Salmo salar). Six exons were deduced by comparison with the corresponding cDNA sequence from O. mykiss (almost 98% homology with Om-II).

Predictive validation study of the Edinburgh Postnatal Depression Scale in the first week after delivery and risk analysis for postnatal depression.

BACKGROUND: Postnatal depression is a major public health problem. The aim of this study is to validate the use of the Edinburgh Postnatal Depression Scale (EPDS) in the early postpartum, and to identify the markers for risk of postnatal depression. METHODS: 815 women filled out an EPDS and general information questionnaire between the third and the fifth day postpartum. The women with an EPDS score of >8 and a randomized control group from those with scores of <8 were contacted 8 weeks postpartum. 363 women therefore had a structured diagnostic interview by telephone at 8 weeks postpartum (MINI-DSM-IV), without knowledge of their EPDS scores, to screen for a major or minor depressive episode. RESULTS: The sensitivity of EPDS was measured as 0.82 [0.78-0.86], with a positivity threshold of 9.5/30. For an estimated prevalence for all depressive episodes of 16.1%, the positive predictive value of EPDS was measured as 42.8% [39.1-46.5%]. Multivariate risk analysis using logistical regression identified the following as risk markers for postnatal depression: previous history of depression (postnatal or other), unemployment, premature delivery or stopping breast-feeding in the first month for non-medical reasons. CONCLUSION: The use of EPDS between the third and fifth day postpartum is valid. An EPDS score of >10 should be completed by a clinical assessment and suitable management. The risk markers identified here are clinical indices that can be used for first-line early screening by non-psychiatric health workers.

Does the Edinburgh Postnatal Depression Scale measure anxiety?

OBJECTIVE: The existence of a separate anxiety and depression dimension within the Edinburgh Postnatal Depression Scale (EPDS) has been reported previously. However, the concurrent validity of this anxiety subscale was never evaluated. We investigated whether (1) this existence of an anxiety subscale could be confirmed and (2) it more highly correlated with other measures of anxiety than the total EPDS. METHODS: The SCL-90-R, the EPDS, and the State-Trait Anxiety Inventory (STAI) were filled out by 197 pregnant women. A principal component analysis (PCA) was used for confirmation of the subscales and correlations were computed between the (subscales of the) EPDS and the other measures of anxiety. RESULTS: The existence of an anxiety scale within the EPDS was confirmed. However, this subscale did not yield higher correlations with other measures of anxiety than did the total EPDS. CONCLUSION: Investigators using the EPDS to screen for depression should realise that the instrument does not exclusively measure depression. It seems that both anxiety symptoms and depressive symptoms are more accurately measured when using the total 10-item EPDS than when using the subscales.

Overexpression of transforming growth factor-beta3 immunohistochemical staining in extramammary Paget's disease, but downregulated expression in Bowen's disease.

BACKGROUND: The role of transforming growth factor-beta (TGF-beta) in carcinogenesis is complex, with some reports indicating a tumor inhibition role and others indicating a tumor promotion role. In particular, TGF-beta3 is thought to play a key role in controlling epithelial homeostasis. Immunopositive p53 has been demonstrated in a variety of human malignant tumors and its role in oncogenesis and tumor progression is thought to be important. Extramammary Paget's disease (EPD) and Bowen's disease are skin cancers of unknown histogenesis. OBJECTIVE: To clarify the role of TGF-beta3 and p53 in EPD and Bowen's disease and to better understand the origin of these disorders. METHODS: Specimens were obtained from 12 patients with EPD and 12 patients with Bowen's disease seen at our clinic between 1993 and 2000. TGF-beta3 and p53 immunohistochemical staining was performed. RESULTS: In three of the 12 EPD patients and five of the 12 Bowen's disease patients, positive p53 staining was detected. In contrast, TGF-beta3 overexpression was detected in all EPD patients, whereas downregulated TGF-beta3 expression was detected in all Bowen's disease patients. CONCLUSIONS: The present data suggest different roles for TGF-beta3 in abnormal epidermal cells in EPD and Bowen's disease. Thus, TGF-beta3 expression may be modulated differently via a p53-dependent or -independent pathway in the pathogenesis of EPD and Bowen's disease. Moreover, high TGF-beta expression appears to be a useful indicator of tumor activity in EPD.

