Precancerous lesions of the cervix uteri in infertile women.

A study of 318 patients with tubal infertility and a control group of 200 unselected infertile women yielded 14 (4.4%) and 1 (0.5%), respectively, with precancerous lesions of the cervix uteri. The one patient in the control group with severe dysplasia was later shown to have tubal infertility. The overall incidence of premalignant lesions of the cervix uteri as reported to the National Cancer Registry of Norway was 0.1% for the age group and period studied. Women with tubal infertility represent a small but comparatively high risk group for the development of precancerous lesions of the cervix uteri.

Mucosubstances in the cervix uteri of the Indian langur monkey, Presbytis entellus entellus (Dufrense).

Mucosubstances in the cervix uteri of the Indian langur monkey, Presbytis entellus entellus was studied during non-pregnancy, pregnancy and lactation. Cells of the cervical epithelium and glands elaborated neutral, sialo- and sulfomucins during non-pregnancy. The concentration of these mucins was low during early pregnancy but increased during late pregnancy. The concentration of mucins was higher during later part of pregnancy than during non-pregnant stage which decline during lactation. Significance of mucins in cervix uteri and probable hormonal control over their elaboration is discussed.

[Cytological changes in the cervix uteri and bacterial vaginosis].

Bacterial vaginosis (BV) is a highly prevalent infectious disease causing numerous complications in obstetrics, gynecology, and in the newborns. The necessity of investigating BV is explained by its increasing incidence, including its asymptomatic forms, its probable sexual transmission, and its possible role in the etiology and pathogenesis of diseases of the cervix uteri because of exposure of the squamous epithelium of the cervix uteri, permanently regenerating and therefore highly sensitive to unfavorable effects, to high concentrations of opportunistic microorganisms. Our findings indicate that women with BV, particularly those with signs of another infection, are at risk of diseases of the cervix uteri. The risk group can be detected by simple and available methods for detecting BV with obligatory cytological studies, particularly in cases with inflammatory processes, and additional tests for associated infections (chlamydiosis, herpes simplex, etc.), whose role in the pathogenesis of cervical diseases is well known.

Ultrasonic assessment of efficacy of radiotherapy in cases of carcinoma of the cervix uteri by using rectosonography.

To assess the efficacy of radiotherapy in carcinoma of the cervix uteri, we used transrectal radial scan ultrasonography with the purpose of making an objective evaluation. The subjects were 29 patients with carcinoma of the cervix uteri who had radiotherapy between October 1984 and October 1986. Six cases (non-effective cases) showed localized relapse within 2 years of the end of radiotherapy. Transrectal radial scans were performed before, during, and at the end of radiotherapy, as well as two and six months after the end of radiotherapy. The characteristic ultrasound findings in the non-effective cases after radiotherapy were as follows: 1) increasing width of the parametrial echo; 2) a band or massive type parametrial echo; 3) a high intensity parametrial echo; 4) a rough contour of the parametrial echo, and 5) an increase in the size of the uterine cervix. It is considered that rectosonography is a useful diagnostic method for checking the therapeutic effect of radiotherapy in cervical carcinoma, and for the early detection of patients with poor prognosis.

Alveolar soft part sarcoma of the cervix uteri. A case report.

A case of alveolar soft part sarcoma (ASPS) of the cervix uteri of a 32-year old woman is described. Its differential diagnosis from histologically similar tumours is discussed. ASPS has been reported in the cervix uteri in one instance only in the literature. In spite of their unusual localization, these cases have to be recognized in order to avoid improper treatment.

[Determination of the proliferative activity of the cervix uteri epithelium in papillomavirus infection].

The results of examinations of 35 patients aged 17 to 42 years who had abnormalities in the cervix uteri were analyzed. Colposcopy detected an atypical epithelium in 100% of cases. To verify the diagnosis, biopsy specimens from the cervix uteri were morphologically studied prior to and following treatment. Before treatment, there was a slight proliferative activity of the epithelium in 45% of cases and its high proliferative activity in 15% in uncomplicated papilloma viral infection (PVI). A follow-up of complicated PVI indicated increases in the expression of a proliferative nuclear antigen (PVA) with higher dysplasia: a moderate response in 75% of D3 + PVI cases and a higher response in 60% of preinvasive cancers. Analysis of PCNA-immunological reactivity of biopsy specimens taken after treatment estimates the efficiency of the chosen complex of treatments: there was a negative result of immunohistochemical responses.

Concomitant chemobrachyradiotherapy with ifosfamide and cisplatin followed by consolidation chemotherapy in the treatment of locally advanced adenocarcinoma or adenosquamous carcinoma of the cervix uteri.

The optimal treatment of women with locally advanced adenocarcinoma or adenosquamous carcinoma of the cervix uteri is still undefined. We report a series of four consecutive patients with locally advanced adeno- or adenosquamous carcinomas of the uterine cervix (FIGO Stages IB-IIIB) treated by concomitant chemobrachyradiotherapy with ifosfamide and cisplatin followed by one to four cycles of consolidation chemotherapy with the same drug combination. After completion of this treatment all patients showed complete clinical remission. Now, after a median follow-up of 40 (range: 13.5-61) months all patients still present with no evidence of disease. Despite the low number of patients in this series we may conclude that concomitant chemobrachyradiotherapy with ifosfamide and cisplatin followed by consolidation chemotherapy with the same drug combination is an efficacious treatment of patients with locally advanced adeno- or adenosquamous carcinomas of the cervix uteri.

