Vascular problems involving the brachiocephalic vessels may occur from atherosclerosis, trauma, vasculitis, infection, and procedures for congenital heart disease. A wide variety of symptoms and signs may result from such disorders. This study is a report of five unusual manifestations of brachiocephalic vascular diseases. Diagnosis, management, and previous surgical experience is discussed relating to each case illustrating the complexity and variability of brachiocephalic vascular pathology.

The degree of the participation of the right and the left lung in the respiration act was determined by the spirographic method with the use of the lateral test in 160 patients with various pathologies of the major cerebral vessels. In patients with pathological convolution of the vessels, stenoses, saccate and arteriovenous aneurisms in the internal carotid artery basin, the vital capacity of the right lung was found to be diminished (the diminution being statistically valid) regardless of the side of the cerebrovascular pathology.
Venous compression is a clinical entity distinct from deep vein thrombosis although the clinical signs may be indistinguishable. Reasons for venous compression are tumors, scars, hematomas, postoperative changes and anatomic variations. The differential diagnosis between compression and thrombosis is important because therapy and prognosis differ markedly between the two patient groups. Ultrasound, computed tomography and magnetic resonance tomography are the diagnostic tools of choice because they offer not only information about the intraluminal situation but also about the extraluminal pathology.

Arterial remodeling is currently being recognized as an important determinant in vascular pathology in which narrowing of the lumen is the predominant feature. Not only expansive remodeling (enlargement), but also constrictive remodeling (shrinkage) is observed in de novo atherosclerosis, in restenosis and in transplant vasculopathy. Expansive remodeling prevents and constrictive remodeling enhances luminal narrowing by plaque formation or intimal hyperplasia. The mechanisms of the opposite remodeling modes is unknown. Insight into the processes that determine the direction of local arterial remodeling may help to develop new strategies to prevent arterial occlusive disease. In the present paper the current status of research in the field of arterial remodeling in cardiovascular disease is reviewed. Mechanisms of arterial remodeling, potential interventions to influence the mode of remodeling as well as the methodological limitations of remodeling studies are discussed.

A case of a 96 year old woman with an incarcerated gallbladder in an epigastric hernia causing acute acalculous cholecystitis is described. At operation the gallbladder was replaced in its anatomical site and the hernia was repaired. This unique case of acute acalculous cholecystitis demonstrates cystic duct obstruction and cystic vessel compression as possible aetiologies for this pathology.

Ultrastructural change was investigated in the small intestinal mucosa of dogs after experimental dissection of the cranial mesenteric plexus and/or obstruction of the lymphatics. In group with a combination of dissection of the plexus and obstruction of the lymphatics (Group A): Marked mucosal edema and dilatation of intercellular spaces of epithelial cell were observed after the 6th postoperative hours (POH), and numerous pinocytotic vesicles appeared after 24th POH. After the 5th postoperative days, rough glycocalyx and irregular microvilli were recognized in epithelial cells. As a characteristic change, pinocytotic vesicles fused together and formed a complex. However, no lipid droplet were accumulated in epithelial cells. In group with obstruction of lymphatics (Group B): Epithelial cells were full in lipid droplets. Based on these evidences, it was suggested that diarrhea and nutritional disorder were mainly caused by the disturbance of absorption and water transport in epithelial cells, involving severe pathological changes in their ultrastructure.