[German language version and validation of the Edinburgh postnatal depression scale].

BACKGROUND AND OBJECTIVE: There is no valid method in the German literature for assessing postpartum depressive disorders. This study was undertaken to translate into German, validate and test the reliability of the Edinburgh postnatal depression scale (EPDS). PATIENTS AND METHODS: Randomly selected women after childbirth (n = 110) underwent (on the fourth postpartum day) a semistructured interview after first having answered the translated EPDS questionnaire. The diagnosis of depressive disorder was made according to the the criteria for psychological disorders in the ICD-10. For validation the results of the EPDS were compared with the clinical diagnosis of depression. The calculation of sensitivity, specificity and positive prognostic value was related to the respective EPDS results. In addition the EPDS data were analyses as to their reliability. RESULTS: The average age of the tested women was 28.6 years; 72% were married and 45% were primiparae. For an EPDS total score threshold value of 9.5 the sensitivity was 0.96, the specificity 1.0, and a positive prognostic value of 1.0. In the reliability analysis for EPDS the Guttmann split-half reliability was 0.82 and the alpha-coefficient 0.81. CONCLUSIONS: The German version of the EPDS with ten questions is an "application friendly" as well as proven to be a valid and reliable method for supporting the diagnosis of postpartum depressive disorder. It is suitable for both clinical and research use.

Abnormal responses of electronic pocket dosimeters caused by high frequency electromagnetic fields emitted from digital cellular telephones.

High frequency electromagnetic fields emitted from digital cellular telephones (cell phones) occasionally cause abnormally high and erroneous indicated dose readings on electronic pocket dosimeters (EPDs). Electric field strength distribution around a cell phone transmitting 1.5 GHz band with a maximum power of 0.8 W was analyzed by using an isotropic probe with tri-axial dipole antennas. Five types of EPDs were exposed to the fields for 50 s under configurations relative to the cell phone. The electric field distribution expanded around the phone's antenna and had a maximum electric field strength of 36.5 +/- 0.3 V m(-1). The cell phone gave rise to erroneous indicated dose readings on four out of five EPDs. The maximum value of erroneous indicated dosage for 50 s reached 1,283 microSv, which was about 2.6% of the annual effective dose limit of 50 mSv. The electromagnetic susceptibility of the EPDs was higher in the sections where the semiconductor detectors or electric circuit boards were located. The distance required to prevent electromagnetic interference differed for each EPD and ranged from 2.0 to 21.0 cm from the cell phone. The electric and magnetic field immunity levels of the EPDs varied from 9.2 V m(-1) to greater than 37.6 V m(-1), and from 0.03 A m(-1) to greater than 0.51 A m(-1). The EPDs displayed erroneous dose readings during exposure but recovered their normal performance after the cell phone ceased transmitting. The electromagnetic immunity levels of the EPDs were either equal to or greater than the IEC-standard. The immunity levels should be enhanced greater than the IEC-standard from the standpoint of radiation protection. The simplest and most reliable measure to prevent potential malfunction is to prohibit the radiation workers from carrying cell phones to their workplace.

Extramammary Paget's disease--a proliferation of adnexal origin?

AIM: To investigate a possible follicular origin of extramammary Paget's disease (EPD). EPD is a predominantly intraepidermal tumour with extensive involvement of adnexal structures and high recurrence rates suggesting a follicular stem cell origin. Cytokeratin (CK) 15 and CK19 are considered markers for follicular stem cells located in the hair follicle bulge region. METHODS AND RESULTS: Formalin-fixed paraffin-embedded tissues of 12 cases of primary EPD (three anal, nine vulvar) were studied immunohistochemically with antibodies to CK15 and CK19. All cases of EPD showed polygonal Paget cells in the interfollicular epidermis, hair follicles, sebaceous and apocrine glands distributed individually, in nests and in gland-like areas. The polygonal Paget cells were intimately associated with small, flat, mitotically active, 'compressed' keratinocytes. The large Paget cells uniformly expressed CK19 in 12/12 EPD. The small 'compressed' keratinocytes showed strong cytoplasmic CK15 staining in 9/12 EPD with focal accentuation, while the polygonal Paget cells were negative. CONCLUSIONS: These histological and immunohistochemical observations allow the following conclusions: (i) the small, flat, 'compressed' keratinocytes are an integral part of EPD; (ii) the dual cell population is reminiscent of sebaceous glands with mature sebocytes and germinative keratinocytes; (iii) since both cell types express cytokeratins typical for follicular differentiation, EPD may be a proliferation of adnexal stem cells residing in the infundibulo-sebaceous unit of hair follicles and adnexal structures.

