A 10 year follow-up of 109 patients with histologic Crohn's disease and anal lesions is reported. Fourteen patients (13 percent) have died, 7 from unrelated disorders. Ten required excision of the rectum, but only 5 for perianal disease (4.5 percent). Of the remaining 85 patients, 61 have been followed up to proctoscopy and rectal biopsy. Anal skin tags were still evident in 25 of 37 patients (68 percent), but new tags have appeared in only 2 patients. Ten of 53 fissures (19 percent) were still present at 10 years, and there were no new fissures. Seven of 21 patients (33 percent) still had fistulas but were asymptomatic; the remainder of the fistulas had healed spontaneously (8) or after operation (6). New fistulas have appeared in five patients. None of the patients have been in continent. These results indicate that perianal manifestations of Crohn's disease pursue a relatively benign course and are rarely an indication for proctectomy.
The vast majority of the pallidal neurons of the hamster, pigeon, caiman and turtle basal telencephalon were positively labeled by an antiserum against LANT-6, a neurotensin-like hexapeptide. In sharks also, LANT-6-positive neurons were observed in the apparent equivalent of the globus pallidus. These results, which imply the coexistence of a LANT-6-like peptide with gamma-aminobutyric acid (GABA) in pallidal neurons, suggest that a LANT-6-like peptide may be an important and evolutionarily conserved neurotransmitter/neuromodulator in pallidal neurons.

A randomized trial to evaluate day hospital care as an alternative to inpatient treatment in the acute phase of psychiatric illness was abandoned when too few patients were admitted to the trial to enable any valid generalization to be made concerning the total at-risk population. In accounting for the failure of the experimental design, attention is drawn to the inability of the research team to influence either the organization of the unit, whereby junior staff had no contact with the day hospital, or the prevailing belief among senior staff that the two treatment settings serve distinctive, not alternative, functions. The implications of our failure for the feasibility of conducting a successful and externally valid controlled trial in this research setting are explored.
The protist kinetid is considered a conservative indicator of phylogenetic relationship. This conclusion is supported by the apparently inverse relationship between the conservation of structure and its level in the organizational hierarchy. A comparative examination of features of protist kinetids indicates that pattern is strongly conserved; within and among flagellate taxa there is more variability in pattern than within the ciliates. It is concluded that the ancestral flagellate kinetid is a pair of orthogonally oriented kinetosomes having one striated rootlet and at least one microtubular ribbon. The ancestral kinetid of the ciliates is also a dikinetid structure, though represented by a paired set of parallel kinetosomes, probably similar to that found today in karyorelictid ciliates, presumptive descendants of the ancestral stock.