Progesterone receptor expression in human cervix uteri.

Progesterone receptor (PR) expression was studied in human cervix uteri from 21 cases. The study was performed on fresh frozen material with application of mouse anti-rabbit PR antibody and the immunoperoxidase method. Most cervical PR-s were present in the glandular epithelium and in smooth muscle cells. The results suggest that PR expression in the cervical glands is hormone-dependent; it was strongest in the presence of estrogen-stimulated endometrium, weaker with progesterone stimulation and absent in atrophy. The ectocervical epithelium contained relatively few progesterone receptors.

Serological responses to papillomavirus group-specific antigens in women with neoplasia of the cervix uteri.

Certain types of human papillomaviruses have been linked to the development of carcinoma of the cervix uteri. We have analyzed 114 serum specimens from women with cervical intraepithelial neoplasia (CIN) or carcinoma of the cervix uteri for the presence of serum antibodies against purified, disrupted bovine papillomavirus (BPV). The titers of immunoglobulin A (IgA) antibodies against BPV were slightly elevated (P less than 0.025) in the sera from CIN or cervical carcinoma patients compared with the titers of 139 serum specimens from sex- and age-matched healthy controls. In contrast, both the IgG and IgM serum antibody titers against BPV were significantly decreased for CIN and cervical carcinoma patients compared with those of healthy controls (P less than 0.001 and P less than 0.005, respectively). These results suggest that the difference between IgA and IgG or IgM antibodies to papillomavirus group-specific antigens may represent interesting serological parameters that could possibly be used in the epidemiologic study of women at risk for CIN.

[Diagnosis, therapy, and prognosis of primary reticulum cell sarcoma of the cervix uteri (author's transl)].

Until now 12 cases of primary reticulum cell sarcoma of the cervix uteri have been described in the literature. Two more cases are reported. Diagnosis is only possible after exclusion of a systemic disease. Principle rules for therapy cannot be given by these few cases. However it seems that combined surgical and radiological treatment provides the best chances for longer surviving. Experiences in chemotherapeutical treatment of reticulum cell sarcoma of the cervix uteri are not yet published. Four of the 14 patients mentioned in this study died from relapse. The prognosis of this type of cervical tumor seems to be rather good.

[Use of direct and serologic methods to detect Chlamydia trachomatis infections in cervix uteri of women].

Investigation included 254 women (18-75 years of age). In this number, 74 patients constituted a group with pathological changes in cervix uteri, while 180 women were seemingly healthy. The material was evaluated by a method of direct immunofluorescence (IF) with monoclonal antibody serum and a culture on McCoy cells. Antibodies in blood serum was detected by an immunoenzymatic test ELISA. Out of total number of 256 patients tested, elementary bodies of C. trachomatis were found in cylindrical cells of cervix uteri by the IF test in 54 (21.25%) of women. Analogical percentage of positive results was obtained by application of culture on McCoy cells. Positive serological results were found in 70 patients (27.55%). In group of 75 patients infected with C. trachomatis, 33 cases (44.59%) were confirmed by application of IF and cell culture, whereas positive serological results were found in 39 (52.70%) of investigated patients. In the group of seemingly healthy women in 11.60% elementary bodies of C. trachomatis were detected and in 17.22% of cases serological results were positive.

Primitive neuroectodermal tumor of the cervix uteri: a case report -- changing concepts in therapy.

BACKGROUND: Peripheral primitive neuroectodermal tumor (PNET) of the cervix uteri is extremely rare. Between 1987 and 2002, there have been eight cases described in the English literature. The treatment policies in these eight cases differed considerably, partly due to the rarity of the disease and to differing time periods of diagnosis and treatment. CASE: At the end of 2002, a 21-year-old woman presented with a PNET of the cervix uteri at our institute, the Erasmus Medical Center. For the appropriate treatment in this case, we reviewed the literature and decided that the treatment should be different from the local surgical treatment followed by additional treatments as most of the earlier reports describe. CONCLUSION: In view of the current knowledge of PNET belonging to the family of Ewing's sarcoma, and the improvement of treatment outcome in these tumors due to dose-intensive neo-adjuvant chemotherapy, patients with PNET of the cervix should be treated in accordance to the protocol for bony Ewing's sarcoma with multimodality therapy by means of induction chemotherapy, surgery, and consolidation chemotherapy.

[The cytological characteristics of the cells of the multilayer squamous epithelium of the cervix uteri in relation to the association of the pathological processes with the human papilloma virus].

An analysis of cytological material in benign (160 observations) and dysplastic (101 observations) processes, as well as in cervix outset cancer (38 observations) with the aim of search for koilocytic atypia markers in the cells, existing due to HPV infection, has been carried out. High association degree of the virus infection with the material under study has been ascertained. It was found out that the increase of koilocytosis phenotypic markers expression rate is a reliable criterion of cervix uteri pathologic processes progression. The signs of permanent koilocytic atypia in women with dysplasia should be considered as prognostic marker of malignant transformation of cervix uteri epithelium.