Characterization of the molecular mechanisms involved in the differential production of erythrose-4-phosphate dehydrogenase, 3-phosphoglycerate kinase and class II fructose-1,6-bisphosphate aldolase in Escherichia coli.

A gapA-pgk gene tandem coding the glyceraldehyde 3-phosphate dehydrogenase and 3-phosphoglycerate kinase, is most frequently found in bacteria. However, in Enterobacteriaceae, gapA is replaced by an epd open reading frame (ORF) coding an erythrose-4-phosphate dehydrogenase and an fbaA ORF coding the class II fructose-1,6-bisphosphate aldolase follows pgk. Although epd expression is very low in Escherichia coli, we show that, in the presence of glucose, the 3 epd, pgk and fbaA ORFs are efficiently cotranscribed from promoter epd P0. Conservation of promoter epd P0 is likely due to its important role in modulation of the metabolic flux during glycolysis and gluconeogenesis. As a consequence, we found that the epd translation initiation region and ORF have been adapted in order to limit epd translation and to create an efficient RNase E entry site. We also show that fbaA is cotranscribed with pgk, from promoter epd P0 or an internal pgk P1 promoter of the extended -10 class. The differential expression of pgk and fbaA also depends upon an RNase E segmentation process, leading to individual mRNAs with different stabilities. The secondary structures of the RNA regions containing the RNase E sites were experimentally determined which brings important information on the structural features of RNase E ectopic sites.

Postnatal depression and elation among mothers and their partners: prevalence and predictors.

BACKGROUND: Correlates and predictors of mood disturbances at 3 days and 6 weeks postpartum were assessed in Irish mothers and their partners. METHOD: The Edinburgh Postnatal Depression Scale (EPDS) and the Highs Scale were used to assess 370 mothers and their partners. Socio-demographic, clinical and obstetric data were collected at patients' first antenatal visit. Factors associated with EPDS scores of > or = 13 and Highs score of > or = 8 were examined. RESULTS: On the EPDS 11.4% of mothers scored > or = 13 at 3 days postpartum and 11% at 6 weeks, while 18.3% of mothers scored > or = 8 on the Highs Scale at 3 days and 9% at 6 weeks. Scores on the EPDS and Highs Scale were interrelated. Factors associated with EPDS scores of > or = 13 at 6 weeks were single status, unemployment, unplanned pregnancy, public status and bottlefeeding. The best predictors of EPDS > or = 13 at 6 weeks were mothers' scores on the EPDS and the Highs Scale at 3 days. Three per cent of partners scored > or = 13 on the EPDS at 3 days postpartum and 1.2% at 6 weeks. CONCLUSIONS: Factors associated with mothers' mood disturbance were readily identifiable and collected routinely at antenatal intake. Mothers' mood within 3 days of delivery was the best predictor of later postnatal depression. Paternal mood disturbance was rare. Certain women may be at increased risk for postnatal mood disturbance and may be amenable to early identification and intervention.

Extramammary Paget disease is characterized by the consistent lack of estrogen and progesterone receptors but frequently expresses androgen receptor.

Extramammary Paget disease (EPD) is an uncommon cutaneous malignant neoplasm that arises in areas rich in apocrine glands (perineum, vulva, and axilla). Apocrine gland origin or apocrine differentiation of cells of EPD has been suggested. Estrongen, progesterone, and androgen hormone receptors have been reported to exhibit a characteristic pattern of expression in mammary apocrine type carcinomas; however, their expression in EPD has not been elucidated fully. By using immunohistochemical methods, we studied the expression of steroid receptors in EPD on formalin-fixed paraffin-embedded tissue samples from 28 patients with EPD without associated visceral malignant neoplasms or adnexal carcinoma. Androgen receptor (AR) was identified in 15 of 28 cases. The proportion of AR-positive cells varied from 1% to more than 75%; 8 cases expressed AR in more than 10% of cells. Strong AR expression also was seen in the invasive carcinoma arising from 1 case of EPD. All cases lacked immunohistochemically detectable estrogen and progesterone receptors. The immunophenotype characteristic of apocrine carcinomas (AR-positive, estrogen receptor-negative, progesterone receptor-negative) was seen in a substantial proportion of EPD cases. Results suggest that AR expression is a factor in pathogenesis of EPD. This may be important for the therapy of recurrent or invasive disease.

Efficacy of alternative multivariate best linear unbiased prediction models for genetic evaluation of swine.

A comparison of the accuracy of alternative BLUP evaluations of swine performance data is reported. A simulated data set of performance for days to market, backfat, number born alive, number weaned, and litter weight in six herds was used for the evaluation. The data structure was derived by using the performance records of six herds sampled from the American Yorkshire Club's STAGES (Swine Testing and Genetic Evaluation System) database. For growth traits, 10,360 pig records from 129 sires and 897 dams were recorded. For maternal traits, records on 2,598 litters of 1,209 sows from 147 sires and 585 dams were included. The actual observed performance of each record was removed and replaced with simulated performance. These simulated data were analyzed by within- and across-herd BLUP models using STAGES and Animal Model (AM) procedures. Results indicate that the alternative BLUP procedures produced similar estimates. Correlations between STAGES and AM EPD ranged from .84 to .95. Correlations between STAGES EPD and true genetic value (G) ranged from .41 to .74, and correlations between AM EPD and G ranged from .40 to .74. On average, AM EPD had a .04 larger correlation with G than did STAGES EPD, although the difference in the correlations was not significant (P greater than .05). Trends in EPD for sires, dams, and pigs or sows were the same. Likewise, standard errors of prediction for AM EPD averaged 4% smaller than those for STAGES EPD. Computationally, the AM procedures used 15 to 20 times as much processing time as did STAGES procedures.

Across-breed expected progeny differences: use of within-breed expected progeny differences to adjust breed evaluations for sire sampling and genetic trend.

Data on 2,034 F1 calves sired by Angus, Hereford, Polled Hereford, Charolais, Limousin, Simmental, Gelbvieh, and Tarentaise bulls with Hereford or Angus dams and data on 3,686 three-breed-cross calves with 700 F1 dams of the same breed crosses were used for this study. Traits analyzed were birth, weaning, yearling, and 420-d weights (BWT, WW, YW, and W420, respectively) of F1 calves and WW of three-breed-cross calves. Expected progeny differences from national cattle evaluation programs for sires of F1 calves and cows for BWT, WW, YW, and net maternal ability (milk) were used to assess their value in prediction of crossbred performance. Regressions of actual F1 calf performance on sire EPD were positive for BWT (1.09 +/- .12 kg/kg of BWT EPD), WW (.79 +/- .14 kg/kg of WW EPD), YW (1.44 +/- .16 kg/kg of YW EPD), and W420 (1.66 kg/kg of YW EPD). These regression coefficients were similar to the expected value of 1.0 for BWT and WW but were larger than expected for YW and W420. Regressions of actual three-breed-cross calf WW on milk and WW EPD of their maternal grandsires were .95 +/- .14 and .42 +/- .10 kg/kg, respectively, and differed little from their expectations of 1.0 and .5, respectively. Observed breed of sire means for each trait were adjusted for sire sampling by using EPD regressions to adjust them to the average EPD of all sires of each breed born in 1970.(ABSTRACT TRUNCATED AT 250 WORDS)

Relationship of maternal milk expected progeny differences to actual milk yield and calf weaning weight.

Calf weaning weights and estimates of 205-d milk yield (n = 317) were obtained from 90 Polled Hereford (PH) and 80 Simmental (SM) cows to evaluate the relationship of these measures with maternal milk and total maternal EPD from national cattle evaluations. An inverse parabolic exponential function predicted 205-d milk yields for each cow from five estimates of daily milk yield taken once per month via calf weigh-suckle-weigh. Changes in calves' 205-d adjusted weights were greater than predicted (P < .02) by maternal milk EPD of PH and SM dams and PH maternal grandsires (MGS) but were similar to changes predicted by SM MGS (P > .50). These changes were also greater than those predicted by total maternal EPD of PH dams and MGS (P < .02) but were similar to those predicted in SM dams and MGS (P > .38). Interactions of maternal milk and total maternal EPD with sex of calf and year did not influence calves' 205-d adjusted weights (P > .10). A positive relationship was found in both breeds between 205-d milk yield EPD (calculated from 205-d milk yield estimates) and maternal milk EPD from national cattle evaluations. Objectively assessing the magnitude of this relationship was hindered by problems in estimating heritability and interpreting correlations between EPD. These data indicate that maternal milk and total maternal EPD from national cattle evaluations are reasonably good predictors of genetic differences in milk yield and weaning weight.

Relationship of sire expected progeny differences to milk yield in Brangus cows.

Milk yield from 160 Brangus cows sired by 65 Brangus bulls was measured over a 3-yr period with a single-cow milking machine to estimate the relationship of actual milk yield of daughters and their calves' BW with cow sire EPD for milk during the preweaning period. Milk yield was measured six times per year at an average 49, 78, 109, 138, 168, and 198 d postpartum. The regression of daughters' milk yield on sire milk EPD was quadratic (P < 0.01), and the initial linear portion of the curve differed among months (P < 0.05) at an average cow BW. Similarly, the regression of 6-mo average 24-h milk yield on sire milk EPD was curvilinear (P < 0.05). When cow BW was fitted as a covariate in the regression of 6-mo average 24-h milk yield on sire milk EPD, there was an interaction of cow BW with linear sire milk EPD and quadratic sire milk EPD (P < 0.10). The associated response surface suggested that the regression was primarily linear in cows weighing < or = 520 kg and curvilinear in cows weighing >520 kg. A trend existed for the regression of calf 205-d weight on grandsire milk EPD to be curvilinear (P < 0.21); however, the regression of calf 205-d weight on milk yield of their dam was linear (P < 0.01). Results from these data suggest that genetic potential for milk yield, and possibly the associated effects on calf BW transmitted through the grandsire, may have a practical maximum because of nutritional limitations that prevent the expression of genetic potential beyond that level, particularly in heavier cows, which suggests the need to match sire milk EPD and cow BW with production environment.

[High rate of early hospitalization in healthy newborns].

OBJECTIVE: The length of hospital stay of healthy term newborns and their mothers varies in different developed countries. The American Academy of Pediatrics defines early postpartum discharge (EPD) as a discharge occurring within 48 hours of postpartum. EPD has been advocated by patients as part of the humanization of care after delivery and by health services as a more efficient management of resources. Controversies in relation to EPD focus on its impact on initiation and maintenance of breastfeeding, the possible increase of readmissions of newborns with jaundice and the influence on newborn screening for endocrine and metabolic disorders. PATIENTS AND METHODS: Five years ago we started an EPD program for healthy term newborns. We present a descriptive observational study including a series of 2798 consecutive live newborns over a period of 19 months (April 1996 to October 1997). Data about breastfeeding at the time of discharge, coverage of hypothyroidism and phenylketonuria screening and readmissions for newborn jaundice were collected during this period. RESULTS: During the defined period of time, 2798 live newborns were registered. Of these, 2109 (75.38%) were included in the EPD group, the majority of them (75.86%) between 24 and 40 hours postpartum. Breastfeeding was implemented in 95.82% of the newborns, 3.56% of the mothers decided to use artificial formulas and 0.52% were prescribed artificial formulas due to health problems in the mother. In relation to newborn screening of endocrine and metabolic diseases, we found similar coverage of hypothyroid screening compared to the other 7 maternities in our province (public and private) and of phenylketonuria screening compared to the other 5 primary care districts. Regarding newborn jaundice, we detected 47 readmissions, which is 2.23% of the total EPD. These newborns were treated with phototherapy and none required exchange transfusion. The mean value of total serum bilirubin at the time of readmission was 18.7 mg/dl, with a range between 15.1 and 22.6 mg/dl. CONCLUSIONS: In our experience, 75.38% of live newborns were included in a EPD program that has been shown to be safe in relation to controversial subjects, although the limitations of an observational study must be taken into consideration. The safety of this program is inferred by the high proportion of breastfeeding on EPD (95.82%), coverage of endocrine and metabolic screening comparable to other surrounding hospitals and adequate control of hyperbilirubinemia in the newborn period.
